Abstract

The Manual of Chronic Total Occlusion Percutaneous Coronary Interventions: A Step-by-Step Approach, Third Edition is a useful reference for percutaneous coronary intervention (PCI) with chronic total occlusion (CTO). Abstract

Background Alongside hydrocephalus, cerebral abscess poses as space-occupying lesion altering consciousness and intracranial pressure. Cerebral abscesses commonly presented vague clinical symptoms in infants, including weeks of fever and late-onset neurologic deterioration. However, limited studies denote cerebral abscesses, especially in children under one year old. Objective We reported a case of a 10-month-old male infant with cerebral abscess and hydrocephalus.Case description A 10-month-old male infant was taken to the hospital due to 6 days progressing fever. Neither clinical nor laboratory findings were abnormal. After being discharged from the hospital, the patient has admitted again 2 days later, reserved for 5 days of fever until unfortunate symptoms of raised intracranial pressure coincide with hypothermic body temperature. A brain CT-Scan showed an area of hypodense lesion in the parieto-occipital region suspected of brain abscess- resulting in non-communicating hydrocephalus. After a total of 12 days of intensive treatment with antibiotics, anti-convulsant, and steroid agents, the patient was comatose and passed away due to respiratory failure. Conclusion Brain abscesses should be investigated thoroughly despite the probable absence of a pathognomonic feature. Radiology imaging ought to demonstrate the early process of abscess pathogenesis. To the best of our knowledge, this is the first case report published in Indonesia of an infant with a cerebral abscess resulting in hydrocephalus.

Abstract

Background Diabetes mellitus (DM) is a disease caused by impaired glucose metabolism, failure of the pancreas to produce the insulin hormone or the body's inability to respond to the insulin hormone. The neurologic complications are neuropathic or nociceptive pain. Neuropathic pain can cause the decrease in patient's quality of life. Knowing the spread of neuropathic pain based on anatomical predilection may help clinicians conduct early diagnosis and improve the patient's quality of life. Objective This study aim to determine the characteristics, prevalence and location of pain in patients with type 2 diabetes mellitus and non-diabetes mellitus at Puskesmas Danurejan I Yogyakarta. Methods A descriptive quantitative study on 42 secondary data from previous study in Puskesmas Danurejan I. The inclusion criteria were data of patients with neuropathic pain (electric shock-like pain, stabbing pain) and nociceptive pain  (burning sensation pain) who were > 40 years old. The exclusion criteria were incomplete data and recorded consumed non-steroidal anti-inflammatory drugs in the last of 4 days in previous study. Univariate and bivariate analysis were used in this study. Results Patients with DM accounted for 50% of cases by 21 people. Patients with DM had the most frequent comorbidities, such as hypertension (42.8%) and obesity (42.8%), Most patients with DM reported pain (85.7%) with neuropathic back pain (27.7%). Based on the type of pain, patients with DM most frequently reported neuropathic pain (66.7%) (p=0.001). Conclusion There was a relationship between diabetes mellitus and the incidence of neuropathic pain but not nociceptive pain. Keywords: diabetes mellitus; nociceptive pain; neuropathic pain.

Abstract

Background Knowledge of medical science is essential to basic medical clerkship students who should prepare for a clinical clerkship. Eye disease is still one of the biggest health problems globally, especially those which cause blindness, one of which is related to refractive disorders. Purpose This study aims to explore knowledge about refractive abnormalities to basic medical clerkship students of UKRIDA in 2022. Method It employs a descriptive research method, and the questionnaires are created based on a Likert scale and shared on Google Forms. Result The number of participants is 80 people with an age range of 20-25 years with 67.5% of women. A total of 43.75% of students have a good level of knowledge, and 56.25% of them hold a sufficient level of knowledge about refractive disorders. Meanwhile, 86.25% of them have a good understanding about eye anatomy and refractive examination. 75.75% have a good level of knowledge about myopia. 32.5% have a good level of knowledge about hypermetropia. And, as many as 35% students have a good understanding of astigmatism. Further, a total of 37.5% of students have a good level of knowledge about refractive anomalies. Conclusion A total of 43.75% of students have a good level of knowledge, and 56.25% have a sufficient level of knowledge about refractive disorders. Abstract

Background: Myeloproliferative disorders can rise a risk of thrombotic events. Ischemic stroke is one of presenting symptoms can be found in such patients. These coexisting conditions can burden patients with more severe complications.Objectives: To discuss a case of polycythemia vera with multiple episodes of ischemic stroke.Case description: The patient was a 70-year-old female with chief complaint of sudden rotatory dizziness with moderate to severe intensity. Nausea, vomiting, slurred speech, and facial muscle weakness were also reported. Onset of symptoms started 3 hours before hospital admission. Medical history including previous ischemic stroke in 2020 and 2015 with remaining right extremity weakness. Physical examination showed weakness, hypertension. Nystagmus horizontal bidirectional and upper motor neuron lesion of left CN VII and XII, weakness in right upper and lower extremities were observed. Physiological reflexes were increased, and pathological reflex was found. Head CT showed hypodense lesion led to her ischemic stroke diagnosis. Blood test showed increased hemoglobin count, leukocytosis but normal platelet count. Increased cell numbers and variations of size, morphology and distribution were found in peripheral smear. This finding was also supported with bone marrow examination which showed hypercellularity, panmyelosis with dysplasia of all hematopoietic lineage. Patient was diagnosed with polycythemia vera and treated accordingly.Conclusion: Further examination of hematology, such as peripheral smear or bone marrow examination should be considered in patients with recurrent episodes of stroke to rule in underlying myeloproliferative disorders.Appropriate treatment and routine hematology follow up are in need to increase awareness of future thrombotic events or leukemic transformations.

Abstract

Background Insomnia is the most common sleep disorder in the world. The incidence of insomnia in some medical students in Indonesia can also reach half of the population. Meanwhile, sleep is the phase where oxygen transport and neuron performance occur optimally. Objective This study aims to determine the association between the degree of insomnia and the grade point average predicate for Duta Wacana Christian University, Yogyakarta medical students. Methods The analytical study with a cross-sectional design was conducted in this research. The number of participants were 213 medical students from 2016, 2017, and 2018 batches who were taken randomly. The degree of insomnia data was taken by the insomnia severity index questionnaire. Results Through the questionnaire, 47.83% of male students and 39.67% of female students experienced sub-threshold insomnia, 7.6% of male students and 7.44% of female students experienced moderate-severity insomnia, and 0.82. % of female students have severe insomnia. Data were analyzed using Chi-square statistical test. Based on the results of the Chi-square analysis, the p-value obtained is 0.058 (p-value > 0.05) so the results are not in accordance with the hypothesis Conclusion It can be concluded from these results that the degree of insomnia does not associate with the grade point average predicate for medical students of Duta Wacana Christian University, Yogyakarta. Abstract

Overall, these two studies provide valuable insights into efforts to reduce pain burden in patients with diabetes mellitus and patients undergoing bronchoscopic procedures. In the context of diabetes mellitus, understanding the characteristics of pain in patients can help in better management. Meanwhile, the use of music as a non-pharmacological therapy in bronchoscopy procedures shows potential to reduce pain and anxiety in patients. However, further research is needed to gain a deeper understanding, as well as to overcome existing limitations, and provide optimal benefits for patients.

Abstract

Background Acne vulgaris is a chronic inflammatory disease of the pilosebaceous follicles. This disease is usually found in more than 85% of young adults worldwide. Acne vulgaris can cause psychological morbidity and also greatly affects socio-economic and the quality of life. Patients often have already tried several treatments, but still do not get good results. Therefore, it is important to identify the type of lesion and then choose the right treatment for acne vulgaris. Objective To know the profile of acne vulgaris patients at the Dermatology and Venereology Clinics of Bethesda Hospital Yogyakarta, from July 2019 to December 2020. Methods This study was a descriptive-retrospective study, using secondary data from the medical records of acne vulgaris patients. Data will be collected in the percentage frequency distribution table. Results This research was done in the medical records department of Bethesda Hospital Yogyakarta from February to April 2022. The number of samples taken by total sampling technique that matched the inclusion and exclusion criteria was 151 records. The highest percentage of acne vulgaris patients were female 72.2% (109 people), aged 20-24 years 57.0% (86), had inflammatory skin lesions 54.3% (82), received combination of topical and systemic treatment 80.8% (122), the most topical treatment prescribed was niacinamide 80.8% (122), the most systemic treatment was antibiotics 41.7% (63), the most systemic antibiotic was minocycline 73.0% (46), the highest visiting frequency was once (without control) 57.6% (87), and irregular follow-up visits 60.9% (92). Conclusion Based on the study conducted, patients with acne vulgaris were dominated by females, aged 20-24 years. One of the most prominent symptoms was inflammatory skin lesions. The patients have received combinations of topical and systemic treatment. The most used topical treatment was niacinamide; the most used systemic treatment was antibiotics; and the most used systemic antibiotic was minocycline. The highest trend for visiting frequency were irregular controls followed by one-time doctor visit without control. Keywords: Acne Vulgaris, Demography, Therapeutics, Control

The content of this book is comprehensive and complete in only one book which allows the reader to understand step-by-step medical education, or simply to look for a certain point of information because in this book each section is divided into chapters and is supported by appendix pages which make it easier to find the study material. Readers' understanding is easier to obtain with this book because the English used is easy to understand, but the small font size may be quite tiring for some readers. Reviewers recommend this book as a reliable and easy-to-understand reference, especially for preclinical and clinical medical educators.

Abstract

Background Sampling in bronchoscopy can lead to acute consequences, such as intrabronchial bleeding, bronchospasm, and pneumothorax. Bronchoscopies make patients uneasy because they fear discomfort and difficulty breathing. Hypnotherapy, music therapy, and aromatherapy can help during bronchoscopies. Musicure is considered to assist people to grow well by sending "passive" musical impulses.Objective The goal of this study was to see how well musicure helped bronchoscopy patients deal with pain.Methods A quasi-experimental design with a pre-test, post-test, and control group was used. Patients with lung illness will undergo bronchoscopy and consecutive sampling at the Dr. Moewardi Surakarta Hospital in February-March 2020. The study subjects were divided into a treatment group with Musicure and a control group without music. Both groups were assessed with pain visual acuity score (VAS)  at baseline and after bronchoscopy.Results A total of 32 subjects with lung diseases whom bronchoscopy in this study. The treatment group showed a decreased pain VAS scale (-14.71±12.31). The treatment group pre-test pain score obtained 31.76±12.86, and the VAS post-scale average of 17.06±12.63. The changes in the VAS scale post-pretreatment group showed a decrease in average -14.71±12.31. The treatment group (p=0.002) obtained a p<0.05, which means that the treatment group experienced a significant change in the VAS scale. There was a significant difference compared to the control (p<0.05).Conclusion Musicure is effective in inhibiting perception of pain that it will lower the VAS scale,  therefore decreasing pain score.  Pain in patients who undergo bronchoscopy will be more controlled, which makes it easier to act and decrease side effects after the action.
