Abstract

Knee osteoarthritis (OA) is a degenerative joint disease commonly observed among middle-aged and older adults. This study aimed to identify the characteristics of patients with knee osteoarthritis at Bhayangkara Hospital Makassar in 2025 based on demographic characteristics, body mass index (BMI), radiographic classification, diagnosis location, and treatment. This study used a retrospective descriptive observational design based on medical records with a cross-sectional approach. Data were obtained from the medical records of 60 patients who met the study criteria and were analyzed using univariate analysis with frequency and percentage distributions. The results showed that patients aged 45–59 years constituted the largest age group (50.0%), while female patients accounted for 63.3%. Housewives represented the largest occupational group (41.7%). Regarding BMI, 45.0% of patients were classified as overweight and 23.3% as obese, resulting in 68.3% being classified as overweight or obese overall. Based on the Kellgren–Lawrence classification, Grade 2 was the most frequently observed radiographic grade (48.3%), while bilateral knee OA was the most common diagnosis location (76.7%). All patients were documented as receiving pharmacological treatment (100%), and no patients were documented as undergoing surgical treatment. In conclusion, patients with knee OA were predominantly women aged 45–59 years, housewives, patients classified as overweight or obese, patients with Grade 2 OA, bilateral involvement, and pharmacological treatment.

Abstract

Gestational Trophoblastic Neoplasia (GTN) is a malignant gestational disorder characterized by abnormal trophoblastic proliferation and elevated B-hCG levels. At markedly increased concentrations, B-hCG may exert thyrotropic activity through stimulation of the thyroid-stimulating hormone (TSH) receptor and contribute to clinically significant hyperthyroidism. GTN in perimenopausal women presents particular diagnostic and therapeutic challenges because its manifestations may overlap with common gastrointestinal complaints and physiological changes associated with the menopausal transition. This case report describes a 50-year-old P2A0 perimenopausal woman with GTN complicated by severe thyrotoxicosis and persistent disease following surgical management. The patient initially presented with progressive lower abdominal discomfort, nausea, recurrent vomiting, and mild vaginal spotting and was initially diagnosed with dyspepsia. Further evaluation revealed a uterine mass, serum B-hCG >50,000 mIU/mL, suppressed TSH, and markedly elevated FT4 >7.77 ng/dL, consistent with severe thyrotoxicosis. Preoperative treatment included propylthiouracil, Lugol’s iodine, dexamethasone, and propranolol to reduce the perioperative risk associated with severe thyrotoxicosis. The patient subsequently underwent total abdominal hysterectomy with bilateral salpingo-oophorectomy (TAH-BSO). Histopathological examination demonstrated malignant trophoblastic proliferation with myometrial invasion. Following surgery, B-hCG levels initially declined substantially but subsequently showed a secondary rise and plateau, consistent with persistent GTN according to FIGO criteria. The patient was then treated with methotrexate-etoposide chemotherapy with leucovorin rescue, with subsequent clinical improvement. This case highlights the importance of considering GTN in perimenopausal women presenting with atypical gastrointestinal or gynecological symptoms, particularly in the presence of markedly elevated B-hCG. Evaluation of thyroid function is important in patients with high B-hCG levels because B-hCG-mediated stimulation of the TSH receptor may represent a likely mechanism of severe thyrotoxicosis. Serial postoperative B-hCG monitoring is also essential for early detection of persistent disease and timely initiation of further therapy.

Abstract

Genu osteoarthritis (OAG) is a degenerative joint disease influenced by mechanical and metabolic factors. Uric acid levels may be associated with inflammatory and metabolic processes accompanying OAG; however, the relationship between uric acid levels and the clinical characteristics and severity of OAG remains inconsistent. This study aimed to analyze the association of uric acid levels with age, sex, body mass index (BMI), and OAG severity based on the Kellgren-Lawrence classification among patients with OAG at Ibnu Sina Hospital Makassar in 2026. This analytical observational study used a cross-sectional design and included 41 patients with OAG. Uric acid levels were measured from capillary blood using a GCU meter, while BMI was obtained from anthropometric measurements and OAG severity was determined from medical records based on the Kellgren-Lawrence classification. Bivariate analysis was performed using the Chi-Square test. Hyperuricemia was identified in 21 patients (51.2%), while 24 patients (58.5%) had Grade 3–4 OAG. A significant association was found between BMI and uric acid levels (p=0.014), with all patients classified as obesity class II having hyperuricemia. No statistically significant association was found between uric acid levels and age (p=0.342), sex (p=0.277), or OAG severity (p=0.208). In this study, uric acid levels were associated with BMI but were not statistically associated with age, sex, or OAG severity. These findings should be interpreted in consideration of the limited sample size and cross-sectional design.

Abstract

The Emergency Installation (IGD) operates 24 hours and must deliver fast, accurate, and high-quality care, yet increasing patient visits and acuity may cause overcrowding, prolonged waiting times, safety risks, and reduced satisfaction. This study aimed to analyse factors associated with patient waiting time at the Emergency Installation of Ibnu Sina Makassar Hospital Yayasan-UMI, focusing on registration, human resources, and facilities/infrastructure. An analytical descriptive cross-sectional study was conducted in August 2025, with data collected across morning, afternoon, and night shifts through service-flow observation and questionnaires. A total of 90 respondents (estimated population 900) were included using the Slovin formula (10% error) and total sampling based on inclusion criteria. Waiting time was defined as the duration from registration until receiving services or transfer. Data were analysed using univariate statistics and Chi-square tests in SPSS, and ethical approval recommendation was obtained (No: 502/A.1/KEP-UMI/VII/2025). Most respondents experienced fast waiting time (56/90; 62.22%), while 37.78% experienced slow waiting time. Registration was rated fast by 62.22% of respondents, human resources were rated adequate by 72.22%, and facilities/infrastructure by 82.22%. Significant relationships were found between waiting time and registration (p=0.005), human resources (p=0.007), and facilities/infrastructure (p=0.005). The findings suggest that improving registration efficiency, staffing distribution, and facility readiness can reduce waiting time and strengthen service quality. The novelty of this study lies in simultaneously examining these three operational factors in one integrated analysis within this hospital’s IGD, addressing a local research gap.

Abstract

Landfill-related pollution may degrade environmental quality and pose health risks, particularly among vulnerable populations such as pregnant women. This study aimed to analyze water quality and describe the health profile of pregnant women in the Tamangapa Community Health Center area according to the proximity of their homes to the landfill. This quantitative descriptive study included 13 pregnant women in their second and third trimesters who met the study criteria. Water quality data were obtained through field measurements, while maternal health and antenatal care data were obtained from measurements and medical records. Data were analyzed using univariate descriptive analysis. The results showed that 76.9% of water samples had elevated temperature, 38.5% were acidic, 30.8% were turbid, and 84.6% had high specific gravity. A total of 76.9% of respondents lived within the predefined close-distance category from the landfill. Based on mid-upper arm circumference, 30.8% of respondents were at risk of chronic energy deficiency. Most respondents had blood pressure within the normal range (84.6%), while mild anemia was found in 53.8% and moderate anemia in 15.4%. The average achievement of the assessed 10T antenatal care components was 85.4%, with laboratory examinations having the lowest achievement (69.2%). The findings indicate changes in several water-quality parameters and the presence of maternal health problems among the respondents. However, due to the descriptive design and limited sample size, this study cannot establish a causal relationship between water quality, residential distance, and maternal health outcomes.

Abstract

The quality of pharmaceutical services and patient satisfaction are two interrelated indicators in assessing the success of healthcare services in community health centers, hospitals, and pharmacies. Research in Indonesia tends to discuss both separately within one type of facility, so cross-comparisons among the three facilities are still limited. This review aims to map, compare, and synthesize scientific evidence regarding the quality of pharmaceutical services and patient satisfaction in Indonesia, while also identifying service dimensions, measurement methods, related factors, and research gaps. Article searches were conducted through Google Scholar, Garuda, and SINTA from July 5–10, 2026, covering articles from 2021–2026 and at least indexed SINTA 3. Out of the initial 55 articles, 22 met the criteria, consisting of eight community health center articles, seven hospital articles, and seven pharmacy articles. The five-dimensional SERVQUAL became the most widely used instrument, both on its own and in combination with other methods. Patients generally fall into the satisfied category, although gaps in performance and expectations are still found, especially regarding the availability of medication, clarity of medication information, and waiting times. The findings indicate the need for strengthening pharmaceutical resources and standardizing the evaluation of service quality across healthcare facilities in Indonesia.

Abstract

Hemophilia is a genetic disorder that causes blood clotting impairment and can affect children's quality of life. Nutritional status plays an important role in the management of children with hemophilia as it influences treatment efficacy and complication risk. This study aimed to describe the nutritional status of children with hemophilia at "IBNU SINA" YW-UMI Hospital Makassar during the 2023-2024 period. This quantitative descriptive study involved 87 hemophilia patients who were grouped by hemophilia type, age, and nutritional status. Nutritional status was assessed using WHO growth charts for children aged <5 years and CDC growth charts for those aged ?5 years. The results showed that the majority of patients had hemophilia A (83%), while hemophilia B was found in 17% of patients. Age distribution showed that 52% of patients were in the adolescent group (10-18 years), followed by the children's group aged 5-9 years (24%). Normal nutritional status was observed in 66.7% of patients aged 0-59 months and 48.5% of patients aged 5-18 years. Overall, 26.4% of patients were undernourished and 9.2% were obese. This study provides important information regarding the variation in nutritional status among children with hemophilia and serves as a foundation for more effective nutritional intervention management. Further research with a prospective cohort design is recommended to understand the factors influencing nutritional status in hemophilia patients.

Abstract

Gout arthritis is an inflammatory disease caused by the deposition of monosodium urate crystals in the joints and surrounding tissues in the setting of hyperuricemia. Demographic and clinical characteristics, kidney function, and treatment patterns need to be identified to describe the profile of patients and support appropriate management. This study aimed to determine the characteristics of patients with gout arthritis at Ibnu Sina YW-UMI Makassar Teaching Hospital in 2025. This was a descriptive retrospective cross-sectional study using secondary data obtained from medical records. All patients with gout arthritis in 2025 who met the inclusion criteria were included using total sampling, resulting in 25 patients. Data were analyzed descriptively and presented as frequency distributions. Most patients were male (84%), and the largest age group was 46–55 years (36%). Regarding body mass index, 56% of patients were in the normal category, while 44% were classified as overweight or obese. Recorded anti-inflammatory therapies included colchicine (76%), nonsteroidal anti-inflammatory drugs (NSAIDs) (56%), and prednisone (24%), while the urate-lowering therapy allopurinol was recorded in 80% of patients. Combination therapy was received by 80% of patients, with the combination of NSAIDs, colchicine, and allopurinol being the most frequently recorded regimen (30%). Tophus was found in 40% of patients, while a history of recurrent attacks was recorded in 28%. Elevated creatinine levels were found in 61.9% of male patients, whereas all female patients had normal creatinine levels. This study demonstrates that patients with gout arthritis at Ibnu Sina YW-UMI Makassar Teaching Hospital in 2025 were predominantly male, with variations in body mass index, tophus, recurrent attacks, kidney function, and treatment patterns.

Abstract

Postoperative pain following gynecological laparotomy is a clinical problem requiring optimal analgesic management. Intravenous ibuprofen and paracetamol can be used as components of multimodal analgesia; however, comparative evidence regarding their effects on pain intensity and inflammatory response in patients undergoing gynecological laparotomy remains limited. This study aimed to compare postoperative pain intensity and changes in the Neutrophil-to-Lymphocyte Ratio (NLR) in patients undergoing gynecological laparotomy who received preemptive intravenous ibuprofen or paracetamol. This experimental pre-post test study included 32 patients divided into ibuprofen (n=16) and paracetamol (n=16) groups. Pain intensity was assessed using the Numeric Rating Scale (NRS) before surgery and at 2, 6, 12, and 24 hours postoperatively. NLR was assessed preoperatively and 24 hours postoperatively. Between-group comparisons were performed using statistical tests according to data distribution. No statistically significant differences in NRS scores were found between the groups at the preoperative, 2-, 6-, 12-, or 24-hour assessments (p>0.05). At 6 hours postoperatively, the mean NRS score was lower in the ibuprofen group than in the paracetamol group, but the difference was not statistically significant (2.88±0.62 vs. 3.31±0.79; p=0.102). The mean change in NLR also did not differ significantly between the paracetamol and ibuprofen groups (8.60±16.91 vs. 8.79±15.08; p=0.836). Thus, this study found no statistically significant differences between preemptive intravenous ibuprofen and paracetamol in postoperative pain intensity or changes in NLR following gynecological laparotomy.

Abstract

Knee osteoarthritis is a degenerative joint disease that commonly occurs in the elderly and has a significant impact on pain, function, and patients’ quality of life. To date, no pharmacological therapy has been proven to modify the course of the disease; therefore, medical rehabilitation remains the main non-pharmacological approach. This study aimed to determine the effect of medical rehabilitation on improving the quality of life of patients with knee osteoarthritis at Ibnu Sina Hospital, Makassar. This study employed an analytic observational design with a retrospective cross-sectional approach. The subjects were 27 patients with knee osteoarthritis who received treatment at the Medical Rehabilitation Unit of Ibnu Sina Hospital, Makassar, during 2023–2024. Data were obtained from medical records and the WHOQOL-BREF questionnaire. Data analysis was performed using univariate and bivariate analyses with the chi-square test. Most respondents were aged ?70 years (48.2%) and were female (66.7%). The majority of patients underwent medical rehabilitation (81.5%), with exercise therapy as the main modality. A total of 74.1% of patients had a good quality of life. There was a significant association between medical rehabilitation and quality of life in patients with knee osteoarthritis (p = 0.009), with an odds ratio (OR) of 12.222. Medical rehabilitation has a significant effect on improving the quality of life of patients with knee osteoarthritis at Ibnu Sina Hospital, Makassar.
