Abstract

Fungus comb is a structure made by termite colony Macrotermitinae with lots of potential because it contains secondary metabolites such as steroids, terpenoids, and saponins and compounds containing antioxidant and anti-tyrosinase properties. This study aimed to determine fungus comb extracts' fingerprint pattern, antioxidant capacity, and anti-tyrosinase activity. Thin layer chromatography (TLC) was used to determine fingerprint patterns. The resulting TLC pattern was processed with ImageJ and converted into a densitogram. The fungus comb was extracted with n-hexane and ethyl acetate. TLC results showed that fungus comb extracts contain steroids and terpenoids and have antioxidant properties. The antioxidant capacity assay was carried out with 2,2-diphenyl-1-picrylhydrazyl reagent. N-hexane extract had an antioxidant capacity value of 9.07 mg ascorbic acid equivalent (AAE)/g extract, while ethyl acetate extract had 6.04 mg AAE/g extract. Anti-tyrosinase activity assay was carried out with L-tyrosine and L-3,4-dihydroxyphenylalanine (L-DOPA) as substrates. N-hexane extract had a better anti-tyrosinase activity with IC50 of 1804 µg/mL for L-tyrosine and 6784 µg/mL for L-DOPA.

Abstract

Extrapulmonary Tuberculosis (EPTB), caused by Mycobacterium tuberculosis, affects organs other than the lungs, most commonly the lymph nodes and pleura. Although pulmonary tuberculosis (TB) is more prevalent in Indonesia, EPTB remains a significant yet often underreported condition. Its diagnosis depends on histological, microbiological, and clinical evidence, with treatment typically initiated based on a physician's judgment. The diverse symptoms of EPTB, depending on the affected organs, contribute to its frequent underdiagnosis. Histopathological examination is the gold standard for EPTB diagnosis, particularly valuable when microbiological tests yield inconclusive results, and is characterized by well- and poorly-organized granulomas. Methods this study reviewed six cases of EPTB, focusing on clinical presentations, radiological findings, and histopathological results. The patients presented with lumps in various locations, including the wrist, spine, neck, abdomen, thigh/groin, and scrotum. Ultrasound and CT scan were employed to assess organ involvement, and tissue biopsies were analyzed histopathologically. The cases exhibited diverse symptoms and clinical findings. Blood tests and chest radiographs were normal in all patients. Imaging studies revealed nonspecific hypoechoic patterns, omental thickening, and prominent mesenteric lymph nodes in some cases. Histopathological analysis consistently identified granulomas in all biopsy samples. Some specimens showed extensive caseous necrosis with Langhans giant cells, while others exhibited fibrotic tissue with well-formed granulomas and minimal caseous necrosis. Conclusion diagnosing EPTB remains challenging due to its paucibacillary nature and the limitations of current diagnostic tools. Histopathological and microbiological evaluations of biopsy specimens are essential to improving diagnostic accuracy and reducing the morbidity and mortality associated with EPTB.

Abstract

Medicinal plant utilization in the clinical management of metabolic disorders, including Diabetes Mellitus (DM) and its associated complications, has gained considerable scientific attention in recent decades. Their perceived tolerability positions them as viable complementary or adjunct options to standard pharmacological regimens. Purwoceng (Pimpinella pruatjan Molk.) is a plant indigenous to Indonesia that demonstrates notably high concentrations of antioxidant constituents. Given that oxidative stress constitutes a fundamental driver in the pathophysiology of DM, it is implicated in the development of complications including diabetic peripheral neuropathy. Objective: To review existing evidence on the potential of purwoceng in reducing blood glucose levels and repairing nerve damage in diabetic neuropathy, including its mechanisms, active compounds, and toxicity profile. Methods: A literature review was conducted using keywords including Pimpinella pruatjan/Molk, Purwoceng, Antioxidant, Diabetes Mellitus, Diabetic Neuropathy, and secondary metabolites. Relevant studies were selected and critically reviewed. Results: Several studies identified secondary metabolites in purwoceng, including phenolics, flavonoids, tannins, coumarins, saponins, terpenes, and steroids. These compounds exhibit antioxidant and anti-inflammatory properties, suggesting potential benefits in reducing hyperglycemia and preventing or repairing nerve damage in diabetic neuropathy. Available evidence also indicates no significant acute toxicity in renal and hepatic organs. Conclusion: Purwoceng exhibits promising potential as an adjunctive therapeutic agent in the management of Diabetes Mellitus and its neuropathic complication, mediated principally through antioxidant and anti-inflammatory mechanisms, with a favorable preliminary safety profile. Rigorous clinical investigations are warranted to substantiate its efficacy and safety in human subjects.
