Abstract

Hypertension is a health problem that increases mortality and morbidity and the economic burden of healthcare in Indonesia. Analysis of Cost of Illness (COI) must be conducted from different perspectives to provide additional information for policymakers about the economic impact caused by hypertension. This review aims to explain the components of healthcare costs in hypertension based on different perspectives, namely patient, payer, and healthcare providers. The results of this study show that direct and non-medical costs are the largest component of healthcare costs. Drug, doctor consultations, supporting examinations (laboratory tests), healthcare workers, administration costs are the highest direct medical costs. Travel and food expenses are components of direct non-medical costs. The lost productivity of patients or caregivers is calculated using the human capital approach and occupies health expenses as indirect costs. This can help the government and other researchers have the same perception in determining the components of the health costs under study. A study of COI can help the government control hypertension with promotive and preventive efforts and improve the government's public health program for hypertension.

Abstract

Pharmacy technician as health workers in pharmacies have high risk of transmitting the COVID-19 virus. They often have direct contact to public or COVID-19 patients who come to pharmacies. This study aimed to determine the compliance level of pharmacy technician to the COVID-19 health protocols and its correlations to the level of knowledge, organizational-environment and self-efficacy. This study was designed descriptive correlational used self-administered questionnaire to collect data with cross sectional study. The participants were selected with simple random sampling among pharmacy technician who worked in community pharmacies in Sleman. A total of 98 pharmacy technician were participated in this study. The compliance levels were divided into low, medium and high. Most participants were in medium level of compliance (61.2%), followed by high level (35.7%) and only 3.1% participants in low level. Data analysis was carried out by cross-sectional and statistical test with SPSS by comparing the three factors to the level of compliance. Statistical tests on organizational-environmental factors and self-efficacy showed a significance value (p<0.05) with a Pearson Correlation value of 0.393 and 0.350. The level of knowledge showed not significance value (p>0.05) with a Pearson Correlation value of 0.152. There was a correlation between organizational-environmental factors and self-efficacy with the the compliance level of pharmacist assistants to the COVID-19 health protocols. While the level of knowledge showed results that there is no correlation.

Abstract

Penelitian ini akan melihat apakah puskesmas-puskesmas di wilayah kabupaten Bantul, DIY membutuhkan apoteker. Analisis terhadap kebutuhan ini didasarkan pada tiga macam responden. Responden pertama adalah tenaga farmasi, yang kedua adalah pasien dan yang ketiga sebagai pendukung adalah kepala puskesmas. Pengambilan data dilakukan terhadap pasien dan tenaga farmasi dengan alat berupa kuesioner. Sedangkan pengambilan data terhadap responden kepala puskesmas dilakukan dengan pengisian kuisioner dan wawancara. Hasil penelitian menunjukkan bahwa paling tidak ada 7 alasan mengapa apoteker diperlukan di puskesmas, yaitu dari aspek pengelolaan obat, pelayanan resep, tingkat beban kerja seorang Asisten Apoteker, item materi informasi obat yang diberikan kepada pasien, ketidak adanya konseling, pengadaan obat yang lebih ekonomis dan adanya share informasi obat dengan tenaga kesehatan yang lain.

Abstract

Diabetes mellitus (DM) is chronic disease that requires routine lifelong treatment. Uncontrolled blood sugar levels in DM patients can cause complications. This has an impact on the burden of health costs. The aim of this study to conduct a narrative review of the cost of illness in type 2 DM patients in various countries. The English articles used in this study were obtained from Pubmed, ScienceDirect, and Wiley that published in the period 2011 to 2021. The cost reported in the articles were local currency that later converted into US dollars according to the time of the study. Based on the narrative review, there are 19 articles that meet the criteria. In the article, there are varied perspectives, methods, and cost categories. The total cost per year for type 2 DM ranged from US$246.1 to US$5894.1, direct, and indirect costs per patient per year ranged from US$ 104.6 to US$5246.4 and US$82.9 to US$2299.4. Type 2 diabetes mellitus increases the burden of health costs. The presence of complications in type 2 DM patients requires higher health costs compared to uncomplicated type 2 DM patients and non-DM patient

Abstract

A good information system will support the implementation of the tasks and functions of the organization. This systematic review aims to identify and characterize the methods and attributes in usability evaluation and identify measurement techniques in usability evaluation. This systematic review uses the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) methodology. The search was carried out using two databases, namely PubMed and Science Direct, by limiting information systems related to health published from 2015 to 2021. Articles that met the criteria were evaluated for methods, attributes, measurement techniques, and usability criteria used. Article quality was assessed using a modified Downs and Black checklist. The search identified 132 articles. The results of abstract screening and duplication-checking obtained 128 articles. The screening was carried out through a complete review of the inclusion and exclusion criteria, obtained 9 articles that met the criteria. There are five methods used for usability evaluation, namely Think aloud (TA), Heuristic evaluation (HE), System Usability Scale (SUS), User viewpoint, and Cognitive Walkthrough Method. Six problem definitions used in usability evaluation are ISO, Nielsen, user-based, TAUCHI, 19 item usability questions, and successful completion of scenarios. The measurement technique is done by qualitative and quantitative analysis. Effectiveness, efficiency, and satisfaction are criteria that are always assessed from almost all articles reviewed. The heuristic evaluation method with quantitative measurement techniques using an evaluator in evaluating usability based on scenarios then proceeding with qualitative measurement techniques to deepen problems and provide solutions is the most ideal usability evaluation measurement technique.

Abstract

Educators and Education Personnel (EEP) are one of the priorities that take precedence in the stages of the vaccination program as a form of preventing the spread of COVID-19. In August 2022, there were cases of exposure to COVID-19 where the first program reached 86%, the cause was compliance with implementing health protocols. This study aims to determine the level of compliance in implementing the COVID-19 prevention health protocol and identify the factors that influence the level of compliance. This research is a quantitative non-experimental study with a cross-sectional study approach. Questionnaires were distributed to 522 EEP by purposive sampling, 339 EEP respondents were obtained with a response rate of 64.94%, divided into 290 EEP as research samples and 49 samples used for the reliability test. Data collection was carried out in September-November 2022 using an online questionnaire via the GoogleTM form. A reliable questionnaire was used with a Cronbach's α value 0.864. The results were tested of normality then analyzed by using the JAMOVI application. For parametric data, the student t-test was used, while for non-parametric data, Mann-Whitney and Kruskal-Wallis data were used. Spearman correlation is used to determine the factors that influence the level of EEP compliance. As results the level of EEP health protocol compliance was 67.9% in the medium category. Factors that influence the level of PTK compliance are the organizational environment (p-value 0.001) and self-efficacy (p-value 0.001). Meanwhile, the level of knowledge (p-value 0.093) showed no significant correlation on the level of EEP compliance.

Abstract

Background: Dispensing antibiotics without a prescription at community pharmacies is a significant contributor to the ongoing global public health issue of antibiotic resistance. Objectives: To estimate the proportion of antibiotics that are dispensed without a prescription in community pharmacies in various Indonesian cities. Methods: A literature review was conducted via PubMed, Science Direct, Google Scholar, Garuda, and Neliti for articles published between January 2007 and December 2022 combined with Boolean operators. The literature search keywords were (simulated patientsOR mystery shopper OR sample patients OR dummy patients) AND ("antibiotics without prescription OR non-prescription antibiotics OR self-medication of antibiotics). The keywords are also used in Indonesian language (Bahasa), including "simulasi pasien” OR "sampel pasien” AND "antibiotik tanpa resep” OR "swamedikasi antibiotik”. Results: Seven studies from various cities have complied with the inclusion criteria and were considered when reviewing 199 articles. The findings of our studies were consistent with the extensive use of non-prescription antibiotics throughout the review. A simulation patient study design was used in all seven studies in this review. Amoxicillin recorded the highest percentage of dispensing without a prescription, while other drugs often purchased include chloramphenicol, ciprofloxacin, and cefadroxil. Among the studies reviewed, one study utilized the pre-test and post-test methods, while the others did not. Conclusion: The lack of prescriptions for antibiotics dispensing has often occurred in community pharmacies throughout Indonesia. The community pharmacist's role is needed as the final gate of pharmaceutical services in providing rational treatment and controlling the dispense of antibiotics without a prescription.

Abstract

Salah satu faktor yang mendorong peserta JKN untuk mengunjungi fasilitas kesehatan adalah persepsinya terhadap sakit, pengobatan, dan kepuasan peserta tersebut terhadap pelayanan kesehatan, khususnya di fasilitas kesehatan tingkat pertama. Kepuasan pasien merupakan salah satu indikator keberhasilan dalam pemberian pelayanan kesehatan. untuk mengetahui gambaran persepsi sakit, pengobatan, dan kepuasan peserta JKN terhadap pelayanan kesehatan di fasilitas kesehatan tingkat pertama dan mengetahui hubungan antara karakter sosiodemografi dengan ketiga varibael tersebut. Penelitian ini merupakan penelitian observasional analitik dengan pengumpulan data secara cross-sectional. Pengambilan data dengan teknik convenience sampling.. Metode pengumpulan data menggunakan metode kuesioner berbasis online. Minimal jumlah sampel pada penelitian ini sebanyak 385 responden peserta JKN untuk memperoleh 95% Confidence Interval (CI) dan margin error 0,05. Hasil dari penelitian terdapat 390 responden yang masuk kedalam kriteria inklusi. Pada persepsi sakit, persepsi terhadap pengobatan, dan kepuasan sebagian besar responden memiliki persepsi positif yaitu sejumlah 373 responden (95,6%), 374 responden (95,8%), dan 363 responden (93%). Variabel yang berhubungan adalah jumlah penghasilan keluarga dan jumlah tanggungan dalam keluarga dengan persepsi sakit, dengan p-value 0,043 (<0,05). Mayoritas responden memiliki persepsi positif tentang sakit dan pengobatan. Sebagian responden juga merasa puas terhadap pelayanan kesehatan di Fasilitas Pelayanan Kesehatan Tingkat Pertama.

Abstract

COVID-19 has become a worldwide pandemic, especially in Indonesia. The high number of COVID-19 cases in Indonesia has a financial impact, as the government must shoulder the cost of dealing with it. The study aimed to determine the direct cost and total cost (cost of illness) of COVID-19 patients at RSA UGM Yogyakarta from a hospital perspective, as well as the differences in total direct cost of hospitalized COVID-19 patients based on age, gender, length of stay, severity, and comorbidity. The cross-sectional study design of the analytic observational research method is based on a hospital perspective. Data was collected retrospectively from the patient's medical record, detailed healthcare expenditure data, and patient claim files. The data was analyzed using univariate, bivariate, and multivariate methods. The study's findings requested the number of inpatient COVID-19 patients as many as 104 samples that satisfied the inclusion specifications, as well as the total direct medical costs of Rp. 2.346.150.137 with an average cost of Rp. 22.559.154 for each patient. The most expensive components were pharmaceutical and BMHP prices of Rp. 958.410.599 (40.85%) and medical support costs of Rp. 725.163.700 (30.91%). Age, length of stay, severity of comorbidities, and usage of antivirals all have an impact on direct medical costs.Keywords: COVID-19, hospital costs, hospital perspective, hospitalization

Abstract

Pengelolaan vaksin di Puskesmas perlu diperhatikan agar ketersediaan vaksin di fasilitas kesehatan dapat terjamin. Fasilitas pelayanan kesehatan yang menyediakan vaksin harus memperhatikan berbagai proses pengelolaan vaksin. Tenaga kefarmasian memiliki peran dalam menjamin keamanan, mutu dan khasiat vaksin mulai dari penerimaan, penyimpanan dan distribusi hingga saat digunakan. Penelitian ini bertujuan untuk mengetahui persepsi dan pengalaman tenaga kefarmasian terhadap pengelolaan vaksin di Puskesmas Kabupaten Kotawaringin Timur. Penelitian ini menggunakan desain penelitian kualitatif. Pengumpulan data diperoleh dari wawancara mendalam kepada 15 partisipan yang terdiri dari 9 Apoteker dan 6 Tenaga Teknis Kefarmasian (TTK) yang dipilih dengan menggunakan teknik purposive sampling. Hasil wawancara dianalisis dengan menggunakan analisis tematik. Hasil penelitian ini memberikan pemahaman yang lebih mendalam tentang peran tenaga kefarmasian dalam pengelolaan vaksin seperti menjamin ketersediaan, mutu dan keamanan vaksin serta melakukan pengelolaan vaksin yang lebih terkontrol. Pengalaman tenaga kefarmasian yaitu memperoleh peningkatan kapasitas diri, menerapkan kebijakan one gate policy dan melakukan pengelolaan vaksin kedaluarsa. Penggunaan aplikasi Smile memungkinkan pencatatan vaksin secara riil time dan efektif. Praktik pengelolaan vaksin meliputi proses dokumentasi, penyimpanan, distribusi dan monitoring dan evaluasi vaksin secara terpadu dengan program Imunisasi. Partisipan mengalami beberapa hambatan seperti perlu pengembangan aplikasi, koordinasi dan kolaborasi dengan tenaga kesehatan, penambahan jumlah Sumber Daya Manusia (SDM) dan supply listrik.
