Abstract

Besides serological cross-reactivity and co-infection between dengue and SARS-CoV-2, diagnostic challenges arise because many people have had positive dengue IgG in dengue-endemic regions, and patients usually come after 4-5 days of fever. We report a case of 9-year-old girl, diagnosed with dengue with warning signs on admission because of fever, severe abdominal pain, thrombocytopenia, and positive dengue IgG. The subsequent course of the illness was not consistent with dengue infection because the shock happened during the febrile phase concurrent with rising leucocyte and platelet counts, the fever continued for longer than seven days, and there was bilateral non-purulent conjunctivitis with subconjunctival hemorrhage. She was diagnosed with MIS-C because of positive SARS-CoV-2 IgG and hyperinflammatory markers. Her clinical condition improved progressively after steroid administration. Clinical awareness about MIS-C is required to avoid misdiagnosis and improper treatment during or after the COVID-19 pandemic. Every clinician should consider MIS-C as a differential diagnosis if patients have inconsistencies with the course of dengue illness.

Abstract

Despite progressive development of antibiotics and surgical approaches, outcomes in patients with intracranial abscess remained far from ideal. This paper aimed to describe challenges in the diagnosis and treatment of intracranial abscess in developing countries based on study from a tertiary hospital in Indonesia. A retrospective study was performed and included patients who were suspected of intracranial abscess at a tertiary referral hospital in Indonesia between January 1, 2012 and January 1, 2020. Clinical, radiological and laboratory characteristics were analyzed. Two-hundred-and-seventy-seven patients were initially enrolled. Age (p = 0.001), GCS at admission (p = 0.009), comorbidity with hypoalbuminemia (p = 0.003) and hydrocephalus (p = 0.009) are significantly associated with patients’ GCS when discharged.  Results from culture and resistance tests could only be retrieved from 66 patients, with 33 (50%) obtained negative results, while the other 33 (50%) results were positive, consisted of 20 (60.6%) were of gram-negative bacteria and 13 (39.4%) were of gram-positive bacteria. Treatments that are directed to hypoalbuminemia and hydrocephalus should also be prioritized when any of these comorbidities exist.

Abstract

Oxidative stress caused by free radicals and long-term antipsychotic use in chronic schizophrenic patients may result in liver cell damage and impaired liver function. Folic acid is an exogenous antioxidant that can boost the antioxidant defense mechanism (Glutathione Reductase), lowering free radicals and improving liver function. Determine the correlation between adjuvant folic acid and liver function in chronic schizophrenic patients. This was a placebo-controlled, double-blinded randomized control trial. The study sample included 36 patients in the control group and 36 patients in the treatment group. Subjects were patients who had suffered from schizophrenia for at least 2 years and received antipsychotics. Research subjects in the treatment group received folic acid supplementation. The intervention was conducted for 21 days during hospitalization. The level of AST (U/L) and ALT (U/L) was measured before and after supplementation. Following folic acid supplementation, AST levels were significantly improved (26.950 U/L compared to prior to treatment 33.09 U/L, p= <0.001). ALT levels after treatment were increased but there was no significant difference observed (p= 0.835). Folic acid supplementation improves AST levels significantly.

Abstract

Ejection fraction (EF) is the percentage of the difference between the end-diastolic volume and the end-systolic volume divided by the end-diastolic volume. This measurement is important in determining how well the heart is pumping out blood and in diagnosing heart failure (HF). Normally the percentage of EF is above 50%. A decrease in EF will make it difficult to perform anesthesia during surgery due to life-threatening irregular heartbeats, leading to sudden cardiac arrest and sudden death. Patients with a very low ejection fraction require identification of risk factors, preoperative evaluation and optimization, medical therapy, monitoring, and appropriate anesthetic technique and drugs. In this report, we present a 67-year-old woman with HFmrEF (estimated ejection fraction 46%) and coronary artery disease who underwent partial hip replacement surgery under general anesthesia. General anesthesia was performed on the patient and induction used fentanyl, midazolam, and atracurium. Management of patients is aimed at maintaining cardiac output and avoiding myocardial depression by maintaining a balance between oxygen supply and demand. Therefore good anesthetic management is needed to maintain the hemodynamic stability of the patient.

Abstract

Bacteria require optimum environmental conditions to support cell growth and reproduction. States that are less than optimal cause bacteria to adapt and can infect humans for invasion of other organs. The speed of bacteria to infect is influenced by the rate and time of generation of bacteria in multiplying cells. This study aims to determine the effect of variations in pH (4,8; 5,8; 6,8; 7,8; and 8,8) in Eosin Methylene Blue Agar (EMBA) media on the generation time of Escherichia coli bacteria. Researchers used the True Experiment Post Only Control Design and One Way ANOVA test. The results showed that at a variation of pH 4,8 there was no growth; pH 5,8 with a generation time of 1,5-24,9; pH 6,8 with a generation time of 1,5-21,7 minutes; pH 7,8 with a generation time of 1,6-22,3 minutes; and pH 8,8 with a generation time of 1,6-22,8 minutes. The study concludes is that there is a significant effect of the variation in pH of the EMBA media on the generation time of Escherichia coli bacteria with a p-value of 0,000 < α 0,05, with the fastest generation time at pH 6,8.

Abstract

Thyrotropin-stimulating hormone-producing adenoma (TSHoma) is a rare cause of hyperthyroidism. Patients with this condition are often diagnosed when the tumor has already grown significantly, as the diagnosis of TSHoma is frequently delayed and confused with primary hyperthyroidism. Our case describes a 60-year-old woman with a goiter who was referred to our center from the otolaryngology department. Her thyroid panel in endocrinology showed elevated free hormone levels (fT3: 4.6 mg/ml and fT4: 1.93 mg/ml) and an unsuppressed TSH: 12.85 μ mIU/l, suggesting central hyperthyroidism. A thyroid gland sample obtained by a fine needle aspiration biopsy did not show malignant cells. An MRI scan of the pituitary gland revealed a macroadenoma with KNOSP grade 1. She underwent transsphenoidal surgery for the tumor. Histopathology revealed an acidophilic adenoma with the expression of TSH and prolactin by immunostaining. The proliferation rate, as indicated by MIB-1 staining, was only 0.6%. The decrease in thyroid hormones after the operation confirmed the effectiveness of the surgery. In conclusion, TSHoma, a rare tumor associated with hyperthyroidism, requires a careful diagnosis for effective management. Our article serves as reference material for understanding cases of pituitary-secreting tumors in Indonesia.

Abstract

Dengue Hemorrhagic Fever (DHF) is an arthropod borne viral disease. The main vector of the disease are the Aedes mosquitoes. Indonesia as one of the tropical nations in South Asia falls into one of the countries in which DHF is an endemic. The notable clinical symptoms are fever usually 400c with a  biphasic pattern in 6% of DHF cases, body ache, muscles, retroorbital and joints, nausea, and vomiting. Fluid accumulation, due to plasma leakage, may occur in the abdominal cavities, pleural space, and even the lungs. The rise of non-communicable diseases like diabetes mellitus has impacted the clinical progression of DHF by increasing the risk of developing into dengue shock syndrome and atypical clinical manifestation of dengue plasma leakage and bleeding disorder. This case report discusses one of these atypical clinical manifestations of DHF. The case is an elderly female presenting in the emergency room with a DHF, Diabetes mellitus type 2, cardiomegaly, and during inpatient care developed a pulmonary Oedema.  The patient experienced prolonged fever and on the 7th day of care develops a pulmonary oedema, the patient is then moved to the intensive care unit, and with proper treatment and monitoring was able to recover from the disease

Abstract

We aimed to investigate the level of red cell distribution width (RDW), in type-2 diabetic patients with atrial fibrillation. Patients aged 18 to 80 years old who were admitted to the internal medicine and cardiology outpatient clinics were enrolled. The data of the patient were retrospectively scanned from hospital database. 105 people were included in the study as 63 patients with diabetes mellitus and atrial fibrillation and 42 patients with diabetes mellitus and no atrial fibrillation. It was found that the age factor was statistically significant in men, women, and all patients between the groups, and Hb, hba1c, and LDL were statistically significant in men and all patients. The  level of RDW is investigated  in 2 groups of diabetic patients with and without atrial fibrillation. However, there was no significant difference between the two groups.

Abstract

During the corona virus pandemic outbreak, there are a number of vaccinations approved. They are distributed and used in human beings worldwide. Aim of this study is to analyze the changes in pattern of uptake mood during different dose courses in EU /EEA population. Therefore, there was use of the following methods: The data from official European community website was collected and analyzed with the software for total number of doses distributed and total number of doses applied. Similarly, cumulative percentage of uptake of vaccine from primary course, first booster, second booster and third booster were compared. Results are showing that there is difference of around 0.5 billion doses between the doses distributed and doses used. The uptake interest decreased from primary dose to third booster dose from 85% to 0% among the countries over the time. The conclusion was found as follows: there are discrepancies between the total number of doses distributed and used. There is very strong loss of uptake interest in the populations in EU/EEA with different dose courses. One must study the reasons for such a strong loss of uptake interest and economic losses caused leading to burden on the tax payers.

Abstract

Diarrhea is the second most common disease worldwide after Acute Respiratory Infection (ARI). One of the causes of diarrhea is the gastrointestinal infection caused by disease bacteria (microbe) such as Salmonella sp., Escherichia coli, Shigella sp., and Vibrio cholerae. The earthworm species Lubricus sp can be used as an alternative medication believed to be able to cure digestive diseases such as typhus, dysentery, and other indigestion like gastritis. This research is laboratory experimental research. The antibacterial activity is measured by determining the Minimum Inhibitory Concentration (MIC) and Minimum Bactericidal Concentration (MBC) values of earthworm infusion (Lumbricus rubellus) using the tube dilution test. The bacteria used in this research are Escherichia coli, Shigella dysentriae, and Vibrio cholerae, isolated from the feces of diarrhea patients. This research aims to examine the earthworm infusion antibacterial (Lumbricus rubellus) activity to several bacteria that commonly cause diarrhea. The research results show that the average MIC value of earthworm infusion (Lumbricus rubellus) against Escherichia coli is >50%, Shigella dysentriae 0,098%, and Vibrio cholerae >50%. The average MBC value of earthworm infusion against Shigella dysentriae is 50%. .
