Abstract

Introduction: Pulmonary tuberculosis (TB) cases in East Jawa increased sharply by 73.3% in 2022, with Surabaya among the highest contributors. Nearly half of TB patients have comorbidities, which complicate therapy and may reduce medication adherence. Non-adherence to anti-tuberculosis drugs (OAT) can decrease treatment success and quality of life (QoL). This study aims to analyze the effect between OAT adherence and QoL in pulmonary TB patients with and without comorbidities.Methods: This cross-sectional analytic study was conducted at Pegirian Surabaya Health Center and Husada Prima Surabaya Hospital from September to December 2024. Seventy-two respondents were recruited and evenly divided into two groups, with thirty-six patients in each group. Adherence was measure using 5-Tepat Questionnaire and QoL was assessed using AqoL-4D instrument. Data were analyzed with Chi-Square and Mann-Whitney tests.Results: Among TB patients without comorbidities, 75% were adherent to OAT, compared to 61.1% in the group with comorbidities. There was no statistically significant difference in adherence rates between groups (p = 0.206). However, patients without comorbidities who were adherent had a significantly higher median quality of life score of 92.00 (p = 0.001), while adherent patients with comorbidities had a median score of 78.00 (p = 0.000).Conclusion: Adherence OAT treatment significantly improves the quality of life of pulmonary TB patients, particularly among those with comorbidities. These findings underscore the importance of interventions to enhance adherence as part of comprehensive TB management strategies.

Abstract

Introduction: The consumption of high fat and sugar can increase the risk of metabolic syndrome. Hylocereus polyrhizus is known to be able to lower total cholesterol levels in the blood. This study aims to determine the effect of Hylocereus polyrhizus extract on total cholesterol levels in Wistar rats with the metabolic syndrome model.Methods: This study used laboratory experimental research using a pretest and posttest with a control group design. The sample consisted of 30 Wistar rats according to the inclusion criteria which were divided into 5 groups: K1 (positive control), K2 (negative control), K3, K4, and K5 (treatment group). The treatment group was given Hylocereus polyrhizus extract with doses of 60, 120, and 180 mg/200grW. Total cholesterol levels were measured on the 8th, 36th, and 64th day. The data were analyzed using the parameterized One-way ANOVA test followed by Tukey's HSD posthoc test, paired T-test, and Pearson's correlation test.Results: The average total cholesterol after being given by Hylocereus polyrhizus extract in K3 (137 ± 2.82 mg/dL), K4 (109 ± 2.31 mg/dL), and K5 (100 ± 2.38 mg/dL) was lower than the negative control group. Data analysis showed significant differences (p<0.05) in total cholesterol levels before and after treatment in all groups. The Pearson correlation test explained that the variation dosage of Hylocereus polyrhizus extract had a very strong correlation with a positive correlation with total cholesterol levels.Conclusion: Induction of Hylocereus polyrhizus extract at doses of 60, 120, and 180 mg/200grW was able to significantly reduce total cholesterol levels.

Abstract

Introduction: Visual acuity is vital for human's daily life. Glaucoma is a disease that affects visual acuity. In Indonesia, the number of glaucoma cases is quite high and its therapy focuses on eye health indicators. Whereas, Many studies have demonstrated that the quality of life of glaucoma patients diminishes. This research aimed to determine the quality of life of primary glaucoma patients in Dr. Moewardi hospital.Method: This research was conducted using observational analytic method with cross sectional approach. Sampling process uses probability sampling with purposive sampling method. The subjects of this study were primary glaucoma patients in Dr. Moewardi hospital from July to November 2022. The sample size obtained from the formula was 38 people. Data were analyzed with independent t-test and one-way ANOVA. Result: From the data analysis of GQL-15 questionnaire's answer, it was found that there was a decrease in quality of life marked by higher GQL-15 questionnaire score in older patients, longer duration of diagnosis, partially or totally blind and no history of surgery. There was no increase in GQL-15 questionnaire result on patients with higher level of education. GQL-15 score in patients with longer duration of diagnosis (p-value = 0.011) and lower visual acuity (p-value = 0,000) was statistically significant.Conclusion: Older patient groups, patients with longer duration of diagnosis, blind patients (unilateral or bilateral), and those who had not undergone glaucoma surgery has a worse quality of life. Patients’ visual acuity and duration of diagnosis are significantly correlated with their quality of life.

Abstract

Introduction: Stunting is a significant issue both globally and nationally. In Karanganyar, an area with a high stunting risk, the performance of the stunting reduction acceleration team is crucial. The information, motivation, and behavioral skill (IMB) model can help explain improvements in individual performance. This study aims to examine the relationships between information, motivation, and behavioral skills and the work performance of the stunting reduction acceleration team in Karanganyar.Methods: This quantitative research uses a cross-sectional design and includes 213 team members selected through simple random sampling in 2023. Data were analyzed using univariate, bivariate, and multivariate methods.Results: The findings indicate that behavioral skills (p = 0.027) show significant positive correlations with work performance. However, there is no significant correlation between information (p = 0.646) and work performance, as does motivation (p = 0.757). The overall interaction model is not significant (p = 0.677).Conclusion: In summary, behavioral skills alone show a positive relationship with performance, whereas information and motivation do not. Collectively, information, motivation, and behavioral skills do not significantly impact the performance of the stunting reduction acceleration team. To enhance team performance, it is essential to prioritize training that develops behavioral skills, alongside providing structured information and implementing motivational strategies, such as rewards and recognition, to sustain team engagement.

Abstract

Introduction: Chronic stress disrupts neuroimmune homeostasis, driving neuroinflammation and neuronal damage. Despite growing interest in natural interventions, comprehensive reviews on ashwagandha’s dual neuro-immunomodulatory mechanisms remain limited. This review examines ashwagandha’s potential in mitigating chronic stress-induced neuroimmune dysregulation.Methods: A review was conducted by searching PubMed, Scopus, and Google Scholar for relevant preclinical and clinical studies published between 2015 and 2025. Studies were selected based on their focus on Ashwagandha’s effects on the hypothalamic–pituitary–adrenal (HPA) axis, cytokine expression, oxidative stress, and neuroprotection. Data were extracted and synthesized thematically.Results: Ashwagandha was found to downregulate pro-inflammatory cytokines (TNF-α, IL-6), inhibit NF-κB signaling, and enhance antioxidant enzymes such as superoxide dismutase (SOD) and catalase (CAT). Clinical and animal studies reported cortisol reduction, attenuation of microglial activation, increased brain-derived neurotrophic factor (BDNF) levels, improved T-cell proliferation and NK cell activity, and restoration of Th1/Th2 balance. These mechanisms were associated with improved mood, cognition, and stress resilience.Conclusion: Ashwagandha demonstrates multimodal neuroimmune modulation, offering a promising natural therapeutic for stress-related disorders. Standardized extracts warrant further investigation for clinical translation.

Abstract

Introduction: Vertebrobasilar artery stenosis can be extracranial or intracranial, and accounts for up to 20% of posterior circulation ischemic strokes. The gold standard for diagnosing is Digital Subtraction Angiography (DSA), an invasive diagnostic method by injecting a contrast agent into the blood vessels leading to the brain through a catheter. This study aims to determine the profile of patients with intracranial and extracranial vertebrobasilar artery stenosis who underwent DSA procedures.Methods: This descriptive research was conducted in RSUD Dr. Moewardi in patients who underwent DSA procedures for the period January 2020 to March 2022 and found vertebrobasilar artery stenosis. The study samples were grouped based on the location of the stenosis; intracranial and extracranial. The data collected included age, gender, symptoms, risk factors, degree of stenosis, and angiographic findings.Results: Of 42 subjects, 33 (78.6%) patients were male. There are 23 (54.8%) patients with intracranial vertebrobasilar stenosis and 19 (45.2%) patients in extracranial. The main symptoms included hemiparesis, vertigo, hemiesthesia, loss of consiciousness, aphasia, dysartria, and dysphagia. The risk factors were 36 (85.7%) patients with hypertension, 18 (42.9%) patients with diabetes mellitus, 18 (42.9%) patients with a history of smoking, and 13 (31%) patients with hyperlipidemia. A total of 8 patients underwent stenting/angioplasty.Conclusion: The prevalence of vertebrobasilar artery stenosis increases with age, most commonly occurring at the age of 50-59 years..The most common risk factor for vertebrobasilar artery stenosis is hypertension. Management recommendations include a combination of antiplatelet and endovascular therapy.

Abstract

Introduction: The surge in medical schools in Indonesia necessitates continuous quality improvement, particularly in curriculum and assessment. This study examines the correlation between first-year medical students' performance in basic medical science laboratory activities and their Grade Point Average (GPA).Methods: A retrospective observational study was conducted using secondary data from a new Medical School in Bekasi, Indonesia. student performance was reflected through the average final laboratory scores in the first year, while the GPA encompasses both medical and non-medical courses, totaling 21 credit hours. Data were collected from the university's information system database. Pearson's correlation test was used for normally distributed data, while Spearman's test was applied for non-normally distributed data.Results: The study included 27 first-year medical students with complete laboratory activity scores and GPA from the first two semesters. Significant positive correlations were found between laboratory performance in all subjects and GPA: Anatomy (R = 0.789, p < 0.0001), Histology (R = 0.7810, p < 0.0001), Microbiology (R = 0.6781, p = 0.0001), and Anatomical Pathology (R = 0.7024, p < 0.0001). These results highlight a strong correlation between laboratory performance and GPA in new medical school in Bekasi.Conclusion: This study emphasizes the importance of laboratory-based experiential learning in enhancing academic achievement among first-year medical students. Despite some limitations of this study, future research should explore the additional factors and include more medical schools to better understand the impact of laboratory performance on academic success.

Abstract

Introduction: The secretome of human dermal fibroblasts consists of diverse bioactive molecules including proteins and extracellular vesicles (EVs), playing a key role in skin regeneration and homeostasis. Molecular weight-based fractionation provides insights into the functional heterogeneity of the conditioned medium (CM). This study aimed to characterize size-fractionated CM from fibroblasts to reveal its molecular properties and potential bioactivity.Methods: A descriptive laboratory study was conducted using conditioned medium from passage 5 human dermal fibroblasts cultured in serum-free conditions. Tangential flow filtration (TFF) was applied to fractionate the CM into low (LMW), medium (MMW), and high molecular weight (HMW) components. Nanoparticle tracking analysis (NTA) quantified EV particle concentration and size, while total protein content was assessed by BCA assay. Experiments were performed in triplicate (n=3).Results: Particle concentrations increased with molecular weight, with HMW fractions showing the highest counts but slightly smaller average particle sizes compared to LMW and MMW fractions. Total protein concentration also rose with molecular weight, peaking in HMW fractions, indicating enrichment of larger protein complexes and vesicles. Fractionation maintained consistent particle size distribution across fractions, supporting effective separation.Conclusion: Fibroblast secretome exhibits heterogeneous EV and protein populations distinguishable by molecular weight fractionation. The enriched higher molecular weight fractions contain more EVs and proteins, suggesting distinct biological roles. Combining TFF with further purification steps may enhance separation purity, facilitating therapeutic applications of secretome components.

Abstract

Introduction: World Health Organization (WHO) determines Listeria monocytogenes as one of the four major bacteria that cause foodborne disease. During L. monocytogenes infection, TNF-α with IFN-γ activates macrophages for the inflammatory process. In the middle of infection, NK cells produce IL-10 to prevent excessive inflammation. Andrographis paniculata (sambiloto in Indonesia) has been known as a traditional medicine for generations. It has been previously known that administration of A. paniculata to mice with rheumatoid arthritis showed an increase in IL-10 cytokine activity in spleen samples. In addition, it also downregulates the production of inflammatory mediators such as Nitric Oxide (NO) and TNF-α. The extracts are converted into nano-sized particles to be easily absorbed by cells, thus allowing the delivery of active substances to the targeted location. This study aims to determine A. paniculata's effect on IL-10 and TNF-α cytokines in rats infected with L. monocytogenes.Methods: This study used 30 male Wistar rats infected with L. monocytogenes (except in the normal group). The animals were divided into 6 groups, i.e. Normal group, Negative Control group (K-), EAP200, nEAP100, nEAP200, and nEAP400. Crude extract of A. paniculata 200 mg/kg BW, nanoparticles of A. paniculata extract 100 mg/kg BW, 200 mg/kg BW, and 400 mg/kg BW were administered for 7 days to the EAP200, nEAP100, nEAP200, and nEAP400 groups, respectively.Results: The IL-10 and TNF-α levels in EAP200, nEAP100, nEAP200, and nEAP400 were significantly lower than the K- group. The lowest levels of IL-10 and TNF-α were in the nEAP400 group.Conclusion: Administration of A. paniculata extract nanoparticles decreased IL-10 and TNF-α levels in infected Wistar rats. The lowest levels of IL-10 and TNF-α were found in the nanoparticle group which received 400 mg/kg BW.

Abstract

Introduction: The obesity pandemic is a source of problems in both developed countries. The obesity is associated with complications before and during pregnancy and even infant death. The study aims to find out the issue of obesity among women of reproductive age, particularly concerning its impact on maternal and fetal outcomes during pregnancy.Methods: This is a cross-sectional observational study using secondary data with medical records taken at Sebelas Maret University Hospital (UNS Hospital). Subjects were obese pregnant women who gave birth at UNS Hospital. Data from 284 single births were added maternal parameters, obstetric parameters, and newborn parameters were taken and then analyzed univariately, and bivariate analysis was carried out using the Kruskall-Wallis test and the Spearman test for comparison and correlation of each variable.Results: This study finds lower incidences of postdate pregnancy and instrumental deliveries among obese mothers, but a higher rate of emergency cesarean sections. Newborn anthropometric measurements increase with maternal BMI, indicating a direct association between maternal obesity and newborn health (p = 0.001), while APGAR scores show no significant differences among maternal BMI groups. The findings underscore the importance of addressing maternal obesity to improve both maternal and newborn health outcomes.Conclusion: The research indicates a correlation between maternal BMI, maternal obstetric characteristics, newborn anthropometric measurements, and APGAR scores. This suggests that interventions aimed at promoting healthy maternal BMI, beginning before conception, continuing throughout pregnancy, and extending into the post-pregnancy period, could potentially improve newborn health outcomes and give long-term health benefits.
