Abstract

Drugs supply management aims to control the drugs supply and make sure that the drugs in good condition, fairly distributed, and fulfill the types and quantity of the drugs needed by health service. Health department of Cilacap District faced many obstacles in drugs supply management during 2008-2010. Therefore, evaluation of drugs supply management in health department of Kabupaten Cilacap during 2008-2010 certainly needed to make the better drugs supply. The research conducted with observational method using descriptive monitoring. Data were collected retrospectively using data of planning, drugs supply and storage, and drugs consumption in health department of Cilacap District. Indicators that used in the research were indicators that determined by health Department of Indonesia in 2006. Primary data were gathered through interview with 3 groups: policy group, drugs supply group, and drugs consumption group or drugs management. Values of drugs management indicators –based on pedoman supervisi dan evaluasi obat publik dan perbekalan kesehatan were gathered through evaluation. The evaluation results show that in 2008, 2009, 2010, the indicators were not fit with the standard indicator, such as: indicator of drugs’ planning accuracy (147%, 108% and 145%), drugs charge per person in Cilacap District (Rp.1.342, Rp.2.000, Rp.4.615) accuracy level of financing the drugs supply (94,60%, 95,04%, 98,30%), and percentage of financing drugs supply (24,8%, 14,8%, 49%). Indicators that fit with the standard indicators were such as: Level of drugs supply (16, 5, and 7 months), accuracy level of drugs item with DOEN 52,4% during 2008-2010, generic drugs supply (89,6%, 79,1%, 88,2%), accuracy level of drugs distribution 100% during 2008-2010, accuracy level of LPLPO delivery time, drugs supply is fit with the drugs demand (109%, 160%, 167%), and drugs charge per visit per prescription (Rp.7.512, Rp. 8.180, Rp. 7.536). Keywords: Drugs Supply Management, Evaluation, Indicators, Health Department of Kabupaten Cilacap

Abstract

Surgical Site Infection / SSI is the result of the contamination of bacteria which enter the body as the surgery is in progress. High risk of Surgical Site Infection occurs in gastrointestinal surgery and is influenced by some characteristics, such as the use of antimicrobial prophylaxis. This research was aimed at identifying the use of antibiotics, identifying the percentage of the occurrence of Surgical Site Infection on the patients of gastrointestinal surgery. The research is a cross sectional research conducted prospectively at PKU Muhammadiyah Hospital Yogyakarta. The research subjects were all inpatients suffering gastrointestinal surgery and using antibiotic during the period of January until March 2012, and there are 68 patients fulfill the inclusion criteria. The inclusion criteria of the research is inpatient suffering gastrointestinal surgery, surgical site is visible, and use antibiotic for preventing Surgical Site Infection. Data that were analyzed comprised the use pattern of prophylactic antibiotics both perisurgery and post-surgery, and the analysis of the occurrence of Surgical Site Infection. The result of the research indicated that prophylactic antibiotics that was given to 14 patients with the most frequently used antibiotic was ceftriaxone (7.35%); perisurgical antibiotics that was given to 16 patients with the most frequently used antibiotics was the combination of ceftriaxone and metronidazole (8.82%), post-surgical antibiotics was given to all patients intravenously or orally in which the most frequently used intravenous antibiotics was ceftriaxone (30.88%) and the most frequently used oral was the quinolones (33.33%) and the most frequently used antibiotic type was cefadroxil (25%). The Surgical Site Infection occurred to 2 patients (2.94%) in contaminated surgery, with the comorbidity of non-diabetic, and they did not use prophylactic antibiotics.Key words: gastrointestinal surgery, occurrence of surgical site infection, prophylactic antibiotics

Abstract

The aim of this study were to know the profile and evaluate the aspects of planning, demand and acceptance of drugs and to know the level of suitability between planning and realization of the drugs acceptance in inpatient primary health care services in District of Sleman on 2008-2010. Descriptive analytical approach with retrospective and concurrent data collection was used as design of this study. Research subjects were primary health care services in District of Sleman that met inclusion criteria. Primary data were collected using interviews and secondary data were collected from documents relating to the planning process, demand, and drugs acceptance. Analysis of qualitative data was presented in the textual form. Quantitative data was summarized and calculated the value of each indicator by Depkes (2006) from year to year then compared with the existing literature. Based on the calculation of drug management indicators, there was none of indicator drugs in all primary health care services during 2008-2010 with 100% accuracy of planning, most of the drug planning did not suitable with the actual acceptance in every month; in the average, level of drug availability in primary health care services of this study was happen in safe category (12-18 months); the highest average percentage of suitability with DOEN in all primary health care services was happen in 2009 (90.7%); average percentage of suitability between drug availability with patterns of disease that closest to the ideal value for all of primary health care services was happen in 2009 (106.5%), and still quite a lot of drugs were defective/expired on the primary health care services with a total loss due to defective drugs/expired at three primary health care services of the research Rp 39,086,514.6 within three years.Keywords : Drug planning, drug availability, primary health service, Sleman

Abstract

Decree of Minister of Health Number 1197/menkes/SK/X/2004 about Standard of Hospital Pharmacy Service, states that the Hospital Pharmacy Service was integral part of hospital health care system which oriented to patient care, provision of drugs quality, clinical pharmacy services, which affordable for all levels of society. Departement of Hospital Pharmacy (IFRS) was the only division were fully responsible for the management and control of all pharmaceuticals and other medical supplies which circulated and used in hospitals. Therefore IFRS should make appropriate strategies that start with the analysis of internal and external environments known as SWOT analysis to formulate strategy that can be implemented in the service of the IFRS. The research carried out by primary and secondary data collection regarding IFRS and direct observation in field conducted by the researchers. Research can be determined which factors into strengths, weaknesses, opportunities and threats that exist on Inpatient Drug Distribution System, which then made the determination of alternative strategies to be applied by using SWOT analysis and matrix QSP. The research results showed that the calculation results of internal evaluation matrix, strengths value of Inpatient Drug Distribution System in IFRS was greater than the weaknesses value at 0.32. The results of external factor evaluation matrix also the probability value was greater than the threat value at 0.06. Therefore Drug Distribution System of Inpatient at Prof. Dr. Margono Soekarjo Hospital located in quadrant 1, which supports policy of aggressive growth. Selection strategy by QSP matrix obtained results to improve the quality and quantity of human resources at 7.01; to establish patient visite program, at 7.43; to improve service quality in order to be more  competitive with other pharmacies and hospitals at 7.28; and implement UDD distribution system at 7.66.Keywords: SWOT Analysis, Drug Distribution System of Inpatient.

Abstract

The use of management information systems applications (SIM) based on computers is still very rare in health facilities such as pharmacies. Ussually, the drug management is done manually so the accuracy of drug management is not good. Based on that problem, research on the application of management information system is needed to see the performance of pharmacies with SIM based on computerize and non-computeize. This study aimed to know the performance of pharmacies in terms of the efficiency of the management of the drug, human resources efficiency and the quickness service of SIM application with computerized and non-computerized. The research metthod was descriptive analytic design by using case studies at the pharmacy X with computerized SIM and Pharmacy Y with non computerized SIM. The data analysis for the management of drug used efficiency of medication management indicators, human resource efficiency used workload analysis, and the speed of measurement time service used dispensing time. The result illustrated that the efficiency of the management of computerized pharmacy medication was better than the non computerized pharmacies. In terms of human resources efficiency levels, as seen from the composition of the human resources required indicated that non computerized pharmacies needed more human resources than computerized pharmacies, as well as the rate speed of service to patients showed that computerized pharmacies was quicker in the service. Keywords: Pharmacy, SIM computerized, SIM non computerized

Abstract

One indicator of the success of individual health care is patient satisfaction. Customer satisfaction was defined as a response to evaluate the expectations of service prior to the actual performance perceived. The aimed of this study were to know the quality of outpatient satisfaction and to know the gap on outpatient satisfaction to the quality of based on servqual dimensions, that is tangible, empathy, assurance, responsiveness andreliability in study. The method of this study was a non experimental descriptive through observation of patient satisfaction. Data were collected with concurrent in sampling technique respondens. Data were collected with a questionnaire in November 2011. Data were analized with instrument using Product Moment Persons corelation, continued with calculation of  servqual score gap betwen performance and expectation. The results showed that the perceived service was not achieve the desired expectations of patients. Largest gap value is showed on the dimension of empathy with a value of -0.27. Meanwhile, the gap value on tangible, assurance,  reliability, and responsiveness dimension were -0.25,  -0.17,  -0.16, -0.15 respectively.Keywords: Service quality, satisfaction, gap, on an out patient basis have not beenable to satisfy

Abstract

Diabetes mellitus may cause significant problems on quality of life through the increasing risks of the occurrence of various complications. Lack of knowledge regarding disease and medication, disobedience of the patient to medication and dissatisfaction with the therapy also contribute to the poor blood glucose control. The aim of this study was to determine medication adherence and its association with blood glycemic control (HbA1c value). This research belongs to cross-sectional research in which sampling was conducted prospectively  by performing purposive sampling on type 2 DM patients in Endocrinology Polyclinic of DR Sardjito Hospital from November 2011 until January 2012. Assessment of therapy adherence was conducted using MMAS-8 and data of blood glucose control were collected through examination of HbA1c value. Questionnaires completed by study subjects as much as 109 respondents were analyzed using bivariate analysis the chi-square test. The result showed that there was only 41,3% patients who adhered to the therapy, while 58,7% was considered to disobey the therapy. In addition, respondents with good glycemic control (<6,5%) were only 41,3%, while the remaining 58,7% was considered poor. There was significant correlation of the therapy type on the adherence level (r =-0,449; p<0,05). Moreover, there was significant correlation of disease duration on the adherence level (r =-0,584; p<0,05). There was significant correlation of the adherence level on the HbA1c value (r =0,931; p<0,05). Keywords: Diabetes mellitus, therapy adherence and HbA1c value.

Abstract

There are some factors that influence human resources management, they are the employee’s attribution, working motivation, and organizational commitment. The purpose of this research is to determine the influence of the employee’s attribution toward human resources management policies to employee’s motivation and organizational commitment in PKU Muhammadiyah Yogyakarta Hospital. This research used questionnaire to 51 employees as subjects and had been working at least 1 year in PKU Muhammadiyah Yogyakarta Hospital. The questionnaires that used in this research consists of questionnaire for employee’s attribution toward human resources management policies, working motivation questionnaire, and organizational commitment questionnaire. To test validity and realibility used 25 respondents, then analyzed with regression analysis to determine the influence of the employee’s attribution toward human resources management policies to working motivation and organizational commitment used SPSS 17 for windows program for analyzing data. The results showed that employee’s attribution toward human resources management policies has positive influence 94,9 % to motivation employees with regression equation Y = 6,425 + 0,967X, employee’s attribution toward human resources management policies has positive influence to organizational commitment 92,6% with regression equation Y=10,133 + 0,735X, and working motivation correlates positively with organizational commitment with r 0,980.Keywords : Employee’s attributions, working motivation, organizational commitment

Abstract

Complementary drugs usage has increased since the last 20 years. Complementary drugs are often used together with prescription drugs. Drugs from natural origin are not free from adverse effects. This study aimed to know drugs from natural origin usage in elderly at geriatric polyclinic of RSUP Dr. Sardjito Yogyakarta, the appropriateness of  information received by patients, were potential interactions, adverse effects reaction.This study design was cross-sectional with purposive sampling technique. Data collected with questionnaire and medical record. Sample size which fulfilled inclusion criteria was 100 samples.The result showed that amount of drugs from natural origin usage in geriatric polyclinic of RSUP Dr. Sardjito Yogyakarta was 43%. The patient purposes in usage natural drugs were 63% as complement, 30% as supplement, and 7% as alternative. Patient characteristics which correlated with drugs from natural origin usage were gender (p<0.05) and occupation (p<0.05). Information received by patients, from friends or family 49%, advertisement 26%, health-care practitioners 16%, and their own knowledge 9%. Inappropriatness betwen patient purpose in using drugs with scientific evidence were 25% for supplement and 25% for complement and alternative. Potential interaction happened between bay leaves and lisinopril that caused hypertension. Adverse effect happened Holothuria sp. juice usage that caused allergic reaction and propolis usage that caused fever.Keywords : Drugs from natural origin, interactions, adverse effects

Abstract

The overall good service system is by patient satisfaction-oriented system, besides the hospital facility aspects, the role of doctors, paramedical and non medical is very important as they will determine the perception of performance and perceived performance of the services provided patients. Asking a good service needs motivation supported by awareness on the importance of excellent service. The study was done in order to see the level of employee knowledge about the excellent service, looking at the relationship of employee knowledge level for the excellent service with the awareness level for excellent level, and to see how the influence of awareness of excellent service to the employee working motivation in Supiori Local Public Hospital. The subjects for this research consists of 37 permanent employee of Supiori Local Public Hospital, then analized using visual binning of SPSS 17 for windows program to see the employee knowledge level on the excellent service, the method of correlation analysis to see the correlation of employees knowledge level for the excellent service, and regression analysis method to see how the influence of awareness for excellent service by the employee to the Supiori Local Public Hospital using SPSS 17 for windows. The results showed that the respondents’ knowledge level classified as good one are 59.5% and 40.5% were considered not good. Knowledge of excellent service with awareness level for excellent service correlate with r-count of 0.692. The awareness effect of the excellent service to the working motivation by the employees in Supiori Local Public Hospital is 12.7% with the regression equation Y = 12.943 + 0.324 X.Keywords : Excellent service, work motivation
