Abstract

Analysis on Pharmaceutical Management Information System (PMIS) application should be conducted for measuring or estimating the potential benefit purposes, observing potential problems that organization and its members might be faced, especially on pharmaceutical inventory management of a hospital. The results of the analysis may be used as the references in order to improve the existing potentials, so the PMIS will be able to carrying on the objective, vision and mission(s) of an organization. The HOT-Fit analysis model plays important role within information system namely Human, Organization, and Technology and harmonization among them in order to create Net Benefit from PMIS implementation. This research was conducted in Pharmaceutical Departement of Dr. Yap Eye Hospital Yogyakarta. This research was conducted within non-experimental quantitative- qualitative approach and applied case study framework. Questionnaires were analyzed by applying descriptive statistic methodology, Pearson correlation analysis, regression, multivariate statistic, and path analysis. Meanwhile, qualitative data were analyzed in order to establish the resume. The results of research showed that application of information system had not fulfilled the user expectation yet. On other side, harmonization among user-organization- technology really needs improvement to happen. There were strong correlations between Human-Technology and Organization-Technology. Human and Technology simultaneously affected the Net Benefit by 88,7%. Human and Organization simultaneously affected the Net Benefit by 87,8%. Organization- Technology simultaneously affected the Net Benefit by 75,3%. The most affective variable toward PMIS successful implementation were Human and Technology which means that certain improvement in the quality of technology & users ability would improve the probability to success of PMIS implementation in Dr. Yap Eye Hospital Yogyakarta. Keywords : Information system analysis, HOT-Fit, Dr. Yap Eye Hospital Yogyakarta

Abstract

The implementation of INA-DRG for Jamkesmas Class III patient at the hospital was expected to be able to increase the access and quality of health service to the poor in order to realize the optimal health service effectively and efficiently. The aims of this research were to analyze the medical cost of heart failure patients, to investigate the affect of patient’s factors, comorbidities, severity level and the duration of treatment on the cost of heart failure treatment and to investigate the difference of average cost for the heart failure patient’s treatment based on the rate of RSUD Dr. Moewardi with the rate of INA-DRG package. This research was descriptive according to the hospital perspective. The data collection method conducted retrospectively. The research subject was the patients who were hospitalized in class 3 and the member of Jamkesmas at RSUD Dr. Moewardi at period of 2009 who suffered from heart failure and fulfilled the inclusive requirement. The data analysis was done with cross tab and linear regression, the cost average comparison and LOS from INA-DRGs package. The p values of statistic result test among sex, age, number of comorbid, severity level and LOS on the medical cost were 0.851, 0.960, 0.694, 0,0621 and 0.000. The average of heart failure medical cost based on RSUD Dr. Moewardi rate on the level of severity I was Rp. 1,870,231 ± Rp.1,506,661, while on the level of severity II was Rp. 2,382,737 ± Rp. 2,132,236 and on the level of severity III was Rp. 2,513,826 ± Rp. 1,664,057. Real ALOS on the level of severity I was 6.92 days, while on the level of severity II was 7.35 days and on the level of severity III was 8.10 days. The research result shown that there was no relation between the patient’s character, number of comorbid and severity level on the hospital medical cost, while the treatment duration had corelation with the medical cost. The average of heart failure treatment cost on the level of severity I, II, and III was lower compared to INA-DRG package rate. It shown that the hospital succeeds to use medical facilities efficiently so that the tarrif will not surpass it of INA – DRG, while the difference of real ALOS and ALOS INA-DRG was the highest on the level of severity III. It has showed that the hospital has done efficiency and has been able to decrease the duration of the heart failure inpatient treatment at the hospital.Key words: INA-DRG, heart failure, Jamkesmas

Abstract

The implementation of INA-DRGs system for the third-class patients of Jamkesmas (Public Health Insurance Program) in hospital is expectedly able to increase the access and the health service quality for the poor and underprivileged people in effectively and efficiently realizing the optimal health service. This research was aimed to obtain the average cost of medical treatment of cervical cancer and also learn the compatibility between cost of medical treatment of cervical cancer in RSUD Dr. Moewardi and the constancy of INA-DRGs based health financing. This was a descriptive research under the perspective of hospital. Data was collected retrospectively by investigating the medical record document of patients with cervical cancer and the data of the medical treatment cost of patients. The research subject was cervical cancer case with the third-class patients of JAMKESMAS in RSUD Dr. Moewardi in the period of January-July 2010 that was in line with the inclusion criteria. Further, the data analysis was done by cross tab with Chi Square test and by comparing the average cost and ALOS (Average length of stay) through the constancy of INA-DRG. The Chi Square Test shows a correlation between LOS, patient factor (the number of parities), and disease factors (co-morbidity and severity level) with the health care cost for cervical cancer in RSUD. Dr. Moewardi. The health care average cost of cervical cancer based on the tariff of RSUD Dr. Moewardi for the severity level I included: IDR2.893.243 for surgery, IDR3.313.342 for chemotherapy, IDR3.988.027 for radiotherapy and IDR10.039.072 for radical hysterectomy. Meanwhile, in the severity level II, it included IDR5.885.859 for surgery, IDR4.474.084 for chemotherapy. In the severity level III, it includes IDR13.347.209 for radical hysterectomy. The ALOS was longer compared to ALOS of INA-DRG except in the severity level III. This then gives an effect on the health care cost for cervical cancer that in general was higher than the tariff of INA-DRG. In conclusion, implementation of INA-DRG for the treatment of cervical cancer in patients of JAMKESMAS in RSUD Dr. Moewardi was in efficient. Keywords: INA-DRG, cervical cancer, JAMKESMAS

Abstract

Diabetes is a chronic disease that can significantly increase a person›s risk of having complications. These complications not only cause the inability of the body that will affect the quality of life, but also need big cost. This research was conducted to analyze the costs and effectiveness of therapies of outpatient with type 2 diabetes mellitus. A crosssectional observational method was performed on 100 patient who were admitted to the Department of Endocrinology of the Sleman Hospital between December 2010 to January 2011. Data were taken in 2 ways: concurrent and retrospective, with accidental sampling technique. The subject of this research were outpatient with type 2 diabetes mellitus at RSUD Sleman Yogyakarta, who have used the antidiabetic therapy for at least three months of therapy before the quality of life measurement. The costs that are measured are direct medical costs, antidiabetic costs, complication costs, laboratory examination costs, and administration costs. The effectiveness of treatment parameters was measured based on the percentage of patients with fasting blood glucose level (GDP) and post-prandial blood glucose (GDPP) which reached the target in the third month of therapy, and achievement of the quality of life research subjects. The Lowest total cost of therapy was the use of glibenclamide, it was 49.730,00, the highest total cost of therapy was the use of insulin, it was Rp. 411.045,50. T test results with 95% confidence level indicates that the patient characteristics and medication adherence complications affect the patients quality of life, whereas non-pharmacological therapy (exercise and diet) did not significantly influence the patients quality of life. The results of measurements of quality of life showed metformin achieved the best quality of life than the other antidiabetic, i.e. 72.53. Based on the effectiveness of the three parameters (measurement of quality of life and levels of GDP and GDPP) note that metformin therapy tend to be more effective compared with other antidiabetic therapies.Key words: Diabetes mellitus type 2, cost analysis, effectiveness, antidiabetic, RSUD Sleman Yogyakarta.
