Abstract

This is a randomized, two-arm, double-blind, placebo-controlled study that will be performed among older adults in Kelantan, Malaysia, for 12 weeks. This study aims to determine the effectiveness of a 12-week Cosmos caudatus (C. caudatus) extract supplementation on dietary intake, cognition, mental health, sleep quality, disability, gut microbiota, physical activity, frailty, metabolites, and protein synthesis pathways among older adults with sarcopenia. This study will involve 64 older adults diagnosed with sarcopenia that will be enrolled and randomly allocated with 1:1 ratio, where 32 subjects required in both intervention and placebo group. The intervention group will receive 500 mg/day of C. caudatus supplementation, while the placebo group will be given 500 mg/day of maltodextrin. Study outcomes including sarcopenia status, dementia, motoric cognitive risk, dietary pattern, anthropometric data, physical fitness, mental health, disability and sleep quality will be measured. Blood will be taken for analysis of protein synthesis pathways (using blood plasma) and untargeted metabolomics at baseline and the 12th week. Fecal sample will be collected from a subsample of 24 subjects for gut microbiota analysis. This study is one of the pioneers randomized controlled trials to evaluate C. caudatus efficacy on various health related outcomes among sarcopenic community-dwelling elderly. This study findings are hoped to have the potential to improve health awareness of ulam or fresh salad consumption, specifically C. caudatus and prove its effectiveness as a nutraceutical product for older adults with sarcopenia. The Australian New Zealand Clinical Trials Registry (ANZCTR) has this trial listed as registered with code (ACTRN12623000046606p).

Abstract

The interaction that occurs between warfarin and antibiotics is a pharmacokinetic interaction which cause bleeding. This study aims to determine whether there is a relationship between the use of cephalosporins and an increase in APTT and PT in cardiovascular patients. This retrospective observational study employs case-control analysis to examine APTT and PT features in cardiovascular patients receiving single anticoagulant or anticoagulant + cephalosporin therapy for the period JulyDecember 2020. The sampling method used in this study is a convenient sampling method that meets the inclusion and exclusion criteria. Analysis of the data used is Logistic Regression. The study found among 65 male and 15 female, 40 patients used anticoagulant, and another 40 patients used anticoagulant and cephalosporine. Among 40 patients who used combination anticoagulant and cephalosporin, 22 of them had experienced of increasing APTT and PT. Cephalosporins showed a significant correlation with APTT and PT, with a P-value of 0.015. From this study, it can be concluded that the use of cephalosporins increases in APTT and PT values in cardiovascular patients who are given anticoagulant drugs and cephalosporins

Abstract

This study aims to determine the effectiveness of clinical pathway implementation in pediatric patients with acute diarrhea with mild-moderate dehydration on clinical outcomes, length of stay, use of quality and quantity of antibiotics at North Jakarta Teaching Hospital. This study is a quantitative study using a retrospective cohort case study method. The samples in this study were pediatric patients with a diagnosis of acute diarrhea with mild-moderate dehydration who were hospitalized at the North Jakarta Teaching Hospital for the period January-December 2020 and JanuaryDecember 2021 before and after the implementation of the clinical pathway, totaling 274 patients. The data used is secondary data in the form of medical records of pediatric patients with acute diarrhea with mild-moderate dehydration who were hospitalized at the North Jakarta Teaching Hospital. The data that has been collected is then processed and analyzed using the chi-square test. The results showed Qualitative use of antibiotics with the Gyssens method after the application of clinical pathway is better than before the application of clinical pathway. Quantitative antibiotic administration with the DDD method in pediatric patients with acute diarrhea with dehydration before and after the application of clinical pathway the highest was ceftriaxone antibiotics after the application of clinical pathway with DDD/100 patient-days value increased compared to before clinical pathway.
