Abstract

Medical records are a mandatory and important document in the management of hospital services. Based on the Minister of Health Regulation No. 24 of 2022, it is described that medical records consist of confidential patient identity data containing records of examination, treatment, medical actions, and other services that have been provided according to patient needs. Problems that occur in various departments include incomplete documents in the preparation, coding errors, and the wrong location of files in the filling selection. Inadequate storage space and sub-optimal filling systems also contribute to lost records and inefficient retrieval. This study aims to look at the medical record file management process at the UPTD Special Lung Hospital North Sumatra Province. This research uses descriptive qualitative research methods. The results showed that the UPTD Special Lung Hospital North Sumatra Province still uses hybrid medical record file, and electronic medical records at the coding and indexing stages, and the supporting factors of the medical record file management process include human resources, suitability of Standard Operational Procedures (SOP), facilities and infrastructure, and reporting carried out at the UPTD Special Lung Hospital North Sumatra Province still has obstacles in the medical record file management process, namely the assembling process, coding and indexing, and the medical record file storange process.

Abstract

Leptospirosis is a zoonotic disease that is transmitted through contact with the urine of infected animals, especially rats, and often occurs in environments with poor sanitation. This study aims to analyze behavioral risk factors and environmental conditions that contribute to the spread of leptospirosis in Pematang Pasir Village, Teluk Nibung District, Tanjung Balai City in 2024. The study used a cross-sectional design involving 50 respondents aged 17 years and older from Wards I and VI. Data were collected through interviews using questionnaires that assessed respondents' knowledge, attitudes, and actions related to leptospirosis, as well as direct observation of environmental conditions such as waterlogging, the presence of rats, and sanitary conditions. The results showed that the majority of respondents (88%) had low knowledge about leptospirosis, although 80% implemented good personal hygiene behaviors. Only 18% of respondents carried out adequate rat control measures. Environmental observations revealed that 96% of respondents experienced puddles around their homes, and 100% indicated that their neighborhoods were flooded frequently. The presence of garbage around the house was found in 98% of respondents, and 98% of respondents often saw rats in or around the house. Most respondents (90%) had a house ≥ 2 meters away from the sewer, with 92% reporting good road conditions. Only 24% of respondents have pets, and 4% live close to ponds (< 700 meters). Factors such as poor sanitation, the presence of rodents, and the high risk of flooding are the main factors that affect the risk of spreading leptospirosis in the region. This study emphasizes the importance of increasing public education about leptospirosis and its prevention measures, as well as environmental improvement efforts to reduce the risk of infection in the affected areas.

Abstract

The production process in palm oil mills is inseparable from potential hazards in the workplace such as the FFB Receiving Station area (Scales, Sorting/Loading ramp), then the Boiling Station (Sterilizer), the Grinding Station (Threshing/Theresser/autofeeder), the Felt/Pumping Station (Screw Press, Slamming Station, Clarification Station, CPO Storage Tank (Storange Tank), Boiler Station and Engine Room Station (Power House), Demint Plant Station. Efforts that can be made are by conducting risk management through the identification of potential occupational safety and health hazards, using the HIRARC method. The goal of HIRARC is to recognize potential hazards and various operational problems in each process. Based on the risk assessment, there are four categories of risk levels identified, namely risk levels, namely Low Risk, Modarate Risk, High Risk, and Extreme Risk. The risk rating provides a level for each risk category, which results in the most dangerous risk, namely the Stockpile Tank Station and Engine Room get a level of 15 High Risk, then at the Boiling, Slamming, Clarification, Steam Boiler Station get a level of 12 High Risk. Risk Control that must be carried out on potential hazards at PT. X is Required to Wear Complete PPE that has been given (gloves, anti-slip shoes, mask, ear plug/ear, helmet, safety glasses), Obey signs, Work according to SOPs and Work instructions, clean the workplace area, monitor to work according to procedures, Carry out periodic Maintanetance, and the author hopes that at PT. X establishes new policies and regulations for workers by providing penalties such as issuing first warning letters or fines for workers who do not comply with SOPs and do not wear PPE. Instead, appreciation is given to workers who comply with SOPs and use PPE properly and correctly.

Abstract

Occupational safety and health is an effort to prevent or reduce work accidents through stopping/minimizing risks or hazard factors in order to achieve work targets or creation. Based on data from the International Labor Organization (ILO), every year 2.78 million workers die due to accidents and diseases at work. Based on this number, around 2.4 million (86.3 percent) die from occupational diseases, while more than 380,000 (13.7 percent) from work accidents. Therefore, Personal Protective Equipment (PPE) is a crucial aspect in reducing the level of hazard risk to workers. This study aims to understand the level of compliance of workers wearing PPE at the work site. This research is qualitative research and case study research design. Based on the results of interviews and observations carried out, it is understood that the level of compliance of informants wearing PPE is still fairly moderate, this matter is evident from the informant's statement that there are still many incomplete use of PPE required when working.

Abstract

Nurse caring is a crucial aspect of healthcare services that significantly impacts patient satisfaction. In the Regional General Hospital Puruk Cahu, the general surgery ward experiences a high number of patient visits, highlighting the importance of understanding the role of nurse caring in influencing patient satisfaction. This study aims to analyze the relationship between nurse caring and patient satisfaction levels in the general surgery ward of the Regional General Hospital Puruk Cahu. This study employed a descriptive correlational quantitative approach with a cross-sectional design. Respondents consisted of adult patients treated in the general surgery ward of the Regional General Hospital Puruk Cahu during September–October 2024. Data were collected using questionnaires on nurse caring and patient satisfaction and analyzed with correlation tests. The majority of respondents (63.3%) rated nurse caring as very high, and 53.3% expressed feeling very satisfied with the care provided. However, the correlation test results indicated no significant relationship between nurse caring and patient satisfaction (p-value = 0.407). While nurse caring was perceived as very high by patients, there was no significant correlation with their satisfaction levels. Other factors may influence patient satisfaction, emphasizing the need to enhance nurse caring quality through targeted training and a holistic approach to achieve optimal nursing care services.
