Abstract

Background: Obesity is a nutritional problem that has become one of the most pressing global public health challenges of the 21st century. According to the SKI 2023 results, the prevalence of obesity among individuals aged 18 and older in Sumatera Utara was 24.3%. Several factors influencing are broadly related to issues of nutritional intake and dietary diversity. Objective: To analyze relationship between nutrient intake and dietary diversity with incidence obesity among undergraduate students in Public Health program at the Universitas Sumatera Utara 2023/2024. Methods: This study used cross-sectional design with 245 participants. Data collected included food recall, BMI, WHR, and IDDS score calculations. The relationship was analyzed using Chi-Square test. Results and discussion: Most subjects had normal nutritional status, but prevalence of obesity 21.6% and risk of central obesity 14.7% were still found. The adequacy levels of energy, fat, and carbohydrates were predominantly in the severe deficiency category, while protein adequacy tended toward the normal category, and nearly all subjects fiber deficiency. Most students had moderate dietary diversity. The analysis results showed no significant association between adequacy levels of energy, protein, carbohydrates, fiber, and dietary diversity with the prevalence of obesity or risk of central obesity. However, there was significant association between fat intake levels and risk of central obesity. Conclusion: It’s important to control fat intake and improve dietary quality by increasing fiber consumption and dietary diversity, along with controlling other factors not examined in this study (physical activity, sleep quality, stress, genetic factors, and eating habits).

Abstract

This study aims to analyze the effect of Occupational Safety and Health (OSH) promotion through social media on Personal Protective Equipment (PPE) compliance among online drivers in Kota Medan. This research employed a quantitative approach with a quasi-experimental design using a one-group pretest-posttest method. A total of 30 respondents were selected using accidental sampling. Data were collected through questionnaires and observation, with intervention delivered via social media platforms such as TikTok and Instagram. The results showed that the mean PPE compliance score increased from 58.20 in the pretest to 74.60 in the posttest, with a difference of 16.40. The Paired Sample T-Test indicated a p-value of 0.000 (p < 0.05), demonstrating a significant effect of social media-based OSH promotion on PPE compliance. In conclusion, social media is effective as a tool for OSH promotion in improving safe behavior among online drivers. Therefore, social media utilization is recommended as an innovative strategy to enhance PPE compliance among informal sector workers.

Abstract

Digital transformation in the healthcare sector is a strategic phenomenon that plays a role in reshaping administrative systems and increasing hospital competitiveness in the digital era. This study aims to comprehensively analyze the role of digital administrative transformation in improving hospital strategic competitiveness through a literature review approach. The method used is a literature review with stages including topic identification, literature search, selection based on inclusion and exclusion criteria, and data analysis and synthesis. The literature search was conducted systematically through academic databases such as Google Scholar and ScienceDirect using relevant keywords. The selected literature was limited to publications within the last 5 years and came from credible scientific sources, such as reputable journals, academic books, and government policy documents. A total of 10 literatures that met the criteria were analyzed using content analysis techniques to identify patterns, themes, and relationships between concepts. The results of the study indicate five main themes, namely: (1) digital transformation as a process of administrative reconstruction based on an integrated system, (2) operational efficiency as the dominant impact of digitalization, (3) data as a basis for strategic decision-making, (4) improving service quality as a representation of hospital competitiveness, and (5) digital transformation as a contextual and non-linear process. In conclusion, digital administrative transformation is a holistic strategy that focuses not only on technology adoption but also encompasses the integration of systems, human resources, and organizational processes to enhance a hospital's strategic competitiveness. Therefore, it is recommended that hospitals strengthen human resource capacity, develop an integrated digital infrastructure, and optimize data utilization in strategic decision-making to support the sustainability of digital transformation.

Abstract

Pneumonia remains a leading cause of morbidity and mortality among children under five, particularly in            developing countries. The Pneumococcal Conjugate Vaccine (PCV) is effective in preventing Streptococcus             pneumoniae infection; however, its utilization in Indonesia, including at Sei Rampah Primary Health Center, remains low. This study aimed to analyze factors influencing PCV utilization and its association with pneumonia incidence. A cross-sectional analytic design was applied to 262 mothers with children aged 0–59 months selected through simple random sampling. Data were collected using questionnaires and medical records, and analyzed using univariate, Chi-square, and logistic regression tests (p < 0.05; 95% CI). The results showed that knowledge, attitudes, and access to healthcare services were significantly associated with PCV utilization, with access being the most dominant factor (OR = 3.08; 95% CI: 1.75–5.42). Family support was significant in bivariate analysis but not in multivariate analysis. PCV utilization significantly reduced the risk of pneumonia (OR = 0.28; 95% CI: 0.13–0.60; p = 0.001). In conclusion, PCV utilization is mainly influenced by access to healthcare, knowledge, and attitudes, and has a protective effect against pneumonia. Strengthening health education and improving service access are essential to increase coverage

Abstract

BPJS Kesehatan Smart Claim implementation at Santa Elisabeth Hospital Medan has progressed to Star 4, including online pharmacy bridging or e-prescription integration. This study aimed to analyze the effect of this implementation on service efficiency and effectiveness. It used a qualitative approach with an intrinsic case study design. Eighteen informants were selected through purposive-homogeneous sampling. Data were collected from August to December 2025 through semi-structured in-depth interviews, limited participatory observation of the end-to-end service and claim workflow, and document review. The data were analyzed thematically using methodological and theoretical triangulation. The findings show that Smart Claim improves data access, supports partial paperless processes, and accelerates service and claim processes under certain conditions. The claim cycle is perceived to be shorter, and pharmacy processing becomes faster after e-prescription integration. Tangibles and responsiveness tend to improve, whereas reliability and assurance still fluctuate during network and bridging disruptions, slow devices, and data mismatch or mapping problems that trigger rework, cross-unit escalation, and pending or returned claims. This study concludes that Smart Claim improves service efficiency and effectiveness conditionally, depending on infrastructure stability, staff readiness, and claim data governance.

Abstract

Pulmonary tuberculosis (TB) remains a major global public health problem, with suboptimal treatment success rates in many regions. This study aimed to analyze individual and behavioral factors affecting the success of pulmonary TB treatment at Sukaramai Public Health Center, Pakpak Bharat Regency. This study employed an analytical observational design with a cross-sectional approach. The population consisted of 328 TB patients, with a sample of 180 respondents selected using purposive sampling. The variables studied included age, sex, education, knowledge, and medication adherence. Data were analyzed using univariate, bivariate (Chi-square test), and multivariate (logistic regression) analyses with a significance level of 0.05. The results showed that most respondents were aged >40 years (63.9%), female (56.7%), had low education levels (71.1%), and had poor knowledge (50.6%). Medication adherence was relatively good, with 59.4% of respondents classified as adherent. Bivariate analysis indicated significant associations between knowledge (p=0.002) and medication adherence (p=0.019) with treatment success. However, multivariate analysis revealed that only medication adherence had a significant effect on treatment success (OR=3.12; 95% CI: 1.65–5.90; p=0.001), while knowledge was not statistically significant. In conclusion, medication adherence is the most dominant factor influencing the success of pulmonary TB treatment. Therefore, interventions should focus on improving patient adherence through continuous health education, family support, and strengthening treatment supervision systems to enhance treatment outcomes.

Abstract

Diabetes mellitus is a metabolic disease characterized by elevated blood glucose levels, with type II diabetes mellitus contributing the majority of cases due to unhealthy dietary patterns, low levels of knowledge, and poor adherence in disease management. Proper dietary management is essential to control blood glucose levels and prevent complications. This study aimed to determine the relationship between the level of knowledge and dietary adherence with blood glucose levels among patients with type II diabetes mellitus in the inpatient ward of RSUD Tarutung. This study used a quantitative method with an analytic approach and a cross-sectional design. The population consisted of 54 patients, and total sampling was applied. Data were collected using a questionnaire and analyzed using the Spearman correlation test. The results show that there is a significant relationship between knowledge level and blood glucose levels (p=0.000; r=-0.598) as well as between dietary adherence and blood glucose levels (p=0.001; r=-0.434). Both variables show moderate negative correlations, indicating that better knowledge and higher dietary adherence are associated with better-controlled blood glucose levels. In conclusion, knowledge and dietary adherence play an important role in controlling blood glucose levels among patients with type II diabetes mellitus. It is recommended that health professionals enhance patient education and that future researchers examine other factors influencing blood glucose levels using more diverse research designs.

Abstract

Health is a fundamental right and an important indicator of a country’s development. The Indonesian government, through the National Health Insurance (JKN) program administered by BPJS Kesehatan, has implemented a tiered referral system to improve the efficiency and quality of healthcare services. This study aims to analyze the impact of implementing the tiered referral system on outpatient visits at RSUD Dr. Ferdinand Lumban Tobing Sibolga, as well as its implications for hospital services, resources, and management. This study employed a quantitative analytic comparative design using a cross-sectional approach combined with retrospective secondary data analysis from 2013 to 2023. The sample included all outpatient visit data using total sampling. Data analysis was conducted using univariate analysis to describe data distribution and bivariate analysis with a paired t-test to examine differences in visits before and after the implementation of the tiered referral system. The results showed a decrease in outpatient visits by 11.31% (from 74,310 to 65,902 visits), with most mild cases being handled at primary healthcare facilities (FKTP), while the hospital managed more complex cases. Hospital revenue remained stable due to an increase in specialized services. Implementation challenges included inappropriate referrals, patient perceptions of complicated referral procedures, long waiting times, and differences in service between JKN and non-JKN patients, which affected patient satisfaction and healthcare-seeking behavior. In conclusion, the tiered referral system has multidimensional impacts on hospitals and patients. Strengthening coordination among healthcare facilities, enhancing the capacity of healthcare workers, and providing education and socialization to the community are necessary to improve patient understanding, satisfaction, and the effective utilization of healthcare services.

Abstract

Developing nations continue to face the disproportionate burden of anemia worldwide. Fatigue and impaired cognitive performance in teenagers, as well as impaired motor and behavioral development in children, are all symptoms of this disorder. There is a substantial risk of iron insufficiency in teenage females since their daily iron needs are about 11 mg. In order to sustain development and compensate for iron losses that occur naturally, including during menstruation, the body's iron needs rise throughout adolescence. Adolescent girls at SMA Swasta Nusantara Lubuk Pakam were the subjects of this research, which sought to examine the correlation between anemia rates and variables such nutritional intake and knowledge. The Spearman Correlation test was used to analyze the data in a cross-sectional research. With a p-value of 0.000 (<0.05), the statistical test findings demonstrated a strong correlation between dietary consumption and knowledge and the occurrence of anemia. With p-values for carbohydrates (1.000), protein (0.716), and fat (0.062) all higher than 0.05, there was no statistically significant correlation between macronutrient consumption and the prevalence of anemia. Because of this, researcher may infer that dietary carbs, proteins, and fats do not contribute to the prevalence of anemia, and we reject the alternative hypothesis (Ha) for this variable.

Abstract

Husband participation in family planning (FP) programs is a key factor in the success of population control and the improvement of family reproductive health. However, the level of husband involvement in FP programs in Indonesia remains relatively low and is influenced by various factors. This study aims to analyze the relationship between knowledge, attitudes, the role of health workers, and sociocultural factors with husband participation in family planning programs. This study employs a quantitative research design with a cross-sectional approach. The study population consists of married couples of reproductive age, with the sample selected using purposive sampling. Data collection was conducted using a structured questionnaire. Data analysis was performed using univariate and bivariate methods with the chi-square test, as well as multivariate analysis using multiple logistic regression to identify the dominant factors influencing husbands’ participation in family planning programs. The results indicate a significant relationship between knowledge, attitudes, and sociocultural factors and husbands’ participation in family planning programs (p < 0.05). Meanwhile, health workers did not show a significant association with husbands’ participation (p > 0.05). The knowledge variable was the factor most strongly associated with husbands’ participation in the family planning program, with a 1.65-fold higher likelihood of participating compared to husbands with lower knowledge levels, whereas attitudes and sociocultural factors did not show a significant influence after controlling for other variables. This study indicates that husbands’ participation in family planning programs is influenced by individual and environmental factors, particularly husbands’ knowledge regarding family planning. Therefore, efforts to enhance education and outreach focused on husbands, as well as sociocultural approaches involving families and communities, are needed to increase men’s involvement in family planning programs.
