Abstract

Osteosarcoma, which rarely originates from the axial bones such as pelvis, presents a major challenge in limb preservation. We report a 9-year-old girl with osteosarcoma on her buttock. She underwent wide excision “modified buttockectomy” to resect the tumour with sciatic nerve involvement. One year after surgery, she was able to walk and no recurrence was found. Keywords: buttockectomy, osteosarcoma ABSTRAKOsteosarkoma, tulang aksial seperti pelvis jarang sekali ditemukan, dan menjadi tantangan besar dalam upaya penyelamatan ekstremitas. Kami laporkan satu kasus osteosarkoma pada tulang panggul-daearah bokong pada seorang anak perempuan berusia 9 tahun. Pasien menjalani eksisi luas “buttockectomy” yang dimodifikasi untuk mengangkat tumor beserta nervus ischiadikus yang terlibat. Pada pengamatan satu tahun pasca pembedahan, pasien dapat berjalan dan tidak ditemukan tanda-tanda rekurensi. Kata Kunci: buttockectomy, osteosarkoma

Abstract

Introduction A freshwater fish Ophiocephalus striatus (OS) or known locally to Indonesian as haruan, has potentials to retain musclemass of critically ill patients following its high content of amino acids, fatty acids, minerals, and vitamins. This study was conducted to investigate the effect of OS on quantitative measurements of biceps brachii and rectus femoris in patients with mechanical ventilation. Methods This was a double-blinded randomized controlled trial at tertiary hospital between July- September 2019. The study included patients aged 18-65 years old with mechanical ventilation whereas patients with chronic kidney disease, paraparesis, or neuromuscular disorders were excluded. Forty-two patients were divided into extract group (n=19) who received 15 g of OS extract daily and control group (n=23) who received none. Quantitative measurements of both muscles were determined by measuring cross sectional area (CSA) using ultrasonography by one radiologist. Data were collected at 1st and 7th day, while the intervention was administered in-between. Independent sample T-test, Mann-Whitney test, and multivariate study using general linear model (GLM) were used for statistical analysis. Results The changes of CSA on biceps brachii and rectus femoris muscles from before to after intervention between extract group compared to control group were 5,30 ± 2,41 to 6,09 ± 1,81 vs. 5,16 ± 2,50 to 4,16 ± 1,94 (p=0.002) and 2,45 (1,10 – 5,90) to 3,47 (1,13 – 8,18) vs 3,85 (1,09 – 9,06) to 2,78 (1,06 – 7,27) (p=0.230), respectively. Multivariate analysis showed significant differences between extract and control group for both muscles with p value of <0.001 and 0.001 respectively. Conclusion Supplementation of Ophiocephalus striatus extract was associated with a significant increase of biceps brachii and rectus femoris CSA in patients with mechanical ventilation. Abstract

Tulang merupakan salah satu organ yang sering menjadi lokasi metastasis kanker, misalnya kanker payudara, prostat, dan paru. Metastasis tulang berpotensi memicu peningkatan mortalitas dan morbiditas, serta penurunan kualitas hidup. Gejala dan tanda klinis metastasis tulang mencakup nyeri, fraktur patologis, hiperkalsemia, serta kompresi medula spinalis. Oleh karena itu, deteksi dini dan tata laksana metastasis tulang yang tepat diperlukan untuk mencegah komplikasi lebih lanjut. Pertimbangan dalam memilih modalitas pemeriksaan radiologi, sebagai langkah utama dalam diagnosis, harus dilakukan secara rasional berdasarkan lokasi lesi metastasis tulang. Selain itu, pemeriksaan alkalin fosfatase dan biopsi juga dapat menjadi pilihan pemeriksaan penunjang. Tata laksana metastasis tulang mencakup terapi suportif, seperti kontrol nyeri, pemberian bifosfonat, pemberian denosumab atau terapi sistemik, penggunaan radiofarmaka, dan kontrol lokal, yaitu radiasi eksterna atau pembedahan.

Abstract

Spinal cord injury is a common type of injury that can be highly fatal. Clinically, spinal cord injury can be assessed using standardized physical examination scoring from International Standards for Neurological and Functional Classification of Spinal Cord Injury Patients, which is the American Spinal Injury Association (ASIA) impairment scale or often called as AIS. The results of this scoring will then be combined with magnetic resonance imaging (MRI) examination to determine the diagnosis and prognosis. In this case, we report a 20-year-old male complaining weakness in four extremities after traffic accident while driving a motorcycle. Based on neurological examination, the patient can barely feel a touch sensation and cannot move both his arms and legs at all. Cervical X-ray examination showed dislocation fracture on C5 level. Further MRI examination showed edema and spinal cord hemorrhage around fracture area. The findings of edema and spinal cord hemorrhage on MRI examination are useful in diagnosing and predicting the outcome of patient with spinal cord injury. Abstract

Background: Sarcopenia has a notable impact on the prognosis of hepatocellular carcinoma (HCC). The prevalence of sarcopenia in patients with HCC is relatively high, with studies reporting rates between 39% and 41.7%. A substantial proportion of individuals diagnosed with HCC also exhibit muscle wasting and reduced muscle function. This study aims to determine the prevalence of sarcopenia in Indonesian patients with HCC (based on the Japanese Socuety of Hepatology (JSH) JSH criteria) and to examine its correlation with poor performance status.Methods: We performed a cross-sectional study of 85 HCC patients between January and October 2021). The skeletal muscle index at L3 (L3 SMI) was measured on CT scans (applying JSH cut-offs for sarcopenia). Clinical data, including Eastern Cooperative Oncology Group (ECOG) performance status, were collected. Bivariate analyses and logistic regression were performed to identify factors associated with sarcopenia.Results: A total of 85 HCC patients (median age: 52 years) were included in the analysis. Sarcopenia was observed in 49.4% of the cohort. Bivariate analysis revealed a statistically significant association between sarcopenia and an ECOG performance status of 2+ (p = 0.003), as well as a MELD-Na score of ≥15 (p = 0.023). In multivariate analysis, poor ECOG-PS remained independently associated with sarcopenia (adjusted OR 4.17; 95% CI 1.50–11.56, p = 0.006).Conclusion: Sarcopenia is highly prevalent among Indonesian HCC patients, and it is strongly associated with poor ECOG performance status.

Abstract

BACKGROUND Temporomandibular joint (TMJ) dislocation is uncommon, yet it is associated with a significant negative effect on the patient’s quality of life. Magnetic resonance imaging (MRI), a gold standard for diagnosing TMJ dislocation, is expensive, time-consuming, and cannot be performed on patients with pacemakers and metallic prostheses. On the other hand, high-resolution ultrasonography (HRUS) has low cost, high accessibility, and is less time-consuming. This study aimed to compare the accuracy and reliability of HRUS to MRI in diagnosing TMJ dislocation. METHODS A comprehensive literature search was conducted using PubMed, Cochrane Library, ScienceDirect, and EBSCOhost databases. Keywords such as temporomandibular joint dislocation, temporomandibular displacement, MRI, and ultrasonography were utilized for the search. The articles obtained were then selected based on the inclusion and exclusion criteria, and the quality assessment was conducted using the QUADAS-2 tool. RESULTS 5 studies were included in this systematic review. The critical appraisal results showed sensitivity ranging from 74.3–93.7%, specificity from 84.2–100%, positive predictive value from 68.2–100%, negative predictive value from 64.0–98.3%, and accuracy from 77.7–91.7% of HRUS compared to MRI. CONCLUSIONS HRUS is a reliable method for diagnosing TMJ dislocation. However, MRI is still necessary in selected and more advanced cases.

Abstract

The Coronavirus disease 2019 (COVID-19) pandemic impacts pulmonary cancer management since it shares similar clinical features and creates fear among patients to visit hospitals due to possible in-hospital disease transmission. We report a patient who presented with a rare case of a pulmonary neuroendocrine tumor with an ocular involvement, which, unfortunately, experienced a delay in diagnostics. The first hospitalization was due to superior vena cava syndrome, pleural and pericardial effusions, and swollen left eye. The patient was diagnosed with pulmonary cancer, released after the symptoms were relieved, and expected to visit a referral hospital for further diagnostics and treatments. The patient returned two weeks later with progressing disease, an ocular metastasis, and a reactive serum IgM/IgG to SARS-CoV-2; serial qPCR tests consistently returned negative. The patient was treated with the best supportive care before succumbing to death. Biopsy showed pulmonary tumor cells consistent with a neuroendocrine tumor. Fear of the pandemic makes patients reluctant to seek help from medical facilities. Pulmonary TBC has similar symptoms to pulmonary cancer, which can pose another challenge in diagnosing pulmonary cancer in TB-endemic countries. Thus, patients often present with advanced-stage pulmonary cancer with rare ocular metastasis, as in this report.
