Abstract

Background: Uric acid can be used as a biomarker to distinguish transudates from pleural fluid exudates. Uric acid is soluble in liquid media, elevated levels of uric acid in blood plasma become a predisposing factor for uric crystal deposits in tissues and body fluids. Increased permeability due to changes in capillary pleural pressure in the formation of transudates is a cause of increased levels of uric acid in pleural fluid. The aim of the study was to analyze the examination of pleural fluid uric acid levels and the ratio of uric acid levels to pleural fluid to serum to distinguish exudates and transudates in pleural effusion. Methods: A cross sectional diagnostic test with a cross sectional study design in pleural effusion patients at RSUD Dr. Moewardi Surakarta in July-August 2019. Subjects were examined for pleural fluid levels, serum uric acid ratio, and pleural fluid analysis. Results: A total of 30 pleural effusion patients were included in this study. The level of agreement between the uric acid pleural fluid against Light criteria was in the moderate category and statistically significant (Kappa = 0.595; P=0.001). The level of agreement between the ratio of uric acid to pleural fluid and serum to Light's criteria was weak and not statistically significant (Kappa = 0.200; p = 0.195) Conclusion: The level of concordance between pleural fluid uric acid with Light criteria is in the medium category and statistically significant. There is no match between the ratio of serum pleural fluid uric acid levels with Light criteria. (J Respir Indo. 2020; 40(2): 82-6)

Abstract

Backgrounds: Type 2 diabetes mellitus (DM) is a common comorbidity of COPD. COPD may be considered as a risk factor for new onset type 2 DM via multiple pathophysiological alterations such as systemic inflammation, smoking, oxidative stress, obesity and inhaled corticosteroid use. Exact prevalence of DM in COPD patients in Indonesia still unclear. Co-morbid conditions like DM have great impact on the outcome of COPD in the form of severity, morbidity and mortality. Methods: A cross sectional study was done in Asthma-COPD clinic Persahabatan Hospital from February to March 2017 to screen COPD patients for DM. Sixty-four subjects were recruited. Interview, physical examination and laboratory testing were performed in all subjects. Results: A total of 64 patients with COPD (Males=60, Female=4) with mean age 65±8.7 were screened for DM. Patients with known history of DM were 12 subjects (18.8%) and were enrolled as known DM cases. Remaining 52 subjects (81.3%) whose DM status was unclear were screened by random or fasting blood sugar and HbA1c. Two subjects (3.1%) were considered as newly diagnosed DM cases. Prevalence of DM in present study was 21.9%. Number of patients with pre-diabetes was 16 subjects (25%). There was no significant relationship between gender, age, smoking, nutritional status, airflow limitation and inhaled corticosteroid use in occurrence of DM among COPD patients. Conclusions: Prevalence of DM in COPD patients in the present study is 21.9%. It is important to screen all COPD patients for DM routinely. (J Respir Indo. 2020; 40(1): 48-57)

Abstract

Background: Pneumonia is an infectious disease with highest morbidity and mortality in the world. The research objectives is to determine the effect of Nigella sativa cement extract in procalcitonin, TNF-α levels, and clinical improvement in pneumonia patients. Methods: This study is a clinical trial quasi experimental design with pre and post test approach in the treatment group and the comparison group. Samples were taken by consecutive sampling between April and May 2016 a total of 30 patients were divided into 2 groups:the treatment group and the comparison group. The treatment group was given additional therapy capsules Nigella sativa cement extract, the comparison group was given the standard therapy. Procalcitonin levels and TNF-α is checked when the patient was admitted and when clinical improvement is achieved. Results: Decreased levels of procalcitonin in the treatment group (8.969±13.591 ng/dl) and a comparison group (1.907±6293 ng/dl) was statistically significant (p= 0.014; 95 % CI= 0.222-1.770). Decreased levels of TNF-α was not significantly different between treatment groups (58.759(1.840-166.50) pg/dl) and a comparison group (57.485 (35.410-81.650) pg/dl), with p= 0.395. The achievement of clinical improvement in the treatment group 4.60±8.28 days and the comparison group 5.53 ±1.45 today, statistically no difference between the two groups. Conclusion: Capsule Nigella sativa cement extract provides benefit for declining levels of procalcitonin and accelerate achievement of clinically stable in pneumonia patients. Abstract

Backgrounds: People who work in the bird market have a high exposure risk of air pollution in large numbers, particulate pollutans including organic dust, loose feathers, insects or ticks, food aerosol particles, bird’s excreta (amonia) and various of gram bacteria, fungal and virus. Exposure to particles will stimulate the immune system against harmful pathogens in the form of an inflammatory response. An infection or injury will stimulate secretion of KL-6/MUC1 (Krebs von den Lungen-6 is a high molecular weight, mucin-like glycoprotein in human (MUC1)) by AEC type II and bronchial epithelial cells in response to cell damage and regeneration. Methods: This was an observational analytic cross-sectional study on 35 subjects, which examined and analyzed the characteristics of workers, percentage count of neutrophils, lymphocytes in induced sputum and KL-6/MUC1 serum levels using ELISA sandwich. Results: Mean KL-6/MUC1 serum levels in the bird market workers increased (1152.67+583.92 U/ml) from normal levels (105.3-401.2 U/ml). Abstract

Backgrounds: Tuberculosis (TB) is still a public health problem in the world.Vitamin C is a water-soluble micronutrient, protects the host from reactive oxygen and reactive nitrogen intermediates generated during mycobacterial infection, involved in the synthesis of collagen, iron transport, and acts as physiological antioxidant. Vitamin C plays role in killing Mycobacterium tuberculosis by fenton reaction. This study was to determine the effect of vitamin C supplementation on chest X-ray (CXR) improvement in pulmonary TB patients during intensive phase. Methods: This was a quasy experimental study with single blind-randomized controlled trial design. Subjects were divided into 2 groups, first group was pulmonary TB patients treated with anti tuberculosis drugs (ATD) plus vitamin C and the second group was treated with ATD plus placebo. This study was conducted in several public health center in Medan City. The subjects were pulmonary TB patients with positive AFB sputum in intensive phase from December 2017 to May 2018. Data were analized using t-test dependent. Results: From 80 total subjects, 53 males (66%) and 27 females (34%). After 2 months of treatment, improvements in CXR images were found in the ATD plus vitamin C group, about 21 subjects (52.5%) had no more lesions on CXR, while the same condition was found on 15 subjects (37.5%) in the placebo group. Abstract

Backgrouds: Multi-drug resistance tuberculosis (MDR-TB) adds the burden of tuberculosis (TB). Depression is a common comorbidity in TB patients. Prevalence of depression among MDR-TB patients was higher, i.e. 11-70%. Prevalence of depression among TB patients in Dr. Kariadi General Hospital was 51.9%. Previous study reported the determinants related to the depression in MDR-TB patients. This study explored the prevalence of depression in MDR-TB patients and its determinants. Methods: This study was a cross-sectional study conducted from July-August 2019. Data consisted of primary and secondary data from Dr. Kariadi General Hospital patients. Sample in this study involved 72 respondents, over 16 years old from 151 registered MDR-TB patients. Depression status was determined using Depression Anxiety Stress Scales obtained from the medical record. Abstract

Backgorunds: Mediastinal germ cell tumors are relatively rare. These tumors can be benign or malignant, which has a poor prognosis malignant. Nowadays, multimodality therapy can improve the survival rate of patients’ mediastinal germ cell tumors. This study aims to determine the characteristics and factors affecting the 1-year survival rate of mediastinal germ cell tumors patients in the Persahabatan Hospital, Jakarta. Methods: The study was conducted using retrospective cohort of mediastinal germ cell tumors patients medical records in Persahabatan Hospital, Jakarta from January 1, 2007 to December 31, 2012. Data obtained were analyzed using Kaplan Meier analysis of survival. Results: As much as 60 subjects participated in this study. Mediastinal germ cell tumors were found more in male (80%) with a median age of 21 years and especially in the age group of 20-29 years (43.3%). Most clinical symptoms found were shortness of breath (66.7%), most patients came with performance status (PS) 2 (50%). The majority of the mediastinal germ cell tumors found were teratomas (53.3%), followed with nonseminoma (40%) and seminomas (6.7%). Factors affecting the survival rate of mediastinal germ cell tumors were performance status of, the patients, treatment, complications and location of the tumors. One-year survival rate of mediastinal germ cell tumors in this study was 41.67% and affected by patients’ performance status, tumor location, complications and treatment. Conclusion: Mediastinal germ cell tumors were mostly found in male, with the chief complain of dypnea and mostly was teratoma. Mediastinal germ cell tumors have high mortality rate. The 1-year survival was influenced by patient’s performance status, tumor location, complications and treatment. (J Respir Indo. 2020; 40(2): 97-102)

Abstract

Backgrounds: Most patients with lung cancer was diagnosted in advanced stage (around 57%). Those diagnosed at early stage were only 15%. To increase the cure rate and life expectancy, lung cancer detection should be performed early. Melanoma-associated antigen 3 (MAGE-A3) is a testicular cancer antigen and is widely expressed in various types of tumor tissue. MAGE-A3 is expressed in about 35-40% of NSCLC. Previous research reported the expression of MAGE-A3 in lung cancer was 30-50%. To detect tumor antigens, CT antigens of the MAGE family can be detected through real time polymerase chain reaction (RT-PCR). This study aimed to analyze the relationship between MAGE A3 expression and histopathology type from forceps biopsy specimens in NSCLC patients. Methods: This study was a cross-sectional study conducted on 14 patients with lung cancer in September 2018 to February 2019. Subjects underwent force biopsy with guidance from bronchoscopy in the pulmonary surgery room Dr. Soetomo. The analysis used was the Fisher's exact test. Results: There were six subjects with histopathology type of adenocarcinoma (42.9%) and eight subjects (57.1%) with the histopathology of squamous cell carcinoma. Expression of MAGE-A3 was positive in 5 subjects (35.7%). Conclusions: There was no significant relationship between the expression of MAGE-A3 and the type of histopathology. Abstract

Backgrounds: Professional divers have a greater lung volume than non-professional divers in higher force vital capacity (FVC) and force expiratory flow in 1 second (FEV1) values. The purpose of this study is to analyse the effect of pressure and duration of diving on changes in lung physiology in professional divers and non-professional divers. Methods: This is a cross sectional study on the personnel of the Indonesian Navy Dislambair Koarmada II Surabaya and RSAL personnel of dr. Ramelan Navy Hospital in Lakesla Surabaya in March-April 2019. Samples was collected with purposive sampling. This study used unpaired subjects with the independent t-test statistic analysis if the data is normally distributed and the Mann-Whitney U test if the data is not normally distributed Results: There was a significant difference in the professional divers group compared to non-professional divers group in the values of FVC (P=0.042) and FEV1 (P=0.040) at 1.3 ATA for 10 minutes and the FEV1 (P=0.049) for 20 minutes. No significant differences in FVC (P=0.092) at 1.3 ATA pressure for 20 minutes. There were no significant differences in FVC (P=0.865), FEV1 (P=0.659) at 1.5 ATA pressure for 10 minutes and FVC (P=0.858) and FEV1 (P=0.857) for 20 minutes. Conclusions: The different pressure in non-professional group could reduce the FEV1 value. There was a difference in the FVC value of the professional diver group and non-professional divers at a pressure of 1.5 ATA for 10 minutes. (J Respir Indo. 2020; 40(2): 103-12)
