Abstract

The coronavirus disease 2019 (COVID-19) pandemic has encouraged global vaccine research, yet vaccine effectiveness in the elderly remains a concern due to immunosenescence. The aim of this study was to compare the cytokine response elicited by an inactivated virus-based COVID-19 vaccine between elderly and young adults, focusing on key cytokines involved in cellular and humoral immunity: tumor necrosis factor-alpha (TNF-α), interleukin (IL)-2, IL-6, IL-10, and interferon-gamma (IFN-γ). A cross-sectional study was conducted in the Jakarta-Bogor region of Indonesia from January 2023 to December 2023. The study population was divided into two age cohorts: elderly (60−85 years) and younger adults (30−40 years). Blood samples were collected twice, after the first booster dose and four weeks after the second booster dose. Serum cytokine concentrations were measured using Luminex assays with microparticles conjugated to monoclonal antibodies against TNF-α, IL-2, IL-6, IL-10, and IFN-γ. Comparisons of the cytokine levels were conducted using Student’s t-tests or Mann-Whitney U tests as appropriate. A total of 74 individuals were included, with 37 each in the elderly and young adult groups. The results showed significant differences in cytokine responses between the two age groups. After the first booster, the levels of IL-6 and IFN-γ were significantly higher in young adults compared to the elderly. After the second booster, the levels of IL-6 were still significantly higher in the young adult group compared to the elderly group (p=0.001). Data indicated that after the second booster dose, the levels of TNF-α increased significantly in the young adult group only (p=0.004), while the levels of IL-2 (p=0.040) and IFN-γ (p=0.006) increased in the elderly group only compared to after the first dose. IL-10 levels increased in both groups (both had p=0.020). This study highlights that young adults had stronger pro-inflammatory responses, while the elderly relied more on IL-2 and IFN-γ for T-cell immunity, suggesting the need for vaccination strategies for the elderly to optimize immune responses.

Abstract

Background: Anatomy is a fundamental component of medical education, playing a pivotal role in understanding the structure and function of the human body. Cadaver-based anatomy learning remains widely used in many medical faculties; meanwhile, plastination as an alternative method, offering efficiency and practicality. This study aimed to evaluate the effectiveness of cadaver and plastination learning media in improving anatomical understanding among first-year medical students.Methods: This was a cross-sectional quantitative study with a pretest-posttest control group design conducted in November-December 2023. The study included 69 first-semester medical students from the Universitas Pertahanan, Republic of Indonesia who had not previously received musculoskeletal anatomy lectures. Participants were divided into cadaver and plastination groups. A questionnaire was distributed before and after the anatomy class. Data were analyzed with the Mann-Whitney, Wilcoxon, and paired sample t-test.Results: Both cadaver and plastination groups showed a significant increase in post-test scores (p<0.05), indicating that each method effectively improved students’ anatomical understanding. However, there was no significant difference between the two groups (p-value=0.253), suggesting comparable effectiveness between cadaveric and plastinated teaching media.Conclusions: Cadaveric and plastinated learning media are equally effective in improving anatomical knowledge among first-semester medical students. A thorough understanding of human anatomy is essential not only for clinical competency but also for advancing knowledge related to wellness, healthy aging, and the management of degenerative diseases.

Abstract

Malaria remains a significant global health concern, with 249 million cases and 608,000 deaths reported in 2022. Early detection, especially in endemic and resource-limited settings, is crucial to interrupt transmission. Rapid Diagnostic Tests (RDTs) are widely used due to their speed and practicality, requiring no laboratory infrastructure. This review aimed to evaluate the diagnostic accuracy (sensitivity and specificity) of conventional RDTs (HRP2, pLDH, combination) and ultra-sensitive RDTs (uRDTs), and compare their effectiveness across different populations and clinical settings. Literature searches were conducted in PubMed, ScienceDirect, and Cochrane (2020–2025), including studies in English or Indonesian. A total of 30 studies were included based on the PRISMA guidelines, with study populations encompassing children, neonates, adults, pregnant women, clinical patients, and community members in both endemic and imported settings (involving more than 50,000 individuals in total). Most RDTs demonstrated high specificity (>90%), but sensitivity varied widely (0 to >95%), influenced by parasitemia level and population characteristics. HRP2-based RDTs were effective for Plasmodium falciparum, but HRP2 gene deletions caused false negatives. pLDH-based RDTs performed better in multi-species infections. U-RDTs improved the detection of low-density parasitemia, especially in pregnant women and asymptomatic individuals. However, sensitivity declined in neonates and adults with low parasitemia. While some RDTs outperformed microscopy in clinical sensitivity, PCR remains the gold standard, especially for detecting subclinical infections. In conclusion, RDTs provide rapid and specific detection, particularly for moderate to high-density P. falciparum infections. However, in low-density or asymptomatic cases, their limited sensitivity necessitates confirmatory testing using microscopy or PCR.

Abstract

Old age according to the World Health Organization in 2017, is someone who has entered the age of 60 years and over. Due to various aging processes that occur in the elderly, the elderly will experience many complaints, one of which is bone pain and anemia. Various conditions in the elderly can cause bone pain and anemia, including: osteoporosis, osteomalacia, renal osteodystrophy, osteonecrosis, malignancy or bone metastases. From these conditions, a lymphocyte profile can be seen using a white blood cell differential scattergram in areas A, B, C, D, and E, according to the purpose of the study, wanted to know the scattergram profile of lymphocytes in the elderly with bone pain and anemia. This research method uses descriptive observation with a cross-sectional design method. The study was conducted from February-June 2020. The research subjects were elderly patients who experienced bone pain and anemia. Bone pain was measured by the Numeric Rating Scale on a scale of 1-10. Anemia was measured by examining hemoglobin on a hematology analyzer, then scattergram analysis was carried out through WDF using Sysmex XN 1000. The results of this study showed that the scattergram profile of lymphocytes in most elderly subjects with bone pain and anemia was in the SSC (A2, A3), SFL area. (B2, C2, D3, E3), which means that there are many changes in the lymphocyte profile, more atypical lymphocyte cells and suspicion of abnormal plasma cells or lymphocyte cells.

Abstract

Diabetes Mellitus (DM) is a chronic metabolic disease with a high global prevalence. A serious complication, diabetic foot, can cause high morbidity and an increased risk of amputation. In Indonesia, about 15% of DM patients develop diabetic foot, with an amputation rate reaching 30%. Hyperbaric Oxygen Therapy (HBOT) is offered as an adjunctive therapy to improve tissue oxygenation, angiogenesis, and wound healing. This study evaluates the effectiveness of HBOT in improving diabetic foot based on the Wagner score through a retrospective observational design using secondary medical records from 2021–2024. A total of 30 patients were analyzed using the Friedman and Wilcoxon tests. The results showed a significant association between HBOT and diabetic foot improvement (p < 0.001), particularly in patients undergoing more than 10 therapy sessions, who exhibited significant Wagner score improvement (p < 0.001). These findings confirm HBOT’s role in reducing diabetic foot severity. Komplikasi serius, yaitu kaki diabetik, dapat menimbulkan morbiditas tinggi dan risiko amputasi; di Indonesia, sekitar 15% pasien DM mengalami kaki diabetik dengan angka amputasi mencapai 30%. Terapi Oksigen Hiperbarik (TOHB) ditawarkan sebagai terapi tambahan untuk meningkatkan oksigenasi jaringan, angiogenesis, dan penyembuhan luka. Penelitian ini mengevaluasi efektivitas TOHB terhadap perbaikan kaki diabetik berdasarkan skor Wagner melalui desain observasional retrospektif menggunakan data sekunder rekam medis pasien tahun 2021–2024. Sebanyak 30 pasien dianalisis dengan uji Friedman dan Wilcoxon. Hasil menunjukkan hubungan signifikan antara TOHB dan perbaikan kaki diabetik (p < 0,001), terutama pada pasien yang menjalani ?10 sesi terapi yang mengalami peningkatan skor Wagner signifikan (p < 0,001). Temuan ini mengonfirmasi bahwa TOHB berkontribusi signifikan menurunkan derajat keparahan kaki diabetik. Kata Kunci:         Terapi Oksigen Hiperbarik, Terapi Tambahan, Kaki Diabetik

Abstract

Community service aims to determine the health picture of Indonesian soldiers in the 1st Infantry Division of Kostrad Cilodong. Therefore, researchers conduct general health screening by measuring body mass index, blood pressure, and glucose levels in the blood. This study used a descriptive method. This method describes a subject or object under study in depth, detail, and breadth. This study used two types of data: primary and secondary. The results of community service as a whole, when viewed from blood pressure measurement data and glucose levels in the blood, can be said to be normal. The routine of Indonesian soldiers is like regular exercise, a healthy and stable diet, and adequate rest influences to reduce risk factors for diabetes mellitus.

Abstract

Obesity is a medical condition characterized by excessive body fat accumulation, which is a major risk factor for hypertension, heart disease, stroke, and diabetes. Waist Height to Ratio (WHtR) is an anthropometric indicator used to predict hypertension risk. This study analyzes the relationship between WHtR and hypertension in operational and non-operational military personnel. The observational analytic study uses a cross-sectional approach and a prospective research design, employing non-parametric tests like Pearson's Chi-Square and Fisher's Exact. With a simple random sampling technique, 74 subjects were selected. Results show a positive correlation between WHtR and hypertension in both groups, indicating that higher WHtR values increase hypertension risk. Fisher's Exact test yielded two-tailed p-value = 0.031 and one-tailed p-value = 0.022, both significant at < 0.05. The study concludes that there is a significant relationship between WHtR and hypertension. Kondisi ini salah satu faktor risiko utama hipertensi, penyakit jantung, stroke, dan diabetes. Waist Height to Ratio (WHtR) salah satu indikator antropometri yang digunakan untuk memprediksi risiko hipertensi. Penelitian ini bertujuan menganalisis hubungan WHtR sebagai indikator obesitas terhadap hipertensi pada prajurit TNI satuan operasional dan non operasional. Metode yang digunakan observasional analitik dengan pendekatan studi cross sectional dan rancangan penelitian prospektif dan menggunakan uji non-parametrik yaitu uji Chi Square Pearson dan Fisher’s Exact. Serta menggunakan teknik penelitian simple random sampling dengan jumlah sampel 74 subjek. Hasil penelitian terdapat korelasi positif antara pengukuran WHtR dengan hipertensi pada kedua satuan tersebut, sehingga menunjukkan nilai WHtR yang tinggi berbanding lurus dengan peningkatan risiko hipertensi. Pada Uji Fisher’s Exact didapatkan p-value (two-tailed) = 0,031, p-value (one-tailed) = 0,022 dengan nilai signifikan < 0,05. Kesimpulan penelitian terdapat hubungan antara indikator WHtR terhadap hipertensi. Kata Kunci:         Obesitas, Hipertensi, Antropometri, WHtR, Prajurit TNI

Abstract

The 2022 WHO report reveals that 81% of adolescents and 27.5% of adults are insufficiently active, with Indonesia's rate rising from 26.1% in 2013 to 33.5% in 2018. This study aims the link between physical activity and cognitive function among 43 staff members at the Indonesian Defense University (UNHAN RI) using a cross-sectional design and consecutive sampling. Data were collected through the Indonesian version of the Montreal Cognitive Assessment (MoCA-INA) via interviews and the International Physical Activity Questionnaire (IPAQ) from December 2023 to January 2024. Chi-square and Fisher tests (p < 0.05) analyzed the relationships between physical activity, cognitive function, gender, and age. Results showed low physical activity led to a 44.4% decline in cognitive function, while moderate and high activity showed declines of 20% and 47.1%, respectively. However, no significant relationships were found between physical activity and cognitive function, age, or gender (p > 0.05). Keywords:          Cognitive Function, IPAQ, MoCA, Physical Activity   Abstrak Laporan WHO tahun 2022 menyoroti bahwa 81% remaja dan 27,5% orang dewasa kurang aktif, dengan angka di Indonesia meningkat dari 26,1% pada tahun 2013 menjadi 33,5% pada tahun 2018. Penelitian ini bertujuan untuk mengetahui hubungan antara aktivitas fisik dan fungsi kognitif pada 43 staf di Indonesia Universitas Pertahanan (UNHAN RI) dengan menggunakan desain cross-sectional dan konsekutif sampling. Data dikumpulkan melalui Montreal Cognitive Assessment versi Indonesia (MoCA-INA) dengan wawancara untuk mengisi kuesioner dan International Physical Activity Questionnaire (IPAQ) dengan mengisi kuisioner dari Desember 2023 hingga Januari 2024. Uji chi-square dan Fisher (p <0,05) menganalisis hubungan antara aktivitas fisik, kognitif fungsi, jenis kelamin, dan usia. Temuan menunjukkan bahwa aktivitas fisik yang rendah menyebabkan penurunan fungsi kognitif sebesar 44,4%, sedangkan aktivitas sedang dan tinggi menunjukkan penurunan masing-masing sebesar 20% dan 47,1%. Namun tidak terdapat hubungan yang signifikan antara aktivitas fisik dengan fungsi kognitif, usia, atau jenis kelamin (p > 0,05). Kata Kunci:         fungsi kognitif, IPAQ, MoCA, aktivitas fisik

Abstract

Hipertensi merupakan salah satu Penyakit Tidak Menular atau PTM yang menjadi faktor risiko utama penyakit kardiovaskular dan juga obesitas. Faktor risiko untuk perkembangan hipertensi primer diantaranya adalah riwayat keluarga, meningkatnya usia dan indeks massa tubuh, berat badan lahir rendah, serta peningkatan konsumsi natrium. Pengabdian kepada Masyarakat dengan tema “Penyakit Tidak Menular pada Usia Remaja di Universitas Ibn Khaldun” dilaksanakan pada 28 Oktober 2024. Adapun peserta yang ditargetkan pada kegiatan ini meliputi mahasiswa dan mahasiswi ya ng berusia 17-25 tahun di Universitas Ibn Khaldun. Kegiatan yang dilakukan berupa penyuluhan tentang hipertensi yang dilakukan secara langsung dengan tahapan paparan materi oleh tim yang terdiri dari mahasiswa Prodi Sarjana Kedokteran FK Militer Unhan beserta dosen sekaligus dokter, dilanjutkan dengan sesi tanya jawab, dan diakhiri dengan kesimpulan. Peserta berjumlah ±800 peserta. Pada pelaksanaan terlihat peserta berperan aktif dan diharapkan para peserta dapat mengetahui langkah preventif dan dapat meningkatkan kesadaran terhadap penyakit hipertensi meskipun penyakit tersebut tidak menular.

Abstract

Dislipidemia merupakan salah satu penyakit yang meningkat prevalensinya pada remaja dan dapat terjadi dikarenakan perubahan profil lemak pada seseorang seperti tingginya kolesterol total, LDL-kolesterol, trigliserida, atau penurunan HDL-kolesterol dengan penyebab primer (faktor genetik) atau sekunder (faktor lingkungan). Perubahan tersebut dapat terjadi secara tunggal atau terdapat komplikasi dengan faktor risiko yang dapat memicu perkembangan atherosclerosis. Pengabdian kepada Masyarakat dengan tema “Penyakit Tidak Menular pada Usia Remaja di Universitas Ibn Khaldun” dilaksanakan pada 28 Oktober 2024. Adapun peserta yang ditargetkan pada kegiatan ini adalah remaja di usia 17-25 tahun atau mahasiswa di Universitas Ibn Khaldun. Kegiatan yang dilakukan berupa penyuluhan tentang dislipidemia yang dilakukan secara langsung dengan tahapan paparan materi oleh tim yang terdiri dari mahasiswa Prodi Sarjana Kedokteran FK Militer Unhan beserta dosen sekaligus dokter, dilanjutkan dengan sesi tanya jawab, dan diakhiri dengan kesimpulan. Peserta berjumlah ±800 peserta. Pada pelaksanaan terlihat peserta berperan aktif dan antusias, pengetahuan para peserta menjadi meningkat terutama tentang awareness dislipidemia.
