Abstract

Tetralogy of Fallot (TOF) is the most common cyanotic heart disease with anomalies consist of: ventricular septal defect (VSD), overriding aorta, pulmonary stenosis and hypertrophic of the right ventricular. It also has various anomalies type such as TOF with absent pulmonary valve, TOF with pulmonary atresia, TOF with atrioventricular septal defect, TOF with coronary artery anomalies, etc which will be discussed in detail. Proper diagnosis is required when dealing with the TOF continue with radiological examination. Adequate history taking and correct physical examination are necessary in dealing with congenital heart defects, especially TOF case. In this overview, we also discuss about the comprehensive management, from tet spell management, palliative management of TOF that required shunt with various technique, and surgical total corrective of TOF with many approach such as transannular accompany with right ventriculotomy or transatrial with transpulmonary or beating heart surgery technique to prevent reperfusion injury complication.

Abstract

Background: Depression is a common mental disorder characterized by constant sadness and loss of interest in activities that are usually liked, accompanied by an inability to carry out daily activities. The Elderly group experiences depression due to physical changes and environmental social conditions. Other risk factors that can cause depression in the elderly include biological factors, marital status, and psychosocial factors. Biological and psychosocial factors have a relationship with changes in the levels of hormones and neurotransmitters in the body. Changes in hormone levels and neurotransmitters can cause changes in blood pressure. Blood pressure is normal if the systolic blood pressure is less than 120 mmHg and the diastolic blood pressure is less than 80 mmHg and high blood pressure if the systolic blood pressure is more than 140 mmHg and the diastolic blood pressure is more than 90 mmHg. Objective: To determine the relationship of depression to blood pressure in the elderly at the Tresna Werdha Social Home Khusnul Khotimah Pekanbaru in 2018. Research Methods: The method used in this study is analytic observation with a cross-sectional design. The sampling technique used the total sampling method, where in this study, the population was a sample, but after the exclusion, the number of samples obtained was 41 respondents. Data collection was carried out by observations made by interviewing respondents using questionnaires and blood pressure measurements using a Sphygmomanometer and stethoscope. Results: Obtained p values (Sig.) Systolic blood pressure and diastolic blood pressure showed several 0.021 and 0.019. This p (Sig.) The value is small of 0.05, and this indicates a significant relationship of depression to blood pressure in the elderly at the Tresna Werdha Khusnul Social Home Khotimah Pekanbaru in 2018. Conclusion: There is a significant relationship between blood pressure depression in the elderly at PSTW Khusnul Khotimah Pekanbaru in 2018.

Abstract

Background: Chronic complications of diabetes mellitus have a significant role in increased morbidity, mortality, disability, and health cost as the population increases every year. Promotive and preventive actions are needed to decrease the prevalence of peripheral diabetic neuropathy. The screening tool for peripheral diabetic neuropathy is Michigan Neuropathy Screening Instrument (MNSI), which consists questionnaire and physical examination.Objective: The purpose of this study was to understand the association of the duration of type 2 diabetes mellitus with the prevalence of peripheral diabetic neuropathy in the Outpatient Unit of Gotong Royong Hospital Surabaya. Method: A cross-sectional study was done using 50 patients in the Outpatient Unit of Gotong Royong Hospital Surabaya. The sampling technique used consecutive sampling with filling out the Michigan Neuropathy Screening Instrument (MNSI) questionnaire. The score of the MNSI questionnaire is analyzed using Chi-Square Test.Result: Among 50 subjects who participated in the study, most were women (78%). The prevalence of patients with peripheral diabetic neuropathy was 28% from all subjects. This chronic complication is divided into two groups based on the duration of diabetes mellitus type 2 with the prevalence of peripheral diabetic neuropathy with a duration of diabetes <5 years (18,2%) and peripheral diabetic neuropathy with the duration of diabetes more than equal to five years old (35,7%). From the analysis with Chi-Square, we did not find a significant association between the duration of type 2 diabetes mellitus and peripheral diabetic neuropathy (p = 0.004).Conclusion: There was no statistically significant association between the duration of type 2 diabetes mellitus and the prevalence of peripheral diabetic neuropathy.

Abstract

Background: Aging is synonymous with decreased body function due to natural factors (aging process) and diseases. The elderly who are unable to do their activities can cause dependence on the caregiver. In carrying out its role, the caregiver has burdens that can affect their life quality. Objective: To measure the correlation between the burden and the quality of life in caregivers of dependent elderly in Prawirodirjan Sub-district, Gondomanan District, Yogyakarta Municipality.Materials and Methods: This study used a cross-sectional method with a total sample of 33 participants. Caregiver burden was measured using the Zarit Burden Interview (ZBI) and Caregiver Reaction Assessment (CRA) questionnaires. Measurements of quality of life were conducted using the WHO QoL BREF questionnaire. Spearman rank correlation analysis was administered to conduct statistical analysis.Results: There were very significant negative correlations between the caregiver’s burden (ZBI) and WHO QoL BREF physical health domain, psychological domain, social relations domain, and environment domain. There were very significant positive correlations between the caregiver’s burden self-esteem caregiver’s domain (CRA) and WHO QoL BREF psychological domain. There was a significant negative correlation between the caregiver's burden of the family support domain (CRA) and WHO QoL BREF physical health domain, psychological domain, social relations domain, and environment domain. There was a significant negative correlation between the caregiver’s burden impacts on the financial domain (CRA) and WHO QoL BREF environment domain. There was a very significant negative correlation between the caregiver’s burden of daily schedule impact domain (CRA) and WHO QoL BREF environment domain as well as there was a very significant negative correlation between the caregiver’s burden of health impact domain (CRA) and WHO QoL BREF physical health domain.Conclusion: There is a significant correlation between burden and the quality of life of dependent elderly caregivers in Prawirodirjan Village, Gondomanan District, Yogyakarta Municipality. The higher the burden of a caregiver, the lower the caregiver's quality of life, and conversely.

Abstract

Introduction: Dementia is a chronic and progressive intellectual function syndrome in the elderly. According to WHO, the prevalence of dementia in the world in 2018 reached 50 million people and is estimated to reach 152 million in 2050. Rehabilitation is an effort to minimize disturbances in the life quality of dementia patients. However, The COVID-19 pandemic has emerged as a new obstacle in providing rehabilitation services for dementia patients. Telerehabilitation is very useful in helping rehabilitate dementia patients in the era of the COVID-19 pandemic.Purpose: The technology use of telerehabilitation in geriatrics dementia is potentially improving medical access to deliver rehabilitation therapy of pandemic COVID-19.Method: This study was conducted by literature review from Science Direct, PubMed, and The American Journal of Geriatric Psychiatry library database, with search terms: "Telemedicine," "Telerehabilitation," "Dementia," "Geriatric" by Boolean search operators. There are 226 records identified and screened by title and abstract. Their studies are finally fulfilled our criteria and will be reviewed.Result: We examined three studies and presented qualitative and quantitative scores for two telerehabilitation methods. Two methods are commonly used for video conference and virtual reality. Video conferences are still the most capable method for dementia telerehabilitation.Conclusion: The technology use of telerehabilitation in geriatrics dementia is potentially improving medical access to deliver rehabilitation therapy to patients and caregivers at a distance during this pandemic.

Abstract

Background: Postpartum bleeding is one of the main causes of mortality in mothers in 2017. Based on Profil Kesehatan Provinsi Jawa Timur's data in 2017, about 28,26% of mothers' mortality percentage is because of postpartum bleeding. Some of the factors affecting the duration of puerperium bleeding are the initiation of early breastfeeding and exclusive breastfeeding. Early breastfeeding initiation can stimulate the oxytocin hormone to contract the uterine and help complete the extrusion of the placental site and sloughing of decidua tissue (lochia). Objective: This study aims to know the correlation of 2 months full of breastfeeding on early initiated breastfeeding group with puerperium bleeding. Method: This study is an observational analytical study where the design is cohort retrospective, and the 60 samples used in this research are taken with simple random technique sampling. This research was carried out by collecting information from secondary data, and the whole process was done in Gotong Royong Hospital Surabaya from July to September 2020. This research used the Mann-Whitney U test to analyze and find the correlation of both variables. Results: Based on the statistic test, there is no correlation between both variables, resulting in p = 0,211. Conclusion: there is no significant correlation between 2 months full of breastfeeding on the early initiated breastfeeding group with puerperium bleeding.

Abstract

Introduction: Nutritional problems in pregnant women in Indonesia reach 50%, this is still a concern. Based on Basic Health Research (Riskesdas) 2018, 17.5% CED pregnant women in Central Java. Mid-Upper Arm Circumference (MUAC) is an examination to determine nutritional status of pregnant women. Poor nutritional status can cause anemic neonates. According to research at RSUP Dr. Hasan Sadikin Bandung 2018, prevalence of anemic neonates is 14.5% which is still a problem.Objectives: This study aimed to determine relationship between MUAC in third trimester and incidence of anemic neonates.Methods: This study used a cohort study of pregnant women and neonates at Dr. Oen Solo Baru. The sampling technique was purposive sampling. The samples were 80 respondents with the independent variable being MUAC in third trimester and the dependent variable being anemic neonates. Data were analyzed using Spearman's correlative test.Results: In 40 (50%) respondents with normal MUAC which is 23.5 – 28.5 cm, there were 31 (77.5%) normal haemoglobin (Hb) neonates and 9 (22.5%) anemic neonates. In 40 people (50%) were malnourished which is <23.5 cm, there were 13 (32.5%) normal Hb levels neonates and 27 (67.5%) anemic neonates. Based on statistical tests, there is a relationship between third trimester MUAC and incidence of anemic neonates with p = 0.000 (<0.05) and the correlation coefficient value is 0.452 which indicates a sufficient correlation.Conclusions: The conclusion is there is a relationship between MUAC for pregnant women in third trimester and incidence of anemic neonates.

Abstract

Background: The elderly have a higher risk of developing hypertension. The elderly will experience a decrease in physical activity due to a decrease in stamina and body strength due to aging. This situation can increase the risk of non-communicable diseases such as coronary heart disease (CHD) and affect blood cholesterol levels in the body. Objective: This study aimed to understand the correlation of physical activity intensity with the ratio of total cholesterol to HDL in hypertension elderly at Puskesmas Gedangan Sidoarjo.Methods: This study is an analytic study with the research subjects elderly with hypertension at the Puskesmas Gedangan Sidoarjo with a cross-sectional study design. Using Spearman's correlative test with the Statistical Product and Service Solution (SPSS) program, data analysis was carried out. Results: There is a negative correlation between physical activity and the ratio of total cholesterol to HDL in the elderly with hypertension with p = 0.004 (<0.05) and the correlation value (τ) = -0.424, which indicates a sufficient correlation value. Of 45 respondents, 11 people (24.4%) had low physical activity intensity, 21 people (46.7%) had moderate physical activity intensity, 13 people had (28.9%) heavy physical activity intensity, 29 people (64.4%) had a total cholesterol/HDL ratio ≤ 5, and 16 people (35.6%) had a total cholesterol/HDL ratio >5. Conclusion: There is a negative correlation between physical activity intensity and the ratio of total cholesterol to HDL in the elderly with hypertension at Puskesmas Gedangan Sidoarjo, which means the higher the intensity of physical activity carried out by the hypertensive elderly, the lower the total cholesterol/HDL ratio.

Abstract

Introduction: Implementing infection prevention protocols during the COVID-19 pandemic in Indonesia hinders the public from accessing health care services. This condition causes virtual health services to become more accessible than conventional health services, thus encouraging healthcare providers to adopt telemedicine as a method of service delivery. However, as a relatively new technology in Indonesia, public acceptance is crucial to predict the level of public willingness to adopt telemedicine. Yet, a little study in Indonesia has tried to assess it. Purposes: The purpose of this research is to measure the acceptability of telemedicine services in Jakarta metropolitan area, the factors that determine it, and whether the level of acceptance correlates with the usage frequency of telemedicine. Methods: Data used in this study is acquired with an online survey from February to March 2022, targeting residents of the Greater Jakarta area. Instruments used in this survey are demographic data form and SUTAQ questionnaire translated into Indonesian. Of the total 454 respondents, 200 have used telemedicine in the last six months. Respondents were then divided into several groups based on age, gender, monthly income, education level, chronic conditions, health insurance membership, and frequency of telemedicine use. Bivariate analysis was conducted using chi-square test for categorical data and t-test for numerical data. Results: The assessment of the SUTAQ scores indicates good acceptance of telemedicine in all demographic groups. Increased accessibility to health services and satisfactory user experience have contributed to its good acceptance. However, we also found that the frequency of use was not correlated with the level of acceptance, but positively correlated with the presence of chronic conditions. Conclusions: Telemedicine is well received by users who live in the Greater Jakarta area. Users find it easier to access health services and are satisfied with the health services provided through telemedicine.

Abstract

Background: In the elderly, there are anatomical, physiological, and psychological changes that can result in reduced sleep time in the elderly, and are thought to affect the quality of life of the elderly.Objective: This study aimed to determine the relationship between sleep time and quality of life in the elderly.Methods: This study used an observational analytic method with a cross-sectional approach. The sample in this study was 35 elderly people in East Manding Village. The sampling technique used was purposive sampling by determining the inclusion and exclusion criteria. The tool used is the WHOQOL-BREF test to measure the quality of life of the elderly and uses the sleep time provisions from the Ministry of Health (Ministry of Health) to measure sleep time in the elderly.Results: Data analysis used a contingency correlation test with a significance value of p<0.05. The correlation between sleep time and quality of life in the physical condition domain was positive, moderate, significant (r = 0.508 with p = 0.000); with the domain of psychological condition is positive, moderate strength, significant (r = 0.408 with p = 0.03); with the environmental factor domain being positive, moderate in strength, significant (r = 0.510 with p = 0.002); while in the domain of social relations there is no significant correlation.Conclusion: Sleep time is positively, moderately, and significantly correlated with quality of life in the domains of physical conditions, psychological conditions, and environmental factors. This means that the higher the sleep time, the better the quality of life in physical, psychological, and environmental conditions. These findings contribute to science and health promotion in geriatrics for the welfare of the elderly.
