Abstract

Background: Lichen planus is a chronic muccocutaneous inflammatory disorder. Oral lichen planus (OLP) has certain specific characteristics in its clinical presentation, but can also be presented in forms resembling other diseases. This may introduce difficulty in the diagnostic process. It has been reported that OLP affects between 0.1 to 4% individuals, predominantly women and people over 40 years old. Case Report: A 46-year-old woman complained of a non-healing ulcer present for 1.5 months. Intraorally, multiple white striae with prominent central areas of brownish hyperpigmentation were apparent on the right and left buccal mucosa, right and left buccal sulcus, and lateral and dorsum of the tongue. A biopsy was completed and the histopathology features confirmed the diagnosis of OLP. For the initial treatment, a topical corticosteroid was prescribed, followed by steroid mouthwash. Response to this treatment was positive. OLP has been frequently reported to affect women over the age of 40, with psychological stress as a primary predisposing factor. The clinical presentation is characteristic of Wickham’s striae with erosive areas. However, a biopsy and histopathological examination is mandatory to confirm the diagnosis. The clinical feature of pigmented OLP has been reported and confirmed by microscopic finding of band-like lymphocytic appearance which is the pathognomonic features of OLP, along with basal cell liquefaction degeneration, and melanin in continence at the lamina propria. Conclusion: Pigmented OLP is a variant of erosive OLP. Anamnesis, clinical presentation, and histopathological examination confirm diagnosis.

Abstract

Background: Angiosarcoma is a rare malignant tumor, which most often occurs in the skin and soft tissue. Its occurrence in the head and neck region, and the intraoral region in particular, is uncommon. The aim of this case report is to highlight the importance of histopathological examination and to underscore the characteristic features of angiosarcoma. Case report: A 52-year-old female presented with a growth in the right maxillary portion of the interdental region of her molar teeth. The growth had been present for approximately one month. A computerized tomography (CT) scan showed metastatic tumors in the lungs, liver, bone, and soft tissue of maxillary gingiva. Intraoral examination found a soft and mobile pedunculated ovoid mass at the right palatal gingiva; the mass was reddish-blue in color. An incisional biopsy was performed to confirm the diagnosis and plan for further treatment. Histopathologic examination revealed malignant endothelial cells in a vasoformative growth pattern, with numerous vascular channels lined with neoplastic endothelium-forming intraluminal buds, projections, and papillae. These projections were interspersed with the tumor cells. Immunohistochemistry analysis showed these endothelial cells demonstrated strong positive immunoreactivity with CD31, factor VIII, and FLi-1. A diagnosis of angiosarcoma was made. Discussion: Because intraoral angiosarcoma is rare and its clinical presentation can resemble other vascular lesions, it can be a challenging diagnosis for clinicians and pathologists. In the present case, the diagnosis of intraoral angiosarcoma was made based on clinical and histopathology findings. The microscopic features of malignant endothelial cells and the presence of intraluminal projections, supported by positive immunoreactivity with CD31, factor VIII, and FLi-1, confirmed the diagnosis. Conclusion: Angiosarcoma poses a diagnostic challenge to dental practitioners and oral pathologists due to the rarity of occurrence in the oral cavity. Histopathological examination is critical for an accurate diagnosis.

Abstract

Kanker mulut merupakan salah satu keganasan dengan morbiditas yang tinggi dan tingkat kelangsungan hidup yang buruk. Jumlahnya masih meningkat di semua negara termasuk Indonesia. Kanker mulut merupakan keganasan yang dapat dicegah dengan melihat gejala klinis yang dikenal sebagai pra-kanker. Deteksi dini adalah salah satu solusinya. Pengabdian masyarakat ini dilakukan secara daring dilakukan pada komunitas penyintas kanker Love and Healthy Tangerang untuk meningkatkan pengetahuan tentang kanker mulut. Informasi tentang pengenalan gejala klinis dini kanker mulut, faktor risiko, pemeriksa mulut sendiri (SAMURI), dan cara menjaga kesehatan rongga mulut diberikan kepada 50 peserta selama 3 jam. Semua peserta adalah perempuan dengan rentang usia 32-64 tahun. Pelatihan dievaluasi dengan melakukan pre-test dan post-test. Terdapat peningkatan nilai rata-rata pre-test 66,38 + 1,73 dan post-test 80,43 + 1,77, dan terdapat perbedaan yang signifikan pada tingkat pengetahuan pada responden sebelum dan sesudah pelatihan (p=0,000). Antusiasme peserta sangat baik, terlihat dari 70 pertanyaan yang diajukan selama pelatihan. Dengan diadakannya pelatihan deteksi dini kanker mulut dengan SAMURI, diharapkan informasi tersebut dapat bermanfaat sehingga angka kejadian kanker mulut dapat ditekan.

Abstract

Background:Diabetes mellitus (DM) is a chronic metabolic disease that can be life-threatening. The prevalence of DM increases every year along with obesity. There are a lot of oral manifestations of DM so dentists play an important role in the treatment of DM. The most common oral manifestations found in DM patients is periodontal disease which has a two-way relationship with DM. Dentists can also be the first to know the presence of DM in their patients. Therefore, it is important for dentists to have good knowledge and attitudes regarding the oral health of DM patients. Objective:To determine the knowledge profile and attitudes of dentists regarding the oral health of DM patients. Methods:Descriptive observational study using a validated questionnaire consisting of 30 questions about knowledge and 12 statements about the attitude of dentists regarding the oral health of DM patients.. Questionnaires were distributed via google form to dentists who are members of the West Jakarta PDGI. Results:The number of respondents in this study were 62 dentists, members of the West Jakarta PDGI. 100% of respondents agree that DM will affect the wound healing process and cause loose teeth. However, 45.16% of respondents did not agree that periodontal treatment (scaling and root planing) can improve glycemic control in DM patients. Only 46.77% of respondents always do scaling and only 19.35% of respondents do root planing for DM patients. 64.52% of respondents do not always update their knowledge about DM. Conclusion: Most dentists understand the prognosis, complications, and risk factors for DM, but do not really understand that periodontal treatment can help improve glycemic control.

Abstract

Background: Oral cancer in Indonesia still has a fairly high prevalence. The appearance of oral cancer can be initiated by the presence of Oral Potentially Malignant Disorder (OPMD) lesions, which is a disorder that may become a malignancy with a condition that is often preceded by the development of cancer of the lips and oral cavity. The knowledge and attitudes of dental education students regarding OPMD need to be achieved well so that it can be applied when practicing as a dentist. Objective: To describe the knowledge and attitudes of dental students in Indonesia regarding Oral Potentially Malignant Disorder. Method: This is a descriptive observational study with a cross-sectional design. The research was conducted by distributing an online questionnaire through a google form consisting of 47 questions about knowledge and attitudes to 211 clinical dental students faculty of dentistry in Indonesia. Respondents are classified into three levels based on their responses to the questionnaire, such as high, medium, and low. Results: A total of 211 respondents were 83.4% female and 16.6% male, of whom were aged ≤ 21 years old (16.6%), 22 – 24 years old (60.2%) and ≥ 25 years old (23.2%). A total of 125 subjects (59.2%) had high knowledge category and 86 subjects (40.8%) had a moderate category. Meanwhile, 195 subjects (92.4%) had attitudes in the good category, 13 subjects (6.2%) in the moderate category and 3 subjects (1.4%) in the poor category. Conclusion: The knowledge of OPMD among dental students in Indonesia were high with scores of 18.45 + 0.21, whereas the attitudes were good with scores of 17.10 + 0.17.

Abstract

Background: Oral cancer is one of the most causes of morbidity and mortality due to malignancy in Indonesia. This problem usually occurs because the cancer is found late in its development. Oral Potentially Malignant Disorders (OPMD) can be detected and treated as early as possible to prevent their transformation into oral cancer by dentists with a good knowledge and attitude about OPMD. Aim: To describe the knowledge and attitudes of dentists in Indonesia regarding OPMD. Method: Descriptive observational study with a cross-sectional design using an online questionnaire (google form) consisting of 47 questions about knowledge and attitude to 167 dentists in Indonesia. The knowledge was categorized into high, moderate, and low levels. The attitude was categorized into good, moderate, and low levels. Results: This study collects data from 167 people according to the inclusion criteria, consisting of 124 women and 43 men, of whom were aged 20-29 years (58 people), 30-49 years (97 people), and >50 years (12 people). A total of 121 respondents have a high level of knowledge and 46 respondents in the moderate category meanwhile, 124 respondents have good attitude categories, 42 respondents in the moderate categories, and one respondent in the low one. Conclusion: The dentist in Indonesia has a high level of knowledge with an average score of 18.86+0.23 and a good level of attitude with an average score of 15+0.18.

Abstract

Background: Candida–related lesions can present as denture sore mouth intraorally and cheilitis angular extra orally. Some predisposing factors include ill-fitting dentures, malnutrition, and poor oral hygiene. Objective: To determine the proper diagnosis and management of sore mouth dentures and cheilitis angular in patients with ill-fitting dentures and bad oral hygiene. Case report: A 59-year-old male patient presented with a burning sensation on the hard palate and sores at the corners of the lips, which were aggravated by the meal and opening of the mouth. No history of systemic diseases was found. Extra-orally erythema fissure lesions were found surrounded by multiple white areas at the corners of the lips bilaterally and were painful. Intra-oral examination showed erythema macular lesions that extend to the gingiva and palate mucosa that contact the denture base. Nystatin oral suspension combined with fluconazole, and multivitamins as supportive therapy. Educational suggestions such as the replacement of dentures and improve oral hygiene conditions were given. Conclusion: Denture sore mouth and cheilitis angular in this case are caused by Candida albicans related to the insufficient denture and poor oral hygiene. Management for this case by providing causative treatment with single or combine antifungal drugs along with proper communication, instructions, and education regarding the diseases.

Abstract

The oral cavity is a mirror of a person's health that can show local and systemic health problems. Dental and oral health problems, especially caries, periodontal disease, and cancer are reported as diseases and disorders in dental and oral health that are still experienced by the population in the world and Indonesia. It is known from a preliminary survey, that family members of WKRI community partner Ranting Birgitta have quite a lot of oral health problems but cannot freely come to the dentist for treatment during the pandemic. The aim of this community service was (1) to increase the knowledge about the problem of caries, bleeding gums, bad breath and the prevention (2) to increase the knowledge of causes and manifestations of various disorders that arise due to systemic medications and how to overcome them. The activity was carried out offline through the educational materials in the form of presentations, demonstrations, and posters to members of the WKRI community of Birgitta branches and guests, on February 5, 2023, at Grha St. Mikael, Kranji, Bekasi. A total of 43 people (32 participants and 11 PkM teams) participated in this activity. The pre-test and post-test results attended by 29 participants showed a significant increase in knowledge by 1.2 times (p<0.001) of the material presented, with an average pre-test score of 54.66 and 78.10 for the average post-test score. Overall, the activity went smoothly. It is highly suggested that similar activities can be held again with a wider scope of participants.

Abstract

Background: Oral potentially malignant disorder (OPMD) is a lesion with a high potential to turn into oral squamous cell carcinoma (OSCC). The discovery of OPMD lesions precedes most cases of OSCC. Sensitive and specific biomarkers can help in the early detection of high-risk patients, one of which is CD8+, part of the cytotoxic T lymphocytes, which have a role in eliminating cancer cells. Several studies use infiltration density and CD8+ T cells' activity as biomarkers in malignancy. This study aims to observe the expression of CD8+ in OPMD lesions and correlate with risk factors Method: Twelve paraffin blocks samples consist of OPMD lesions. One oral lichen planus (OLP) as the positive control. Samples were stained using HE to observe the degree of dysplasia and immunohistochemistry to observe the expression of CD8+ T cells. Observation of results using a microscope with 100x magnification to select the field of view and 400x magnification to count the number of positive CD8+ T cells with a cut-off point of 500 cells from a total of 5 fields of view and determined by weak intensity (

Abstract

Background: Oral Allergy Syndrome (OAS) is a localized IgE-mediated food allergy triggered by certain fruits and vegetables in patients with atopic history. This case report describes two siblings with Oral Allergy Syndrome (OAS) presenting as allergic angioedema triggered by specific food allergens.  Case Presentation:  Case 1 involves a 25-year-old female who developed lip swelling, a burning sensation, numbness, multiple oral ulcers on the lower lip, and surrounding redness 30 minutes after consuming mint sauce in an Indian dish. Case 2 details a 21-year-old male who experienced immediate lip and soft palate swelling with numbness after eating Chinese food suspected to contain nuts. Discussion: Both siblings share a history of childhood atopy, predisposing them to IgE-mediated reactions. Their symptoms were characteristic of localized allergic responses in the oral mucosa, consistent with OAS. Case Management: Management involved a combined therapeutic approach: topical corticosteroids were applied to the affected oral areas to reduce local inflammation and promote healing of the mucosal lesions, while oral corticosteroids provided systemic anti-inflammatory effects. Additionally, cetirizine, an H1 receptor antagonist, was administered to block histamine-mediated symptoms such as swelling and itching. Patients were also advised to avoid mint-containing and peanut-containing foods as predisposing factors. Conclusion: This report highlights not only the clinical presentation and successful management of OAS in individuals with familial atopy but also underscores the potential genetic predisposition contributing to food-induced allergic reactions.
