Abstract

Introduciton Since then the pathogenesis of NPC has been intensively studied, specifically aimed at geography and racial variation. In recent years many environmental and biological factors have shown a risky relationship to the occurrence of NPC and the latest research results indicate the role of genetic and viral factors in the development of this disease Objectives Etiology of nasopharyngeal cancer (NPC) is multifactorial, and many of these factors overlap where one factor may occur together with other elements as a cause. Cigarette smoking is thought to also play a role in the development of NPC. Methods The study design was in case of control with an analytical approach during February-October 2007. To find out the relationship between Cigarette smoking NPC, a case-control study was conducted, with a sample of 96 patients with NPC as case and 96 people as a control. Case group and control samples were taken from RSUP H. Adam Malik Medan and RSU Dr. Pirngadi Medan. Results The majority of NPC patients are: Men (compared with women 2.84: 1), 50-59 years (29.2%), and farming (32.3%). The Batak ethnic is the largest ethnic group suffering from NPC 54 people (56.3%) and followed in the second place the most are Javanese (29.2%). The most histopathological type is WHO type 3 (38.6%). Most stages were III (58.4%), followed by stage IV (40.6%), stage II (1%), and there was none of stage I.In univariate logistic regression showed a significant relationship between smokers with consumption of cigarettes 11-20 cigarettes per day with OR=2.530 (p=0.021) with the occurrence of NPC. However, in the multivariate logistic regression analysis, the number of cigarettes per day did not show a significant relationship (p=0.587). The results of multivariate logistic regression analysis, nasopharyngeal carcinoma were significantly associated with people who had started smoking before the age of 20 years (p=0.000; OR 5.35 and CI 95% 2.290-12.499), consuming habits of salted fish before 10 years of age with sometimes frequency consumption of salted fish p=0.000; OR 7.766 (95% CI 2.937-20.538), often p=0.000; OR 16.515 (95% CI 5.3000-51.463), and the habit of using firewood p=0.014; OR 3.147 (95% CI 1.260-7.860). There was no significant relationship between the duration of cigarette smoking (p=0.293), a number of cigarettes (p=0.021) and the types of cigarettes smoked (p=0.081) with the incidence of NPC. Conclusion Cigarette smoking as a risk factor for NPC cannot act stand-alone as a risk factor, but there is a role for other factors that also influence as a risk factor.

Abstract

The coronavirus pandemic (COVID-19) is designated as a Public Health Emergency that is troubling the world, much remains unknown about the virus that causes COVID-19, but in general, this virus is transmitted through direct contact with droplets from the respiratory tract of an infected person (which exits through coughs). And sneezes). People can also become infected by touching a surface contaminated with this virus and then touching their face (eg, eyes, nose, mouth). The school health program (SHP) is a health program implemented at the school level. The main activities of SHP are carried out through health education, health services, and fostering a healthy school environment with the primary target of students, educators, educators, and the school community. Efforts to maintain health can be started from the school environment, which emphasizes changing behavior. Therefore this SHP program is essential because it includes early health education. This community service aims to provide education and socialization to prevent the spread of the COVID-19 virus to teachers and students based on UKS at Amanah SMPS, Binjai District, Langkat Regency. Before being given education, assessing the COVID-19 prevention behavior that they have implemented is carried out using a valid questionnaire. Then proceed with educational activities. The educational method carried out was by lecturing and discussing the prevention of COVID-19 and simulating the proper use of masks, after which several participants were asked to practice it and witnessed by other participants. It is hoped that with this community, serving students and teachers will increase their understanding and be able to prevent COVID-19 both for themselves and prevention in the community.

Abstract

Background Leukemia is the most common malignancy in childhood. Malnutrition is the main nutritional disorder occurring in children with cancer. Nutritional supportive care is a medical modality that has been associated with improved tolerance to chemotherapy, survival, and quality of life, as well as decreased risk of infection in children undergoing cancer therapy. Objective To examine differences in induction phase outcomes in well-nourished and malnourished pediatric acute lymphoblastic leukemia (ALL) patients. Methods This prospective study was conducted in newly-diagnosed ALL patients aged 1 to 18 years who underwent induction phase chemotherapy at Haji Adam Malik General Hospital, Medan, North Sumatera. Mid-upper arm circumference (MUAC) was used to determine nutritional status. Patient characteristics such as complete blood count (CBC) at the time of diagnosis, occurrence of febrile neutropenia, duration of induction, length of hospital stay, and remission status were collected. We analyzed differences in CBC, frequency of febrile neutropenia, as well as duration of induction and hospital stay between well-nourished and malnourished patients. We also assessed the difference in remission attainment between the two groups. Results There were 21 subjects in each group. Compared to well-nourished subjects, malnourished ones had a higher median incidence of febrile neutropenia [1 (range 0 to 30) vs. 3 (range 0 to 4); P=0.04], longer mean induction duration [64.2 (SD 11.5) vs. 71.2 (SD 10.6) days; P=0.046], and longer median hospital stay [30 (range 19 to 56) vs. 36 (range 22 to 49) days; P=0.001] compared to well-nourished subjects. There was no significant difference in CBC parameters and remission status after induction between the two groups. Conclusion Malnourished pediatric ALL patients had a significantly higher incidence of febrile neutropenia, duration of induction phase, and duration of hospital stay compared to well-nourished ALL patients.

Abstract

This study aims to determine the relationship of family support to medication adherence in MDR-TB patients. This research method is descriptive correlation with cross sectional research design. The research sampling technique is total sampling. The results showed that adherence to taking medication in MDR-TB patients was more commonly found in good family support, namely as many as 23 respondents (92.6%), the results of the chi-square test obtained a p-value = 0.000 meaning there was a relationship between family support medication adherence in MDR-TB patients. In conclusion, there is a relationship between family support and medication adherence in MDR TB patients. Keywords: Family Support, Medication Compliance, MDR-TB.

Abstract

This study aims to identify an effective health promotion model in efforts to prevent stunting through the Maternal and Child Health program at the Tembilahan Health Center, Riau Province. This research is a quantitative research using Quasi Experiments with a pre and post-test control group design approach. The research sample was 102 women of childbearing age, namely 34 people in each group who were taken using the consecutive sampling technique. Data collection using a questionnaire. Univariate and bivariate data analysis using paired t-test statistics. The results showed that there were differences in the increase in knowledge in the treatment group p=0.000 (p<0.05) and there were differences in attitudes in the treatment group and the control group p=0.000 (p<0.05). In conclusion, a health promotion model that is effective in providing increased knowledge is a health promotion model of lectures and leaflet media and a health promotion model that is effective in providing attitude changes is a health promotion model of lectures and video media. Abstract

Depresi merupakan salah satu masalah kesehatan mental yang sering dialami oleh lansia akibat perubahan biologis, psikologis, dan sosial yang terjadi selama proses penuaan. Kondisi depresi yang tidak ditangani dapat menurunkan kualitas hidup lansia. Salah satu intervensi nonfarmakologis yang dapat digunakan untuk mengurangi depresi adalah terapi tertawa, yang mampu meningkatkan hormon endorfin, menurunkan stres, serta memperbaiki suasana hati dan interaksi sosial lansia. Penelitian ini bertujuan untuk mengetahui pengaruh terapi tertawa terhadap tingkat depresi pada lansia di UPTD Pelayanan Sosial Lanjut Usia Binjai. Metode: Penelitian ini menggunakan desain quasi experimental dengan pendekatan two group pretest posttest design. Populasi penelitian terdiri dari lansia di UPTD Pelayanan Sosial Lanjut Usia Binjai dan Panti Sosial Karya Kasih Medan. Sampel berjumlah 84 responden yang dibagi menjadi kelompok intervensi (42 responden) dan kelompok kontrol (42 responden). Tingkat depresi diukur menggunakan instrumen Geriatric Depression Scale (GDS). Kelompok intervensi diberikan terapi tertawa selama satu bulan dengan frekuensi tiga kali seminggu selama 20–30 menit per sesi, sedangkan kelompok kontrol menjalani aktivitas rutin panti. Analisis data menggunakan uji dependent t-test dan independent t-test dengan tingkat kemaknaan α = 0,05. Hasil penelitian menunjukkan bahwa terdapat penurunan tingkat depresi pada kelompok intervensi setelah diberikan terapi tertawa dibandingkan sebelum intervensi. Selain itu, terdapat perbedaan rerata tingkat depresi yang signifikan antara kelompok intervensi dan kelompok kontrol setelah perlakuan. Kesimpulan terapi tertawa berpengaruh efektif dalam menurunkan tingkat depresi pada lansia. Intervensi ini dapat dijadikan sebagai terapi komplementer yang mudah, aman, dan ekonomis untuk meningkatkan kesehatan mental serta kualitas hidup lansia.

Abstract

Neonatal sepsis is one of the most common causes of neonatal death in the first month of life. Neonatal sepsis often shows atypical clinical features include temperature instability, lethargy, skin changes, feeding problems, and many others. Isolation of bacteria from the culture is the gold standard for diagnosing sepsis, but this approach is frequently time-consuming and even in cases of negative blood cultures, sepsis cannot be conclusively ruled out, posing a major challenge in its diagnosis and management. Early diagnosis and timely intervention are essential to improve the prognosis of neonatal sepsis. Aim: The aim of this study is to discuss how infection markers can aid in the early identification of neonatal sepsis and what are the best infection markers to predict neonatal sepsis mortality. Method: This study assessed the diagnostic accuracies of infection marker used in the diagnosis of neonatal sepsis. Included studies were retrieved by searching three major databases (Google Scholar, Pubmed, and Science Direct) using MeSH words and relevant references, and reviewed based on the inclusion/exclusion criteria. The article discusses potential advancements and future prospects for biomarkers. An excellent marker can guide antibiotic administration and duration if sepsis is confirmed, reducing unnecessary use. The marker should have high sensitivity, specificity, positive predictive value, negative predictive value and accuracy.

Abstract

Antibiotics have been overused, the unwise use of antibiotics has become a special concern and has led to resistance. Almost 50% of antibiotic therapy is initiated inappropriately and without correct identification of the etiological agent. The high number of inappropriate antibiotic prescriptions contributes to the occurrence of antibiotic resistance and increased risk of death. MDR pathogens result in a 40% increase in mortality in hospital-acquired infections. The purpose of this study was to discuss the pattern of the resistance of antibiotic therapy in pediatric patients admitted to the PICU. Method: The search strategy was conducted through three databases (Google Scholar, Pubmed, Science Direct) which published during 2015-2025, using MeSH words “Antibiotic Resistance” AND “Pediatric intensive care unit”. There were thousand studies found, then by using the PRISMA flowchart, which total ten studies were included. PICU has a higher ratio of pathogen composition and bacterial resistance. Many factors can lead to antibiotic resistance, it might be caused by unnecessary or suboptimal prescribed antibiotic therapy from community before, previous health care and the hospital itself. The high number of inappropriate antibiotic prescriptions contributes to the occurrence of antibiotic resistance. Antibiotic resistance can lead to longer hospital stays and increased risk of mortality.

Abstract

In children and adolescents, depressive symptoms often presenting as irritability, anhedonia, or behavioral changes. Despite significant advancements in survival rates due to improved therapeutic protocols, the psychological burden experienced by children with ALL.Objective: To evaluate the relationship between age and the presence of depressive symptoms in children diagnosed with ALL.Methods: A cross-sectional study was done using consecutive sampling with 40 children aged from 7 to 18 years old acquired, conducted from August to October 2024 at the outpatient unit of Adam Malik Hospital, Medan. Forty children aged 7–18 years with a confirmed diagnosis of ALL were included. Depressive symptoms were assessed using the Patient Health Questionnaire-9 (PHQ-9). Categorical variables were analyzed using the chi-square test, followed by bivariate to identify significant predictors of depression.Results: Among the 40 participants, the most age group were children ≤ 10 years old (65%). A statistically significant association was found between age group and the presence of depressive symptoms (p = 0.001, OR= 33,3). Conclusions: Age is significantly associated with the severity and presence of depressive symptoms in pediatric patients with ALL. Older children were more likely to report higher levels of depression. Psychological evaluation and targeted mental health support may be especially important in older age groups.

Abstract

Background: Malaria remains a significant public health concern, particularly in endemic regions, is frequently associated with anemia in children. Accurate hemoglobin measurement is crucial for diagnosing anemia, especially in resource-limited settings. This study compares the accuracy of SD Biosensor Standard G6PD Analyzer with Hemocue HB301 System for hemoglobin measurement among children in malaria-endemic areas. Objective: To determine the sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV), positive likelihood ratio (LR+), negative likelihood ratio (LR-), pretest odds, post-test odds, and overall accuracy of the SD Biosensor Standard G6PD Analyzer compared to the Hemocue HB301 System. Method: This cross-sectional diagnostic study involved 325 children aged 6-18 years in Kualuh Leidong District. Secondary data were obtained from a previous study conducted in August 2024, using a total sampling technique. Result: The mean hemoglobin level measured by SD Biosensor Standard G6PD Analyzer (14,22±2,21 mg/dL) was higher than that measured by Hemocue HB301 System (13,45±1,63 mg/dL), with significant difference (p=0,001). The prevalence of anemia detected by SD Biosensor and Hemocue was 12,3% and 11,4%, respectively. The SD Biosensor Standard G6PD Analyzer demonstrated a sensitivity of 70,27%, specificity of 95,14%, PPV of 65%, NPV of 96,14% and an overall accuracy of 92,31%. Conclusion: The SD Biosensor Standard G6PD Analyzer exhibited relatively low sensitivity but high specificity compared to Hemocue HB301 System in measuring hemoglobin levels among children in malaria-endemic areas. Further research is needed to compare both devices against the gold-standard automated hematology analyzer and to identify the factors influencing measurement accuracy.
