Abstract

Introduction: Knee osteoarthritis is a degenerative joint disease that is most common in adults and the elderly in many countries. Incidence of osteoarthritis is increased steadily over the last 15 years. Currently, there is still no simple therapy to cure osteoarthritis disease. Therefore, it is very important to find the risk factor of osteoarthritis to do etiology prevention. Aim: The purpose of this study is to discover the correlation of body mass index and age with disability rate in patients with Grade II-IV Knee Osteoarthritis. Methods: This research is an observational analytical study using cross sectional approach. The sampling techniques used was purposive sampling. Data collection was done by interviewing respondents using KOOS-PS questionnaire. Result: There is a significant, strong correlation between body mass index and disability rate in patients with knee joint OA (r=0.661). However, there is no significant correlation between age and knee joint OA disability (r=0.150). A moderate correlation is found, respectively, between body mass index and the grade of knee joint OA (r=0.424) and also between age and the grade of knee joint OA (r=0.352). Conclusion: There is a significant correlation between body mass index and disability rate of patients with knee joint OA.

Abstract

Introduction: Posterior tibial tendon dysfunction is one of the most common, problems of the foot and ankle. Tenosynovitis of the posterior tibial tendon (PTT) is an often unrecognized form of PTT dysfunction. Case: A 54-year-old woman presented with left ankle pain that began while morning walk three days prior. She noted that the left ankle hurt with even light touch and the pain was unrelieved with sodium diclofenac. She denied any history of trauma. She was seen in the outpatient clinic for this condition. On examination, a three centimeter area of pain was found posterior to the medial malleolus and parallel to the PTT. She also had a stage I flat foot and mild soft tissue swelling around medial malleolus region on her radiograph examination. Ultrasound examination was done with the result of anechoic fluid visible in the peritendinous space around the PTT. The patient received diagnosis of PTT tenosynovitis, with the foot and ankle disability index (FADI) score was 58.7. Platelet rich plasma (PRP) injection was done twice with an interlude of two weeks. The pain subsided and the following FADI score was 84.6. Outcome: Patient showed improvement in her left ankle PTT tenosynovitis after two PRP injection. Conclusion: This case report highlights the efficacy of PRP as a modality in managing PTT tenosynovitis.

Abstract

Penelitian ini bertujuan untuk menganalisis tanggung jawab hukum dokter ortopedi dalam pelaksanaan operasi bedah minimal invasif (MIS) pada pasien BPJS Kesehatan dalam kerangka sistem Jaminan Kesehatan Nasional (JKN). Meskipun MIS menawarkan berbagai keunggulan seperti pengurangan trauma jaringan dan pemulihan lebih cepat, penerapannya di Indonesia dihadapkan pada tantangan, seperti keterbatasan fasilitas dan sumber daya medis dalam sistem JKN. Penelitian ini menggunakan metode normatif dengan studi literatur dan analisis dokumen hukum, termasuk Undang-Undang Praktik Kedokteran dan Kitab Undang-Undang Hukum Perdata terkait malpraktik. Hasil penelitian menunjukkan bahwa dokter ortopedi memiliki tanggung jawab hukum besar dalam menjalankan prosedur MIS, termasuk kewajiban memberikan informasi yang jelas kepada pasien dan memperoleh persetujuan yang sah (informed consent). Kendala sistemik dalam JKN berpotensi meningkatkan risiko komplikasi medis dan gugatan hukum. Oleh karena itu, diperlukan penguatan pengawasan dan pelatihan untuk meminimalkan risiko hukum dan memastikan kualitas pelayanan medis.

Abstract

BackgroundCheckrein deformity is a rare abnormality comprising flexor hallucis longus (FHL) tendon entrapment in the posterior foot due to post-traumatic or ischemic retraction of the FHLtendon following soft tissue trauma. The diagnosis is essentially clinical, but complemented by imaging to rule out unrecognized causes and evaluate the fracture healing process. This case report is the first Indonesian study to describe release of the flexor retinaculum and Z-plasty (lengthening) of flexor digitorum longus (FDL) and FHL tendons through a medial retromalleolar approach to repair the deformity. Case DescriptionWe present the case of a 51-year-old male who complained of clawing of his right first and second toes. The interest of this study lies in the fact that this patient had never experienced ankle or distal tibial fractures, only a trivial injury two years back when the patient kicked a stone at work and had a bruise on his right second toe with no fracture. Exploration of FHL tendons was performed at the level of the midfoot. The patient was planned for surgery with a medial retromalleolar ankle approach, which facilitates the release of the flexor retinaculum, in conjunction with a Z-tenotomy on the FHL and FDL tendons. ConclusionA correction was achieved, and two months post-surgery there were no recurrences of the deformity. However, this surgical procedure requires more cases to support an evaluation of its effectiveness. We suggest that exploration at the ankle and midfoot should be the primary surgical intervention in similar cases of checkrein deformity.
