Abstract

Coronavirus Disease 2019 (COVID-19), which is caused by the SARS Coronavirus 2 (SARS-CoV-2), has affected over 200 countries worldwide. First case of COVID-19 wasfound in Wuhan, China, precisely around December 2019. COVID-19, especially in thosewith underlying health conditions or comorbidities, has an increasingly rapid and severeprogression, often leading to death. This virus is a single-strand RNA coronavirus, binding the angiotensin-converting enzyme 2 (ACE2) receptor which enters human cells. Coronavirusdisease has been reported to affect cardiac and vascular organs. cardiomyocyte death andinflammation are results of a direct mechanism that involves viral infiltration into myocardialtissue. Some cardiovascular manifestations of myocardial injuries associated with COVID-19are arrhythmias, myocardial infarction, heart failure, and elevated biomarkers (cardiactroponin I and brain-type natriuretic peptide). Some of this manifestation requires immediate intervention or surgery. Criteria are needed for hospitals or institutions that mostly maintain cardiac surgery services and surgery-urgent patients depending on severity of the disease and hospital resources. These criteria also limit the risk of exposure to patients and healthcare workers and allocate resources appropriately to those in greatest need. This paper aims to share our discussion and give an overview of patients undergoing cardiac surgery, providing clinicians with recommendations to triage and plan these procedures during the COVID-19 outbreak effectively.

Abstract

Introduction: The incidence of acute and chronic injuries increases every year. Most injuriesin the world population, as many as 48%, are caused by trauma or surgery. the highestincidence of wounds in the world based on the etiology was from surgery, which was 110.30million cases. One of the handlings of wounds on the skin can be done by giving herbalmedicines, which can be easily accessed by the public, such as binahong leaves and papayaleaves. Purpose: To analyze the effectiveness of giving 40% binahong leaf extract cream and 40%papaya leaf extract cream on the wound distance in the white rat wound healing process. Method: this study is an experimental study with post-test only control group designapproach. The sampling technique used in this study was a random sampling method wherethe number of samples was calculated using the Federer formula, with a total sample of 28white rats. The distance of the wound was measured using a micrometer. Results: Twenty-eight wound distance on rats were measured as subjects of the study andthen treated with binahong leaf extract cream and papaya leaf extract cream, then the wounddistance of each treatment was measured using a micrometer. According to the results, 40%binahong leaf extract cream and 40% papaya leaf extract cream showed better effectivenessin reducing the wound distance of white rat incision than the negative control group (creambase) and positive control group (gentamicin ointment). According to the Mann Whitney testcomparing the P1 group (40% binahong leaf extract cream) and P2 (40% papaya leaf extractcream), p-value >0.05, which is 0.543, which means that P1 has no significant meandifference with P2. Conclusion: There was no significant difference in effectiveness between the treatment of40% binahong leaf extract cream and 40% papaya leaf extract cream.

Abstract

A novel coronavirus pneumonia outbreak began in Wuhan, Hubei Province, in December 2019. It has spread rapidly from China to worldwide. The COVID-19 pandemic of 2020 posed a historic challenge to health care systems around the world, healthcare systems needed to consider providing clinical services to other patients in need. The specialty of general thoracic surgery includes the management of serious diseases such as chest wall congenital anomaly, lung cancers, esophageal cancers, mediastinal tumor, chest wall infection, and trauma. Thoracic surgery is one of high risk procedure in this pandemic, however, a high level of evidence supports the surgical management of potential patients with thoracic disease and anomaly is still lacking. Critical determinants of robust thoracic surgery service provision are pre-existing plans for epidemic response. "Flatten the curve" as an aggressive action is needed. Prioritizations of thoracic surgery cases are needed to differ between elective and emergency cases to limit any contamination. Before surgery, important pre-operative assessments were conducted aims to identify patients' high risk and adjust the procedure. example of a recommendation, if urgent / emergency surgery with symptoms clear clinical pneumonia or rapid test (+). The lessons learned can apply to the other areas during this pandemic, and the world, in preparation for the next one.

Abstract

Introduction: According to data from the Department of Health (2018) , infant mortality rate(IMR) in the first semester was as many as 10.294 cases, and the maternal mortality rate(MMR) in the first semester was as many as 1712 cases. Continuity and Duration of labor areessential factors in determining the number of IMR and MMR. Several factors affecting laborduration are power, passenger, passage, and maternity mother psychological. Purpose: To understand the correlation between primipara mothers' anxiety levels with thelength of labor progress in the first active phases. Method: This research used observational analytic method with a cross-sectional design anda purposive sampling technique. The research period was two months, with total respondentswere 40 people. Statistical analysis was done using Spearman rank correlation test. Results: 40 pregnant primipara mothers underwent labor at Mataram City CommunityHealth Centre in June - August 2019. We found about four respondents have heavy levels ofanxiety, in which all of them had no progress in delivery labor. Based on the statistical testsusing the spearman rank, there is a strong, positive (Ï€ = 0.714) and significant correlation (P =0,000) between anxiety level of primipara mothers with the length of labor progress in thefirst active phase. Conclusion: There is a significant correlation between primipara mother anxiety levels withthe period length of labor progress in the early active phases at Babakan community healthcenter Mataram City. Researchers have shown that the higher the anxiety level, the longer thelabor progress, and vice versa. This result must be explored further because of anxiety thataffects mothers' condition during labor and affects uterine contractions and cervical dilatationthat determine the smooth labor process.
