Abstract

Pre-frail dan frail elderly are susceptible to dysphagia. Dysphagia leads to vulnerability to aspiration pneumonia, undernutrition, and dehydration, which negatively affect physical condition and quality of life. Expiratory muscle strength training (EMST) is an intervention for dysphagia. However, the effects of EMST on pre-frail elderly are still unclear. This study aimed to investigate effectiveness of expiratory muscle training on dysphagia risk of pre-frail elderly. The study design was a randomized clinical trial. Twenty-four pre-frail elderly at Rindang Asih I and II Elderly Social Service Institution randomly assigned into experimental group (n=12) or control group (n=12). The experimental group received expiratory muscle training using PEP Threshold® device 5 times per week for 6 weeks. Both of group also received typical multicomponent exercise for elderly 3 times per week for 6 weeks. Outcome measure was risk of dysphagia assessed using Swallowing Disturbance Questionnaire (SDQ). The intra-group analysis showed significant differences pre- and post-intervention in experimental group but not in control group (p<0,001 and p=0,107, respectively). Inter-group analysis showed significant differences between experimental and control group post-intervention (p<0,001). This finding indicate that expiratory muscle training effective on reducing dysphagia risk in pre-frail elderly.

Abstract

Endometriosis is a chronic inflammatory disease characterized by the presence of endometrial tissue/lesion outside of the uterus. Unveiling pathogenesis and pathophysiology of the disease remains an elusive target to be discovered. Among many other mechanistic theories, menstrual retrograde theory is favorable in explaining endometriosis which corroborated with certain survival factors that allow endometrial cells to grow and persist. This review set out to provide an update on oocyte and embryo quality derived from women with endometriosis who undergo IVF cycles. Database searching was conducted using PUBMED and the keyword used for literature searching was endometriosis, oocyte quality, and embryo quality. As a chronic inflammation occurs in reproductive tract, endometriosis has been demonstrated to negatively impact the oocyte quality and endometrial receptivity that leads to infertility. Account for about 10-40% of women with endometriosis administered in-vitro fertilization (IVF) programs to achieve pregnancy and healthy live-birth babies. Through IVF, oocytes can be fertilized and cultured in-vitro; thus, avoiding pra-implantation embryos to grow under unfavorable in-vivo tubal and endometrium environments due to endometriosis. IVF could be a method of choice for infertility treatment owing to endometriosis. Improved quality of oocyte, embryo as well as clinical pregnancy could be attained depending on endometriosis severity.

Abstract

Soil-transmitted helminth (STH) infections most commonly infect preschool and school-age children. The prevalence of STH infection remains high, i.e., 73 million children are infected in Indonesia. The most commonly infecting worms are roundworms (Ascaris lumbricoides), whipworms (Trichuris trichiura), and hookworms (Ancylostoma duodenale and Necator americanus). STH infection is transmitted through soil contaminated by STH eggs or larvae. Personal hygiene, availability of clean water, and defecation behavior are factors in the development of STH infection. Prolonged infection can cause malnutrition, anemia, and slow growth. This study aims to examine the association between nutritional status, age, and gender on the incidence of STH infection. This research is a descriptive-analytical study with a cross-sectional approach. Fifteen (42.86%) of 35 preschool and school-age children's stool samples were positive for Ascaris lumbricoides eggs. The nutritional status of the children was grouped into underweight, normal, and overweight. The body mass index (BMI) calculation resulted in 65.71% (23/35) underweight, 31.43% (11/35) normal, and 2.86% (1/35) overweight children. The Spearmen test shows that there is no significant correlation between nutritional status (p=0.517), age (p=0.614), and gender (p=0.637) on STH infection.

Abstract

Pregnant women suffering from undiagnosed or inadequately treated depression during their pregnancy face increased postpartum risks and jeopardize the health and safety of both mother and fetus. To understand the management of depression in pregnant women at Gamping II Community Health Centre. This qualitative research employed a case study approach. Informants were selected using heterogeneous purposive sampling, comprising 9 participants: a pregnant woman, a head of the community health centre, a doctor, two midwives, a psychologist, a psychiatric nurse, and two pharmacists. The study reveals that the depression management provided to pregnant women is not comprehensive due to the lack of post-referral care, resulting in inadequate monitoring and evaluation. There is a lack of knowledge about using mental health screening tools, understanding care for depressed mothers, and implementing guidelines for managing depression in pregnant women. These factors hinder the provision of comprehensive care. Pregnancy care for women with depression requires comprehensive health management strategies. Comprehensive care is crucial to mitigate the impact of mental health disorders experienced during pregnancy

Abstract

Collodion baby with cholestasis is an uncommon condition. The term "collodion baby" refers to newborns with thick skin covering their entire body. Neonatal cholestasis is a form of conjugated cholestasis hyperbilirubinemia caused by reduced bile flow. The condition causes persistent jaundice (over two weeks) and elevated conjugated bilirubin. Genetic abnormalities or gene mutations have been reported in neonatal ichthyosis syndrome with cholestasis. A case report of a collodion baby, a rare congenital condition characterized by a parchment-like membrane covering the entire body, was brought to Dr. Soetomo General Academic Hospital. Prolonged jaundice was one of the other findings. A skin biopsy found characteristics consistent with the diagnosis of collodion in a newborn. The paediatric team managed this case, which was treated in collaboration with a dermatologist. This case showed that newborn collodion syndrome required skin care, cholestasis screening, and jaundice management from birth.

Abstract

Malaria is a major infectious disease worldwide, and vaccination is essential for disease control. The Plasmodium falciparum Erythrocyte Membrane Protein-1 (PfEMP1) is a potential malaria vaccine candidate due to its involvement in pathogenesis. Injection of DBL2β-PfEMP1 recombinant protein in animal models induces IgG and CD4+ production and inhibits binding with host endothelial receptors. This study aimed to analyze the spleen immune response by measuring the white pulp diameter. The experimental study used Wistar rats (Rattus norvegicus) that were divided into four groups, a control group and three treatment groups, which were injected with 100, 150, and 200 μg of DBL2β-PfEMP1 protein. Injection was done thrice with three-week intervals (days 0, 21, and 42). On day 56, rats were euthanized, and spleens were prepared for histology examination. The white pulp diameter increased along with increasing the dose of protein. The ANOVA test showed a significant difference between groups (p=0.001). The posthoc Bonferroni test showed a significant difference between the control and the 150 and 200 μg groups and the 100 μg and 200 μg groups. In conclusion, the DBL2β-PfEMP1 recombinant protein injection increased the spleen white pulp diameter in Wistar rats, and the 200 μg dose resulted in the highest increase.
