Abstract

Background: The rising global prevalence of obesity and its related health issues make it crucial to explore all potential therapeutic options. Coconut oil (CNO) has been extensively studied for its health benefits, yet no scoping review has specifically assessed its potential as an anti-obesity agent, particularly among obese subjects. Objective: This review aims to outline the characteristics and findings regarding the use of coconut oil as an anti-obesity agent in both experimental animal and human studies. Method: The review included original studies on the potential of coconut oil as an anti-obesity agent, published in English or Indonesian between 2011 and 2022. A comprehensive search was performed across databases such as PubMed, ScienceDirect, Google Scholar, SpringerLink, and Portal Garuda. Articles were selected following the PRISMA-ScR flow diagram, with subsequent data extraction, analysis, and synthesis conducted.Results: Eight articles met the inclusion criteria, comprising four preclinical trials (involving obese animal models) and four human studies that recruited obese people. The preclinical trials used virgin coconut oil (VCO), while the human studies utilized both VCO and coconut oil (CNO). In preclinical trials, VCO administration was associated with reductions in low-density lipoprotein cholesterol (LDL-C), total cholesterol (TC), and triglyceride (TG) levels. In clinical trials, consumption of VCO and CNO was linked to increased high-density lipoprotein cholesterol (HDL-C) levels and reductions in LDL-C, TC, TG, and anthropometric measures (body weight, body mass index, waist circumference, waist-to-hip ratio, and body fat mass). However, the effects of VCO and CNO on LDL-C, TC, and TG were inconsistent.Conclusion: Coconut oil, particularly in the form of VCO, has shown potential as a supplement for managing obesity. However, this scoping review highlights inconsistencies in the effects of coconut oil on lipid profiles (LDL-C, TC, TG), indicating the need for further research to clarify these outcomes.

Abstract

The growing elderly population in Indonesia highlights the need for support systems that can ensure their safety and well-being, particularly for those living alone. Living independently increases the vulnerability of older adults to risks such as falls, sudden illness, and emergency conditions that require immediate assistance. However, the use of simple, elderly-friendly emergency technologies at the community level remains limited. This community service program in Purbayan village in Yogyakarta City initiated the development of the Emergency Bracelet for the Elderly Living Alone (GEULIS), an innovative solution based on Internet of Things (IoT) technology. The prototype bracelet was designed with a simple emergency push button system capable of sending alerts via WhatsApp to family members or community health workers, when the elderly require help. The program comprised need assessments, designing and developing the prototype, and pilot testing with elderly individuals living alone. The results demonstrated that the device functioned effectively in transmitting emergency signals. While the design was relatively wearable and user-friendly, further refinement is needed to optimize ease of use. This program highlights the potential for continued development and broader application of GEULIS within the community.

Abstract

Latar Belakang: Ketaatan pengobatan hipertensi merupakan prediktor penting dalam keberhasilan terapi hipertensi. Namun, data menunjukkan rendahnya ketaatan minum obat pada pasien, yang berdampak pada peningkatan risiko komplikasi (misal, stroke) dan menurunkan kualitas hidup Tujuan: menggambarkan hasil publikasi terkini mengenai metode intervensi peningkatan ketaatan minum obat antihipertensi di Indonesia. Metode: Penelusuran artikel dilakukan dengan pendekatan untuk Scoping Review dengan pencarian kata kunci di Garuda (“kepatuhan hipertensi”, “ketaatan hipertensi” dan PubMed ((adherence OR compliance) AND (hypertension OR blood pressure) AND Indonesia), untuk artikel terpublikasi di tahun 2017 sampai Juli 2023. Hasil: Terdapat 39 artikel yang memenuhi kriteria, 2 diantaranya dipublikasikan di jurnal internasional. Proporsi artikel yang dipublikasi di jurnal nasional terakreditasi adalah 75,7% (28 dari 37 jurnal nasional). Sebagian besar penelitian dilakukan pada pasien puskesmas baik yang termasuk di program pelayanan penyakit kronis (prolanis), rawat jalan maupun rawat inap. Selain di puskesmas, rumah sakit/klinik, penelitian dilakukan pada pasien di apotek. Program posyandu dan posbindu menjadi wahana penelitian intervensi ketaatan pengobatan di masyarakat. Intervensi edukasi melalui berbagai pendekatan dan media merupakan metode yang paling banyak dilaporkan; berupa edukasi saja atau kombinasi dengan metode lain. Pemberian edukasi terutama dilakukan oleh apoteker. Layanan kunjungan rumah (pharmacy home care) dilaporkan efektif meningkatkan ketaatan pengobatan. Selain media edukasi konvensional (poster, leaflet), dilaporkan juga penggunaan video dan media interaktif lain berbasis digital. Sistem pengingat minum obat diberikan antara lain melalui self-reminder card, kotak obat harian, kalender fungsional serta pengingat melalui layanan pesan singkat di gawai. Pendekatan intervensi yang disesuaikan dengan faktor penghambat ketaatan (targeted and tailored intervention) dilaporkan efektif untuk meningkatkan ketaatan pengobatan. Selain ketaatan pengobatan, beberapa penelitian melaporkan juga luaran lain seperti penurunan tekanan darah atau peningkatan pengetahuan pasien. Terdapat 2 artikel yang melaporkan tidak efektifnya intervensi edukasi saja untuk meningkatkan ketaatan minum obat. Simpulan: Hasil review ini menunjukkan intervensi peningkatan ketaatan minum obat terutama dilakukan melalui metode edukasi, dengan apoteker memegang peran utama. Pemanfaatan teknologi dan kombinasi metode intervensi yang sesuai dengan pasien perlu dikembangkan di penelitian selanjutnya.

Abstract

Colorectal cancer (CRC), a malignancy that affects the colon and rectum, is influenced by gene mutations, epigenetic changes, local inflammation, and lifestyle risk factors. New cases of CRC globally account for 10%, with mortality at 9.4%, while Indonesia occupies the top four incidence and mortality cases. A family history of CRC requires preventive measures, including consuming functional foods to maintain the colon microenvironment. This article aims to review the potency and toxicity effects of omega-3 fish fatty acids as a chemopreventive agent against CRC. The data sources were original articles about omega-3 fish fatty acids as CRC chemopreventive agents, published in 2012-2022 and in English or Indonesian. Databases used are Ovid, ScienceDirect, ProQuest, PubMed, Springer Link, EBSCO, and Google Scholar using Boolean search. The selection of studies followed the PRISMA-ScR method. Of the nine articles selected, fish omega-3 fatty acids had the potential as a chemopreventive agent against CRC, with several variations in daily dose ranges accompanied by a good lifestyle. Oral omega-3 fish supplementation gave benefits in the range of 96 mg–2,000 mg of eicosapentaenoic acid (EPA) and between 360 mg–1,000 mg of docosahexaenoic acid (DHA), with a daily intake varying between 0–14 grams/day. One study reported a toxic effect of fish omega-3 fatty acids, which raised the risk of post-operative infection after parenteral (intravenous) administration. There are no reported side effects across eight studies following oral preparation administration. The omega-3 fish fatty acid shows potential as a chemopreventive therapy for CRC; further studies are required to explore the parenteral administration-associated toxic effects.
