Abstract

Background: Women's involvement in decision-making related to health services has a significant impact on improving maternal and child health, as well as strengthening women's empowerment. In 2022, the Maternal Mortality Rate (MMR) in Pasaman Barat District was 195 per 100,000 live births, ranking first among districts and cities in West Sumatra Province. Objective: This study aims to identify factors associated with women's physical independence in accessing maternal and child health (MCH) services. Methods: This is an analytical study using a cross-sectional design. The sample size was 106 participants. Results: 3.2% of respondents had low independence in utilizing MCH services. Residential area (p-value = 0.038; AOR = 0.321) and knowledge about MCH (p-value = 0.001; AOR = 7.015) were the dominant factors associated with women's physical independence in utilizing MCH services. Meanwhile, husband's education, number of family members, mother's occupation, age group, mother's education, husband's occupation, family income, and access to health facilities were confounding variables in this study. Conclusion: The low level of women's autonomy in utilizing KIA services is primarily influenced by the area of residence and the level of knowledge about KIA. Educational efforts, training of health workers, and cross-sectoral collaboration are needed to improve access and gender equality in health services.

Abstract

Background: Postpartum mothers experience a drop in beta-endorphin levels on day 4. Beta-endorphins play an important role in analgesics, milk production, and preventing postpartum depression. Objective: This study aimed to determine the effectiveness of core stability exercise on the level of beta- endorphin hormones in postpartum mothers. Methods: The research design used a pretest and posttest controlled group design. Involving 42 respondents, namely postpartum mothers at Halmahera and Ngesrep Health Centers, Semarang. Respondents were divided into 2 groups equally by simple random sampling, namely: experimental group (core stability exercise) and control group (postpartum exercise). Then the effect before and after the intervention on beta-endorphin hormone levels will be seen by controlling anxiety scores. Analysis using independent T test and MANCOVA.Results: The mean increase in beta-endorphin hormone level in the experimental group was 140 ng/ml, while in the control group it was 43.1 ng/ml. Core stability exercise is more effective for increasing beta-endorphin hormone levels after controlling anxiety compared to postpartum exercise with a p value of 0.023.Conclusion: Core stability exercise is effective in increasing beta-endorphin hormone levels in postpartum mothers. This research serves as a guideline for midwives to apply core stability exercise when providing midwifery care to postpartum mothers.

Abstract

Background: Aging causes a decline in physiological functions that affect balance, increases the risk of falling, and triggers fear of falling in older adults. The Otago Exercise Program (OEP) is designed to improve balance and reduce fear of falling, although its effectiveness is still being studied. Objective: The study aims to evaluate the effectiveness of the OEP in improving balance and reducing fear of falling in older adults through a systematic literature review. Methods: This study used a systematic literature review method following the PRISMA guidelines and the PICO framework. Literature searches were conducted across six major databases using the keywords “Otago Exercise Program,” “balance,” “fear of falling,” and “older adults.” A total of 1,289 articles were identified, and 1,262 articles were screened after duplicates were removed. Of these, 1,245 articles were excluded for not meeting the inclusion criteria. Twelve articles were analyzed using the Critical Appraisal Skills Programme (CASP) 2024 instrument. Results: The 12 selected studies originated from various countries, including India, Switzerland, Greece, Turkey, Israel, China, Spain, Cyprus, and Iran. The results showed that the OEP consistently improved both static and dynamic balance. Additionally, most studies reported a significant reduction in fear of falling. Conclusion: OEP has been proven effective in improving balance and reducing fear of falling in the elderly. Further research with consistent duration and long-term monitoring is needed to enhance the generalizability of the results.

Abstract

Background: Povidone iodine is widely used in wound healing, but it can cause irritation and cellular toxicity. Clitoria ternatea L. has the potential to contain compounds such as rutin, quercetin 3-O-rhamnoside, ternatin, and anthocyanins as natural alternatives in wound healing. In this study, the extract used contains anthocyanins. Objective: The study aimed to evaluate the effects of Clitoria ternatea L. extract cream on the expression of Interleukin-6, Epidermal Growth Factor, and re-epithelialization in Sprague Dawley rat incision wounds. Methods: An in-vivo study with a post-test only control design was conducted on five treatment groups, including a control group without wounds (healthy), povidone iodine, and Clitoria ternatea L. extract cream (5%, 10%, 15%). IL-6 and EGF were analyzed using immunohistochemistry, while re-epithelialization was assessed using hematoxylin-eosin staining. Results: Clitoria ternatea L. extract increased IL-6 and EGF expression, with the highest expression at a 10% concentration. Re-epithelialization was faster in the treatment groups compared to both the positive and negative controls. Conclusion: The 10% Clitoria ternatea L. extract cream demonstrated efficacy comparable to povidone iodine in incision wound healing.

Abstract

Background: One in five Indonesian elderly was frail. Multicomponent exercise is one of the recommended strategies for frailty management in the elderly. However, it would be difficult for the elders to practice the exercise without proper exercise guidelines. In this study, a Multicomponent Exercise Program (MEP) module has been developed and implemented as exercise guidelines, but its effectiveness to improve physical performance of frail elderly has not been tested. Objective: The study aimed to determine the effectiveness of the MEP in the module to improve frailty, hemodynamic parameters, and balance in pre-frail and frail elderly. Methods: Quasi-experimental method was used in the study involving 17 participants (73.47 + 8.43 years). The MEP was conducted 3 times per week for 12 weeks, consisted of flexibility, balance, aerobic, and strength training. Categorical data were analyzed using McNemar's test while analysis of continuous data using paired t-test or Wilcoxon Signed Rank, α = 0.05.Results: The MEP decreased frailty scores, systolic, diastolic blood pressure, and mean arterial pressure (p < 0.000), and improved balance (p = 0.023) in the elderly. Furthermore, the program reduced the frailty status, as evidenced by 41.18% of frail participants becoming pre-frail and 5.88% participants shifting from pre-frail to robustness. Conclusion: The MEP in the module was effective to improve frailty, hemodynamic parameters, and balance in the elderly. However, this needs to be confirmed with larger samples and better research designs.
