Abstract

Background: Nursing care provided to patients, especially in hospitals, is expected to be of high quality, and its implementation is in accordance with the nurse's clinical competence. Objectives: This scoping review aimed to assess the implementation of the clinical competence of nurses in hospitals so that a clear picture of the implementation of these competencies is obtained. In addition, this study can be evidence-based and related to implementing these clinical competencies. Methods: This literature review used PICO framework to develop articles questions (P: nurses, I: not applicable, C: not applicable, and O: application of clinical nursing competency). Articles search for literature review was conducted during April and May 2023. This study used Pubmed and ScienceDirect Databases and other sources. The keywords that used in this scoping review was “nursing competency” OR “clinical nursing competency” AND “evaluation” OR “implementation” AND “hospital”.Results: There were 1,150 articles from the search results, but only seven were included for review. One article is a descriptive comparative study using a qualitative approach, and six articles are cross-sectional studies. The articles are publications within five years (2019-2023). In its implementation, the average clinical competency of nurses is at a low to moderate level, and only one study found was at high nurse competency. The results of the supervisor's clinical competency assessment were significantly lower than those assessed by oneself and the group. In addition, the nurse competency level was moderate, and there was no significant difference in the clinical competency of nurses before and during the COVID-19 pandemic.Conclusions: The implementation of nurses' clinical competence has yet to be optimal, so hospital management needs to improve nurses' clinical competence so that nurses can provide higher quality and professional nursing care in the hospital.

Abstract

Background : Nausea and vomiting in first trimester pregnant women can cause symptoms of prenatal distress and stress which will affect a mother’s quality of life. Nausea and vomiting will also affect the nutritional intake of the mother and fetus. Nutritional needs that are not fulfilled from the beginning of pregnancy can cause fetal growth restriction which can cause short and long term complications and have a negative impact on quality of life.Objectives : This research aims to test the combination aromatherapy product Pepperlav (Peppermint and Lavender) in pregnant women to relieve nausea and vomiting.Methods : This study pretest-posttest control group design method with a total sample of 40 first trimester pregnant women who experienced nausea and vomiting. This research was conducted at the Midwife Independent Practice in the Bantul Region. Data collection was carried out by interviewing and filling out questionnaires that measured nausea and vomiting before and after the intervention. The questionnaire used is a valid and reliable RINVR. Intervention were carried out by administering Pepperlav Aromatherapy Combinations with formulations FIB (40:50:10), F2B (30:60:10), F3B (20:70:10) with VCO based notes that have fulfilled the physical stability, homogeneity, measurement PH, spreadability and adhesion tests, product safety tests and preference tests conducted on women who are not pregnant. Data were analyzed using the Paired t Test. Results : Based on statistical tests with Paired t test analysis p value: 0.001, this shows that the p value <0.05, there is a significant effect on the administration of pepperlav aromatherapy in Formula 1 formulation (a combination of 40% peppermint, 50% Lavender and 10% VCO. Conclusions : Aromatherapy combination of Pepperlav (peppermint and lavender) can relieve nausea and vomiting in pregnant women.

Abstract

Background: Environment consisting of  residential density, house ventilation, room humidity, and lighting, is regarded as one of the factors causing the transmission of pulmonary tuberculosis (TB). The current research aimed to figure out the environmental factors determining the occurrence of pulmonary TB in Surabaya City. Objectives: the purpose of the current study was to figure out the environmental factor determining the incidents of pulmonary tuberculosis (TB) in Surabaya CityMethods : This was observational research carried out by a cross-sectional approach. People with pulmonary TB registered for treatment at the Perak Timur Community Health Center and Jagir Community Health Center in Surabaya City were recruited as the research population. Meanwhile, the research samples were patients with pulmonary TB in both Community Health Centers. Simple random sampling was utilized to collect 130 patients as respondents. The data obtained were then analyzed using univariate and bivariate logistic regression.Results: The bivariate statistical test on the four research variables showed that residential density had OR=0.671, house ventilation had OR=2.874, room humidity had OR=1.231, and house lighting had OR=1.170. Fourth, environmental factors are not the cause of TB disease, because the results of statistical tests show that the OR value is above 0.5Conclusions: It can be concluded that residential density, lighting, and house ventilation did not influence the occurrence of pulmonary tuberculosis. It was more likely that it was caused by factors which do not come from the living environment. Suggestions for future research need to be an air swab examination around the residence of TB patients

Abstract

Background: Inappropriate practice of feeding and complementary foods is one of the causes of malnutrition in children in Indonesia. Providing additional food such as Modisco (Modified Dietetic Skimmed Milk and Coconut Oil) can be an effort to improve the nutritional status of toddlers, besides acupressure therapy can increase appetite so that it can increase the child's weight.Objective: This study aims to determine the difference in the effectiveness of the combination of Modisco and acupressure with only Modisco on increasing toddler’s  weight.Methods: This study used a Quasy Experiment group design with a Pre and Post Test Control Group Design approach. The inclusion criteria were children aged 1-5 years, included in the thin category and did not have heart, liver, and kidney disease. The sample was selected by purposive sampling as many as 30 toddlers who were divided into 2 groups with the provision that group I amounted to 15 toddlers were given complementary foods made from modisco II for 14 days and acupressure 6 times (2 times a week for 3 weeks), while group II amounted to 15 toddlers are only given complementary foods made from modisco II for 14 days. Statistical test using Paired sample T Test.Results: In group I, there was a difference in the average weight of children under five before and after the intervention with p value = 0.001 (p < 0.005), meaning that the combination of modisco and acupressure can increase body weight in toddlers. In group II, there was a difference in the average weight of children under five before and after the intervention with a p value = 0.003 (p < 0.005), which means that modisco can increase body weight in toddlers.Conclusion: From the results of the difference in the average weight of group I and group II with a value of p = 0.013, it means that there is no significant difference between the groups given the combination of modisco plus acupressure and the modisco group on the weight gain of toddlers.

Abstract

Background: The agenda for strengthening the global health architecture is to improve quality of life for premature infants. Premature births increase the morbidity and mortality of newborns, necessitating innovation in primary health care. Objectives: This study was conducted to examine innovation programs with a continuity of care approach in primary health care for premature infants in an effort to strengthen the global health architecture. Methods: A literature review method was used to analyze articles obtained using databases such as Science Direct and Google Scholar. The researcher used the key words “continuity of care” and “premature infants”. The inclusion criteria were full-text journal articles published from January 2020-December 2022 in English-language journals. Results: This study obtained ten articles that correspond to the continuity of care approach for premature infants. The articles included four qualitative studies, three quantitative studies, one mixed-method study, one literature review, and one systematic review. There are three themes related to the continuity of care for premature infants, including (1) Primary healthcare with information continuity, relationships continuity, and management continuity; (2) Continuity of care in the antenatal, natal, and postnatal periods; (3) The continuity of care on premature infant approach improves the outcome of premature infant care, the ability of parents, and health service quality. Conclusions: A continuity of care approach can increase the achievement of adequacy, equity, efficiency, and effectiveness in primary health care for premature infants. Joint synergy between the community, health services, and health policymakers is essential in efforts to strengthen the quality of life for premature infants.

Abstract

Background: Chronic low back pain is defined as pain that persists for 12 weeks or more and is a major health problem worldwide and is associated with high medical costs, decreased productivity, and disability. The use of analgesics with nonsteroidal anti-inflammatory drugs or opioids is one of the most common strategies to manage CLBP, but is not free from various adverse side effects. The use of other effective interventions to control pain including non-opioid medications, topical treatments, physical modalities, and complementary and alternative therapies is needed to reduce the side effects resulting from the use of pharmacological therapies. Auricular Point Acupressure is one of the complementary strategies of Traditional Chinese Medicine (TCM) that is widely used clinically to treat pain. APA may be useful for acute or chronic pain, either alone or in combination with other treatments.Objectives: To analyze the effectiveness of Auricular Point Acupressure for chronic low back pain.Methods: Literature review through online databases namely: Google Scholar, PubMed, EBSCOhost, and Scopus on articles with free full-text criteria, using Indonesian or English, publication in the last 10 years (2013-2022), and research designs of clinical trials, randomized controlled trials, systematic reviews, meta- analysis. The article search used the keywords (low back pain OR low back pain OR back pain) AND (auriculotherapy OR auricular therapy OR auricular acupressure OR ear acupressure) AND (pain OR disability).Results: 50 articles were found with 5 articles meeting the inclusion criteria. Of the five articles, the majority explained that the use of Auricular Acupressure is relatively safe and effective for reducing chronic low back pain, some articles showed a decrease in disability rates and improved sleep quality.Conclusions: Auricular Point Acupressure can be considered a relatively safe and effective complementary therapy fopr reducing chronic low back pain.

Abstract

Background : Errors in the calculation of drug titration are one of the causes of medical errors that have an impact on the implementation of cardiopulmonary resuscitation in children. Calculations based on age, child weight and incorrect anthropometric data are what cause medical errors in the administration of emergency drugs in cases of pulmonary cardio resuscitation. One strategy in utilizing technological advances to minimize the occurrence of medical errors in drug administration with the titration method is to use a dose calculation application for drug titration.Objectives : The purpose of this literature review is to analyze the effect of using drug administration calculation applications for the prevention of medical errors in emergency cases and pediatric cardiopulmonary resuscitation. Methods : This research method is a literature review using PRISMA analysis with a database of PUBMED, Science Direct, Proquest, Springer, Wiley, Taylor ,and France from 2017-2022. Results :The use of mobile apps for drug administration or titration in cases of cardiovascular emergency children has proven to be effective and efficient because it can speed up the preparation time with dose accuracy to minimize ischemia assembled hypotension to vital organs and improve the hemodynamic status and maintain neurological status. Conclusions: Mobile applications is effective and efficient in preventing medical errors in drug administration with the titration method in cases of pediatric cardiopulmonary resuscitation.

Abstract

Background: In our setting majority of the mothers were discharged on postnatal day 3.but over the past few years the NICU admissions also increased prolonging the hospital stay of mothers leading to increase in the hospital acquired infections for both mother and baby. This study was developed in this context. Objectives: The present study was performed with the purpose of establishing the incidence of neonatal jaundice in term neonates and the role of the various maternal factors being responsible for exaggerated physiological hyperbilirubinemia in term neonates. Methods: Using the NICE gestational age-based phototherapy chart, this study analysed serum bilirubin of the term newborns on their day 2 of life. Total 155 antenatal mothers were taken for study. Associations between maternal risk factors and neonatal jaundice and comparing risk factors in early-term, term, and late-term newborns were assessed using chi-squared or Fisher’s exact testsResults: Incidence of neonatal jaundice is 58.1% in this study. Age, BMI, weight gain of the mother had no significant correlation with neonatal jaundice. Blood group incompatibility had a positive corelation (P=.001) Gestational age at the time of delivery determines the neonatal outcome. (P=.001). As gestation advances the neonatal outcomes are better. Hyperbilirubinemia incidence more at 37 -38 weeks when compared to 39 weeks. Maternal comorbidities, amniotic fluid volume changes, mode of delivery didn’t have statistical significance. when new-born factors are considered birth weight, gender growth problems, blood group has no statistical significance whereas dehydration proved to be significant (P=.001). meconium-stained amniotic fluid[P-0.034] were found to be associated with the development of neonatal jaundice.Conclusions: The prevalence of jaundice is higher in this study. maternal and foetal blood group incompatibility, gestational age at delivery, meconium-stained amniotic fluid, neonatal dehydration was found to be associated with the development of neonatal jaundice.

Abstract

Background: Exclusive breastfeeding coverage in Lampung Province in 2020 was 70.1%, yet it still did not meet the expected target. Many factors influenced the success of exclusive breastfeeding. In providing breast milk, the father played a major role for the success. Father's knowledge and support in breastfeeding issues were the first thing the fathers needed to have to support breastfeeding practices. The fathers' support for the breastfeeding process was still lacking as society believed that breastfeeding was only the mother's duty. The support that a father could provide as a Breastfeeding Father could be in the form of helping with child care, helping with household chores, and providing food or other needs related to the breastfeeding process. Preliminary studies showed a lack of support from fathers for breastfeeding mothers at the Batanghari Lampung Community Health CenterObjective: The purpose of this study was to determine the relationship between the knowledge and the father's support as a breastfeeding father towards exclusive breastfeeding.Method: This research was conducted in the Work Area of the Batanghari Inpatient Health Center, East Lampung in January - February 2023. The sample used was the fathers of infants aged 6-12 months in the Work Area of the Batanghari Inpatient Health Center, East Lampung, as many as 63 respondents obtained using accidental sampling technique . This research was a quantitative study using the Cross Sectional Study approach, analyzed by using the chi square test (α=0.05) for bivariate analysis.Results: The results of this study indicates that there was a real relationship between the father's knowledge as a breastfeeding father and the father's support as a breastfeeding father on exclusive breastfeeding in the Work Area of the Batanghari Inpatient Health Center, East Lampung with p-values of 0.038 and 0.010.Conclusions: There is a significant relationship between father's knowledge and the father's support as a breastfeeding father and exclusive breastfeeding in the Batanghari Inpatient Health Center, East Lampung working area.

Abstract

Background: One of the government programs that focuses on the first 1000 days of life is exclusive breastfeeding for infants aged 0-6 months, but there is often a failure of exclusive breastfeeding at the age of 0-3 days due to pre-lactal feeding, so education during pregnancy is needed to improve the knowledge and attitudes of pregnant women through KEKASIH (Small Group of Cadres with Pregnant Women) classes. Objectives: This study aims to determine the effectiveness of KEKASIH classes on knowledge and attitudes of pregnant women trimester III in remote areas of Enggano Island, Bengkulu Province in 2023.Methods: Quasi-experimental research design with pre- and post-test and control group. Intervention in the form of KEKASIH class, which is a modified class for pregnant women divided into small groups with cadres as facilitators. Independent variables were KEKASIH class, dependent variables were knowledge and attitude. External variables are age, parity, education, distance of pregnancy, employment. The sample was trimester III pregnant women a total of 50 people consisting of the treatrimesterent group is pregnant women in KEKASIH class, the control group is pregnant women who receive conventional health promotion.Results:The results of the study showed that there was a difference in the mean knowledge score before and after exclusive breastfeeding education through KEKASIH class from 27.36 to 49.04, p=0.00. There is no difference in the mean attitude score before and after exclusive breastfeeding education in the control group from 28.16 to 28.96 p=0.00.Conclusions: KEKASIH Class is more effective in improving mothers' knowledge and attitudes about exclusive breastfeeding with a value of p=00. Educational factors affect mothers' knowledge, but gestational distance and parity affect mothers' attitudes about exclusive breastfeeding.
