Abstract

In this company, annual factory repair activities or Turn Around (TA) is a periodic maintenance program of factory facilities, where various work activities are carried out in a certain period of time, and involve so many people with various levels of expertise and work skills. This research was taken using a qualitative research method, namely by describing how the application of Job Safety Analysis (JSA) is carried out on internal part repair work and burner tile replacement in Turn Around (TA) activities in accordance with existing conditions, developing commitments and policies, or ongoing processes. The results of the reseacrh obtained based on in-depth observation and interviews found several potential hazards in this work, including the fall of the workpiece (18 work steps) 2 medium and 16 high, height (13 work steps) 3 medium and 10 high, dusty conditions (19 work steps) 3 medium and 16 high, gas poisoning (16 work steps) 1 medium and 15 high, hot conditions (17 work steps) 3 medium and 14 high, pinched (11 work steps) 2 medium and 9 high, and sparks (5 work steps) 1 medium and 4 high. The application of Job Safety Analysis in evaluating this job shows that comprehensive hazard identification, accurate risk analysis, implementation of effective preventive measures, and adequate training and supervision to ensure work safety

Abstract

A fire emergency response system is a series of procedures that must be carried out when a fire occurs to protect the safety of building occupants. The main objective of this final project is to ensure that the fire emergency response system in the main building of in the fertilizer industry. can function well when a fire occurs and can protect the safety of all main building employees. This research uses a qualitative research method which uses two types of data, namely primary data with in-depth interviews and observations, and secondary data from document reviews, company archives and literature studies. The results and discussion based on variables related to the implementation of the fire emergency response system can be concluded that the informants implemented the fire emergency response system procedures well and in accordance with established regulations. This is proven based on observations that in the main building of PT.X already has a fire emergency response system, such as a fire detection and alarm system, fire extinguishing system, evacuation routes, gathering points and other supporting equipment. Apart from that, there are also evacuation procedures, training and simulations, as well as maintenance of a fire emergency response system that is complete and in accordance with established regulations.

Abstract

The implementation of HIRADC (Hazard Identification Risk Assessment and Determinant Control) is one of the efforts that can be made to control potential hazards that cause work accidents. HIRADC (Hazard Identification Risk Assessment and Determinant Control) is the process of identifying hazards, assessing, and evaluating the risks contained in work activities and environments. The results of the risk evaluation are used to determine hazard control efforts so that they can reduce the level of risk that is likely to cause work accidents. The method used to complete the research results has achieved the expected objectives. The data presented uses a qualitative descriptive approach, which shows K3 in accordance with current conditions, developing policies, ongoing processes, and effects that occur or trends that are occurring in the company. These results are compared with data obtained in the field or from the HSE regarding the implemen- tation procedure of HIRADC (Hazard Identification Risk Assessment and Determi- nant Control) and compared with applicable standards. Based on the identification that has been carried out, 42 potential hazards have been identified from 21 stages of work activities, 6 of which are in the high risk category, 20 are in the low risk category, 16 are in the medium risk category. Furthermore, all these activities or processes are then analyzed in the HIRADC table and converted into percentages for each category to determine the risk priority. The results for the low risk level are 0.47%, the medium risk level is 0.38%, and the high risk level is 0.14%. This company has made efforts to reduce potential hazards in the work environment through substitution, engineering control, administrative control, and finally PPE
