Abstract

Small Dense Low-Density Lipoprotein (sdLDL) and the ApoB/ApoA-I ratio has greater atherogenic potential and is a better marker to predict atherosclerotic blood vessel disease. The purpose of this study was to determine the relationship between the sdLDL and ApoB/ApoA-I ratio to assess the prevalence risk of Peripheral Arterial Disease (PAD) in hypertensive patients. A cross-sectional observational analytic study was performed in 51 hypertension patients with age> 18 years old in Dr. Moewardi Hospital Surakarta from May until June 2018. Patients have measured ABI scores, BMI, blood pressure, lipid profile, ApoB, and ApoA levels. Data were statistically analyzed was using bivariate analysis and multivariate analysis. P-value <0.05 was statistically significant. The prevalence of PAD was 54.90%. Bivariate analysis of age variables (PR: 3.15; 95%Cl: 1.128-8.811; p=0.005), sdLDL (PR: 2; 95%Cl: 0.997-4.013; p=0.03), the ratio of ApoB/ApoA-I (PR: 5.786; 95%Cl: 0.899-37.224; p=0.007), and smoking (PR: 1.896; 95%Cl: 1.210-2.971; p=0.015) was significantly related with PAD. After adjustment of age, smoking, and dyslipidemia variables using multivariate logistic regression analysis, PAD was still related with sdLDL (PR: 10.55; 95%CI: 1.80-61.73; p=0.009), age (PR: 11. 61; 95%CI: 1.83-61.73; p=0.009), and smoking (PR: 11.96; 95%CI: 1.71-83.81; p= 0.013). sdLDL and ApoB/ApoA-I ratio were related to PAD. However, sdLDL, age, and smoking are independent variables of PAD in hypertension patients.

Abstract

Disseminated histoplasmosis is a severe manifestation of fungal infection caused by Histoplasma capsulatum. It usuallyoccurs in a patient with an immunodeficiency state. With the increase of HIV infection and the use of immunosuppressantdrugs lately, its prevalence also increases. A case of 43 years old female with prolonged fever, pancytopenia, and massiveprogressive splenomegaly. The diagnosis of disseminated histoplasmosis and the secondary hemophagocytic syndromewas made based on bone marrow examination that showed increased hemophagocytic processes and multipleintracytoplasmic H.capsulatum. She had been treated with Itraconazole 200 mg for three months. In the first month'sevaluation, her complete blood count improved without any transfusions, and the size of her spleen size decreased. She hadbeen fully recovered after the completion of 3-month treatment.

Abstract

It is reported the incidence of DVT is approximately 84 cases per 100,000 each year, with 30-50% of untreated DVT are atrisk for pulmonary embolism, causing a 12% increase in mortality rate. An accurate and rapid diagnosis of DVT is needed tominimize the risk of complications and prevent unnecessary anticoagulant therapy without waiting for the results of adiagnostic examination using ultrasound. This study aimed to determine the diagnostic value of plasma D-dimer levels onDoppler ultrasound for DVT diagnosis to help clinicians to select a rapid and accurate diagnostic test for DVT diagnosis. Thisresearch was a retrospective study using data from medical records and performed at the Medical Record Installation of Dr.Wahidin Sudirohusodo Hospital, Makassar, by taking data on patients with DVT along with the results of D-dimer andDoppler ultrasound from January to December 2018. D-dimer levels were measured using an immunoturbidimetric methodwith a reference value of<0.5 μg/mL. A total of 33 samples were obtained with a mean of D-dimer value was higher inpositive Doppler (5.29) compared to negative (2.31), although not statistically significant (p> 0.05). Also, the mean of Wellsscore was higher in positive Doppler (4.74) compared to negative (4.17), although not statistically significant (p> 0.05). Thediagnostic values of D-dimer were as follows: sensitivity of 92.6%, specificity of 33.3%, positive predictive value of 86.2%,negative predictive value of 50.0%, and accuracy of 81.8%. D-dimer test can be used both for screening and diagnostic testswith cut-off value ≥ 2 μg/mL.

Abstract

Osteoporosis is a disorder represented by manifestations of low bone mass, decreased bone tissue, and disrupted bonemicroarchitecture. The diagnosis of osteoporosis so far has been based on fracture manifestations after minimal trauma orby detecting low Bone Mineral Density (BMD). Measurement of Receptor Activator of Nuclear Factor-κβ Ligand (RANKL)and Osteoprotegerin (OPG) levels has opened the discourse of a more specific assessment of osteoblast and osteoclastregulation. The RANKL/OPG ratio can represent resorption and bone formation more significantly when correlated withBMD features. This study aimed to analyze the correlation between serum RANKL and OPG levels and ratio with BMD. A totalof 58 post-menopausal females from 13 elderly in Integrated Community Health Care Surabaya and Sidoarjo were enrolled.Data were collected by recording age, onset of menarche, onset of menopause, and Body Mass Index (BMI). Serum RANKLand OPG levels were evaluated using sandwich ELISA from Elabscience®. The RANKL/OPG ratio was obtained from the ratiobetween measured RANKL and OPG levels in serum. The proximal femur and lumbar spine BMDs were measured usingHologic® Discovery™ QDR™ Dual-Energy X-ray Absorptiometry (DEXA). Pearson's correlation test in this study showed nosignificant correlation between BMD and RANKL levels (lumbar: p=0.203; hip: p=0.283). The insignificant result was alsoshown in the correlation between BMD and OPG levels (lumbar: p=0.412; hip: p=0.617). A significant result between lumbarBMD and RANKL/OPG ratio was only found in the osteopenia subjects (p=0.001). The RANKL/OPG ratio had a significantcorrelation only with osteopenia-BMD in post-menopausal females. Therefore, it could be used as supporting data inosteoporosis screening.

Abstract

The technique of supernatant removal in urine sediment examination using the Shih-Yung method can be done by aspirating and decanting. The aspirated technique is the recommended technique. The Central Laboratory Installation of Dr. M. Djamil Hospital, Padang used decanted supernatant removal technique. The variety of preanalytical procedures affect the results of urine sediment examination. This study aimed to analyze the agreement of erythrocyte, leukocyte, and non-hyaline cast sediment examination results using the Shih-Yung method in aspirated and decanted supernatant removal technique. This study was an analytical observational study with a cross-sectional design of 37 urine specimens that met the inclusion and exclusion criteria at the Central Laboratory Installation of Dr. M. Djamil Hospital, Padang from July to September 2020. Examination of erythrocyte, leukocyte, and non-hyaline cast sediment using Shih-Yung method with aspirated and decanted supernatant removal technique. Numeric data were analyzed using the Mann-Whitney U test and Cohen's Kappa test for the degree of agreement, p-value < 0.05 was considered significant. The agreement test results for erythrocyte, leukocyte, and non-hyaline cast sediment in aspirated and decanted supernatant removal technique were (Kappa=0.88, p < 0.05), (Kappa=0.83, p < 0.05), and (Kappa=0.86, p < 0.05), respectively. The degree of agreement test results for erythrocyte, leukocyte, and non-hyaline cast sediment using the Shih-Yung method in aspirated and decanted supernatant removal technique were almost perfect and statistically significant. The technique for supernatant removal in urine sediment examination using the Shih-Yung method can be done by decanting.

Abstract

Acute Kidney Injury (AKI) remains a common complication of post-operative Coronary Artery Bypass Grafting (CABG)related to high morbidity and mortality. Systemic inflammation has been known as a part of the pathogenesis of acute kidneyinjury. This study aimed to analyze the Neutrophil Lymphocyte (N/L) ratio and Platelet Lymphocyte (P/L) ratio in post-operativeAKI. This research was a retrospective study involving 76 patients who underwent CABG surgery in Dr. Wahidin SudirohusodoCentral Hospital, Makassar, between January 2015 and May 2019. The diagnosis of AKI was based on Kidney Disease ImprovingGlobal Outcomes (KDIGO) 2012 criteria. The N/L ratio and P/L ratio were calculated respectively as a neutrophil count ratio tolymphocyte count and platelet count to lymphocyte count. Twenty-five (32.9%) patients developed AKI in the first 48 hours ofthe post-operative period. There was no difference in pre-operative N/L ratio and the P/L ratio between AKI and non-AKIgroups (p > 0.05). Both ratios increased after surgery and were significantly different between the two groups with higher meanratio in non-AKI (16.93±9.16 vs. 21.86±11.13, p=0.040 and 239,24±184,36 vs. 314,49±143,73, p=0.011). Both pre-operativeand post-operative N/L ratio and P/L ratio could not distinguish AKI after CABG.

Abstract

Gram-Negative Bacteria (GNB) have been widely reported to cause worldwide infections and life-threatening. The high prevalence of drug-resistant GNB causes the treatment of GNB to become difficult. This case report describes a stepwise laboratory approach and interpretation for Gram-negative bacteria infection in sepsis patients. An 84-year-old female patient with a history of congestive heart failure, after three weeks of hospitalization, GNB was proven as the cause of sepsis. Laboratory approach for inflammation (C-reactive protein, procalcitonin) was made and confirmed with a positive culture of several specimens (sputum, urine, and blood). The identification of bacterial-culture revealed as Carbapenem-resistance Klebsiella pneumoniae and Extended-spectrum Beta-lactamases Escherichia coli. This case highlights GNB as a potential agent to worsen the infection (sepsis) and also a useful approach for the detection of multidrug-resistant bacteria, particularly in secondary hospital settings. The application and interpretation of integrated clinical and laboratory criteria may bring out better and effective patient management.

Abstract

Cardiovascular disease remains a significant health problem in the Asia Pacific region. Several studies have found that dyslipidemia is a cause of morbidity and mortality and requires high medical costs. Dyslipidemia is a risk factor for atherosclerosis. The most widely used therapy for dyslipidemia is statins. Statins often cause muscle disorders such as myalgia, myopathy, and rhabdomyolysis, which can cause death. A prospective cohort study design was carried out at Airlangga University Hospital, Surabaya, from April to November 2019. A total of 26 sample pairs containing 13 samples were treated with Atorvastatin, and 13 samples were treated with Simvastatin. The subjects were examined for the creatinine kinase activity level using enzymatic methods. The mean creatinine kinase levels in the atorvastatin group before and after treatment was 105.71 IU/L and 100.03 IU/L, respectively, because the subjects were diagnosed with acute coronary syndromes and blood was collected during acute conditions. Median creatinine kinase levels in the Simvastatin group were 85.5 IU/L before therapy and 118.1 IU/L after therapy, indicating significant differences in creatinine kinase levels before and after treatment. Simvastatin is very susceptible to certain drug interactions that can increase the concentration of statins in the serum. There were differences in levels of creatinine kinase activity before and after Simvastatin therapy but not Atorvastatin.
