Abstract

Speech quality is an essential output in assessing the success of a palatoplasty. The goal of a palatoplasty is not merely to create a simple anatomical closure of the palate, but also to create an adequate velopharyngeal mechanism for a normal speech outcome and to prevent abnormal maxillofacial development after surgery. The aim of this study is to find out the difference in speech outcome between post-cleft palate closure patients and patients without cleft palate. An analytical retrospective study was conducted on 22 children (n = 22) with complete unilateral cleft palate, who had been treated using two flap push back technic of palatoplasty during 2014-2017 by purposive sampling method, and 22 children without cleft palate as the control group. The evaluation of speech outcome was done using an assessment of perception by doing a speech pathologist and instrumental examination by taking a lateral cephalometry radiograph. The perception was assessed by the articulation pattern, hypernasality, and speech intelligibility. The lateral cephalometry radiograph was taken at /i/ phonation to measure the distance velum to the posterior pharynx wall. Data were analyzed using Mann Whitney test. The velopharyngeal competence in post-palatoplasty group consisted of 22.8% adequate result, 0.1% marginal result, and 68.1% inadequate result. Meanwhile, in the control group, there were 72.7% adequate and 27.3% inadequate competence. According to the result of the statistical test, this study concluded that there was a significant difference in speech outcome based on articulation pattern, hypernasality, speech intelligibility, and velopharyngeal distance between post-cleft palate closure patients and patients without cleft palate (p < 0.05). Majority of patients after cleft palate closure with two flaps pushback technique had inadequate velopharyngeal competence with moderate-severe hypernasality, severe nasal emission, abnormal speech intelligibility, and velopharyngeal distance ≥ 5.0 mm, whereas the majority of control group had an adequate velopharyngeal competence.

Abstract

Introduction: Odontogenic myxoma is a rare benign odontogenic tumour. Although it grows slowly, odontogenic myxoma is aggressive and has a high recurrence rate after surgical intervention. The tumour is most commonly found in the posterior mandible and is more common in females than in males. Radiographically, odontogenic myxoma presents as a unilocular or multilocular radiolucency with irregular or scalloped borders, frequently accompanied by displacement of teeth in the vicinity of the tumour. The treatment of odontogenic myxoma may be either conservative or radical surgery. Case and Management: A 57-year-old female patient presented with a palpable mass on the lower jaw that had been growing for a period of two years, accompanied by pain and displacement of the lower teeth. A clinical examination revealed the presence of a hard, non-painful, non-bleeding lump with approximate dimensions of 6x5x4 cm on the mandible, accompanied by displacement of the anterior lower teeth. An incisional biopsy led to the diagnosis of an odontogenic myxoma in the left mandibular region. The patient subsequently underwent surgery comprising segmental resection and plate reconstruction. Conclusion: Odontogenic myxoma is a rare odontogenic tumour with a high recurrence rate. In such cases, segmental resection with a margin of ≥ 1 cm and mandibular reconstruction with a plate is considered the treatment of choice.

Abstract

ABSTRACTIntroduction: The highest predilection for jaw tumors is located in the mandible. One of the management of tumors in the mandible is resection. The resection will result in a discontinuity in the jaw and disruption of the stomatognathic system, one of which is a decrease in the motoric function of the masticatory muscles. Electroneuromyography (ENMG) is a type of examination that includes an electroneurography (ENG) examination, which includes a Neural Conduction Study (NCS) based on stimulation value (STIM) and velocity (VEL), and electromyography (EMG) which can be used to assess motoric function impairment of masticatory and facial muscles in patients undergoing mandibular resection. This study analyzed the differences in the motoric function of masticatory and facial muscles post-segmental mandibulectomy and hemimandibulectomy. Methods: This preliminary analytical cross-sectional study compares ten post-segmental mandibulectomy or hemimandibulectomy patients who had undergone mandibular reconstruction in the Oral Surgery Department of Hasan Sadikin Hospital Bandung. Assessment was performed using an AO plate (Arbeitsgemeinschaft für Osteosynthesisfragen) towards the patients. The results were compared using Chi-Square and Mann-Whitney statistical analysis. Results: A decrease in masticatory muscle function was detected using ENMG. NCS and EMG assessments were obtained. There was no significant difference in NCS values (p>0.05) in patients with segmental mandibulectomy (mean STIM 4.2 ± 1.7, VEL 13.23 ± 5.38) and hemimandibulectomy (mean STIM 4.3 ± 1.35, VEL 12.56 ± 4.83), however, a significant difference was found in the EMG values (p=0.025; p<0.05) of the patients with segmental mandibulectomy (mean 70% of patients were normal) and hemimandibulectomy (mean 20% of patients were normal). Conclusion: There are differences in the decreased motoric function of masticatory and facial muscles post-segmental mandibulectomy and hemimandibulectomy.Keywords: motoric function; masticatory muscles; facial muscles; mandibular resection

Abstract

ABSTRACTIntroduction: The popular half-face helmet places the mandible at high risk of fracture during a motorcycle accident. The anatomical shape and position of the mandible make it more frequent to fracture. The purpose of this study was to compare the severity of mandibular trauma in motorcyclists using half face helmets and without using a helmet based on the Mandible Injury Severity Score (MISS). Methods: This research was a prospective cross-sectional study on 60 research subjects at the Emergency Unit of Hasan Sadikin Hospital, Bandung, between July 2019 - February 2020. The data were analyzed with the Mann-Whitney test to see the severity of mandibular trauma using a half face helmet and without a helmet based on MISS (Mandible Injury Severity Score). Results: Of 60 subjects of motorcycle accidents, 58.3% were <25 years old, 38 (63.3%) used half face helmets and 22 (36.7%) non-helmets, 83.3% of the patients were drivers, and 11.7% were passengers. Most fracture sites are parasymphysis, corpus, condylus, symphysis, and subcondylar. 48.3% of subjects had malocclusion, 56.5 with mild displacement, and 43,3% with moderate removal. The average MISS score for half face helmet users was 5.71 and compared to 7.45 for non-helmets. Based on the Mann Whitney test results, the MISS value was not significantly different. Conclusion: Riders who wore half face helmets and those who didn’t wear helmets have the same risk of mandibular trauma based on the MISS. Motorcycle users should use a full face helmet to prevent mandibular fractures during traffic accidents.Keywords: MISS, trauma of mandible, half face helmet, traffic accidents.

Abstract

ABSTRAKPendahuluan: Prosedur sinus lifting merupakan prosedur standar yang dilakukan di regio posterior maksila atrofi sebelum penempatan implan gigi, dengan perforasi membran sinus sebagai salah satu komplikasi yang dapat terjadi akibat keberadaan septa sinus maksilaris. Tujuan dari penelitian ini adalah untuk mengetahui teknik dan modifikasi terbaru dari sinus lifting yang tersedia untuk pasien dengan septa sinus maksilaris. Metode: Penelitian ini merupakan tinjauan sistematis yang mengacu pada kerangka Patient or Problem, Intervention, Control or Comparison, and Outcome (PICO) dan mengikuti pedoman Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA). Pencarian artikel dilakukan pada tiga database dan satu academic search engine, yaitu PubMed, Science Direct, CrossRef, dan Google Scholar, menggunakan kata kunci dan kriteria inklusi. Hasil: Sebanyak dua belas artikel memenuhi kriteria untuk dilakukan review yang menggambarkan beberapa teknik sinus lifting dan modifikasinya yang dapat digunakan dalam menangani pasien dengan septa sinus maksilaris. Pendekatan lateral window adalah teknik dasar yang umum digunakan. Namun, teknik alternatif mencakup pendekatan lateral window menggunakan panduan bedah, pendekatan lateral window dengan teknik floating septum, pendekatan single window palatal, dan pendekatan transcrestal yang dibantu oleh perangkat hidrolik merupakan alternatif lain yang dapat digunakan dalam prosedur sinus lifting pada pasien septa sinus maksilaris. Simpulan: Pendekatan lateral window untuk sinus lifting adalah teknik yang direkomendasikan dan paling umum digunakan untuk pendekatan bedah pada pasien yang memiliki septa sinus maksilaris, dengan hasil yang dapat diprediksi. KATA KUNCI: teknik sinus lifting, modifikasi sinus lifting, septa sinus maksilaris, implan dentalSinus lifting technique on patient with maxillary sinus septa: systematic reviewABSTRACT Introduction: The sinus lifting procedure is a standard procedure performed in the atrophic posterior maxillary region prior to dental implants placement, with sinus membrane perforation as one of the complications that can occur due to the presence of maxillary sinus septa. The aim of this study is to explore the latest techniques and modifications of sinus lifting available for patients with maxillary sinus septa. Methods: This study is a systematic review based on the Patient or Problem, Intervention, Control or Comparison, and Outcome (PICO) framework and follows the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. Article searches were conducted in three databases and one academic search engine: PubMed, Science Direct, CrossRef, and Google Scholar using specific keywords and inclusion criteria. Results: A total of 12 articles met the criteria for a review, describing several sinus lifting techniques and their modifications for patients with maxillary sinus septa. The lateral window approach is the basic technique commonly used. However, alternative techniques include the lateral window approach using a surgical guide, the lateral window approach with the floating septum technique, the single window palatal approach, and the transcrestal approach assisted by hydraulic devices. Conclusion: The lateral window approach for sinus lifting is the recommended and most commonly used technique for surgical intervention in patients with maxillary sinus septa, yielding predictable outcomes. KEY WORDS: Sinus lifting technique, sinus lifting modification, maxillary sinus septa, Implant dental

Abstract

Introduction: Implant placement after tooth loss can prevent an alveolar bone resorption. Bone grafting is used to obtain adequate quantity and quality of alveolar bone to support dental implants. Hydroxyapatite is the primary synthetic bone graft biomaterial, with a composition and structure similar to natural bone. Hydroxyapatite biomaterials have been widely researched and developed. This study aims to summarize the modified HA materials that have been successfully used in supporting dental implant treatment. Methods: This review was conducted using a systematic review method that refers to systematic literature review simplification with PICO framework, based on PRISMA guidelines. Literature searches were performed through Science Direct, PubMed, Google Scholar, SAGE Journals, and Cochrane Library, for articles published between 2016 – 2021. Inclusion criteria for this review consisted of research articles on the use of hydroxyapatite bone graft in dental implant treatment. Results: Analysis was carried out on 11 articles on the use of hydroxyapatite bone graft in dental implant treatment with various material modifications: sintered, carbonated, eggshell-derived, 3D printed, obtained by sponge replica method, and in combination with rhBMP-2, PRF membrane, collagen membrane, and amniotic membrane. Additionally, hydroxyapatite also comes in several forms: porous block, granular, and nano-sized. Treatment success was observed through histology and histomorphometry analysis; SEM, XRD, FTIR, CBCT, and CT-Scan imaging; and ISQ value. Conclusion: This review demonstrates the modified hydroxyapatite, in combination with other materials or in various form, successfully supports dental implant treatment. This success is attributed to good osseointegration between bone and implant, bone growth, and increase in bone thickness, which are influences by the materials composition and morphology.

Abstract

Objective: This case study is a case of infected dentigerous cyst involving all erupted incisor teeth and impacted mandible canine. Diagnosis of results from clinical examination, radiographic and histopathologic.Methods: The initial treatment was antibiotic therapy for 2 week, followed with cyst enucleation, extracted of the all fourth incisors and odontectomy lower left canine, lower premolar and upper right molar through intraoral approach under general anesthesiaResults: The evaluation of 2 weeks post surgery treatment showed the healing was good and the chin region showed no sign of inflammation, the color of the tissue was similar to its surrounding.Conclusion: Last sentence of abstract should be a conclusion or further treatment plan for the case. Abstract

Pendahuluan: Jaringan parut pada labioplasti adalah hasil yang tak dapat dihindarkan dari luka operasi. Jaringan parut ini dapat menimbulkan masalah secara fungsional, kosmetik dan psikologis. Penilaian objektif memberikan pengukuran kuantitatif pada jaringan parut, sedangkan penilaian subjektif bergantung pada penilaian pengamat. Skala yang digunakan untuk mengevaluasi berbagai tipe jaringan parut salah satunya Vancouver Scar Scale (VSS). Penilaian objektif yang digunakan pada penelitian ini adalah nilai Vascular Endothelial Growth Factor (VEGF) diketahui sebagai mediator angiogenesis yang mempromosi penyembuhan luka kutan dan merangsang pembentukan jaringan parut. Tujuan penelitian ini adalah untuk menganalisis korelasi kadar vascular endothelial growth factor saliva dengan gambaran klinis jaringan parut pada pasien pasca operasi. Metode: Penelitian ini merupakan penelitian korelasi analitik dengan desain cross sectional yang menilai korelasi antara korelasi antara kadar VEGF dengan gambaran klinis jaringan parut pada 36 pasien dengan kasus celah bibir unilateral yang telah dilakukan operasi labioplasti di Rumah Sakit Gigi dan Mulut Pendidikan Fakultas Kedokteran Gigi Universitas Padjadjaran. Subjek pada penelitian ini adalah pasien yang memenuhi kriteria inklusi. Pemilihan subjek penelitian untuk kelompok uji dilakukan secara nonprobability sampling. Setelah dilakukan tindakan labioplasti, dilakukan pengukuran kadar VEGF pada hari ke-21, selanjutnya dilakukan penilaian gambaran klinis jaringan parut dengan VSS pada hari ke-90. Data yang terkumpul dianalisis dengan menggunakan uji korelasi Rank Spearman. Hasil: Terdapat korelasi yang kuat dan bermakna secara statistik antara kadar VEGF dengan gambaran klinis jaringan parut. Nilai koefisien korelasi antara VEGF dan gambaran klinis jaringan parut sebesar r=0,804 (p=0.001), yang dapat diartikan sebagai korelasi positif. Simpulan: Terdapat korelasi antara nilai VEGF saliva dengan gambaran klinis jaringan parut pada pasien pasca operasi labioplasti.Correlation between vascular endothelial growth factor (vegf) saliva with clinical features of scar tissue in patients post labioplasty surgery: study cross sectionalIntroduction: Scarring in labioplasty is an unavoidable outcome of a surgical wound. Such scarring can cause functional, aesthetic, and psychological complications. Objective assessments provide quantitative measurements of scarring, whereas subjective assessments rely on observer judgement. The scale used to evaluate various types of scarring is the Vancouver Scar Scale (VSS). The objective assessment used in this study is the value of Vascular Endothelial Growth Factor (VEGF), known as a mediator of angiogenesis that promotes cutaneous wound healing and stimulates scar tissue formation. Methods: This study is an analytical correlation study with cross-sectional design that assesses the relationship between VEGF levels and the clinical picture of scarring in 36 patients with unilateral cleft lip who had undergone labioplasty surgery at the Faculty of Dentistry Teaching Dental and Oral Hospital, Padjadjaran University. The subjects in this study were patients who met the inclusion criteria. The selection of research subjects for the test group was carried out by non-probability sampling. VEGF levels were measured on the 21st postoperative day, and the clinical picture of scar tissue was assessed using the VSS on the 90th postoperative day. The collected data were analyzed using the Spearman rank correlation test. Results: The findings demonstrated a strong and statistically significant positive correlation between salivary VEGF levels and the clinical picture of scarring. The correlation coefficient between VEGF and the clinical picture of scarring was r=0.804 (p=0.001), indicating a significant association. Conclusion: There is a significant positive correlation between salivary VEGF levels and the clinical picture of scarring in patients after labioplasty surgery.

Abstract

Dentigerous cyst is an odontogenic cyst that surrounds the crown of an impacted tooth, caused by fluid accumulation between the reduced enamel epithelium and the enamel surface, usually associated with mandibular third molars. The purpose of this study was to find out the prevalence of dentigerous cyst caused by impaction of mandibular third molar at the oral and maxillofacial surgery department of Hasan Sadikin hospital Bandung. This study was a descriptive survey study. Samples were taken from the data of patient’s medical record from July 2006 until June 2011. The result of this study showed the number of oromaxillofacial cyst cases in the period was as much as 316 (0.78%). Dentigerous cyst was the most common odontogenic cyst (48,64%). Dentigerous cyst most commonly caused by impaction of the mandibular third molar (13,89%). The amount of male and female patients with dentigerous cyst was the same (50% each). The most age group that suffered dentigerous cyst due to impaction of the mandibular third molars was the age group of 41-50 years (40%). The most chosen therapy was enucleation (100%). From the results of this study can be concluded that dentigerous cyst due to impaction of mandibular third molars was the most common case, with the same frequency found in both male and female, the most age group of impacted was the age group of 41-50 years old, and the most chosen therapy was enucleation.

Abstract

Background. Ameloblastoma is a benign odontogenic tumor with an aggressive biological behaviour, and the surgical treatment frequently results in failure for the post operative recurrence. Purpose. The aim of this study was to evaluate the clinical result of the patient with aggressive recurrent ameloblastoma who underwent two times radical surgery to get recurrence free. Case. We report a patient who was diagnosed with ameloblastoma of the mandible 13 years ago and had undertaken operation hemimandibulectomy and mandibular reconstruction. However, recurrence occured and secondary surgical treatment (particularly radical) offers the best chance to the patient. The recurrence of an ameloblastoma mainly displays the ineffectiveness or perhaps lack of success of the main surgical treatment. Conclusion. The method has to be intense as well as radical in order to steer clear of reccurence. For ameloblastoma an ineffectiveness of initial surgical treatment indicates that more radical strategy led to minimum recurrence rate.
