Abstract

Angiotensin-converting enzyme (ACE) is a circulating and membrane-bound enzyme in the renin-angiotensin system. Polymorphism is the presence of two or more alleles at a locus in a population. In patients with heart failure with impaired systolic function, the RAA system is activated. One of the main therapies in heart failure is the administration of angiotensin-converting enzyme blockers. This study aims to examine the relationship between angiotensin-converting enzyme gene polymorphisms with improvement in left ventricular ejection fraction (FEVK).This research is a prospective observational study using a convenient sampling technique, carried out at Mitra Medika Hospital Bandar Klippa, Deli Serdang. The number of samples in the study was 37 people. Each patient was examined for initial echocardiography and at least 1 month after treatment when clinical improvement was seen. All patients received treatment according to the standard guidelines for the management of chronic heart failure according to the European Society of Cardiology (ESC)/Indonesian Cardiovascular Specialist Doctors Association (PERKI). The majority of research subjects were male as much as 81% and the average age of the patients was 56 years. ACE gene polymorphism was dominated by gene II as much as 57%. The average FEVK value in the first measurement was 25.8% and in the second measurement, it increased to 34.4%. There was a significant change between FEVK I measurements by an average of 25% to 34% in the next measurement (p 0.05). In the analysis of the relationship, it was seen that the ACE gene polymorphism did not affect the improvement of the FEVK value.Treatment of chronic heart failure with a decrease in ejection fraction was seen to have a significant effect in increasing FEVK. There is no relationship between angiotensin-converting enzyme gene polymorphisms on response to therapy in patients with chronic heart failure in this study.

Abstract

In schizophrenic patients, symptoms of depression are often found which can lead to adverse effects on the treatment process, such as increasing the length of treatment, increasing the possibility of relapse, non-adherence in taking medication, and can also lead to suicide in schizophrenic patients. In schizophrenic patients, the activity of the central angiotensin converting enzyme (ACE) gene is altered and angiotensin II interacts with central dopamine, thereby contributing to the pathophysiology of schizophrenia. In addition, it is likely that the ACE gene variant is also involved in the development of depressive symptoms in schizophrenic patients. This research is a categorical analytic study with a cross sectional method, the sampling method is non-probability sampling with consecutive sampling type, with a total sample of 42 people. There was no significant difference between depression symptoms and ACE gene polymorphisms with P value = 0.746. There was no significant difference between depression symptoms and ACE gene polymorphisms.

Abstract

High-density lipoprotein (HDL) functions to transport cholesterol from the blood vessel endothelium to the liver and excreted through the digestive tract so that it does not cause dyslipidemia, which is a risk factor for cardiovascular disease. Current treatment for dyslipidemia uses long-term statin drugs, which can cause various side effects, so herbal alternatives such as ketapang leaves (Terminalia catappa L.), which contain flavonoids and have the effect of increasing HDL levels, needed to be evaluated to achieve that reason. This study aims to evaluate whether ketapang leaf extract has an effect on increasing blood HDL levels. This research used experimental animals with a pretest and a posttest with a control group design. It was conducted at the Biochemistry Laboratory and Laboratory Animal Management Unit, Faculty of Medicine, Universitas Muhammadiyah Sumatera Utara, Medan, from September to December 2023. This study used Wistar strain male white rats (Rattus norvegicus) that were induced on a high-fat diet, divided into five groups: negative control, positive control (given simvastatin), treatment 1 (given 21 mg/kgBW ketapang leaf extract), treatment 2 (given 42 mg/kgBW ketapang leaf extract) and treatment 3 (given 84 mg/kgBW ketapang leaf extract). The study results showed a significant difference (p<0.05) in the average HDL levels before and after treatment in the positive control, treatment 2, and therapy three groups. Thus, the study concluded that ketapang leaf extract affected increasing HDL levels of male white rats (Rattus norvegicus).

Abstract

Background: Stroke is the second largest cause of death in the world. Stroke is classified based on its etiology as ischemic stroke and hemorrhagic stroke. Most large-scale studies on the risk of total cholesterol and triglyceride levels in stroke are not distinguished between ischemic and hemorrhagic strokes. Purposes: The purpose of this study was to determine the comparison of triglyceride and total cholesterol levels in ischemic stroke patients with hemorrhagic stroke. Method: Descriptive analytic study using medical records of patients at Medan Haji General Hospital in 2018-2019. The number of ischemic stroke patients is 28 patients and hemorrhagic stroke 28 patients with a total sample of 56 patients. Results: In ischemic stroke patients, the average value of triglyceride levels was 144.75 mg/dL and the average value of total cholesterol was 250.93 mg/dL. In hemorrhagic stroke patients, the average value of triglyceride levels is 126.93 mg/dL and the average total cholesterol level is 174.25 mg/dL. Conclusion: From this study we found a significant difference in total cholesterol between ischemic and hemorrhagic strokes. No significant difference was found in triglycerides between ischemic and hemorrhagic strokes.

Abstract

Abstrak: : Berat badan lebih dan obesitas diketahui dapat mengganggu metabolisme tubuh, diantaranya dalam proses metabolisme glukosa darah. Akumulasi lemak yang berlebih dapat mengganggu metabolisme insulin sehingga menyebabkan hiperinsulinemia dan resistensi insulin. Berat badan lebih dan obesitas dapat ditentukan menggunakan pemeriksaan sederhana seperti indeks massa tubuh, lingkar perut dan lingkar lengan atas. Maka dari itu penelitian ini bertujuan untuk mengetahui apakah ada atau tidak hubungan antara indeks massa tubuh, lingkar perut dan lingkar lengan atas terhadap kadar glukosa darah puasa pada mahasiswa. Penelitian ini merupakan penelitian observasional analitik  dengan desain cross sectional. Tujuan dalam penelitian ini untuk mengetahui ada atau tidak adanya hubungan antara indeks massa tubuh, lingkar perut dan lingkar lengan atas terhadap kadar glukosa darah puasa pada mahasiswa Fakultas Kedokteran Universitas Muhammadiyah Sumatera Utara. Jumlah subjek yang diteliti adalah 40 mahasiswa. Hasil uji memperlihatkan bahwa tidak adanya hubungan pada setiap variabel. Tidak adanya hubungan indeks massa tubuh, lingkar perut dan lingkar lengan atas terhadap kadar glukosa darah puasa pada mahasiswa Fakultas Kedokteran Universitas Muhammadiyah Sumatera Utara.

Abstract

Abstrak: : Status gizi dipengaruhi oleh makanan yang dikonsumsi dan aktivitas fisik setiap individu. Konsumsi makanan tinggi lemak serta aktivitas fisik yang kurang menyebabkan terjadinya penumpukan lemak di dalam tubuh. Lemak yang menumpuk berpotensi terhadap abnormalitas lipid plasma yang merupakan faktor risiko terhadap kejadian aterosklerosis. Terdapat beberapa antropometri sederhana untuk menggambarkan lemak di dalam tubuh seperti pengukuran indeks masa tubuh, lingkar lengan atas dan lingkar perut. Penelitian ini bertujuan untuk mengetahui hubungan indeks masa tubuh, lingkar perut dan lingkar lengan atas terhadap kadar trigliserida pada Mahasiswa Fakultas Kedokteran Universitas Muhammadiyah Sumatera Utara (FK UMSU) Angkatan 2019. Penelitian deskriptif analitik dengan pendekatan cross sectional. Subjek penelitian adalah Mahasiswa FK UMSU Angkatan 2019 yang berjumlah 39 subjek penelitian yang memenuhi kriteria inklusi dan ekslusi. Uji korelasi Spearman menunjukkan bahwa IMT berhubungan dengan kadar trigliserida (P= 0,003), lingkar perut berhubungan dengan kadar trigliserida (P= 0,039) dan lingkar lengan atas berhubungan dengan kadar trigliserida (P= 0,039). Kesimpulan: Indeks masa tubuh, lingkar perut dan lingkar lengan atas berhubungan dengan kadar trigliserida pada Mahasiswa FK UMSU Angkatan 2019 .

Abstract

Abstract: Nutritional status is known to influence low-density lipoprotein (LDL) levels. It can be assessed by measuring body mass index (BMI), waist circumference (WC), and mid-upper arm circumference (MUAC). Individuals with obesity are at greater risk of having elevated LDL levels compared to those with normal nutritional status. In women, hormonal changes, particularly during menopause, can affect lipid metabolism and contribute to higher LDL levels. This study aimed to examine the correlation between BMI, WC, and MUAC with LDL levels in women aged 30-50 years. This descriptive-analytical study used a cross-sectional design involving 38 females’ subjects aged 30-50 years old. Anthropometric measurements and LDL levels were recorded. Pearson’s correlation test was used for data analysis. The results showed a weak but statistically significant positive correlation between BMI and LDL (p = 0,034, r = 0,344), WC and LDL (p= 0,035, r=0,344), and MUAC and LDL (p = 0,046, r = 0,326). This study found a correlation between BMI, WC, and MUAC on LDL levels.

Abstract

Geriatric syndrome is a collection of health conditions often experienced by the elderly, including cognitive problems, mobility, and impaired daily functioning that negatively influence the routine tasks and standard of living of the seniors. To identify risk factors that influence cognitive decline in the elderly and geriatric syndrome. In the implementation of community service activities in Kutalimbaru Village Hamlet III using health education methods that prioritize active community involvement in each program as well as health checks and elderly lifestyle education. This community service activity in Dusun III Kutalimbaru Village was attended by 27 people. There were 9 people with hypertension, 5 people with high cholesterol, 15 people with hyperuricemia, 7 people with obesity, 2 people with high blood sugar. From the results of the health checks that have been carried out, several abnormal data are found, so counseling on healthy lifestyles and routine health checks is needed for the community in Kutalimbaru village hamlet III.

Abstract

Sumber daya manusia dimasa yang akan datang sangat dipengaruhi oleh pola hidup dalam keluarga. Salah satunya adalah pola konsumsi makanan. Pengetahuan akan sangat mempengaruhi keluarga dalam pemilihan bahan makanan, proses makanan hingga pemyajian makanan agar tetap aman untuk dikonsumsi oleh seluruh anggota keluarga agar tercapai status kesehatan yang optimal. Tujuan: meningkatkan pengetahuan masyarakat untuk mengenai berbagai kontaminasi dalam makanan serta dampaknya terhadap kesehatan.  Metode: kegiatan dilakukan di kelurahan Tegal Sari Mandala (TSM) 2 dan 3 dengan total 63 peserta dengan metode: ceramah, tanya jawab secara aktif, pengisian kuesioner sebelum dan sesudah edukasi hingga cara deteksi bahan kimia dalam makanan. Hasil: dari hasil posttest setelah edukasi dijumpai peningkatan pengetahuan berdasarkan skoring yakni pada pengetahuan tinggi dari 36,5% menjadi 76% dan pada pengetahuan rendah dari 63,5% sebelum edukasi menurun menjadi 24% setelah edukasi.
