Abstract

The decline of olfactory function is an early indicator of neurodegenerative diseases, such as Alzheimer’s and Parkinson’s disease. An olfactory function assessment instrument using ten familiar scents with multiple-choice answers has been developed, but several items were inapplicable and had not been standardized. This study aimed to identify various scents that have not been standardized for the aging population with normal cognitive function. This research was a preliminary study with a descriptive cross-sectional approach conducted in Kalianyar village, Jakarta, involving 23 elderly participants with normal cognitive function. An instrument consisting of a list of scents was employed in two steps, with or without assistance. Initially, participants had one chance to smell and try to guess the type of scent given. If the scent was not guessed correctly, participants were then provided with multiple-choice options as a hint. Based on the assessment of twelve scents from the existing research, only nine scents were recognizable. Among them, coffee, lemongrass, oranges, eucalyptus, and menthol were the scents with the most correct answers without assistance. Additionally, there were four scents recognized with or without assistance, such as jasmine, lemon, aromatic ginger, and lime. This study found that only nine scents were familiar and had the potential to be utilized in the future as a screening tool for cognitive impairment.

Abstract

Background: The aging population is growing faster in several countries around the world, including Indonesia. Oral health literacy as a personal-behavioral component has been studied and is regarded an important determinant of health as well as a causative factor for oral health disorders, which are subsequently thought to have an impact on oral health-related quality of life. Objective: The purpose of this study was to investigate the association between oral health literacy and oral health status among elderly in Yogyakarta. Method: This cross-sectional study included 193 respondents aged ?60 years in Yogyakarta City. The data collection was done using interviewer-administered questionnaires (socio-demography data, health history, health behavior, and oral health literacy) and clinical examination. The Modified Gingival Index is used to determine gingival health status during a clinical examination of the oral cavity. Result: The results of this study indicated that there was a significant relationship between oral health literacy and Modified Gingival Index (p=0.01). Understanding participants' oral health literacy levels was critical for developing successful health education materials and intervention programs that enhance oral health. Conclusion: Elderly people with higher oral health literacy have better oral health outcomes.

Abstract

Background: Acute Respiratory Distress Syndrome (ARDS) is one of the most common complications of COVID-19 infection and is closely related with patient mortality. Increased D-dimer levels are considered one of the most consistent clinical indicators of coagulopathy. Theoretically, this event is based on the emergence of a cytokine storm triggering lung tissue damage, pulmonary microvascular endothelial damage and alveolar edema, leading to hypoxia in the body. Objective: To determine the association between ARDS incidence and the increase in D-dimer levels in COVID-19 patients at Bethesda Hospital Yogyakarta. Method: This research was conducted in retrospective cohort design. The data utilized in this study came from the medical records of male and female COVID-19 patients between 19 and 60 years old, who were diagnosed with ARDS by doctors at Bethesda Yogyakarta Hospital. The medical records were collected between June 1, 2021, and September 30, 2021. The data analysis included univariate (descriptive statistics) and bivariate (Chi-Square, Mann-Whitney, Spearman Rank) analyses. Result: There was a significant relationship between COVID-19 ARDS patients and the increased levels of D-dimer in patients undergoing hospitalization at Bethesda Hospital Yogyakarta (p<0,001, OR=4,589, 95%CI=2,104-10,007). Additionally, there was a significant difference in D-dimer levels between ARDS and non-ARDS patients. The severity of ARDS, duration of hospitalization, gender, and age did not have a significant association with the increase in D-dimer levels (p>0,05). Conclusion: There was a significant relationship between the incidence of ARDS and elevation of D-dimer levels in COVID-19 patients at Bethesda Hospital Yogyakarta.

Abstract

The COVID-19 pandemic has significantly altered various sectors, and the scientific community is no exception. It has reshaped the research landscape across numerous disciplines, profoundly impacting how medical research is conducted, prioritized, and funded. As we move into the post-pandemic world, understanding these transformations is crucial for evaluating shifts in medical research and contemplating their implications for future scientific inquiry. This editorial focuses on the pandemic-driven transformations in medical research, including methodological innovations, changes in research focus, and their implications for future research directions. The need to respond rapidly to a rapidly changing environment has accelerated the adoption of digital technologies and remote research methodologies. During lockdowns and periods of social distancing, the continuation of research increasingly relied on virtual collaboration and online data collection. This shift has highlighted how digital technologies can enhance research efficiency and brought attention to the necessity of robust frameworks to address the challenges associated with virtual interactions and remote data collection.

Abstract

Background: Coronavirus Disease-19 (COVID-19), a severe acute respiratory syndrome,  stems from a novel coronavirus known as Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2). The common symptoms and signs of COVID-19 are fever, cough, diarrhea, and myalgia as well as neurological manifestations. Neurological manifestations of COVID-19 in adult patients have been well recognized, unfortunately, there is limited evidence of neurological manifestations of COVID-19 in children, especially in Indonesia, so herewith we describe neurological manifestations in children, to contribute to the knowledge in the fields of pediatric health and neurology, particularly their correlation with COVID-19. Objective: To determine the neurological manifestations in children with COVID-19. Method: An observational analytic study among 124 pediatric patients with COVID-19 at Dr. Kariadi Hospital Semarang. Demographic and clinical data were obtained from medical records. To determine risk factors for the occurrence of neurological manifestation, the chi-square test, Fisher exact test, and Mann-Whitney U-test were performed. Results: Out of 124 subjects, 49 (39.5%) developed neurological manifestations, including headache (10%), seizure (23%), weakness (5%), dizziness (0.8%), vision impairment (0.8%), ataxia (0.8%), coma (11.3%), paresthesia (0.8%), acute encephalopathy (3.2%), sympathetic storming (0.8%), and others (9.7%). Diagnosis of Multisystem Inflammatory Syndrome in Children (MIS-C) was found in 10 (8.1%) patients, in addition, there was a significant association between the diagnosis of MIS-C, comorbid conditions, and outcomes among patients and neurological manifestations. Conclusion: The most common neurological manifestation experienced by pediatric patients with COVID-19 in this study was seizures. Patients with MIS-C and pre-existing conditions are at higher risk of experiencing neurological manifestations. Children with neurological manifestations more frequently presented with worse outcomes.

Abstract

Background: Accelerated aging is associated with increased exposure to reactive oxygen species (ROS), leading to lower collagen and elastin levels in the body. This reduction reduces microvascular activity, causing microcirculatory disorders and skin wrinkles. Administering collagen tripeptides can help improve the microcirculation system. Objective: This study aims to investigate the effects of collagen tripeptide on improving microcirculation, reducing skin wrinkles, and lowering excessive ROS toxicity. Method: The research method used is experimental with a pre-post study approach without using a control group and descriptive quantitative data analysis. We measured microcirculation using a microscope, quantified ROS levels with the QYM-DI microscope, and assessed nasolabial fold depth with an acupuncture needle. Results: The study results indicate that collagen tripeptide significantly improves microcirculation, reduces skin wrinkles, and decreases excessive ROS toxicity. The non-parametric Wilcoxon test results are as follows: (1) there is a significant difference before and after collagen tripeptide consumption in terms of microcirculation improvement (p = 0.000), reduction of skin wrinkles on the nasolabial folds (p = 0.008), and reduction of ROS (p = 0.000). This improves microcirculation, which in turn reduces skin wrinkles on the nasolabial folds. Conclusion: Consuming collagen tripeptide for three months can improve microcirculation, reduce skin wrinkles on the nasolabial folds, and reduces ROS.

Abstract

Background: Lymph node metastasis in breast cancer is a clinical parameter used to predict a patient’s prognosis. Lymph node metastasis is affected by numerous factors, one of which is hypoxic conditions that trigger the expression of HIF-1?, a transcription factor expressed by cells in hypoxia. To date, the role of HIF-1? in lymph node metastasis in breast cancer requires further research. Objective: This review aimed to describe the possible role of HIF-1? in breast cancer lymph node metastasis. Methods: PubMed and Google Scholar databases were searched for articles using Boolean operations with keywords related to HIF-1? expression and lymph node metastasis of breast cancer. We have limited the 10 years of publication to be included in this review. Results: HIF-1? has the potential to induce epithelial-mesenchymal transition, extracellular matrix invasion, matrix degradation, and lymphangiogenesis through various target gene expression and signaling pathways, leading to lymph node metastasis of breast cancer cells.

Abstract

Introduction: This study examined the disaster response system of Universitas Kristen Duta Wacana (UKDW) during the COVID-19 pandemic. As a complex institution with interconnected components, UKDW plays a vital role in disaster mitigation and response. Method: Using a qualitative research approach, this study analyzed data from focus group discussions, in-depth interviews, and questionnaires to assess university preparedness and response to the pandemic. Result: The findings revealed that UKDW's response to COVID-19 involved several key stages: pandemic identification, mitigation and preparedness, changes in learning strategies, and evaluation. While the university demonstrated adaptability and resilience, challenges, such as communication gaps and resource limitations, were identified. Conclusion: The study's recommendations include strengthening disaster response training, improving communication channels, and enhancing collaboration with external partners to enhance UKDW's preparedness for future crises.

Abstract

Background: Cataract is a disease that has become the most common cause of visual impairment in Indonesia. Its incidence in Indonesia is about 210.000 cases/year, while cataract surgery rate is only 80.000 patients/year. One of provinces in Indonesia that has the highest cataract prevalence based on survey results that has been done by Rapid Assessment of Avoidable Blindness (RAAB) in 2014-2016 is East Java. There were 371.599 blindness cases with 81.1% from all the blindness cases caused by cataract. Objective: This study described characteristics and clinical features of phacoemulsification cataract surgery patients in Pasuruan Eye Hospital in 2023. Method: This quantitative research was done by using descriptive observational design and cross-sectional approach. Variable research was taken from medical records in Pasuruan Eye Hospital. Results: There were 55 cataract surgery patients who completed all appointments with an ophthalmologist until D+30 of the surgery. Research results showed that 48 patients (87.2%) obtained good vision after undergoing cataract surgery using the phacoemulsification method. Proportion of cataract events was greater in women (54.5%) than men (45.5%). The most frequent surgical complication after phacoemulsification was corneal edema (23.6%). Conclusion Most cataract surgery results using the phacoemulsification method show good visual acuity. However, there were cataracts that occur at ages less than 50 years. This condition requires action from health services to investigate the factors that influence the occurrence of cataracts at an earlier age.

Abstract

The management of non-obstetric emergency surgeries during pregnancy presents unique challenges that necessitate careful consideration of both maternal and fetal health. This case report outlines the management of a pregnant patient (G3P2A0H2, 10-11 weeks gestation) who presented with acute abdominal pain due to a suspected ovarian cyst torsion. The patient underwent exploratory laparotomy under spinal anesthesia with bupivacaine, morphine, and fentanyl. Postoperative pain was managed using a multimodal analgesic strategy including surgical wound infiltration with 0.25% ropivacaine combined with dexamethasone. This technique was used to minimize opioid use and enhance recovery. The patient experienced stable hemodynamics and minimal postoperative pain (VAS 1-2), and was discharged on the third postoperative day. This case highlights the importance of effective pain management, particularly during non-obstetric surgeries during pregnancy. Regional anesthesia, along with local infiltration of ropivacaine and dexamethasone, provides an effective pain control strategy, contributing to faster recovery and enhanced patient satisfaction while minimizing systemic opioid exposure.
