Abstract

ABSTRACTCollecting duct carcinoma is a rare and highly aggressive subtype of renal cell carcinoma. The incidence rate is less than 1-2% of all renal tumors and usually, affect middle-aged adult, commonly in men. We reported a 76-year-old man complains of an intermittent painless gross hematuria, abdominal mass and left flank pain for approximately three months. The CT abdomen showed a slightly enhancing solid mass in the left kidney and para-aorta lymphadenopathy. Cut surfaces of the kidney showed a solid-cystic and ill-defined greyish-white tumor. Microscopically, tumor formed solid sheets and tubulopapillary structures lined by neoplastic cells, hobnailing nuclei, abnormal mitotic, and a desmoplastic stroma with lymphoplasmacytic infiltration, and the immunochemical profile were PAX8 (+) /p63 (-). Based on these findings, the diagnosis was a collecting duct carcinoma. This tumor arising from the collecting duct of Bellini in the renal medulla, accounts for less than 1-2% of all renal masses and important to be distinguished from other tumors due to differences in prognosis and therapeutic. Histopathological examination is needed to establish the diagnosis. A case of collecting duct carcinoma that occurred in a 76-year-old man has been reported. A definitive diagnosis can only be done with a detailed histopathological examination for patient management benefits. Keywords          : renal cell carcinoma, collecting duct carcinoma, urothelial carcinoma, PAX8, p63

Abstract

ABSTRACT Background: The Mayer–Rokitansky–Kuster–Hauser (MRKH) syndrome is congenital malformation due to utero‐vaginal agenesis. For many years Dr Soetomo Hospital has been applying McIndoe technique using biomaterial amnion. Recently, in collaboration with digestive surgery, neovagina operation using sigmoid was performed. However, no data are available on the complications, anatomic, and functional results of the neovagina operation using sigmoid and amnion. Methods: MRKH patients who have performed neovagina operations from January 2011 to December 2014 were involved in this case review. The anatomical function was measured based on minimal vaginal length of more than 6 cm and width of 3 cm, while sexual performance is measured by Female Sexual Function Index (FSFI), which is above 23, and Female Sexual Distress Score Revised (FSDSR), whose score is < 11. Result: In 4 years period, there were 6 cases of MRKH underwent neovagina (1 used sigmoid and 5 used amnion) at Dr Soetomo Hospital. All of them had a satisfying anatomical and sexual function. Even though neovagina operation significantly reduces sexual distress, but it does not necessarily mean alleviate as seen by the high FSDSR score. Neovagina using amnion has faster operation time and cheaper with the same length of hospital stay than sigmoid neovagina. Sigmoid neovagina has a better vaginal length, lubrication and no need dilatation after operation, but it has higher complication risk. Conclusion: The Neovagina technique should be used at Soetomo Hospital depends on patient wishes and circumstances. For those who have strong financial support and are not willing to do manual dilatation, they should use the sigmoid neovagina, while those who are able to do manual dilatation, then the amnion neovagina is a good choice. Keywords: Neovagina, MRKH, sigmoid, amnion, anatomic function

Abstract

ABSTRACT  Synovial chondromatosis is a case that rarely found epidemiologically. It is a process which is benign in the synovial lining of joints, synovial sheaths, and bursae. It is the metaplastic process of synovium, which converts it into the cartilage and gets detached to become a loose body. Methods of this study are describing a case report of patient of Airlangga University Hospital that has synovial chondromatosis. A 38 years old woman, with a one-year history of pain, edema, and restriction of the left knee joint. Patient symptoms were insidious in onset, which gradually progressed. Decreased range of motion of her knee. The symptoms were mimicking of osteoarthritis. Considering the extensive involvement with multiple nodule masses inside the knee joint, we planned surgical management and open procedure. Total synovectomy was done, synovium and the masses were sent for histopathological examination which confirmed the diagnosis of synovial chondromatosis. In our case, the patient has clinical symptoms mimicking osteoarthritis of genu. Some study reported similar cases. Although synovium osteochondromatosis is a rare case and it should be kept as a differential diagnosis with chronic knee pain with swelling. Synovial chondromatosis is a rare case in the orthopedic patient. Diagnosis of synovial chondromatosis is often made following a thorough history, physical examination, and radiographic examination and histopathologic. In our case report, we present synovial chondromatosis in woman mimicking osteoarthritis.  Patient treated by surgical removal of the loose bodies followed by total synovectomy. Abstract

AbstractGallstones formed due to imbalance of bile components, excessive/ high level of cholesterol, nor bilirubin. Obesity was a condition where body mass index (BMI) was excess, which can cause health problems such as an increase of cholesterol, triglycerides, insulin levels, and blood pressure. There’s growing attention to cholelithiasis in Indonesia, but there wasn’t any data on the prevalence due to the limited amount of study related to cholelithiasis and BMI. This study aimed to analyze the relationship between BMI and cholelithiasis in Dr. Soetomo General Hospital. An analytical descriptive study with the case-control design was conducted, and the data were collected from medical records. The sample was 124 patients from the internal outpatient clinic Dr. Soetomo General Hospital. The data were analyzed using a t-test 2 independent sample. The ratio between female and male patients was 3:1, mean of age was 36-45 years, 45.2% had weight range from 61-70 kg, 51.2% had height range 150-159 cm, the BMI from patient with cholelithiasis specifically 45.3% on at-risk scale and 40% on obese I, the result showed p-value 0.089 (p>0.05), which means that there was no significant differences with the BMI from patients with cholelithiasis and without cholelithiasis. In conclusion, BMI could not be used as a parameter to determine the occurrence of cholelithiasis on an individual. Keywords: body mass index, cholelithiasis, gallstone

Abstract

Abstract Chronic Obstructive Pulmonary Disease (COPD) is a chronic airway inflammation with resulting progressive airflow limitation that has a high incidence, morbidity, and mortality. Roflumilast is an oral phosphodiesterase-4 inhibitor as a therapy to decrease the risk of COPD exacerbations in patients with moderate-severe COPD with a history of chronic bronchitis. Roflumilast can be given orally once daily as a single or combination drug. It can be used as COPD moderate-severe but also more beneficial as COPD mild treatment. The efficacy of Roflumilast can prevent exacerbations of repetition and can fixed lung function both in terms of FEV1 and vital capacity to force. The incidence of side effects, which are diarrhea and digestive disorders. The cost-effectiveness showed that Quality of life patient with Roflumilast is better than the group without additional. This paper aimed to review systematically Roflumilast as COPD treatment for the clinical application. Keywords                   : COPD, Roflumilast, phosphodiesterase-4 inhibitorCorrespondence          : fauna@staff.ubaya.ac.id

Abstract

ABSTRACT          Chloroquine is the first line of medicine in the treatment of malaria. Besides being antimalaria, the chloroquine also can be used as the anti-inflammation in the medicine of arthritis rheumatoid arthritis and lupus erythematosus discoid. Hydroxychloroquine sulfate is 4-aminoquinolin with hydroxylated chloroquine analog, having the same pharmacokinetic as chloroquine which is given orally in hydroxychloroquine sulfate form, processed by gastrointestinal absorption and very faster kidney elimination. The effectiveness of chloroquine and hydroxychloroquine towards COVID-19 in the in vitro experiment showed it could inhibit the duplication of the SARS-CoV-2 virus. The chloroquine function is to stop COVID-19 infection with (EC50) 1,13 μM and (CC50) larger than 100 μM. Meanwhile, the hydroxychloroquine function is to inhibit the attachment and entry of the virus into the host’s cell by enzymatic activation which is the lysosome acidification disorder and antigen presentation as the result of pH increase. Based on the clinical study, the 10 of 12 patients who have lopinavir/ritonavir therapy by virology, the chloroquine group showed RT-PCR negative on day 7, 10, and 14 in compare to lopinavir/ritonavir that showed RT-PCR negative on day 14. On the 9th day, 60% of the patients of chloroquine group showed the CT scan of Lungs image normal instead of the lopinavir/ritonavir at 25%. In the day 14 based on the CT test result, the pulmonary improvement increased twice rather than chloroquine group (Rate Ratio 2.21). It proved that the chloroquine role showed the result of the medicine has a significant effect by cleaning the virus or other clinical matters. The purpose of this literature review is to know the effectiveness quinoline class of drugs which is chloroquine and hydroxychloroquine in COVID-19 disease. Keywords: effectiveness, chloroquine, COVID-19Correspondence: dedydermawan555@gmail.com

Abstract

Abstract Goat bile has been used by some Indonesian people to treat malaria and increase their stamina. This study aimed to prove whether goat bile toxic or not in BALB/c mice and to verify the antimalarial activity of goat bile at various concentrations in mice infected with Plasmodium berghei ANKA. Acute toxicity test was performed using twenty male BALB/c mice with an average body weight of 25 grams, which were divided into four groups. Mice were given 25%, 50%, and 100% goat bile, respectively, while negative control was given distilled water. Any change in weight, odor, color, agitation, appearance, color of urine and feces, coma, and death, were recorded. A different set of mice were infected with P. berghei ANKA. This study conducted using the posttest only control group design with four treatments and five replications. A four day-treatment of goat bile was given by oral gavage to find out its effect on parasitemia level. Infected mice were divided randomly into 4 groups, where the GBNeg group as negative control was given only distilled water. The GB25, GB50, and GB100 groups were treated with 25%, 50%, and 100% goat bile, respectively. The parasitemia was observed daily on Giemsa-stained tail blood smears of each mice. No death or other sign of toxicity was found in goat bile-treated mice. Goat bile showed anti-malarial activity. The parasitemia in all goat bile treated groups was lower compared with the negative control group. The ED50 of goat bile against the growth of parasite was 48,55 %. Goat bile is a potential source of new antimalarial therapies. Further investigations are recommended to yield new anti-malarial drug candidates. Keywords                :    Goat bile, parasitemia, Plasmodium berghei ANKA, ED50Correspondence     :    heny-a@fk.unair.ac.id

Abstract

AbstractCOVID-19 (Coronavirus Disease 2019) has been started in Wuhan, China, and spread worldwide and resulting in many cases of death. COVID-19 attacks the respiratory tract acutely and infected both children and adults. The number of cases in children is less than in adults. By seeing from the clinical aspect, the COVID-19 case in children is milder. There are differences in immunology responses in children and adults where children have higher immunology response of COVID-19 than adults. Meanwhile, if the immunology response is slow in adults, it may cause them infected by COVID-19 with severe symptoms. There are some relations between immunization with immunology response to SARS-CoV-2 where children who already have BCG vaccination has lower infection rates of acute respiratory tract case. This study aims to know the difference between COVID-19 cases that infected adults and children seen in various aspects. Keywords                     : COVID-19,SARS-CoV-2, children, adult, severity, managementCorrespondence         : mohammad_husin@yahoo.com
