Abstract

Chronic Kidney Disease (CKD) or chronic kidney failure is a disease due to decreased kidney function that causes the body to fail to maintain fluid and electrolyte balance metabolism, resulting in urea retention and other nitrogen waste accumulation in the body. This research aims to analyze given this position to lower oxygen consumption and increase maximum lung expansion, the ineffectiveness of the client's breath pattern is more optimal in CKD patients in the Adenium Room of DR. Soebandi Jember Hospital. This type of research is descriptive using the case study approach method. The subject in this case study was one patient affected by CKD with the ineffectiveness of breath patterns. Semifowler position intervention is performed where the position of the head and body is raised by 45°. Result: The application of a semi-fowler position (45° sitting position) for 3x24 hours according to the standard of procedure helps reduce shortness of breath and helps optimize RR in the client so that the problem of ineffectiveness of breath patterns can be resolved. Interventions in regulating the angle of sleep position can meaningfully produce good respiration, so it can be considered one of the interventions to optimize the ineffectiveness of breath patterns.

Abstract

Hyperglycemia is the main symptom in patients with diabetes mellitus. One of the risk factors that can increase blood sugar levels in diabetes mellitus (DM) patients is anxiety and stress, as a result of diabetes mellitus sufferers who experience anxiety, their physical condition does not improve, and their blood sugar levels are always high. Progressive muscle relaxation therapy (PMR) is a therapy that aims to focus the patient's attention so that they can differentiate the feelings that occur when the muscles are relaxed or relaxed from when they are contracting so that the patient can manage their body condition against their anxiety. This study aims to analyze the provision of progressive muscle relaxation therapy to control anxiety levels and blood sugar levels in patients with type 2 diabetes mellitus in the Gardena medical ward of Dr. Soebandi Hospital Jember. This research uses descriptive methods in the form of case studies on a patient with Type 2 DM. Therapy was given once a day for 20-30 minutes. Anxiety levels were measured using the DASS questionnaire, and blood sugar measurements were made using a blood sugar stick. After being given PMR therapy for 3 x 24 hours, the patients experienced a decrease in anxiety levels and blood sugar levels. Providing PMR therapy is effective in treating unstable blood glucose levels and anxiety for patient with Type 2 DM.

Abstract

Background: Management of secretions is one of the main problems faced in the medical condition of respiratory disorders. The use of complementary therapy techniques to treat airway clearance such as the active cycle of breathing technique is still low when compared with other interventions in treating respiratory problems. Aims: The research aims to determine the effectiveness of providing active cycle of breathing technique therapy to overcome nursing problems ineffectiveness airway clearance in nursing care. Method: This type of research is a case study using nursing care methods. The research sample was patients treated in the Catleya room at Dr. Soebandi Jember Hospital. Patients are given nursing intervention in the form of therapy active cycle of breathing technique for 4 consecutive days 2 times a day for 15-20 minutes with 4 cycles. Results: providing active cycle of breathing technique therapy can reduce nursing problems ineffectiveness airway clearance. Providing active cycle of breathing technique therapy effectively shows improvements in conditions such as improved respiratory frequency, reduced shortness of breath, and decreased sputum production. Conclusion: providing active cycle of breathing technique therapy effectively solve nursing problems ineffectiveness airway clearance.

Abstract

Glaucoma is a chronic neurodegenerative disorder of the optic nerve characterized by an increased IOP and progressive. The main threat to patients with visual impairment is the gradual loss of vision. Decreased vision function in glaucoma patients and inability to adapt to changes in body functions will have an impact on the ability to carry out daily activities and will affect the quality of life. The aim of this study was to analyze the quality of life in glaucoma patients at the Jember Regency Balung Regional Hospital. This study used a quantitative descriptive with a cross- sectional approach. 97 respondent were treated at Poli Mata Hospital in Balung Regional Hospital, Jember Regency obtained by purposive sampling technique. Collected data using the National Eye Institute Visual Function Questionnaire-25 (NEI-VFQ 25). Data analysis using univariate. The results of the study were 49 respondents (50,5%) had a qood quality of life and 48 respondents (49,5%) had a less quality of life, near vision is the most problematic subscale in all of domains. The quality of life of glaucoma patients is a good category, which means respondents are satisfied with their health and vision condition does not significantly interfere with daily activities. Respondents can still do their daily activities independently even though slightly limited, both with and without glasses.

Abstract

Abstrak Ulkus diabetik adalah komplikasi DM yang paling sering terjadi. Angka mortalitas diabetisi dengan ulkus diabetik juga semakin meningkat, sehingga dibutuhkan strategi pencegahan untuk mengidentifikasi risiko. Tujuan penelitian adalah teridentifikasi diabetisi yang berisiko mengalami ulkus diabetikum berbasis Diabetic Foot Screening di wilayah Kabupaten Jember. Penelitian ini menggunakan pendekatan kuantitatif dengan jumlah sampel sebesar 100 responden. Teknik sampling yang digunakan adalah multistage random sampling. Instrumen yang digunakan dalam penelitian Michigan Diabetic Neuropathy Score (MDNS), Michigan Neuropathy Screening Instrument (MNSI), Ipswich Touch Test (IpTT) dan Monofilament Test. Alat pengumpulan data berupa ceklist yang telah disesuaikan dengan diabetic foot screening, tensimeter aneroid, dan stetoskop. Penelitian ini dianalisis secara univariat, ditampilkan dalam bentuk tabel distribusi frekuensi. Hasil penelitian memberikan gambaran identifikasi risiko ulkus diabetik berdasarkan nilai neuropathy perifer diabetik, nilai ankle brachial index (ABI), deformitas pada kaki, gangguan mobilisasi, kuku patologis, riwayat ulkus dan amputasi. Diabetisi memiliki risiko ulkus diabetik dengan kategori rendah sebanyak 41 diabetisi (41%), risiko sedang sebanyak 56 diabetisi (56%) dan kategori risiko tinggi sebanyak 2 diabetisi (2%). Diabetisi di Kabupaten Jember memiliki risiko ulkus diabetik. Identifikasi risiko terjadinya ulkus diabetik dibutuhkan oleh diabetisi sebagai rujukan untuk melakukan tindakan preventif agar tidak terjadi perburukan komplikasi dari DM. The mortality rate of people with diabetes who have diabetic ulcers is also increasing, prevention strategies to identify risks are required. The study sought to identify diabetics in Jember that were at risk of developing diabetic ulcers. With a sample size of 100 respondents, this study takes a quantitative approach. The sampling technique used is multistage random sampling. The instruments used were Michigan Diabetic Neuropathy Score, Michigan Neuropathy Screening Instrument, Ipswich Touch Test, and Monofilament Test. Data collection are a checklist form that has been adjusted to the diabetic foot screening, aneroid sphygmomanometer, and stethoscope. This was analyzed univariately and displayed as a frequency distribution table. The results provide an overview of ulcer risk identification based on diabetic peripheral neuropathy values, ankle-brachial index values, foot deformities, impaired mobilization, pathological nails, history of ulcers, and amputations. Diabetes patients have a low risk of diabetic ulcers 41 people (41%), a moderate risk 56 people (56%), and a high risk 2 people (2%). Diabetics in Jember are at risk of developing diabetic ulcers. Identifying the risk of diabetic ulcers is necessary for people with diabetes to take prevention. Keywords: diabetic foot screening, DM type 2, diabetic ulcer

Abstract

Abstrak Perawatan kaki sangat penting untuk menurunkan risiko ulkus kaki diabetik. Perilaku perawatan kaki sebagai bagian dari pengelolaan mandiri sangat ditentukan oleh faktor psikososial. Tujuan penelitian adalah menganalisis faktor psikososial yang mempengaruhi perilaku perawatan kaki penderita diabetes mellitus tipe 2.  Penelitian ini merupakan penelitian kuantitatif dengan desain cross-sectional. Besar sampel adalah 138 responden dengan teknik multistage random sampling. Variabel penelitian adalah faktor psikososial (pengetahuan, koping, distres, dukungan keluarga, dan dukungan perawat) sebagai variabel independen dan perilaku perawatan kaki sebagai variabel dependen. Instrumen penelitian berupa kuesioner, yaitu SKILLDS, Coping Scale, DDS, HDFSS, persepsi peran perawat. dan NAFF. Hasil penelitian menunjukkan bahwa sebagian besar responden berusia 56-65 tahun (48,5%), perempuan (64,4%), pendidikan dasar (39,1%) dan penghasilan rendah (60,1%). Sebagian besar mempunyai pengetahuan, koping, persepsi dukungan keluarga, dan dukungan perawat dalam kategori sedang (45,6%; 67,3%, 53,6%, dan 63,7%), sedangkan distres dalam kategori tinggi (37,6%). Perilaku perawatan kaki dalam kategori sedang (63,0%). Hasil analisis data dengan regresi linier berganda didapatkan bahwa model fit (F=2,419; p=0,039; R2=0,049). Hasil uji partial menunjukkan bahwa hanya pengetahuan yang berpengaruh signifikan terhadap perilaku perawatan kaki (p=0,035). Upaya peningkatan perilaku perawatan kaki dalam pengelolaan mandiri dapat ditingkatkan dengan meningkatkan pengetahuan pasien dalam melakukan perilaku perawatan kaki secara mandiri. Psychosocial factors largely determine foot care behavior as part of self-management. The study aimed to analyze the influence of psychosocial factors on the foot care behavior of people with T2DM.  The study was a quantitative study with a cross-sectional design. The sample size was 138 respondents with a multistage random sampling technique. The psychosocial factors (knowledge, coping, distress, family support, and nurse support) were independent variables, and foot care behavior was the dependent variable. The research instruments were questionnaires (SKILLDS, coping scale, DDS, HDFSS, nurse's role perception, and NAFF). Most respondents have the knowledge, coping, family support perceived, and nurse support perceived in the moderate category (45.6%; 67.3%, 53.6%, and 63.7%), and distress was in the high category (37.6%). In contrast, foot care behavior was in the moderate category (63.0%). The results of data analysis with multiple linear regression found that the model fit (F=2.419; p=0.039; R2= 0.049) and only knowledge significantly affected foot care behavior (p=0.035). Efforts to improve foot care behavior in self-management can be increased by increasing patient knowledge in performing foot care behavior independently.
