Abstract

Background: Iron supplementation in pregnant women is one of the key interventions recommended to prevent anemia and support the health of both the mother and the fetus. The increased need for iron during pregnancy makes supplementation a primary strategy in maternal health care. However, it is important to note that excessive iron intake has the potential to cause side effects such as iron overload, which can increase oxidative stress and insulin resistance in pregnant women. These conditions are suspected to contribute to an increased risk of pregnancy complications, one of which is Gestational Diabetes Mellitus (GDM). Objective: This study aims to determine the effect of iron (Fe) supplementation on the risk of gestational diabetes mellitus during pregnancy. Methods: This study employed a literature review design. Article retrieval was conducted through three databases—Google Scholar, DOAJ, and PubMed—using the keywords “iron supplementation,” “iron supplementation,” and “gestational diabetes risk.” Inclusion criteria included articles published between 2018 and 2023. The search yielded 5 relevant articles for comprehensive review. Results: Analysis of the selected articles indicates a trend toward an increased risk of GDM in pregnant women receiving iron supplementation, particularly when iron levels are already high. Therefore, the administration of iron supplements should be approached with caution if a pregnant woman has GDM. Conclusion: Iron supplementation is associated with an increased risk of GDM; therefore, its use should be tailored to the individual needs of the pregnant woman.

Abstract

Noncommunicable diseases (NCDs) are the leading cause of death globally and continue to rise, including in Indonesia. Unhealthy lifestyle changes among the population are a major factor in the rise of NCDs, necessitating effective promotive and preventive strategies. With the advancement of digital technology, health promotion has undergone a transformation through the use of digital media such as mobile health (mHealth), social media, and short message service (SMS). However, the effectiveness of digital health promotion in preventing NCDs still yields mixed results. This study aims to enhance the effectiveness of digital health promotion in preventing NCDs through a Systematic Literature Review (SLR) approach. The research method follows the PRISMA guidelines, with a literature search based on data from Scopus, PubMed, and ScienceDirect for articles published between 2020 and 2025. Inclusion criteria included Scopus-indexed journals discussing digital interventions in health promotion, while articles in the form of opinions, editorials, and studies without empirical data were excluded. A total of 15 articles met the criteria and were analyzed narratively. The results indicate that digital health promotion is consistently effective in increasing public knowledge and awareness regarding NCD prevention. However, its impact on changes in health behavior remains variable. Mobile health (mHealth)-based interventions demonstrated higher effectiveness in increasing physical activity and reducing body mass index (BMI), while SMS-based interventions were shown to improve adherence to healthy eating patterns. In general, digital health promotion has moderate effectiveness in influencing public health behavior. The study’s conclusions indicate that digital health promotion is a promising strategy for NCD prevention, but requires further development—particularly in integrating behavioral change theory, enhancing user engagement, and ensuring equitable access to technology. Future research is recommended to develop more interactive, personalized, and sustainable interventions to enhance the effectiveness of long-term behavioral.

Abstract

Social media platforms such as Instagram, TikTok, and YouTube have become the main platforms for the promotion of e-cigarettes by influencers, which influences vaping behavior among adolescents. Positive content, such as “vape tricks” and gender-based marketing, reinforces the perception that vaping is safe and attractive. Objective: This study aims to analyze the impact of e-cigarette promotion by influencers on social media on vaping behavior among adolescents (aged 13-23 years) through a systematic review of the literature. Methods: A systematic review was conducted using databases such as ScienceDirect, PubMed, and Google Scholar with keywords such as “e-cigarette promotion”. Inclusion criteria included English-language articles (2020-2025) discussing e-cigarette promotion by influencers on Instagram, TikTok, or YouTube. Literature selection used the PRISMA method, resulting in 10 articles from 835 initial articles after screening for duplication and eligibility criteria. Results: E-cigarette promotion was dominated by content with “vape tricks” and gender-based marketing strategies reinforcing the normalization of vaping. TikTok increased the risk of e-cigarette use, while YouTube promoted product reviews and discounts. The lack of health narratives exacerbated adolescents' knowledge gap about the risks of vaping. Conclusion: Influencer promotion of e-cigarettes on social media increases the normalization and behavior of vaping among adolescents, driven by visual content Strict regulations and social media-based educational campaigns are needed to protect adolescents from the influence of this promotion.

Abstract

Nutritional problems among pregnant women, such as Chronic Energy Deficiency (CED) and anemia, remain significant challenges in efforts to reduce maternal and infant mortality rates in Indonesia. In response to this issue, the Padangmatinggi Public Health Center in Padangsidimpuan City has implemented a Local Food-Based Supplementary Feeding Program (PMT) as a specific nutrition intervention. However, the effectiveness of this program has yet to be comprehensively evaluated. This study aims to assess the implementation of the PMT program using a systems approach, covering the components of input, process, and output.This research adopts a descriptive qualitative method with a case study design. Data were collected through in-depth interviews, observation, and document review involving seven key and supporting informants. The findings indicate that input components—such as human resources, infrastructure, and budget support—are adequate and support the implementation of the program. The intervention process followed established procedures, including the identification of CED-targeted pregnant women based on MUAC <23.5 cm, distribution of supplementary food by health workers and community health volunteers, and routine nutritional status monitoring. The program’s output showed improvements in the nutritional status of pregnant women with CED, as evidenced by increases in body weight and mid-upper arm circumference.Nevertheless, the program faces challenges, particularly in the limited number of community health volunteers and reliance on central government funding, which could hinder its sustainability. Therefore, strategies are needed to strengthen implementation capacity through regular training and the development of local food education modules, along with enhanced efficiency in monitoring systems through digitalization. This study is expected to serve as a reference for policy-making in maternal nutrition interventions at both local and national levels.

Abstract

Chronic kidney failure is a progressive and irreversible decline in kidney function, and it is one of the catastrophic diseases with high financial burden in Indonesia. The increase in chronic kidney failure cases is influenced by the prevalence of hypertension, diabetes mellitus, obesity, as well as changes in community lifestyle. This study aims to identify the characteristics of chronic kidney failure patients undergoing hemodialysis in North Sumatra Province from 2015 to 2020 based on gender, age, type of National Health Insurance membership, and average service claim costs. The study uses a descriptive quantitative approach with a non-experimental cross-sectional design. The data used are secondary data of a 1% sample from BPJS Kesehatan for 2015–2020, which has obtained data usage permission from the BPJS Kesehatan Central Office. The variables analyzed include gender, age, membership segmentation, diagnosis, and claim verification costs.Keywords: chronic kidney disease, hemodialysis, BPJS Kesehatan, claim cost, patient characteristics. The analysis was carried out descriptively by taking into account individual weights to reflect the population. The study results showed that the proportion of chronic kidney failure patients undergoing hemodialysis was mostly male, totaling 66.17 percent. Based on age, the adult group (19–59 years) dominated with 67.79 percent, followed by the elderly proportion at 28.07 percent. The largest proportion by type of participation came from the PBPU group at 62.05 percent, followed by PPU at 17.29 percent. The average cost of chronic kidney failure claims with hemodialysis was Rp816.713,9 with a standard deviation of Rp209.344,8, a minimum claim of Rp95.900, and a maximum of Rp4.219.800. Chronic kidney failure patients in North Sumatra were dominated by the productive age group, males, and PBPU participants, with a fairly high financial burden on the health insurance system. These findings indicate the need to strengthen promotive and preventive efforts against risk factors for chronic diseases to curb the increase in cases and the burden of health care costs

Abstract

The Emergency Department (ED) is an essential unit within hospital services that plays a crucial role in handling patients experiencing emergency situations. The quality of care provided in the ED greatly influences patient safety, the satisfaction of patients’ families, and the overall reputation of the hospital. This study aimed to examine the quality of healthcare services in the Emergency Department of Permata Hati Hospital, Asahan Regency, in 2026 by applying the five SERVQUAL dimensions, namely tangible, reliability, responsiveness, assurance, and empathy. This research used a quantitative method with a cross-sectional study design. The population consisted of family members who accompanied patients in the Emergency Department of Permata Hati Hospital during the research period. A sample of 97 respondents was obtained through probability sampling using an accidental sampling technique. Data collection was conducted using a structured questionnaire, and the data were analyzed using univariate analysis, bivariate analysis with the chi-square test, and multivariate analysis through logistic regression. The findings indicated that the majority of respondents considered the quality of healthcare services in the Emergency Department to be good. The bivariate analysis showed that reliability, responsiveness, assurance, and empathy were significantly associated with the quality of healthcare services, while the tangible dimension did not demonstrate a significant relationship. Furthermore, the multivariate analysis identified responsiveness as the most influential factor affecting the quality of healthcare services in the Emergency Department. In summary, the quality of healthcare services in the Emergency Department of Permata Hati Hospital is determined by several dimensions of service quality, particularly the responsiveness of healthcare professionals. Therefore, hospital management is encouraged to improve service responsiveness, strengthen effective communication, and foster empathetic attitudes among healthcare staff in order to continuously enhance the quality of emergency care services.

Abstract

The implementation of hospital promotion strategies through public relations and brand management at F.L. Tobing General Hospital, Sibolga, is influenced by various inhibiting and supporting factors. The main inhibiting factors include limited marketing budgets, a lack of competent human resources in public relations and brand management, suboptimal use of digital media, intense competition from more aggressive private hospitals, and weak promotional effectiveness evaluation mechanisms. Meanwhile, supporting factors include F.L. Tobing General Hospital's status as a government referral hospital, support from local government policies, opportunities for collaboration with various institutions, and increasing management awareness of the importance of promotion and strengthening brand image to increase hospital competitiveness. This study used a descriptive qualitative approach with triangulation of data sources within the F.L. Tobing General Hospital's work area, involving one key informant and 11 triangulation informants, and data collection techniques through observation, interviews, and documentation. The results showed that the advertising strategy was still dominated by internal media such as banners, posters, and brochures. Inpatient sales promotion is conducted through non-tariff promotions due to regulations, including simplified BPJS and insurance administration, service package information, and supporting facilities. Personal selling through empathetic interactions with healthcare workers, oriented toward patient-centered care, is the most influential strategy in building trust and inpatient care decisions. Direct digital marketing through WhatsApp and a reservation system supports rapid information access and increases patient satisfaction and loyalty. Public relations strategies play a crucial role in building image, encouraging positive word of mouth, and increasing inpatient visits. Continuous improvement of interpersonal communication skills, optimization of digital media, and contextual health education for patients' families are recommended.

Abstract

Tuberculosis (TB) in children remains a significant public health issue, particularly in the context of household transmission. This study aimed to analyze the time-to-event of TB incidence among children and to identify environmental and family behavioral factors influencing its occurrence in Medan Tuntungan District, Medan City, in 2025. This study employed an analytical observational design with a retrospective cohort approach and survival analysis. A total of 40 children aged 6–10 years were selected using purposive sampling. Data were analyzed using Kaplan-Meier methods, Log-Rank tests, and Cox proportional hazards regression. The results showed that the median time to TB incidence was 8 months. Significant factors associated with TB incidence in children included exposure to cigarette smoke (HR=3.12; p<0.05), poor household ventilation (HR=2.78; p<0.05), household crowding (HR=2.15; p<0.05), and inadequate family preventive behavior (HR=2.40; p<0.05). In conclusion, environmental and family behavioral factors play a crucial role in determining the timing of TB incidence in children. Survival analysis provides a more comprehensive understanding of risk periods and determinants. TB prevention strategies should focus on improving household environmental conditions and family health behaviors.

Abstract

Emergency services represent the frontline of hospital care and require optimal response time and high-quality service delivery, with response time and service quality serving as key performance indicators. The achievement of these indicators is strongly influenced by the implementation of effective emergency service management policies. This study aimed to analyze the effect of emergency service management policies on response time and service quality at Bangun Purba Regional General Hospital, Deli Serdang Regency, in 2025. This study employed a quantitative analytical design, with the study population consisting of patients and/or patients’ families who received services at the Emergency Department of Bangun Purba Regional General Hospital. Data were collected through questionnaires and observation, and analyzed using univariate analysis to describe the distribution of study variables and bivariate analysis to examine the relationships between variables. The univariate analysis indicated that emergency service management policies were generally perceived as moderate to good, while response time performance had not fully met the minimum service standards. Bivariate analysis revealed a statistically significant relationship between emergency service management policies and response time (p < 0.05), as well as between management policies and service quality (p < 0.05). Response time was also significantly associated with service quality (p < 0.05), indicating that faster response time was associated with better perceived service quality. In conclusion, emergency service management policies play a crucial role in improving response time and service quality in the emergency department, highlighting the need for strengthened policy implementation, improved adherence to standard operating procedures, and continuous evaluation to enhance service quality and patient safety.

Abstract

High traffic density contributes significantly to air pollution through motor vehicle exhaust emissions, particularly sulfur dioxide (SO₂), which poses a risk of respiratory disorders among outdoor workers such as gas station operators. Climatic conditions and regional topography influence SO₂ concentrations in mountainous and urban areas, as well as the associated health impacts in the form of respiratory disorders. This study aimed to analyze differences in SO₂ concentrations between mountainous and urban areas and to examine the effect of SO₂ exposure on respiratory disorders among gas station operators. A quantitative comparative approach with a cross-sectional study design was employed. The study was conducted at gas stations located in both mountainous and urban areas from October 2024 to February 2025. The study population consisted of all gas station operators totaling 78 individuals, with 39 operators from each area. The sample size was 78 respondents, selected using a total sampling technique. Data were collected through interviews and observations, along with measurements of SO₂ concentrations. Data analysis was carried out in two stages: univariate analysis and bivariate analysis using independent t-tests and logistic regression, with a significance level of 95% (α=0.05). The results showed a significant difference in SO₂ concentrations between mountainous and urban areas (p-value=0.001). Furthermore, SO₂ exposure was found to have a significant effect on respiratory disorders among gas station operators (p-value=0.004; Exp(B)=1.859). SO₂ concentrations were lower in mountainous areas compared to urban areas, indicating that urban environments contribute more substantially to respiratory disorders.It is recommended that PT Pertamina provide appropriate respiratory protective equipment, such as masks, for gas station operators and implement policies mandating their use during working hours.
