Abstract

BACKGROUND: Sarcomatoid bladder carcinoma is scarcely found, with most cases of bladder carcinoma due to bladder stone as a risk factor consisting of squamous cell carcinoma. Only 221 cases were reported between 1973 and 2004 of sarcomatoid bladder cancer.  Therefore, this case report brings out sarcomatoid bladder carcinoma and its clinical relevance. CASE DESCRIPTION: A 58 years old Indonesian man experienced gross hematuria for 3 months, and thus finally brought to the Emergency Room (ER) where he was initiated urinary catheter and given intravenous (IV) fluid along with analgesic parenterally. Emergency abdominal radiograph assessment and blood test were made. Abdominal radiograph assessment suggests a 6 x 6 cm opacity in the bladder and several urolithiasis in the pelvic cavity. Ureum and creatinine increased in the blood test procedure. Based on the characteristic appearance and radiological findings, a diagnosis of vesicolithiasis was made. Following the diagnosis, the patient was urgently operated on (Open vesicolithotomy), and during the stone removal, there was a tangled polyp-like mass found buried underneath the stone. Histological findings of mass and surrounding lymph nodes were made and histopathologically stated the mass was sarcomatoid carcinoma of the bladder and it had metastated to lymph node and pelvic region. CONCLUSION: Considering hematuria in urolithiasis is frequent, the clinician should be aware of any tumor findings especially when the hematuria is massive thus emergency urolithotomy should be considered immediately.

Abstract

Background Handgrip strength and endurance are some of the ways that can be done to examine the muscle quality of adolescents as well as their physical fitness. It is influenced by various internal and external factors, including body mass index, gender, age, and smoking habits. Handgrip strength can be an important parameter for health. Adolescents with lower muscle strength have an increased mortality rate in adulthood. Thus, this study aimed to determine the factors associated with handgrip strength and endurance in adolescence. Methods The study used analytic observational with a cross-sectional approach using consecutive non-random sampling on 58 adolescents between 15-23 years old. The instruments used a scale and microtoise to measure body mass index, and a hand dynamometer to measure handgrip strength and endurance. The data was analyzed by the Chi-square test using the SPSS program with p<0.05. Results Of the 58 respondents consisting of 48.27% men, and 51.72% women, there were 58.62% normal BMI and 70.68% did not smoke. The percentage of respondents who had weak grip strength was lower (44.82%) compared to weak grip endurance (55.17%). There was no significant relationship between gender and handgrip endurance; body mass index and handgrip strength as well as handgrip endurance; cigarette consumption and handgrip strength. Moreover, there was found a significant relationship between gender and handgrip strength (p=0.000) and between cigarette consumption and handgrip endurance (p=0.011). Conclusions Handgrip strength and endurance were more in the weak-medium category. There was no significant relationship between gender and handgrip endurance; body mass index and handgrip strength as well as handgrip endurance; cigarette consumption and handgrip strength. Moreover, there was found a significant relationship between gender and handgrip strength and between cigarette consumption and handgrip endurance.

Abstract

The high mortality rate among COVID-19 patients in the acute respiratory distress syndrome (ARDS) phase led to the administration of immunosuppressive drugs. Corticosteroids could block inflammation caused by cytokine storm, and prevent pneumonia, edema, fibrosis, and ARDS. Even though it was believed to have beneficial effects, corticosteroids can suppress T CD4+ and CD8+ cell-mediated immunity reaction through decreased IFNγ production thus leading to reactivation of latent Tuberculosis (LTBI). Therefore, the usage of corticosteroids in the ARDS phase of COVID-19 patients should be carefully given; pre-screening of LTBI may be done to avoid Tuberculosis reactivation.

Abstract

The use of marijuana/cannabis for therapy, especially for certain neurological diseases, is still causing controversy in various countries, including Indonesia. Various studies continue to be carried out while certain parties feel they have the right to get treatment through marijuana based on testimonials and previous research. The urge to legalize the use of marijuana as an alternative therapy is getting tougher, including in Indonesia. Judging from medical ethics, therapy can be applied if its use is following medical indications, the benefits outweigh the risks, its use respects human dignity, and has been clinically tested with scientific research. This research used literature studies from various references to analyze the medical and ethical aspects of using marijuana as a therapy. Potentially Cannabidiol in cannabis can be an alternative therapy, provided that it has passed the correct and ethical scientific research, which is not yet complete. Thus, the legalization of the use of cannabis/marijuana as a therapy still requires stronger and more valid scientific evidence so that the use of cannabis does not reduce human dignity, especially patients as vulnerable people under the pretext of humanity.

Abstract

Background Anesthesiologists in Indonesia are still in high demand. The large number of patients and the heavy workload faced by residents are accompanied by quite high pressure from various aspects, this can cause anesthesia resident participants to experience fatigue so they are prone to a syndrome called burnout syndrome. This study aims to determine the relationship between fatigue and burnout syndrome among residents in the Anesthesiology and Intensive Therapy Department, Faculty of Medicine, Universitas Sumatera Utara. Methods This study was a cross-sectional study on 106 residents that was performed using a questionnaire consisting of characteristics of study samples, translated Fatigue Assessment Scale (FAS), and Maslach Burnout Inventory-Human Service Survey (MBI-HSS). Results From the analysis, it was discovered that 50% of the residents didn’t experience fatigue with a median score of FAS 21.5, and 84.9% of the residents didn’t experience burnout syndrome with a median score of MBI-HSS was 51,84 ± 19,59. There was a significant relationship between FAS and total score of MBI-HSS, emotional exhaustion, depersonalization, and personal achievement with p-value <0.001. Conclusions There was a significant relationship between FAS and total score of MBI-HSS, emotional exhaustion, depersonalization, and personal achievement.

Abstract

Background Learning concentration is the main capital for students to receive teaching materials and indicators of the success of the learning process. One factor that contributes to study concentration is good sleep quality. The learning process in the technological time involves the use of many devices which will indirectly increase screen time and ultimately be considered to affect learning concentration. This study aimed to assess the relationship between sleep quality and screen time with the learning concentration of high school students. Methods This cross-sectional study was conducted in Serang involving 185 high school students aged 16-18 years who had good verbal and non-verbal communication skills  Sample selection is done by Consecutive Non-Random Sampling. Students who were under several conditions such as unpleasant psychological conditions (loss of family members, and loved ones), mental disorders, and psychotropic medication would not participate in this study. Data were taken directly with a PSQI questionnaire to assess sleep quality, The Army Alpha Test was used to measure study concentration, and a screen time questionnaire to calculate screen time. Data was processed and analyzed with SPSS version 22 with the Chi-Square test used to see the meaning of variable relationships. Results Out of the 185 students studied, 153 (82.7%) students had poor sleep quality and 103 students (55.7%) had high screen time exposure (56 hours/week). However, it was found that 83.2% of the students had good learning concentration. Chi-square test results ≥ showed no relationship between sleep quality and screen time with learning concentration (p>0.05). Conclusions There was no relationship between sleep quality and screen time and high school students learning concentration.

Abstract

Background Dry eye syndrome (DES) is the most common eye disease with a prevalence of around 5 – 50% of the worldwide population. In Indonesia alone, the prevalence of DES is around 27.5%. DES is a multifactorial disorder indicating that inflammation plays an important role in the pathophysiology of DES. The inflammation causes increased oxidative stress which can cause decreased tear production and inflammation on the ocular surface can cause an increase in excessive tear evaporation, causing instability of the tear film. Vitamin D has an anti-inflammation effect. The role of vitamin D in the eye through receptor vitamin D (VDR), which is expressed in the retinal pigment epithelium, retinal photoreceptors, ganglion cells layer, ciliary body, lens, cornea epithelium, and endothelium, as well as in aqueous humor, vitreous humor, and tears film. Vitamin D deficiency is said to have a contribution to DES. For that reason, this study aims to examine the relationship between vitamin D levels with DES. Methods The research design in this study was an observational analytic with a cross-sectional approach, which was followed by FK USAKTI employees in February – March 2021. Fifty-seven employees aged 25 – 60 were according to criteria and willing to take part in this study. OSDI questionnaire, Schimer’s test and tear break-up time (TBUT) were used to examine DES. Examination of vitamin D by measuring 25(OH)D levels. The data were analyzed using a chi-square test with significance level p<0.05. Results The vitamin D levels examination with DES using Schimer’s test was found to be unrelated (p=0.948), comparing vitamin D levels with the TBUT test, it was found significantly related to DES (p=0.030), meanwhile using the OSDI questionnaire was found to be unrelated (p=0.285). Conclusions There is a significant relationship between vitamin D levels and DES through TBUT examination.

Abstract

Background Elders experience an aging process that is marked by a decrease in muscle mass that can result in a decrease in overall muscle strength.  Handgrip strength is a parameter used to assess maximum handgrip which can represent an overall muscle strength. Magnesium has a role for skeletal muscles and also affects muscle performance. This study aims to measure the relationship between magnesium intake and handgrip strength in the elderly. Methods This study is an observational analytical study with a cross-sectional design, involving elders aged  60 years in elderly nursing homes as the study subjects. Elders who were able to communicate actively were included and elders who had physical disability were excluded. The Semi Quantitative Food Frequency Questionnaire (FFQ) was used for assessing magnesium intake and handgrip strength was measured using a handgrip dynamometer. Data were analyzed with Chi-Square statistical test and Fisher’s Exact Test with a significance of < 0.05. Results A group of 71 elders participated in this study. The result of this study shows that subjects with sufficient magnesium intake amounted to 18 subjects (25.4%) and 53 (74.6%) with low magnesium intake.  The distribution of subjects with sufficient handgrip strength amounted to 36 subjects (50.7%) and 35 (49.3%) with low handgrip strength.  Statistical analysis shows that there is no significant relationship between magnesium intake and handgrip strength in elders (p=0.539; p>0.05), while sociodemographic factors of age and gender showed a significant relationship towards handgrip strength (p=0.012 and p=0.028; p<0.05). Conclusions There is no significant relationship between magnesium intake and handgrip strength in elders.  Sociodemographic factors of age and gender showed significant relationships towards handgrip strength.

Abstract

Background Cigarette smoke is the main source of exposure to carbon monoxide (CO) besides air pollution. When cigarette smoke is inhaled, carbon monoxide is absorbed through the lungs, enters the bloodstream, and then binds to hemoglobin to form carboxyhemoglobin (COHb), whose levels in the blood can be measured as a marker of cigarette smoke absorption. Both smoking habits and CO levels can affect lung function, after smoking, blood CO levels will increase and lung function will decrease significantly. Methods This study used an analytic observational method with a cross-sectional design with a total sample of 99 respondents. Data collection in September-December 2022 using the Brinkman Index questionnaire, Fagerstrom Test for Nicotine Dependence (FTND) questionnaire, exhaled CO levels, and spirometry examination. Results 47.5% exhaled CO levels on 0-6 ppm or in a green zone, 77.8% of subjects smoked, 57.6% FTND score was 0-2, 51.5% of subjects had restricted lung function, and 79.5% subjects with obstruction. Exhaled CO levels were strongly correlated with the Brinkman index (r=0.654, p=0.000), and moderately significant correlation with the FTND score (r=0.544, p=0.000). There is no correlation between exhaled CO levels and degree of smoking with lung function. Conclusions Exhaled CO levels have a significant correlation with smoking and nicotine dependence but do not correlate with pulmonary function test.

Abstract

Background Basic Life Support (BLS) is the first aid required for patient cardiac arrest and respiratory arrest, especially those occurring outside the hospital. In Indonesia, approximately 300,000–350,000 people experience cardiac arrest each year, mainly on the roads due to trauma/accidents. This research aims to determine the influence of BLS training on the knowledge and skills of the general public, especially online motorcycle taxi drivers, in providing first aid for cardiac arrest. Methods This research employs a one-group pre-post test design to compare knowledge of basic life support before and after training. The sample consists of 193 individuals, with sample selection using consecutive non-random sampling, and data collection conducted through interviews using a questionnaire. Results The research findings using the Wilcoxon Signed Ranks Test yielded a P-value of 0.001, indicating the acceptance of the alternative hypothesis (Ha). This result indicates that there is a statistically significant effect of basic life support training on the knowledge of online motorcycle taxi drivers around Jakarta. Conclusions There is a significant effect of basic life support training on the knowledge of online motorcycle taxi drivers as well as increasing the ability to act on BHD  in areas around Jakarta.
