Abstract

Balanced Scorecard (BSC) merupakan suatu kerangka kerja yang komprehensif dan koheren yang menerjemahkan visi dan misi perusahaan ke dalam seperangkat ukuran kinerja yang terpadu. Unit Bisnis Apotek PD. Farmasi Ciremai merupakan salah satu unit bisnis yang dimiliki oleh Pemkot Cirebon yang bergerak di bidang farmasi komunitas. Penelitian ini bertujuan untuk mengidentifikasi kinerja Unit Bisnis Apotek PD. Farmasi Ciremai dengan empat perspektif balanced scorecard yaitu perspektif keuangan, pelanggan, proses bisnis internal, serta pembelajaran dan pertumbuhan, dimana hasil pengukuran dapat digunakan sebagai dasar penyusunan peta strategi perusahaan.Penelitian ini merupakan penelitian dengan pendekatan deskriptif non eksperimental menggunakan data kuantitatif yang diperoleh melalui laporan keuangan, observasi langsung, dan survei terhadap resep menggunakan analisis deskriptif evaluatif, dan data kualitatif diperoleh melalui kuesioner dan hasil wawancara mendalam dengan Apoteker dan Direktur PD Farmasi Ciremai. Sebelumnya dilakukan uji validitas dan reliabilitas kuesioner menggunakan analisis product moment pearson dan Alpha Cronbach dengan software SPSS 17.0.Berdasarkan hasil penelitian rata-rata kinerja dari Unit Bisnis Apotek PD. Farmasi Ciremai secara keseluruhan rata-rata indikator dalam kondisi baik; (1) Perspektif keuangan menunjukkan kinerja keuangan yang baik, karena berada diatas rata- rata industri (ROI 10,86%, GPM 16,05%, TATO 2,56x, dan ITOR 8,95x) kecuali nilai NPM (4,26%) masih di bawah rata-rata industri. (2) Perspektif pelanggan, secara keseluruhan tingkat pelayanan kepada pelanggan sudah baik, kondisi ini tercermin dari skor rata-rata kepuasan pelanggan yang tinggi (3,7). (3) Perspektif proses bisnis internal, mampu memberikan fasilitas pelayanan yang baik dengan tingkat ketersediaan obat yang tergolong tinggi (88,41%), tetapi pada pelayanan informasi obat dan waktu tunggu perlu ditingkatkan. (4) Perspektif pertumbuhan dan pembelajaran, mampu meningkatkan motivasi dan semangat kerja serta disiplin kerja karyawan ditunjukkan dengan turunnya persentase ketidakhadiran (2,18%), peningkatan turn over rate (0,08%), peningkatan produktivitas (Rp. 1.091.172/karyawan), dan skor budaya organisasi (3,63) yang tinggi, skor motivasi kerja tinggi (3,66), team work dengan skor 3,52 (tinggi), serta leadership dengan skor 3,65 (tinggi), tetapi skor nilai kepuasan   karyawan 3,23 masuk dalam kategori sedang, sehingga manajemen perlu berupaya untuk lebih meningkatkan lagi kinerjanya dimasa mendatang. Pada penelitian ini pengukuran kinerja unit bisnis apotek adalah suatu tahap untuk melakukan pemetaan strategi unit bisnis apotek PD Farmasi Ciremai dengan rerangka balanced scorecard. Kata kunci: PD. Abstract

One of the breast cancer management is done by hormone therapy. This study aimed to determine the main cost component of therapy, the cost of therapy both in outpatient and inpatient, and also the influence of patient characteristics, factor of therapy, and type of hormone therapy on breast cancer treatment costs at RSUP dr Wahidin Sudirohusodo. The research method was an analitic observational study from hospital perspective. The subjects were outpatient and inpatient with breast cancer in the period January-June 2014 at RSUP dr. Wahidin Sudirohusodo who met the inclusion criteria. The data was collected retrospectively which extracted from medical records and the treatment costdata from breast cancer patients. The data was analyzed by calculating the prevalence of breast cancer patients given hormone therapy, major components of costs, and expenses for each component, both for therapy outpatient or inpatient. The results showed that breast cancer patient which included in inclusion criteria were 105 patient. The range cost for inpatient surgerywas Rp 5.436.756 - Rp 5.646.678 with the largest cost component was the medical substance after use (BHP) which included in the cost of medical action 28,6%. The cost range for inpatient chemotheraphy was Rp 2.546.166 - Rp. 6.823.821 with the largest cost component was the cost of drugs namely 83,5%. The cost range for outpatient was  Rp 1.538.750 - Rp 4.202.935 with the largest cost component was the cost of drugs namely 63,57%. Patient characteristics that influenced the cost of breast cancer were stage (p = 0,001), medical action (p = 0.001), and cycles of therapy (p = 0,027). In addition, the type of hormone therapy also affects the cost of breast cancer (p = 0,047).Keywords: breast cancer, hormone therapy, cost of therapy

Abstract

ISO 9001 is an international standard which commonly used for certification related to the quality management system of company or organization eventhough not all of organization that implemented ISO 9001 seems to get the benefit related to higher organizational performance or consistency of products quality. This research aims to evaluate the implementation of ISO 9001 in the quality management system of PT. Bio Farma (Persero), to find out the influence of the implementation of the ISO 9001 quality management system on the managerial performance and on the consistency of vaccine products quality in PT. Bio Farma (Persero). This research was descriptive analysis retrospective research. The data collected are from the observation on the implementation of ISO 9001, the interview with the related parties, and the document review. The documents reviewed are the manual system guide, Standard Operating Procedure, Report of the Key Indicator Performance, and Report of Annual Product Review (APR). The result of the research shows that PT. Bio Farma (Persero) has already implemented the required elements of the ISO 9001 quality management system were Quality management system (100%), 2) Management responsibility (99,2%), 3) Resources management (100%), 4) Product realization (99,2%), 5) Measurement, analysis and improvement (100%). The implementation of the ISO 9001 quality management system improves the managerial performance of the company, whereas the key performance indicator in the last four years was always achieved, even there was improvement in achievement minimum standard. The consistency of the vaccine quality in the last three years was always achieved as observed from DTP-HB vaccine’s quality parameter that shows the process was capable (Process Capability Index (Cpk) ≥ 1), except for pH parameter within the year of 2007 (Cpk = 0,55) and year of 2009 (Cpk = 0,92); and MWGT parameter within the year of 2007 (Cpk = 0,75). Keywords: quality management system, ISO 9001, managerial performance, consistency of products quality

Abstract

Pharmacy Department is the highest revenue of Panembahan Senopati Bantul Regional General Hospital, but its performance is never measured, so the success of intangible and tangible asset management in achieving the organisational vision and goals is still unknown. This research was conducted to measure and evaluate the performance of Pharmacy Department at Panembahan Senopati Bantul Regional General Hospital using the balanced scorecard. This research was a descriptive non-experimental study. Data were obtained retrospectively and prospectively. Qualitative data were acquired through questionnaires and in-depth interview with Head of the Pharmacy Department. Quantitative data were attained through direct observation, prescription survey, and financial reports. Quantitative data were statistically analyzed using a statistic, while qualitative data were analysed and presented in descriptive form. Performance of learning and growth perspective showed that from human capital view, the proportion of pharmacists and bed was not ideal (1:36); the percentage of employee training had not reached 100%; and the average of employee morale was high (3.18). The percentage of Organization Capital was 75.71% which showed the high culture, leadership, teamwork, and alignment. Meanwhile, Information System Capabilities needed the addition of expired date and drugs dosage databases, internet, and the development of management information system. The performance of internal business process perspective showed that the average of drug availability was 98%; the average dispensing time was 37 minutes for compounded prescription and 29 minutes for non compounded prescription; the average SOP suitability with accreditation standards of hospital was 74%; and there was no documentation for medication error. Performance of the customer’s perspective showed the percentage of patients encountered was 80.97%; the rate of customer satisfaction was 86%; and the rate of customer growth was 1% per month. Performance of the financial perspective found that TOR value was 9 times per year in 2011 and 7 times per year in 2012; the rate of service revenue in 2011 was 35% and 28.35% in 2012; Gross Profit Margin was 26.29% in 2011 and 24.83% in 2012.Keywords: Performance, Balanced Scorecard, Pharmacy Department

Abstract

The prevalence of COPD in Indonesia is increasing. Over the world, COPD is predicted to be the third cause of death in 2020. As the chronic disease, quality of life became one of the outcome treatments of COPD. This research was aimed to validate Indonesian version of St. George's Respiratory Questionnaire (SGRQ) Indonesia. This study used cross sectional design approach from June to July 2016, conducted at Respira Lung Hospital, Yogyakarta. Data was collected from medical record and direct interviews to patients using SGRQ. An inclusion criterion was adult patients with COPD. Some statistical tests were performed to analyze construct validity, known group validity and reliability of Indonesian version of SGRQ. We recruited 41 respondents. The SGRQ meet reliability criteria with the value of Cronbach alpha are > 0.7 for symptom, activity and impact domains. There are 17 questions and 10 questions in symptom, activity and impact domains which do not meet convergent and discriminant validity. The significant difference is found in symptom domain for the 17-48 and 49-80 year age. The SGRQ Indonesian version is not valid enough to be used in clinical practice but still reliable.

Abstract

Pelayanan farmasi merupakan pelayanan penunjang dan merupakan pendapatan utama rumah sakit, dengan kontribusi 90% pelayanan kesehatan merupakan perbekalan farmasi. Banyaknya kesalahan dalam pemberian obat menyebabkan kejadian tidak diharapkan, rumah sakit perlu memperhatikan akreditasi instalasi farmasi dari segi manajemen penggunaan obat (MPO). Tujuan penelitian ini untuk mengetahui tingkat kesesuaian pelayanan farmasi terhadap tujuh standar akreditasi MPO dan strategi pengembangannya dengan metode hanlon berdasarkan standar akreditasi rumah sakit tentang MPO di Instalasi Farmasi Rumah Sakit PKU Muhammadiyah Surakarta. Penelitian ini merupakan rancangan penelitian non eksperimental, data dianalisis secara kuantitatif dan kualitatif. Penelitian dilakukan pada bulan februari sampai bulan maret tahun 2016. Pengumpulan data menggunakan instrumen kuesioner penilaian diri sendiri dan wawancara kepada staf instalasi farmasi rumah sakit yang terlibat berdasarkan tujuh standar MPO. Strategi pengembangan untuk penyelesaian masalah elemen penilaian menggunakan analisis prioritas masalah dengan metode Hanlon. Hasil penelitian tingkat kesesuaian pelayanan farmasi terhadap standar akreditasi MPO adalah MPO1 organisasi dan manajemen sebesar 98,71%, MPO2 seleksi dan pengadaan 98,26%, MPO3 penyimpanan 98,37%, MPO4 pemesanan dan pencatatan 97,83%, MPO5 persiapan dan penyaluran 96,37%, MPO6 pemberian 98,82%, dan MPO7 pemantauan 94,10%. Dari 24 elemen penilaian standar MPO yang dilakukan ada 7 yang belum memenuhi standar maksimal dan memiliki kekurangan. Analisis prioritas masalah elemen penilaian menggunakan metode Hanlon, prioritas secara berturut-turut pertama elemen penilaian MPO6.P3 identifikasi petugas untuk memberikan obat, MPO7.P2 monitoring efek obat, MPO4.P2 identifikasi petugas kompeten, MPO1.P4 pelayanan penggunaan informasi obat, MPO3.P1 penyimpanan produk nutrisi, MPO5.P3 penyiapan produk steril, dan terakhir MPO7.P3 pencatatan atau pelaporan obat yang tidak diharapkan dalam status pasien.

Abstract

The use of management information systems applications (SIM) based on computers is still very rare in health facilities such as pharmacies. Ussually, the drug management is done manually so the accuracy of drug management is not good. Based on that problem, research on the application of management information system is needed to see the performance of pharmacies with SIM based on computerize and non-computeize. This study aimed to know the performance of pharmacies in terms of the efficiency of the management of the drug, human resources efficiency and the quickness service of SIM application with computerized and non-computerized. The research metthod was descriptive analytic design by using case studies at the pharmacy X with computerized SIM and Pharmacy Y with non computerized SIM. The data analysis for the management of drug used efficiency of medication management indicators, human resource efficiency used workload analysis, and the speed of measurement time service used dispensing time. The result illustrated that the efficiency of the management of computerized pharmacy medication was better than the non computerized pharmacies. In terms of human resources efficiency levels, as seen from the composition of the human resources required indicated that non computerized pharmacies needed more human resources than computerized pharmacies, as well as the rate speed of service to patients showed that computerized pharmacies was quicker in the service. Keywords: Pharmacy, SIM computerized, SIM non computerized

Abstract

Warfarin merupakan salah satu antikoagulan yang biasanya digunakan pada pasien stroke iskemik dengan atrial fibrilasi. Sampai saat ini, penggunaan antikoagulan sebagai terapi pencegahan masih dalam penelitian. Hasil yang diharapkan pada pasien ini terlihat dari peningkatan nilai INR (International Normalized Ratio) dan mencapai target terapi. Jenis penelitian adalah cross sectional. Data diambil secara prospektif dari rekam medik pasien yang dirawat di unit stroke dan bangsal rawat inap Dahlia 2. Subyek penelitian adalah pasien stroke iskemik dengan atrial fibrilasi yang menjalani rawat inap sedangkan objek penelitian meliputi rekam medik pasien yang didalamnya tertera nilai INR (International Normalized Ratio) selama pasien menggunakan warfarin dan didukung pula oleh informasi tambahan mengenai kondisi pasien yang diperoleh dari keluarga pasien dan tenaga medis yang terlibat. Analisis data dilakukan secara deskriptif dan korelatif. Analisa data dilakukan dengan membuat grafik hubungan INR (International Normalized Ratio) pasien dengan durasi penggunaan warfarin dari tiap pasien yang disertai juga dengan dosis warfarin, komorbiditas dan terapi lain yang diperoleh pasien selama berada di rumah sakit sampai tercapai atau tidak tercapainya target INR (International Normalized Ratio). Selain itu, dilakukan juga analisis statistik multiple regression linear untuk mengetahui hubungan antara dosis dan durasi penggunaan warfarin terhadap target INR (International Normalized Ratio) 1,5-2,0 pasien selama menggunakan warfarin dan di rawat dirumah sakit. Hasil penelitian menunjukkan adanya hubungan yang bermakna dengan level signifikansi 0,05 (P<0,05) dari dosis dan durasi warfarin terhadap INR (International Normalized Ratio) pada pasien stroke iskemik dengan atrial fibrilasi.Kata kunci: warfarin, INR (International Normalized Ratio), dosis, durasi, stroke iskemik, atrial fibrilasi

Abstract

People often consider that generic drugs are inferior drugs. Knowledge holds big roles in consumer decision making process, including in purchasing generic drugs. Students of health sciences got different understanding compared to students of non health sciences about to knowledge of generic drugs. Therefore, this study aimed to get better understanding and compare product class knowledge, perceived risk, and attitude towards generic drugs in students of health sciences versus students of non health sciences. The population of this non experimental study was Gadjah Mada University students. Sampling technique was using purposive sampling. Samples were UGM students of health sciences and students of non health sciences who met inclusion criteria. Data were collected using questionnaires and analyzed using path analysis and Mann-whitney. Results of this study showed that there was significant negative influence between objective knowledge and perceived risk (standardized coefficient beta value = -0.368), subjective knowledge and perceived risk (standardized coefficient beta value = -0.485), prior experience and perceived risk (standardized coefficient beta value = -0.485), and perceived risk and attitude toward generic drugs (standardized coefficient beta value = -0.489). There were difference between health students and non health students on their product class knowledge, perceived risk and attitude towards generic drugs, which health students got higher product class knowledge and attitude towards generic drugs, while non health students got higher perceived risk compared to health students. Keywords: product class knowledge, perceived risk, attitude towards generic drugs, generic drugs.

Abstract

Jamkesmas is social assistance health services program to poor society. This program is aimed to make cross subsidy health services for all poor society. This research is aimed at identifying the characteristics of patients, verification of Jamkesmas, average discrepancy of therapy cost, and average discrepancy of medicine cost. This research uses non‐experimental descriptive design, which is using proportional random sampling method that collected retrospectively. The result shows that appendicitis patient that need inclution criteria is 37 cases, of acutegastroenteritis (GEA) is 33 cases, of hypertension is 31 cases, of urinary tract infections is 32 cases , and of acute respiratory infection is 38 cases. The total amount of claims of Jamkesmas inpatient in 2008 is Rp 6.152.995.376, of the payable claims is Rp 6.053.265.909, and of the non‐payable claims is Rp 119.729.467. The average discrepancy cost of Jamkesmas verification and direct medical cost on receipt of appendicitis therapy is 56.028 ± 299.686, of acutegastroenteritis (GEA) is Rp ‐ 12.266 ± 204.275,, of hypertension is Rp  118.697 ± 329.572, of urinary tract infections is Rp 155.582 ± 696.428, and of acute respiratory infection is Rp 86.417 ± 156.541. The average discrepancy of medicine cost of Jamkesmas verification and medicine cost on receipt of appendicitis therapy is Rp ‐424.818 ± 444.459, of acutegastroenteritis (GEA) is Rp ‐ 31.110 ± 96.157, of hypertension is Rp ‐67.001 ± 114.541, of urinary tract infections is Rp 41.635 ± 353.945, and of acute respiratory infection is Rp 32.955 ± 132.815. Key words : Cost Analysis, Inpatient, Jamkesmas, RSUD Panembahan Senopati Bantul
