Abstract

ABSTRACTDental caries is one of the dental health problems. Dental caries are formed because there was leftover food that sticks to the teeth, which eventually lead to tooth calcification. Impact, teeth become brittle, hollow, even broken.Dental caries experience a loss of power to make a child chewing and digestive disruption, which resulted in growth of less than the maximum. This study aims to determine the relationship of parents’ behavior towards the maintenance of dental health of children (feeding habits sweet, sticky and drinking milk, teeth maintenance, inspection tooth and mouth) with dental caries in the TK R.A Bustanussholihin in Balun village Turi District of Lamongan. This study Analytics whose data is collected with a cross sectional sample of kindergarten children aged 4–6 years with a total sample of 49 children. The collection of primary data through interviews parents TK R.A Bustanussholihin student using questionnaires and secondary data obtained from the agency are in the region of the profile data TK R.A Bustanussholihin in Balun Village of Turi District of Lamongan. This study used a correlation coefficient of contingency. Based on the results of the contingency coefficient correlation test showed that the factors that have a strong relationship is the habit of feeding the sweet, sticky, and drink milk with a value of P = 0.504. While the factors that have a weak link is the habit of maintaining dental hygiene habits of children and child oral examination . From this study it can be concluded that there is a relationship or a strong correlation between feeding habits sweet, sticky and drinking milk with the incidence of dental caries in children aged 4–6. So as to prevent dental caries severity we need to hold counseling on feeding sweet, soft and gooey on the effect of dental caries as well as how it should be formula feeding or breast milk to the child to prevent rampant caries.Keywords: dental caries, sweetened food, children aged 4–6 years

Abstract

Public Health Center of Ngemplak I is one of Public Health Center at Sleman regency which has hospitalization facilities. Based on Bed Occupancy Rate (BOR) at Public Health Center of Ngemplak I, hospitalization has been utilized optimally. BOR for 2001 was 89,11% and 2002 was 93,8%. Compared with another Public Health Center which has hospitalization facilities at Sleman regency, the utilization of hospitalization facilities at Public Health Center of Ngemplak I is the highest from the other. More over, some people of Prambanan and Kalasan district choose to utilize hospitalization facilities at Public Health Center of Ngemplak I although there is Public Health Center with hospitalization facilities in their district.  The objective of  this research was to know the description of factors that encourage family’s  decision in utilizing hospitalization facilities at Public Health Center of Ngemplak I Sleman. The method of this research was qualitative. The subject of research was family of patient who hospitalized at Public Health Center of Ngemplak I. Sample was defined by using accidental sampling and consist of 30 people. The instruments of this research were questionnaire and guide interview. The result showed Public Health Center of Ngemplak I was used for hospitalized by family with middle to lower socio economic characteristic with average income Rp.200.000,00-Rp.500.000,00, most of responden work as farmer, labour, lower employee; low to middle education; lived in Ngemplak district, distance from home to Community Health Center < 5 km; and paid the treatment cost by themselves. The factors from service provider that encourage family’s decision to utilize hospitalization at Public Health Center of Ngemplak I were cheap, near, quickly treatment, friendly  service, easy administration, good facilities, and complete diagnostic tools. The internal factors that encourage family decision were income, health insurance participation, disease severity, and experience of utilitation  before. Abstract

ABSTRAKPasien dengan diabetes melitus (DM) tipe 2 akan menggambarkan penyakit yang dialami sesuai dengan apa yang ada dalam pikirannya untuk memahami masalah terkait. Persepsi sakit dapat ditingkatkan melalui pendidikan kesehatan oleh perawat tentang bagaimana mengelola penyakit dan manajemen perawatan diri. Penelitian ini bertujuan untuk menganalisa hubungan antara peran perawat sebagai edukator dengan persepsi sakit pada pasien DM tipe 2. Jenis penelitian ini adalah observasional analitik dengan pendekatan cross sectional. Teknik sampling penelitian ini adalah consecutive sampling dengan sampel sebanyak 112 responden. Pengumpulan data menggunakan kuesioner peran perawat sebagai edukator dan kuesioner Brief Illness Perception Questionnaire (BIPQ). Analisis data dilakukan dengan menggunakan uji korelasi Spearman-rank dengan tingkat signifikansi 0,05. Hasil penelitian menunjukkan terdapat hubungan yang signifikan antara peran perawat sebagai edukator dengan persepsi sakit (p value: 0,001; r: -0,433). Tingkat korelasi sedang dan bersifat negatif yang berarti semakin tinggi nilai peran perawat sebagai edukator maka semakin rendah nilai persepsi sakit. Semakin baik peran perawat sebagai edukator, semakin positif persepsi pasien tentang penyakit, yang berarti semakin sedikit ancaman penyakit yang dirasakan oleh pasien. Studi ini menunjukkan bahwa perawat harus dapat meningkatkan penyediaan pendidikan kesehatan untuk pasien dengan DM tipe 2 sehingga pasien dapat memiliki persepsi positif terhadap penyakit.   ABSTRACT Patients with type 2 diabetes mellitus will describe their illness based on what is in their minds to understand related problems. Illness perception can be improved through health education by nurses about how to manage the illness and self-care management. This study aimed to analyze correlation between the role of nurse as educator and illness perception in patients with type 2 diabetes mellitus. This research applied an observational analytic design with cross sectional approach. A total of 112 respondents were obtained by using consecutive sampling technique. Data collection was conducted by administering questionnaires of Role of Nurse as Educator and Brief Illness Perception Questionnaire (BIPQ). Data analysis was performed with Spearman-rank correlation test with significance level of 0,05. The result showed a significant correlation between the role of nurse as educator and illness perception (p value: 0,001; r: -0,433). The correlation was moderate and negative which means the higher the value of the role of nurse as educator, the lower the value of illness perception. The better the role of the nurse as educator, the more positive the patient’s perception about the illness, which means the less threatening the illness is perceived by the patients. This study indicates that nurses should be able to improve the provision of health education to patients with type 2 diabetes mellitus so that patients can have positive perceptions of the illness.

Abstract

ABSTRACT Introduction: Insulin therapy is one of the treatments in type 2 diabetes mellitus. Patients undergoing insulin therapy often experience side effects such as hypoglycemia. As a result, the patient feels a trauma that makes the patient change the insulin dose, ignores insulin injection time and lack of confidence when giving insulin injections independently. This condition makes managing blood sugar levels more difficult to achieve. This study aimed to analyze the correlation between self-compassion and adherence to insulin therapy in patients with type 2 diabetes mellitus at RSD dr. Soebandi Jember. Method: This research applied a descriptive correlational design with a cross-sectional approach. A total of 84 respondents obtained using consecutive sampling. Data collection conducted administering a questionnaire of the Self Compassion Scale (SCS) and the Morisky Medication Adherence Scale (MMAS-8). Data analyzed using the spearman correlation test with a significance level of 0.05. Result and analysis: The result showed a significant positive correlation between self-compassion and adherence to insulin therapy (p value=0.001; r=0.694). Discussion: Patients in managing diabetes need adherence to treatment. The existence of adherence in patients begins with the patient's self-acceptance of the illness. Self-compassion in patients is present when patients can accept themselves and love themselves without looking at the problem they face. The higher self-compassion makes a better adherence to insulin therapy. This study indicates the importance of self-compassion in patients with type 2 diabetes mellitus to improve insulin therapy compliance to control the patient's blood sugar levels. Abstract

Diabetes Mellitus (DM) is a chronic disease that needs a sustainable self-care. Low-level knowledgeof self-care can worsen health condition and cause stress. DSME/S is an ongoing process tofacilitate knowledge, skill, and abilities required to perform self-care. This research aimed to analyzethe effect of DSME/S on stress in patients with type 2 DM. The design was quasi-experimental withrandomized control group pretest-posttest design. The sample size was 30 respondents which wererandomly divided into 15 respondents as the intervention group and 15 respondents as the controlgroup. DSME/S was conducted once per week for 6 weeks with 150 minutes for each encounter.The measurement of stress was measured by using Diabetes Distress Scale (DDS). The data wereanalyzed by using dependent t test and independent t test with significance level of 0.05. The resultof dependent t test revealed a significant difference of stress between pretest and posttest both inthe intervention group (p = 0.000) and in the control group (p = 0.000). Furthermore, independent ttest showed a significant difference between intervention group and control group (p = 0.001) sothat there was a significant effect of DSME/S on stress in patients with type 2 Diabetes Mellitus.Keywords : stress, DSME/S, diabetes mellitus

Abstract

Diabetic wound very easily causes complications in the form of infection caused by bacterialinvasion, and hyperglycemia condition boosts the growth of bacteria. One of the bacteria thatcan cause infection in diabetic wound is Staphylococcus aureus. Diabetic wound care can bedone with honey. Honey contains antibacterial, antioxidant and hydrogen peroxide that can killharmful germs. The purpose of this research was to identify the effect of wound care with honeyon Staphylococcus aureus colonization in diabetic wound. The research design used was a preexperimentwith one group pretest and posttest design. Sample was taken using consecutivesampling with 7 respondents. Data analysis applied dependent t-test and obtained p value of0.000 (p value <α = 0.05); it can be concluded that there is an effect of wound care with honeyon Staphylococcus aureus bacterial colonization in diabetic wounds of patients with DiabetesMellitus in work area of Public Health Center of Rambipuji Jember. The suggestion from thisstudy is to apply honey as a wound care agent because it has antibacterial activity that can helpprevent infection and accelerate wound healing.Keywords: wound care, diabetic wound, honey, Staphylococcus aureus

Abstract

Oxygenation is human major basic need for maintaining body cell metabolism, survival and activities of various body organs. Respiratory system and cardiovascular system change throughout the aging process. Decreased respiratory function will affect the fulfillment of the needs of oxygenation in the elderly. Diaphragmatic breathing exercise is one of methods to maintain and improve respiratory functions in the elderly. This research aimed to determine the effect of diaphragmatic breathing exercise on respiratory functions (RR and PEFR) in the elderly. This research is a pre-experimental research with one group pretest and posttest design. Sampling technique used purposive sampling with 14 elderly given intervention of diaphragmatic breathing exercise once a day for 14 days. Abstract

Age is a factor that can influence sleep quality. Sleep quality decreases with aging and the complaintsof sleep become common among elderly. This study aimed to analyze the effect of laughter therapy onsleep quality of elderly in Long-Term Care Jember. The design of study was randomized control grouppretest-posttest design. The sample was 30 respondents, 15 respondents is intervention group and 15respondents is control group. Sleep quality was measured by using Pittsburgh Sleep Quality Index(PSQI). The data were analyzed by t test, Wilcoxon test, and Mann-Whitney test with significance levelof 0.05. The result revealed a significant difference of sleep quality before and after laughter therapy inthe treatment group (p: 0.001), and there was no significant difference between pretest and posttest inthe control group (p: 0.082). Mann-Whitney test showed a significant difference of sleep qualitybetween the treatment group and the control group after laughter therapy (p: 0.000). The percentageof elderly sleep quality before laughter therapy in the treatment group that is bad 100%, while afterlaughter therapy that is either equal to 100% which means there is an increase in the quality of sleepof 100%. Percentage of quality sleep before therapy in the control group of poor by 100%, while after abad sleep therapy quality percentage is 100%, which means no increase in the quality of sleep in thecontrol group It can be concluded that there was a significant effect of laughter therapy on sleepquality of elderly in Long-Term Care Jember. Abstract

Pathological change experienced by patients with type 2 diabetes mellitus is impaired peripheral blood circulation, especially in the lower limbs and feet. Peripheral blood circulation can be examined by measuring Ankle Brachial Index (ABI). Therapeutic exercise walking is one of physical activities which can improve blood circulation. This research goal was to analyze the effect of therapeutic exercise walking on peripheral blood circulation in patients with type 2 diabetes mellitus. The research design was randomized control group pretest-posttest design. The sample size was 15 respondents as control group and 15 respondents as treatment group. Data was analyzed by using dependent t-test and independent t-test with significant level of 0.05. This result showed a significant increase of ABI after therapeutic exercise walking in the treatment group (p= 0.001). However there was a significant decrease of ABI between pretest and posttest in control group (p=0.010).Independent t-test indicated a significant difference of ABI between treatment group and control group (p=0.000). So, it can be concluded that there is an influence of therapeutic exercise walking on the peripheral blood circulation in patients with type 2 diabetes mellitus. Nurses should apply therapeutic exercise walking to improve peripheral blood circulation in patients with type 2 diabetes mellitus. Abstract

Diabetes distress is an emotional problem that is directly related to the burden and worriesresulted from living with chronic disease. Stress has an impact on self-care behaviour whichaffects metabolic control. This study aimed to analyze the correlation between diabetesdistress and self-care behaviour in people with type 2 diabetes mellitus.This research appliedan observational analytic design with cross sectional approach. A total of 66 respondents wereenrolled in this study by using quota sampling technique. Data collection was conducted byadministering questionnaires of Diabetes Distress Scale (DDS) and Summary of Diabetes SelfCare Activities (SDSCA). Data were analyzed by using Spearman test with significance level of0.05. The result showed that the mean value of diabetes distress and self-care behaviour was2.16 and 3.97 respectively. The p value was 0.000 (p <0.05) with the correlation (r) of -0.629.There was significant correlation between diabetes distress and self-care behaviour. Thecorrelation was strong and negative which means the higher the level of diabetes distress thelower the performance of self-care behaviour. This study suggests the importance of assessingexperienced distress to optimize self-care behaviour in type 2 diabetes patients.Keywords: type 2 diabetes mellitus, diabetes distress, self-care behaviour
