Abstract

Obesitas dan berat badan lebih masih menjadi faktor risiko dari berbagai jenis penyakit seperti diabetes mellitus, penyakit jantung koroner, osteoartritis, dan penyakit refluks gastroesofageal (GERD). Penelitian ini dilakukan untuk mencari hubungan antara berat badan lebih dengan penyakit refluks gastroesofageal. Sebanyak 80 orang pasien Poliklinik Penyakit Dalam Rumah Sakit Umum Mitra Medika Tanjung Mulia Medan diikutkan dalam penelitian ini. Sampel diukur berat badannya dalam kilogram, dan tinggi badannya dalam meter, kemudian dicari Indeks Massa Tubuhnya (IMT) dengan cara membagi berat badan dengan tinggi badan kuadrat. Sampel juga diwawancarai menggunakan kuesioner GERD-Quest (GERD-Q). Selanjutnya data dikumpulkan, diolah, dan dianalisis menggunakan uji Chi-Square dengan aplikasi Statistical Package for Social Science (SPSS) versi 21. 47 (58,75%) dari 80 sampel merupakan orang dengan IMT berat badan lebih dan 33 (41,25%) dengan IMT bukan berat badan lebih. 15 (18,75%) orang ditemukan memperoleh skor GERD-Q 8, yang terdiri dari 8 (53,3%) orang kategori berat badan lebih dan 7 (46,6%) orang bukan kategori berat badan lebih. Data tersebut dijadikan tabel 2x2 yang kemudian dilakukan uji Chi-Square. Diperoleh hasil beda proporsi sebesar 0,04 (p=0,45) dan rasio prevalen (PR) sebesar 0,34 dengan interval kepercayaan (IK) sebesar 0,22-2,51. Dari hasil penelitian ini dapat disimpulkan bahwa tidak terdapat hubungan antara IMT kategori berat badan lebih dengan GERD.

Abstract

Aging is a natural biological process that leads to a decline in physiological functions, particularly in the musculoskeletal system, making older adults more susceptible to joint pain and limitations in functional mobility. A home care approach through non-pharmacological interventions such as elderly exercise is needed as a promotive and preventive effort to maintain independence and quality of life among older adults. This study aimed to implement a home care program through education and practice of elderly exercise to reduce joint pain and improve functional mobility in older adults. The method used was a pre-experimental design with a one-group pretest–posttest approach, conducted through educational, participatory, and applicative strategies that included the stages of preparation, implementation, and evaluation. The participants consisted of 35 older adults aged 60–75 years in Hamlet I, Tanjung Anom Village, Deli Serdang Regency. The intervention involved health education and elderly exercise practice for 15–20 minutes, consisting of warm-up, core movements, and cool-down sessions. Evaluation was carried out by measuring joint pain levels using the Numeric Rating Scale (NRS) and functional mobility using activities of daily living (ADL/IADL) assessments. The results showed a reduction in joint pain levels from moderate to mild and an improvement in functional mobility, as indicated by increased independence of older adults in performing daily activities. It was concluded that the implementation of a home care program through elderly exercise was effective in reducing joint pain and improving functional mobility, and it is recommended to be applied sustainably at the community level with the support of families and health cadres.

Abstract

This study aimed to analyze the effect of cancer stage on hemoglobin levels and to examine the moderating role of chemotherapy cycles in this relationship using Orem’s Self-Care Deficit Theory as a conceptual framework. This analytical cross-sectional study involved 54 cancer patients who had undergone at least one cycle of chemotherapy at H. Adam Malik General Hospital, Medan, Indonesia. Secondary data were obtained from medical records, including cancer stage, number of chemotherapy cycles, and hemoglobin levels. Data were analyzed using a Generalized Linear Model (GLM) with an interaction term to assess moderation effects. The results revealed that cancer stage had a significant negative effect on hemoglobin levels (β = -128.226; p < 0.001), indicating that more advanced cancer stages were associated with lower hemoglobin levels. In contrast, the number of chemotherapy cycles showed no significant effect (β = -0.004; p = 0.609) and did not moderate the relationship between cancer stage and hemoglobin levels (β = 0.025; p = 0.379). These findings suggest that cancer stage is a more dominant factor influencing hemoglobin levels than chemotherapy exposure. From a nursing perspective, the findings highlight the importance of early anemia screening, hematological monitoring, patient education, and self-care support, particularly for patients with advanced-stage cancer. This study was limited by the use of secondary data and a single-center setting, which may restrict the generalizability of the findings. Future studies are recommended to include larger samples, longitudinal designs, and additional clinical variables that may influence hemoglobin levels in cancer patients
