Abstract

Background: Dysmenorrhea was a common menstrual disorder among female university students that could interfere with academic activities. Irritative dietary patterns, such as consuming seblak—high in capsaicin and sodium—were suspected as triggers for menstrual pain. Objective: This study aimed to analyze the association between seblak consumption and dysmenorrhea, considering residential status as an effect modifier. Methodology: A cross-sectional study was conducted involving 207 female students at Universitas Sriwijaya via quota-convenience sampling. Data were collected using a Food Frequency Questionnaire (FFQ) and a structured questionnaire for functional dysmenorrhea. Data analysis involved logistic regression and stratified analysis using STATA 14. Results: Results show that the prevalence of dysmenorrhea is 39.6%. No significant association exists between seblak consumption and dysmenorrhea in the total sample. However, stratified analysis reveals a significant association among students living with parents (OR=2.76; 95% CI: 1.18–6.42; p=0.019), while no significance is found in boarding houses (p=0.570). Interaction testing confirms residential status as a significant effect modifier (p-interaction=0.020). Conclusion: Seblak consumption significantly increases dysmenorrhea risk among students residing with families, but the influence is distorted by environmental factors in boarding houses. Reproductive health education should consider spicy diet patterns, particularly for groups with high dietary stability at home.

Abstract

Maternal mortality remains a public health issue in many countries, including Indonesia. Preeclampsia and hemorrhage are complications during pregnancy that are two of the three leading causes of maternal mortality in Indonesia and East Java Province. This study aims to map the prevalence of preeclampsia and hemorrhage in pregnant women with maternal mortality rates in East Java Province in 2024. This study is an observational cross-sectional study using secondary data on the number of cases of preeclampsia and hemorrhage complications in pregnant women and the total number of maternal deaths in 38 districts/cities in East Java Province in 2023. The analysis was performed using the QGIS application and Pearson and Spearman correlation tests. The highest maternal mortality rates were concentrated in the eastern part of East Java Province. Most of the prevalence of preeclampsia in pregnant women was spread across areas with high maternal mortality rates, while the prevalence of hemorrhage in pregnant women was spread randomly across several areas regardless of the high or low maternal mortality rates. The Pearson correlation test showed a moderate correlation between the prevalence of preeclampsia and maternal mortality rates. The Spearman correlation test showed no correlation between the prevalence of hemorrhage in pregnant women and maternal mortality rates with a weak correlation that was not statistically significant. The statistically and spatially significant variations indicate that other factors contribute to the pattern of maternal mortality in East Java Province. There is a need for strategies to optimize pregnancy check-ups as a measure for early detection of pregnancy complications in order to reduce the risk of delayed treatment, which can result in maternal mortality.

Abstract

Background: Stunting is a linear growth disorder caused by chronic malnutrition and recurrent infections, characterized by a height-for-age Z-score (HAZ) < −2 SD according to WHO standards. Stunting remains a global and national public health problem, including in Indonesia and Deli Serdang Regency. This condition has serious impacts on cognitive development, productivity, and increases the risk of chronic diseases in adulthood. Stunting is influenced by multiple factors, including food intake, household food security, parenting practices, environmental sanitation, and utilization of health services. Objective: To analyze the determinants of stunting in the working area of Aras Kabu Public Health Center, Deli Serdang Regency, in 2025. Methods: This study used a quantitative analytic design. It aimed to analyze the determinants of stunting among children under five. The study was conducted in the working area of Aras Kabu Public Health Center, Deli Serdang Regency. Results: Based on univariate and bivariate analyses, the prevalence of stunting in the Aras Kabu Public Health Center area in 2025 was 19.25% and was significantly associated with food intake, household food security, parenting practices, environmental sanitation, and utilization of health services. These findings indicate that the five factors are important determinants of stunting and should be prioritized in prevention efforts. Therefore, strengthening the role of primary health care through nutrition education, optimization of community health posts (posyandu), growth monitoring, and promotion of clean and healthy living behavior (PHBS) is necessary, along with cross-sector collaboration, active parental involvement, and engagement of health workers in early detection and prevention. Conclusion: Based on the study findings, there is an association between dietary intake, food security, parenting practices, environmental sanitation, and utilization of health services with the incidence of stunting in the working area of Aras Kabu Public Health Center in 2025. Multivariate analysis also showed that dietary intake was the most influential factor associated with stunting.

Abstract

Malaria is one of the infectious diseases that remains a public health problem in many countries, particularly in tropical and subtropical regions. This disease is caused by the Plasmodium parasite, which is transmitted through the bite of an infected female Anopheles mosquito.This study aimed to analyze the influence of social media use on preventive behavior in preventing the spread of malaria. The study employed a cross-sectional design and was conducted at Mutiara Community Health Center, Asahan Regency, in 2025. The research was carried out from January to April 2025. The study population consisted of all patients seeking treatment at Mutiara Community Health Center, with a total sample of 100 respondents selected using a simple random sampling technique. Data analysis was performed using the Chi-square test and logistic regression.The results showed that there was no significant association between gender (p = 0.159; OR = 1.96; 95% CI = 0.863–4.434) and age (p = 0.840; OR = 1.09; 95% CI = 0.464–2.570) with preventive behavior against malaria transmission. However, there was a significant association between educational level (p = 0.009) and social media use (p = 0.047; OR = 2.70; 95% CI = 1.070–6.856) with preventive behavior against malaria transmission.Based on these findings, respondents are expected to increase their awareness and active participation in implementing preventive behaviors to reduce the spread of malaria, such as maintaining environmental cleanliness and using mosquito nets while sleeping.

Abstract

Pharmaceutical personnel play a crucial role in ensuring the quality of healthcare services in hospitals. A high workload, if not properly managed, can negatively affect performance, patient safety, and service quality. This study aims to analyze the workload of pharmacists at the Pharmacy Installation of Tanjung Selamat General Hospital, Langkat, focusing on four main variables: the type and number of tasks, limitations in human resources, duration and work shift patterns, and work adjustment strategies.This study employed a qualitative approach using a descriptive method. Data were collected through in-depth interviews with one key informant (Head of the Pharmacy Installation) and three triangulation informants (Pharmaceutical Technical Personnel and Pharmacy Assistants). Data analysis was conducted through data reduction, data presentation, and conclusion drawing.The results indicate that pharmacists and pharmaceutical personnel at Tanjung Selamat General Hospital handle various types of tasks, including administrative and non-clinical duties such as medical waste management. The limited number of staff leads to task overlap, uneven shift distribution, and an increased risk of work fatigue. The 24-hour shift system has not been optimally implemented due to the disproportionate distribution of personnel. Work adjustment strategies, such as task prioritization and teamwork, have been applied; however, they remain individual-based and are not yet supported by a structured managerial system.In conclusion, the workload of pharmaceutical personnel is considered high and complex. The hospital needs to increase staffing levels, evaluate the shift system, and strengthen supporting systems such as standard operating procedures (SOPs) and information technology. Adjustments in human resource management policies are essential to maintain work efficiency and the quality of pharmaceutical services

Abstract

The implementation of Minimum Service Standards (MSS) in the health sector is an obligation of local governments to ensure the provision of basic health services for the community. This study aims to analyze the implementation of MSS at Mersam Community Health Center, Batanghari Regency, in 2025, viewed from the aspects of communication, resources, disposition, and bureaucratic structure, as well as to identify the supporting and inhibiting factors. This research employed a qualitative approach with a descriptive method through interviews, observations, and documentation.The results indicate that the implementation of MSS has been carried out in accordance with existing regulations; however, it has not been fully optimal. The main obstacles include limited health personnel, inadequate facilities and infrastructure, high workload, and low public awareness of preventive services. Supporting factors include leadership commitment and support from the Health Office. The strategies implemented involve improving coordination, optimizing available resources, and strengthening public outreach. The findings emphasize the importance of strengthening resource capacity and enhancing cross-sectoral coordination to ensure that MSS implementation becomes more effective and sustainable in improving the quality of basic health services at the community health center level.

Abstract

Background: HIV/AIDS remains a serious global health problem because it attacks the immune system and can progress to AIDS without regular antiretroviral (ARV) therapy. Globally, more than 37 million people are living with HIV, and Indonesia is among the countries with relatively low ARV coverage, with only around 10% of people living with HIV/AIDS (PLWHA) receiving treatment. In North Sumatra, Tebing Tinggi City is one of the areas experiencing an increase in cases; however, the utilization of HIV testing services and adherence to ARV medication remain low. A preliminary study at RSUD Dr. H. Kumpulan Pane showed that lack of information, motivation, and behavioral skills caused some patients to be inconsistent in undergoing therapy.Objective: This study aimed to analyze adherence to ARV medication among PLWHA at RSUD Dr. H. Kumpulan Pane, Tebing Tinggi City, in 2025. Methods: This study used a quantitative design with a cross-sectional approach. Results: The results showed that information, motivation, and behavioral skills had a significant effect on adherence to ARV medication among PLWHA at RSUD Dr. H. Kumpulan Pane, Tebing Tinggi City, in 2025. The better the information received, the higher the patient’s motivation, and the better the behavioral skills in managing therapy, the higher the patient’s adherence to ARV treatment. Conclusion: Information, motivation, and behavioral skills are important determinants that contribute to the success of ARV therapy among PLWHA.

Abstract

This study aims to compare the implementation of the role of the Family Assistance Team (TPK) in the prevention and reduction of stunting in Pulau Simardan and Bunga Tanjung Villages, and to identify the supporting and inhibiting factors in its implementation. The study employed a qualitative approach with a comparative phenomenological design. Data were collected through observation, document review, and in-depth interviews with 13 informants consisting of TPK cadres, target families, Family Planning Field Officers (PLKB), and neighborhood heads. Data were analyzed thematically and validated through source and method triangulation.The results indicate that TPK has carried out its mentoring functions routinely and systematically, including nutrition and health education, surveillance of at-risk families, facilitation of health service and social assistance referrals, psychosocial support, as well as recording and reporting through the Elsimil application. Home visits were conducted one to three times per month. Differences in regional characteristics influenced implementation strategies, where collective-based approaches were more effective in areas with strong social cohesion, while individual approaches were more suitable in areas with high population mobility. Supporting factors included basic training, the use of digital applications, and support from community leaders. The main obstacles were limited facilities and infrastructure, low incentives, and uneven digital literacy. This study recommends strengthening the technical and institutional capacity of TPK and integrating cross-sector coordination based on local data to enhance the effectiveness of stunting reduction acceleration programs.

Abstract

Exclusive breastfeeding during the first six months of life is an important indicator of maternal and infant health. However, the coverage of exclusive breastfeeding in the working area of the Sibande Community Health Center, Pakpak Bharat Regency, is still not optimal. One of the factors contributing to the improvement of exclusive breastfeeding coverage is the role of Posyandu cadres as the frontline of community-based health services. This study aims to describe the role of Posyandu cadres, identify the obstacles they face, and formulate strategies to improve exclusive breastfeeding coverage through the optimization of the role of Posyandu cadres. This study employed a qualitative approach with a phenomenological study design. Informants were selected purposively and consisted of 12 participants, including Posyandu cadres, breastfeeding mothers with infants aged 0–6 months, and health workers from the Sibande Community Health Center. Data were collected through in-depth interviews, observation, and documentation studies, and were subsequently analyzed using thematic analysis. The results showed that Posyandu cadres play roles as educators, motivators, companions, and intermediaries between breastfeeding mothers and health workers. However, these roles have not been performed optimally due to limited knowledge and counseling skills related to breastfeeding, the influence of local cultural practices, and the lack of continuous guidance and support. The discussion indicates that optimizing the role of cadres through exclusive breastfeeding counseling training, continuous mentoring, the provision of educational media, and strengthened cross-sector coordination has the potential to improve exclusive breastfeeding coverage. The conclusion of this study highlights that optimizing the role of Posyandu cadres is an important strategy for increasing exclusive breastfeeding coverage. It is recommended that the Community Health Center regularly conduct exclusive breastfeeding counseling training and strengthen supervision and support for Posyandu cadres.

Abstract

Mobile JKN is a digital application developed by BPJS Kesehatan to facilitate participants in accessing various administrative services and information related to the National Health Insurance program, such as participant registration, updating membership data, and obtaining information on healthcare services at primary healthcare facilities and referral hospitals. Although the application was designed to improve access to healthcare services, its utilization among participants remains relatively low. This study aimed to analyze factors associated with the utilization of the Mobile JKN information system among National Health Insurance participants at Grandmed Hospital. This study used a quantitative approach with a cross-sectional design. The study population consisted of 100.040 National Health Insurance participants registered at Grandmed Hospital, and a sample of 100 respondents was determined using the Lemeshow formula. Data were analyzed using univariate analysis, bivariate analysis with the Chi-square test, and multivariate analysis using multiple logistic regression. The results show that the majority of respondents (60%) have not utilized the Mobile JKN application. Bivariate analysis shows significant associations between the utilization of Mobile JKN and system quality, information quality, service quality, frequency of use, and net benefits (p < 0.05). Multivariate analysis shows that information quality is the most dominant factor associated with the utilization of Mobile JKN after controlling for system quality and service quality. These findings indicate that improving the quality of information within the applicati on and strengthening outreach to participants are necessary to enhance the utilization of Mobile JKN and improve access to healthcare services.
