Abstract

Background: The study aims to map TB (Tuberculosis) – HIV (Human Immunodeficiency Virus) patients based on clinical, radiological, and laboratory patterns. Objective: This study is expected to provide previous research investigating radiologic and laboratory examinations in TB-HIV patients. Method: We conducted a cross-sectional study based on medical records of TB-HIV patients at RSPI Sulianti Saroso from January 2004 - March 2017. Samples were 240 TB-HIV patients from a population of 1014 People Living with HIV/AIDS (PLWHA). Inclusion criteria were PLWHA, aged over 18 years, diagnosed with TB with completed medical record status. Result: Characteristics of the majority of patients were in the productive age group (99.2%) with an average age of 33 years and male sex (78%). The main symptom is cough (91.7%) and cough with phlegm (59.1%). Most patients (83.37%) have not received ART (Anti-Retroviral Therapy). Pulmonary physical examination showed that most patients had Ronchi (95.8%) while others got wheezing (5%). Laboratory tests revealed an initial CD4 cell count of 71.6 cells/ml and a negative Acid-Fast Bacillus (AFB) smear (59.2%). The patient's radiology was mainly with an infiltrate (82.5%); most infiltrate was presented in apex (52.4%). Conclusion:  The majority of TB-HIV coinfected patients are young males, presenting with common TB symptoms, low CD4 counts, and negative AFB smears, complicating diagnosis. Radiological findings frequently show infiltrates, particularly in the apex. These findings emphasize the importance of early diagnosis and ART initiation to improve outcomes in TB-HIV coinfected patients

Abstract

Background: Gout arthritis is caused by the deposition of monosodium urate crystals in the joints, which can cause significant pain, swelling, and recurring inflammation, particularly in the big toe. Inflammatory mediators are critical to the beginning and duration of the inflammatory response in gout arthritis. Gout is mediated by TNF-α, ILs, and the NLRP3 inflammasome, which promotes macrophages, monocytes, and neutrophils, leading to inflammatory reactions. TNF-α acts as a potent inflammatory mediator by stimulating the expression of other inflammatory cytokines, adhesion molecules, and matrix metalloproteinases (MMPs). TNF-α interacts with particular receptors and activates intracellular signaling pathways, leading to leukocyte recruitment, NLRP3 inflammasome activation, and the release of other inflammatory mediators. Objective: This article is a review article in which the researchers examine the current state of understanding regarding the role of TNF-α as a major inflammatory mediator in gout arthritis, deepen the understanding of TNF-α in gout inflammation, and explore its potential as a therapeutic target. Method: It involvedsystematically searching relevant databases, selecting studies based on inclusion criteria, and synthesizing findings to identify trends, gaps, and the current state of knowledge on the topic. Result: The results of this review article indicated that TNF-α plays a crucial role in joint inflammation by triggering the release of pro-inflammatory cytokines and stimulating neutrophil infiltration into the joints. Conclusion: Thus, Inhibiting TNF-α can reduce inflammation and improve symptoms in people with gout arthritis.

Abstract

Background: Reduced metabolic function can cause the human body to experience a lack of energy, resulting in the possibility of diseases including diabetes mellitus, hypertension, atherosclerosis, and even coronary heart disease. Objective: This study aims to analyze the incidence of obesity through Body Mass Index on the incidence of diabetes mellitus and Lipid Profile at SMA X Surabaya. Methods: The research design used was cross-sectional. Sampling was carried out using non-probability sampling with a random sampling technique. Data collection was carried out at SMA X Surabaya by 64 teachers and employees of SMA X Surabaya. Respondents were gathered to receive health education, then went through the registration stages, and vital signs, and then 5 mL of venous blood was taken to measure blood sugar levels and lipid profiles, namely total cholesterol and triglyceride levels. This research uses the Chi-Square test statistical test. Result: There is a significant relationship based on the chi square test between body mass index and cholesterol levels and triglyceride levels. because the p-value < 0.05 is 0.041 and 0.050 respectively, but there is no significant relationship between body mass index and glucose levels with a p-value of 0.181. Conclusion: There is a relationship between the incidence of obesity through Body Mass Index and the incidence of Lipid Profile but there is no relationship with the incidence of diabetes mellitus at SMA X Surabaya

Abstract

Background: One well-known hypercoagulable state that raises the risk of venous thromboembolism is pregnancy and it is more common in cesarean delivery. Pulmonary embolisms are difficult to identify based solely on postpartum symptoms. Most of the symptoms are non-specific, therefore they are typically underdiagnosed throughout pregnancy and the postpartum period. Objective: This work presented information on the treatment of high-risk pulmonary embolism in the postpartum phase. Case presentation: This paper reported the case of a 33-year-old female, admitted to the emergency room for shortness of breath. Her personal history showed that she had just given birth to her sixth child by caesarian section a month ago and had a history of hysterectomy due to severe infection. Result: She suffered a high-risk pulmonary embolism in the post-partum period. It takes a strong index of suspicion to diagnose and treat patients in a timely manner. Conclusion: Echocardiography and Multislice Computerized Tomography are the best diagnostic methods. Venous thromboembolism prophylaxis should be taken into account, particularly in patients who have risk factors.

Abstract

Background: Facial skin is the most sensitive part, so it is important to take care of it. Facial skin care is a treatment to keep facial skin elastic and maintain facial skin moisture. Facial acupressure is a method of treatment that begins with massaging the meridian points so that blood flow becomes smooth and facial muscles feel more relaxed. Using honey is a facial skin care method that is made into a mask. Objective: The research aims to determine the effectiveness of the combination of facial acupressure and honey on facial skin moisture. Methods: This research is an analytical experimental study, with a sample of 30 women aged 40 - 50 years. In the first treatment, a moisture test was carried out, followed by the second treatment, the moisture test was measured for the facial acupressure and after the third treatment, the combination of the facial acupressure with a honey mask was subjected to a moisture test.  The research was carried out twice a week for 4 weeks for a total of 8 times. The research was carried out in April – May 2024 at the Wijaya Surabaya clinic & in a doctor's practice and used a Skin Moisture Analyzer as a measuring tool to measure skin moisture. Using statistical analysis with the Mann-Whitney test (α= 0.05) with a p-value of 0.000. Result: The results of the third treatment showed an increase in facial skin moisture until it reached 93.4%, including moist skin and 6.6%, which was considered normal skin criteria. Conclusion: It has been proven that the combination of facial acupressure and honey is effective on facial skin moisture, experiencing very significant changes in humidity starting from the first, second and third treatments.

Abstract

Background: Indonesia is the most significant contributor to Hajj pilgrims, with health risks that are still relatively high, one of which is Hypertension. Lack of knowledge about Hypertension is an obstacle to controlling Hypertension. In addition, half of hypertension-related deaths could be prevented by compliance with taking antihypertensive medication. Objective: This study sought to determine the association between prospective Hajj pilgrims' compliance with medication and their knowledge of hypertension in 2023 who suffer from Hypertension in Batu City. Method: The research used quantitative methods with a cross-sectional design. The sample for this research was 39 Prospective Hajj pilgrims, and total sampling was taken. Result: The results of the study on Prospective Hajj pilgrims who suffer from Hypertension in Batu City showed that the majority of their knowledge about Hypertension was in a good category, 23 people (59.0%), the level of compliance with taking medication was in the low sort 24 people (61.5%); There was no correlation between medication adherence and knowledge of hypertension (p-value 0.185). Conclusion: Non-compliance with taking medication prospective Hajj pilgrims in Batu City can be changed by regularly providing education about the importance of taking medication and the risks that can arise if they do not regularly take hypertension medication

Abstract

Background: Recently, many people have considered electronic cigarettes to be a safer alternative to conventional tobacco cigarettes, even though they still contain harmful substances such as nicotine, which is associated with a decrease in hippocampal neurons. Objective: This study aimed to examine the effect of exposure to e-cigarettes on hippocampal neurons in rats. Methods: An experimental study was performed using 26 tissue samples obtained from Wistar rats. The rats were randomly assigned to four groups and treated over an eight-week period as follows: the control group (K) (n=7) had no vapor exposure; the V0 group (n=6) received vapor without nicotine (0 mg/ml); the V6 group (n=7) was exposed to vapor and 6 mg/ml nicotine; and the V12 group (n=6) was exposed to vapor and 12 mg/ml nicotine. Hippocampal neurons were examined using hematoxylin-eosin staining at 400x magnification across 7 fields of view. Data were analyzed using the Kruskal-Wallis test with the Mann-Whitney post-hoc.  Results: The number of hippocampal neurons was significantly reduced in both the V12 and V0 groups compared to that in the control group (p<0.05). The lowest number of hippocampal neurons was observed in the V0 group (0 mg/ml nicotine), followed by the V12 group (12 mg/ml), and then the V6 group (6 mg/ml).Conclusion: The present study showed that the variation of nicotine in vapor exposure decreases hippocampal neurons in rats.

Abstract

Background: Niacinamide, also called nicotinamide, is the active form of niacin (nicotinic acid, vitamin B3). Niacinamide’s role in medicine continues to be studied due to its extensive effects and low toxicity. Various studies also suggest that niacinamide offers anti-inflammatory effects, improves the skin barrier, and acts as an anti-aging, brightening, and anticancer agent. Cutaneous lupus (CL) is a general term for a group of autoimmune connective tissue disorders affecting the skin that may be associated with systemic lupus erythematosus (SLE). There are plenty of treatment modalities for patients with CL, each with varying efficacy. Therapeutic options for CL can range from topical agents, including corticosteroids and calcineurin inhibitors, to systemic therapy, such as antimalarials, immunosuppressants, retinoids, thalidomide, and biological agents. Niacinamide is a topical agent widely used in various cases, one of which is CL. Objective: This case report aims to discuss the effectiveness of niacinamide as an adjuvant therapy in CL. Case Presentation: A 48-year-old woman with SLE and CL received systemic therapy consisting of methylprednisolone, hydroxychloroquine, and 4% niacinamide gel for 8 weeks. Results: Observations showed clinical improvement in CL lesions without any significant side effects. Conclusion: Therefore, 4% niacinamide gel therapy can be used as an adjuvant therapy.

Abstract

Background: This essay examines the most recent news, namely the passing of a voting committee member (PPS) in the general election (Pemilu), which included candidates for president, vice president, and legislature (Caleg). PPS saw 35 fatalities and 3.909 illnesses. Given that the counting and recapitulation process is still in progress, the likelihood of deaths and illnesses will only rise. Objective: The aim of this research is to find the relationship between hypoxia and stress in PPS officers who died due to physical exhaustion during election activities in Indonesia Methods: Articles containing information on hypoxia, comorbidity, stress, physical exhaustion, and its connection to death were included as inclusion criteria. There was a total of 25 papers that met the inclusion criteria after the literature analysis procedure was completed. Result: Along with the comorbid status, physical exhaustion, and stress play a significant part (comorbidities). This connection arises from the fact that physical exhaustion and stress induce hypoxia, which in turn affects immunity. The drop in blood oxygen levels, or hypoxia, disrupts and dysregulates the innate and adaptive immune systems. The chance of dying can rise when hypoxia and ongoing stress are present. Writing this paper proposes more investigation into the role that hypoxia and stress play as death risk factors. Conclusion: Hypoxia results from stress. what makes HIF the primary transcription factor active. When PPS officers activate their HIF in order to fulfill their mandate and fulfill their obligations under Law Number 7 of 2017 concerning General Elections, there is an elevated risk of death and morbidity.

Abstract

Introduction: Crusted scabies (CS) is characterized by uncontrolled proliferation of mites up to thousands. Complications include glomerulonephritis, rheumatic fever, and secondary infection which can cause bacteremia and sepsis. This case report describes a case of CS that was hospitalized with secondary complications of MRSA infection. Case Presentation: A 44-year-old woman complained of thick scales and itching all over her body for 6 months. Complaints begin with reddish bumps between the fingers, toes, body, genitals, and scalp, accompanied by itching, especially at night. The patient took self-purchased corticosteroids for this complaint for 6 months. Dermatology status all over the body showed erythematous papules plaques with greenish-yellow scales, blackish crusts, hyperkeratosis, and excoriation, also erythematous base erosions and blackish crusts in the right thoracic region. Ectoparasite scrapping examination showed large amounts of Sarcoptes scabiei, eggs, and scybala, so the patient was diagnosed with Crusted scabies. A wound culture and nasal swab showed MRSA. The patient died of septic shock on day 12 of treatment. Discussion: Crusted scabies is a highly contagious form of scabies, characterized by thickening of the skin layer and crusts. Long-term corticosteroid consumption in this case caused a compromised immune response so that the number of mites multiplied rapidly and increased the risk of MRSA infection. MRSA mortality rates range from 5-60%. Conclusion: Ignored cases of scabies can develop into crusted scabies which have a high risk of secondary infection, such as MRSA which can cause serious morbidity and even mortality.
