Abstract

Hypertension is one of the chronic diseases with a very high prevalence worldwide, influenced by various factors such as environment, genetics, lifestyle, and the use of certain medications like birth control pills. Increased blood pressure in hypertensive patients has a significant impact on the risk of serious cardiovascular complications such as heart disease, heart failure, stroke, and kidney failure. Blood pressure control can be achieved thru self- management, which includes medication adherence, healthy lifestyle changes such as regular physical activity, and a low-sodium diet. One effective physical activity for lowering blood pressure in hypertensive patients is brisk walking. Based on this, this study aims to determine the effect of the brisk walking exercise technique on lowering blood pressure in hypertensive patients at RSD dr. A. Dadi Tjokrodipo Bandar Lampung. The method used by the researcher in this study is descriptive with a case study approach. The measuring instrument in this study used a sphygmomanometer, and the brisk walking exercise technique was applied according to the SOP. The research findings indicate a decrease in blood pressure among hypertensive patients after performing brisk walking exercise daily for eight days. The brisk walking exercise technique can lower blood pressure in hypertensive patients at RSD dr A. Dadi Tjokrodipo Bandar Lampung.

Abstract

Tuberkulosis (TBC) merupakan tantangan kesehatan serius di Kabupaten Jember, yang menempati peringkat ketiga tertinggi di Jawa Timur. Keberhasilan pengobatan TBC sering terhambat oleh rendahnya kepatuhan terapi yang berakar dari minimnya pengetahuan, dukungan keluarga, dan tingginya stigma sosial. Kegiatan pengabdian melalui program "OPTIMAL CARE" ini bertujuan memberdayakan keluarga dan kader kesehatan untuk mendukung keberhasilan terapi pasien TBC. Metode yang digunakan adalah pemberdayaan komunitas melalui empat tahapan: identifikasi kebutuhan, perancangan program, implementasi, dan evaluasi. Hasil dari tahap identifikasi dan perancangan menunjukkan adanya kesenjangan pengetahuan yang signifikan, di mana 90% keluarga pasien belum memahami praktik perawatan pendukung di rumah. Selain itu, teridentifikasi pula keterbatasan peran kader yang belum optimal sebagai edukator dan motivator. Berdasarkan temuan tersebut, program ini secara spesifik dirancang untuk meningkatkan kapasitas keluarga dan kader melalui edukasi terstruktur dan pendampingan intensif. Kesimpulan dari tahap awal ini menegaskan bahwa intervensi pada keluarga dan kader merupakan langkah strategis untuk membangun ekosistem pendukung yang kuat guna meningkatkan keberhasilan pengobatan TBC.

Abstract

Repeated arteriovenous fistula cannulation induces significant procedural pain in patients undergoing maintenance hemodialysis, necessitating effective, non-pharmacological pain management strategies. This study aimed to evaluate the efficacy of a simple, nurse-led cold compress therapy in reducing cannulation-related pain among patients with chronic kidney disease (CKD). A descriptive case study was conducted in a hospital hemodialysis unit between June and July 2025. Two adult male patients (aged 45 and 53 years) reporting procedural pain were enrolled. Pain intensity was assessed using the Numeric Rating Scale (NRS) at baseline and following two subsequent hemodialysis sessions. The intervention consisted of applying ice cubes enclosed in a rubber glove to the cannulation site for approximately 10 minutes prior to needle insertion. At baseline, patients reported moderate (NRS 6) and severe (NRS 7) pain, respectively. Following the second intervention session, pain scores decreased to 4 and 5. By the third session, pain further diminished to mild levels (NRS 2 and 3). Notably, no adverse cutaneous or systemic events were observed during the application. Pre-cannulation cold compress therapy represents a feasible, low-cost, and safe nurse-led intervention that progressively mitigates hemodialysis cannulation pain. Larger, randomized controlled trials are warranted to validate these preliminary findings, isolate the analgesic effect, and standardize optimal clinical application protocols.
