Abstract

Cryptococcosis is the fourth most common opportunistic infection among Acquired Immune Deficiency Syndrome (AIDS) patients.About 100% mortality has been reported within two weeks in patient with cryptococcal meningitis but without specific treatment. Theaim of the study was to compare Cryptococcal antigen (CrAg) detection between Cryptococcus Antigen Latex Agglutination System(CALAS) and Lateral Flow Assay (LFA) among AIDS patients. This research was designed as comparative analytic with cross sectionalstudy on 56 serum derived from AIDS patient who visited Teratai Clinic before ARV therapy initiation and who had never been diagnosedas Cryptococcal meningitis. Each sample was tested for CrAg with CALAS and LFA according to the manufacturer instructions. Thisstudy was conducted in the Clinical Pathology Laboratory at Dr. Hasan Sadikin Hospital/Centre Research Unit (CRU) Medical Faculty,Padjajaran University between December 2012March2013. The statistical analysis was done using Chi square test. The result showed thatthere was no significant difference between CALAS and LFA method with p=0.596 (p>0.05), the positive probability of CrAg detectionusing LFA was 0.75 times compared to the CALAS method. For the CrAg detection in the AIDS patients there was no significant differencebetween CALAS and LFA and the positive probability of CrAg for LFA was detected about 0.75 times compared to the CALAS method.

Abstract

Oxidative stress as a consequence of chronic alcoholic intake causes lesions of liver tissue. The objective of this study was to identifythe antioxidant effect of ethanol extract derived from Balinese purple sweet potato on the liver tissue in mice after chronic consumptionof alcohol. Subjects of this study were 32 adults male Swiss mice (12–14 weeks) divided into 4 groups with control group post-testonly design. Post-treatment of blood and liver samples were collected from each group of 8 mice. The MDA liver was quantified withthiobarbituric acid reactive substances (TBARS) method. Samples were collected from control group of 8 mice without sweet potatoextract and alcohol, 8 mice with 1 mg extract of Balinese purple sweet potato, 8 mice with 0.8 gram alcohol, and 8 mice with combineof alcohol and extract of Balinese purple sweet potato, each treatment was carried out every day for 4 weeks. The results showed asignificant increase of serum SGOT, SGPT and liver MDA level, after chronic consumption of alcohol (p = 0.00). The decrease of serumSGOT, SGPT and liver level MDA was significant in the group with extract of Balinese purple sweet potato (p = 0.00). The researchersconcluded that the extract of Balinese purple sweet potato had an antioxidant effect in mice which consumed alcohol chronically. Basedon this study it is concluded that extract of Balinese purple sweet potato has a protective effect on the liver cell in mice which consumedalcohol chronically.

Abstract

Elevated fibrinogen levels is related to the blood hyperviscosity, that causes low blood velocity. Non-contrast-enhanced transcranialDoppler (TCD) is used to evaluate blood flow from the cerebrovascular system. To know the relationship between fibrinogen levels andexamination of TCD in acute ischemic stroke through evaluation. A cross sectional study was admitted from July 2012-Juny 2013.The researchers determined the differences between fibrinogen and TCD in the stroke group and control. The researchers examinedthe relationship between fibrinogen and TCD examination in the stroke group. The fibrinogenwas measured with Clauss method. TheTCD was examined due to middle of the cerebral artery (MCA) and the Internal Carotid one Artery (ICA). The patients were diagnosedas ischemic stroke from head CT-scan. Statistical analyses employed the Independent T test, Anova test and Pearson correlation. Theresearchers had 24 patients and 24 controls that the Fibrinogen levels in stroke group is 549.16±104.84 mg/mL and in the control groupis 385.64±16.80 mg/mL. The researchers examined MCA in the stroke as well as the control and found the mean velocity 43.12±21.03and 56.97±6.22 (p=0.05), the peak velocity 74.17±32.58 and 94.55±14.11 (p=0.05) end diastolic velocity 23.27±12.66 and35.30±7.34 (p=0.00). In ICA, the stroke group and control and found the mean velocity 31.40±8.86 and 43.07±8.06 (p=0.00), thepeak velocity 54.99±11.50 and 75.04±16.04 (p=0.00) end diastolic velocity 18.23±7.67 and 25.64±5.24 (p=0.00). The correlationbetween fibrinogen and TCD in the stroke group was not significant on MCA and ICA, P>0.05. It can be concluded that the differencesbetween fibrinogen levels and TCD in the stroke group and control are significant. But there is no correlation between the fibrinogenand TCD in the stroke group.

Abstract

Diabetic retinopathy is a complication of diabetes mellitus in the form of microvascular (aneurysma) which can cause blindness.Interleukin-6 is a pro - inflammatory cytokine produced by monocyte macrophage in the early inflammation. Chronic inflammationplays the role on artherosclerosis event, because it is directly correlated with all phases of it and cardiovascular disease as well. Theaim of this study is to know the difference in plasma IL-6 levels among patients suffering from diabetes mellitus together with diabeticretinopathy and without diabetic retinopathy. This study is an observational analytical research with cross sectional design. The sampleswere taken randomly as many as 16 patients, divided in each group suffering from or without retinopathy. The diabetic mellitus patientswere established through their related history and were examined randomly for blood glucose and their fundus or retinal abnormality.The level of IL-6 was examined by ELISA method with double antibody sandwich streptavidin - biotin. Based on this study it was foundthat the mean plasma levels of interleukin 6 (IL-6) of the diabetic mellitus patients with retinopathy was 1.165 ± 0.296 pg/mL andthe mean plasma levels of IL-6 with diabetes mellitus without retinopathy was 0.879 ± 0.230 pg/mL. Based on the normality test withShapiro-Wilk the data showed a normal distribution (p>0.05), while the Levene’s homogenity test obtained homogeneous with p= 0.369(p>0,05). The statistical analysis with two independent samples t-test showed that there were significant differences between the groupsof patients who suffered diabetes mellitus with retinopathy and without retinopathy (p=0.005). It can be concluded so far that the meanlevels of interleukin-6 (IL-6) plasma in diabetic mellitus patients with retinopathy is higher compared to patients with the same diseasebut without retinopathy.

Abstract

Toxigenic Clostridium difficile infection, causing a Pseudo Membrane Colitis (PMC) and Clostridium Difficile Associated Diarrhea(CDAD) has increased sharply. The largest risk factor is the use of antibiotics. The purpose of this study was to know how to determinethe prevalence and characteristics of subjects with Toxigenic Clostridium difficile and to assess the ability of the toxin rapid test comparedto real-time PCR. Ninety adult subjects with antibiotic therapy more than two (2) weeks were enrolled in this study. The results of toxinrapid test and real-time PCR were presented in a 2x2 table, statistical test used was Chi square. The prevalence of Toxigenic Clostridiumdifficile based on the toxin rapid test and by real-time PCR was 27.3% and 37.5%, respectively. There were significant differences betweenstool consistency and number of antibiotics used with the detection of Toxigenic Clostridium difficile. There was a relationship betweenthe duration of antibiotic therapy with the detection of Toxigenic Clostridium difficile using real-time PCR (p=0.010, RR=2.116). Thesensitivity, specificity, PPV, NPV, PLR and NLR rapid test against real-time PCR were 69.7%; 98.2%; 95.8%; 84.4%; 39.2 and 0.31,respectively. This study concluded that the prevalence of Clostridium difficile in RSCM was higher compared to that in Malaysia, Thailandand India; the subjects with antibiotic therapy for more than four (4) weeks had a double risk to have Toxigenic Clostridium difficilethan subjects with antibiotic therapy for less than that time (4 weeks). Thus, in this study, toxin rapid test could be used as a tool todetect Toxigenic Clostridium difficile.

Abstract

Ascites is a condition of abnormal fluid accumulation in the peritoneal cavity that can be caused by many underlying diseases. Some studies conclude that ascitic fluid evaluation with transudate-exudate concept has been replaced by greater indicator, known as Serum Ascites Albumin Gradient (SAAG). This study aims to analyze the compatibility of SAAG and ascitic fluid evaluation to the clinical diagnosis of patients in Dr. Wahidin Sudirohusodo Hospital Makassar. The retrospective cross-sectional study was conducted by taking data from all patients with ascites condition examining both ascitic fluid and serum albumin simultaneously or within 3 days at the Dr. Wahidin Sudirohusodo Hospital Makassar from January – August 2018. The statistical analysis was performed for frequency distribution and Kappa statistical test to assess the compatibility of SAAG and ascitic fluid evaluation to clinical diagnosis of the patient. The study was conducted on 68 subjects and found that both ascitic fluid examination and SAAG were significantly compatible to the clinical diagnosis, 25.8% (p <0.05), 42.5% (p <0.001), respectively. SAAG and ascitic fluid evaluation were significantly in accordance with the clinical diagnosis of the patient. The compatibility value of SAAG is greater than ascitic fluid evaluation so that SAAG is more recommended for ascites examination.

Abstract

Varieties of the laboratory test can be applied in the detecting of iron status. These test are divided into conventional hematologicindex and biochemical iron serum. With the development of an hematological analyzer, new parameter has been found to monitor theiron status with reticulocytes hemoglobin analysis (Ret-He). To know the diagnostic accuracy of Ret-He and to conclude whether Ret-Hecan be used as a parameter to detect iron deficiency in pregnant women by obtaining certain information. This study used analyticalcross sectional was conducted at Haji Adam Malik Hospital Medan, in March−June 2013. The Applying samples consisted of 64 thirdtrimester pregnant women which divided into two (2) groups: the noniron deficiency group using 28 subjects and in the iron deficiency36 subjects. In this study, the researchers found AUC for Ret-He about 77.7% with the best cut off value of <31.4 pg. Using the cut offvalue the researchers obtained the sensitivity about 61.1%, specificity 75.0%, Positive Predictive Value (PPV) 75.8%, Negative PredictiveValue (NPV) 60.0%, Likehood Ratio (LR) (+) 2.44 and LR (–) 0.51%. Based on this study, can be concluded, that the diagnostic accuracyRet-He is a good parameter to detect iron deficiency in the pregnant women.

Abstract

Rinitis alergi merupakan penyakit bukan akibat non-infeksi yang ditemukan antara 10−30% penduduk dewasa dunia dan dapatmenyebabkan penurunan mutu kehidupan seseorang. Rinitis alergi merupakan manifestasi alergi tipe 1 atau tipe cepat yang dimediasioleh IgE. Pemeriksaan utama rinitis alergi adalah Skin Prick Test (SPT) dan IgE spesifik. Pemeriksaan IgE spesifik mempunyai kepekaandan kekhasan yang menyerupai SPT, tidak memerlukan tenaga terlatih dan menyebabkan anafilaktik. Penelitian ini untuk mengetahuiadakah kesesuaian nilai diagnostik IgE spesifik menurut metode imunoblot dengan SPT di pasien rinitis alergi dengan mengujinya.Rancangan penelitian adalah potong lintang yang dilakukan terhadap pasien yang datang di Unit Rawat Jalan THT-KL RSUDDr. Soetomo pada bulan Mei sampai dengan Oktober 2014. Pasien dikelompokkan berdasarkan diagnosis rinitis alergi dan yang nonalergidan non-infeksi serta ditetapkan secara klinis, ada riwayat alergi, pemeriksaan fisik, serta tingkat jumlah keseluruhan IgE serumdan atau eosinofil darah. Pemeriksaan SPT dilakukan dengan memakai ekstrak alergen dari Stallergens dan IgE spesifik menurut metodeimunoblot memakai Foresight®. Dalam kajian ini didapatkan empat puluh tiga pasien didiagnosis rinitis akibat alergi. Hasil IgE spesifikmenurut metode imunoblot positif terdapat di 36 (84%) pasien dengan pola alergen terbanyak D1/D2 29 (67%). Kepekaan dan kekhasandiagnostik IgE spesifik menurut metode imunoblot berturut-turut adalah 72,34% dan 46,15%. Kesesuaian nilai diagnostik IgE spesifikmenurut metode imunoblot dengan SPT mempunyai koefisien kappa 0,158. Didasari telitian ini tidak didapatkan kesesuaian antaraIgE spesifik menurut metode imunoblot dengan SPT. Di ketahui pula bahwa IgE spesifik menurut metode imunoblot dapat digunakanbersama-sama dengan SPT dalam mendiagnosis rinitis akibat alergi.

Abstract

Komponen darah Packed Red Cells (PRC) sering ditransfusikan kepada pasien dengan petunjuk untuk memperbaiki danmempertahankan sebaran oksigen ke jaringan. Eritrosit mengalami perubahan metabolik selama masa penyimpanan kantong darah PRCdi Bank Darah yaitu deplesi 2,3-diphosphoglycerate (DPG) dan Adenosine Triphosphat (ATP) yang juga disebut dengan jejas penyimpanandan disebabkan oleh penurunan pH darah, peningkatan kepekatan kalium dan laktat. Tujuan penelitian ini untuk mengetahui pengaruhpenyimpanan terhadap indeks eritrosit pada darah simpan PRC di Bank Darah. Penelitian deskriptif potong lintang dilakukan masawaktu Februari hingga April 2015 dengan menggunakan 35 sampel darah simpan PRC yang disimpan dan batal ditransfusikan laludiperiksa nilai indeks eritrositnya dengan menggunakan alat Hematology Analyzer Sysmex XT-2000i pada hari pertama (hari-1),keempat (hari-4) dan ketujuh (hari-7) penyimpanan di Bank Darah RSUP Dr. Wahidin Sudirohusodo Makassar. Hasil uji statistikFriedman didapatkan perbedaan bermakna di semua tolok ukur indeks eritrosit pada hari pertama, keempat dan ketujuh penyimpanan.Lanjutan dengan Uji Post Hoc Wilcoxon didapatkan perbedaan bermakna tolok ukur MCV antara hari-4 dan hari-7 (p=0,000), hari-1dan hari-7 (p=0,001); MCH antara hari-1 dan hari-7 (p=0,018); MCHC antara hari-1 dan hari-4 (p=0,000), hari-4 dan hari-7(p=0,002), hari-1 dan hari-7 (p=0,000); RDW antara hari-1 dan hari-4 (p=0,011), hari-4 dan hari-7 penyimpanan (p=0,004). HasilPenelitian ini menunjukkan nilai indeks eritrosit darah simpan PRC mengalami perubahan bermakna dari hari pertama hingga ketujuhpenyimpanan di Bank Darah walaupun nilai indeks eritrosit semua tolok ukur masih dalam batas normal.
