Abstract

Modified Radical Mastectomy (MRM) may induce a systemic inflammatory response that can affect postoperative recovery in patients with breast cancer. The Systemic Immune-Inflammatory Index (SII) is an inflammatory biomarker calculated from platelet, neutrophil, and lymphocyte counts. This study aimed to compare postoperative changes in SII among breast cancer patients undergoing MRM who received epidural anesthesia and general anesthesia at Ibnu Sina YW-UMI Hospital. This retrospective analytical observational study used a comparative approach based on medical records of breast cancer patients who underwent MRM between January and December 2024. SII was calculated using the formula (platelet count × neutrophil count)/lymphocyte count, while the change in SII was defined consistently as the postoperative SII minus the preoperative SII (?SII = SII postoperative ? SII preoperative). Statistical analyses included the paired t-test, Wilcoxon test, and Mann–Whitney test according to data distribution. A total of 36 patients were included, comprising 10 patients who received epidural anesthesia and 26 who received general anesthesia. In the epidural group, the mean SII decreased from 839.04 preoperatively to 477.04 postoperatively. In contrast, the mean SII in the general anesthesia group increased from 742.28 to 1,487.24. The comparison of postoperative SII changes between the two anesthesia groups showed a statistically significant difference (p<0.001). These findings indicate different postoperative SII dynamics following MRM between patients receiving epidural and general anesthesia. However, the findings should be interpreted cautiously because of the retrospective design, unequal group sizes, and potential confounding factors that were not fully controlled. Further prospective studies with standardized postoperative blood sampling and adjustment for relevant perioperative confounders are warranted.

Abstract

The Surgical Safety Checklist (SSC) is a patient safety instrument used to enhance communication, coordination, and procedural consistency in surgical care. This study aimed to analyze the relationship between compliance with SSC implementation and the following factors: individual characteristics, human resources and work environment, organizational and management support, facilities and infrastructure, and team leadership. The study was conducted in the Operating Room of Ibnu Sina YW-UMI Hospital, Makassar. An analytical study design with a cross-sectional approach was employed, involving 30 respondents—comprising surgeons, anesthesiologists, and operating room nurses—selected via stratified random sampling. Data were collected using Likert-scale questionnaires and direct observation. Bivariate analysis was performed using Fisher’s Exact Test. Significant relationships were found between SSC implementation compliance and individual factors (p=0.006), human resources and work environment (p=0.029), organizational and management support (p=0.007), facilities and infrastructure (p=0.010), and team leadership (p=0.031). The proportions of compliance within the groups meeting the respective criteria were 88.0%, 63.2%, 56.3%, 58.8%, and 84.6%, respectively. In conclusion, all five factors were significantly associated with compliance in SSC implementation. However, inconsistent relationship patterns observed for some factors require cautious interpretation and further investigation.

Abstract

Diabetes mellitus is a metabolic disease with increasing global prevalence, necessitating the exploration of alternative therapeutic agents from natural sources such as secang wood (Caesalpinia sappan L.). This study aimed to determine the effect of secang wood extract on blood glucose levels of alloxan-induced mice and compare its effectiveness with metformin. A true experimental study with pretest-posttest control group design was conducted on 24 male mice divided into 4 groups: negative control (distilled water), extract dose 75 mg/kgBW, extract dose 150 mg/kgBW, and positive control (metformin 100 mg/kgBW). Diabetes was induced by alloxan 120 mg/kgBW intraperitoneally. Blood glucose measurements were performed at pretest, post-induction, day 7, and day 14. The results showed that alloxan induction successfully increased blood glucose levels in all groups (263.00–296.50 mg/dL). Phytochemical tests confirmed the presence of flavonoids, tannins, alkaloids, saponins, and triterpenoids in secang wood extract. On day 14, the 150 mg/kgBW extract group showed a decrease in blood glucose levels to 137.17 ± 7.39 mg/dL, which was statistically equivalent to metformin (137.00 ± 7.38 mg/dL) and significantly different from the negative control (301.00 ± 17.18 mg/dL) (p<0.05). This effectiveness is attributed to the synergistic action of bioactive compounds, especially brazilin which has a similar mechanism to metformin in inhibiting hepatic gluconeogenesis. It can be concluded that secang wood extract at a dose of 150 mg/kgBW effectively reduces blood glucose levels with potential equivalent to metformin, making it a promising candidate for development as a safe and accessible alternative diabetes therapy from natural sources.

Abstract

Pleural effusion is a pathological accumulation of fluid in the pleural cavity that may result from various infectious and non-infectious conditions. This study aimed to describe the clinicopathological profile of patients with pleural effusion at Ibnu Sina YW-UMI Hospital, Makassar, during the 2024–2025 period. A descriptive retrospective study based on medical records was conducted using total sampling. A total of 44 medical records of patients who met the study criteria were analyzed based on age, sex, etiology, pleural fluid examination, clinical symptoms, cytological findings, effusion location, and nutritional status. The results showed that most patients were adults (57%) and female (61%). Infectious etiology was the most common group (61%), while non-infectious etiology accounted for 39%. A positive Rivalta test was found in 52% of patients, lactate dehydrogenase (LDH) levels >2/3 of the upper limit of normal serum in 50%, and total protein levels >2500 mg/dL in 55%. Dyspnea was the most frequently reported clinical symptom (61%). Cytological findings were predominantly lymphocytic (43%), followed by inflammatory (32%), malignant (14%), and reactive (11%) findings. Pleural effusion was most frequently located on the right side (41%). Regarding nutritional status, 48% of patients had normal nutritional status, followed by underweight (39%) and overweight (14%). Overall, the clinicopathological profile was predominantly characterized by adult and female patients, infectious etiology, dyspnea, lymphocytic cytological findings, and right-sided pleural effusion

Abstract

This study aimed to analyze the relationship between academic burnout and sweet food consumption patterns among undergraduate medical students at the Faculty of Medicine, Universitas Pembangunan Nasional “Veteran” Jakarta. This quantitative, analytical study employed a cross-sectional design and involved 96 students from the Class of 2023, selected using convenience sampling. Data were collected using the Maslach Burnout Inventory–Student Survey (MBI-SS) and a modified Emotional Eating Scale (EES), then analyzed using the Chi-Square test via IBM SPSS Statistics software. The results showed that the majority of respondents fell into the academic burnout score category of at or below the sample median (53.1%). The proportion of respondents with high-level sweet food consumption was greater among those with academic burnout scores above the median compared to those with scores at or below the median (54.3% vs. 40.0%), bivariate analysis revealed no significant relationship between academic burnout and sweet food consumption patterns (p = 0.159). In conclusion, academic burnout is not significantly associated with sweet food consumption patterns as a form of emotional eating among medical students at the Faculty of Medicine, UPN “Veteran” Jakarta.
