Abstract

Abstract Background: The incidence of myoma associated with pregnancy is reported about 0.3–5%. As the maternal age increase, so the incidence of myoma. Myomectomy is the most common surgical procedure performed during cesarean section. Many obstetricians are reluctant to perform this combined procedure as they are worried about the potential morbidity and mortality rates due to uncontrollable bleeding. Objectives of this case series is to investigate the success of myomectomy in cesarean section at Persahabatan Hospital, Jakarta. Methods: Four cases of leiomyoma with pregnancy underwent myomectomy during cesarean section. The indications for cesarean, the average surgery time, the amount of blood loss, the need of transfusion and intensive care, postoperative complications and length of stay were recorded as outcome. Results: The average time taken for surgery was about 100 minutes, average blood loss was approximately 300 ml to 1000 ml, more than usual cesarean section. There were no need of transfusion and intensive care, longer of stay or postpartum hemorrhage in any of the cases. Conclusions: Myomectomy at the time of caesarean section may be a safe and feasible procedure. Caesarean myomectomy, if done in well-equipped centers by an experienced obstetrician is a definite and safe option.

Abstract

Severe sepsis is sepsis with one or more organ dysfunction. One of the causes of sepsis in Intensive Care Unit (ICU) is Hospital-Acquired Pneumonia (HAP). HAP occurs 5-10 cases per 1000 patients on mechanical ventilation. Ventilator Associated Pneumonia (VAP) is predominantly caused by Pseudomonas aeruginosa. Several important factors in the pathogenesis of VAP are barrier to Na+-K+-Cl– transporter-1 (NKCC1), endotracheal tube device without antibiofilm. VAP can be prevented by oral hygiene, endotracheal tube device made from antibiofilm, head up 30 degrees, evaluation of cough ability, swallowing function

Abstract

Kesehatan anak Indonesia setiap tahun semakin membaik. Angka kematian bayi menurun dari 68/1000 kelahiran hidup (tahun 1991) menjadi 22/1000 kelahiran hidup (tahun 2015), angka kematian balita menurun dari 97/1000 kelahiran hidup (tahun 1991) menurun tajam menjadi 26 /1000 keahiran hidup (tahun 2015). Namun, untuk meningkatkan kualitas tumbuh kembang anak menjadi generasi penerus bangsa yang unggul masih menghadapi banyak masalah pemenuhan hak-hak anak sejak masa pembuahan, sampai remaja, yaitu: hak kelangsungan hidup, tumbuh kembang optimal, perlindungan, dan menyatakan pendapat yang harus dipenuhi dengan prinsip non diskriminatif untuk kepentingan terbaik anak.

Abstract

Introduction: Tuberculosis (TB) is an important health problem in developing countries. This infection results neutrophilia and lymphopenia. The neutrophil to lymphocyte ratio (NLR) is an easy, inexpensive, and sensitive examination. The NLR result in tuberculosis patients showed higher compare to normal population, with cut off greater than 2.16. The aim of this study was to know the difference of NLR result in adult TB patients before and after the intensive phase treatment. Methods: We collected all new cases adult TB patients who came to out-patient department in Atma Jaya Hospital, without any HIV infection, chronic kidney disease, diabetes mellitus, chronic obstructive pulmonary disease, and bacterial pneumonia. Baseline characteristic, leukocyte count, neutrophil and lymphocyte count, also NLR were collected before and after intensive phase treatment, then were analyzed using SPSS. Results: There were 44 participants included in this study, 23 (52.3%) of them were male, with median age were 37 years old. Eighty eight percent of the subject had pulmonary tuberculosis. The NLR result before treatment (2.77) was higher compare with the NLR result (2) after intensive phase treatment (p less than 0.001). Conclusion: There was a statistically significant decrease in NLR result after intensive phase treatment of adult TB patients.

Abstract

Introduction: Health education program to recognize and prevent degenerating disease is important for elderly. therefore, a national pension bank in Indonesia set initiative to provide health empowerment program in their waiting room, to optimize momentum when elderly community come and gather regularly in pension bank at every first week of the month. The bank organized health education and medical consultation program in the waiting room, cooperated with trained health educator and physician. This study investigated how elderly satisfy with the programs, and its effect to program and institution image. Method: We investigated 438 elderlies in 10 cities of 14 pension bank branches in Indonesia who participated in health education and consultation program. We assessed their satisfaction level related to health education and its cause using SERVQUAL approaches that consist of tangible, reliability, responsiveness, assurance and empathy. We also investigated how it would trigger individual willingness to spread word of mouth in promoting the health education program and the institution to other elderly, using standardized equation modeling (SEM) analysis. Result: Most of elderly satisfy and understood the both health education content (90%) and considered this program as meaningful program (84%). In health education, the highest factor contributes to elderly satisfaction were tangibles (educator and doctor performance) and reliability factors (regular schedule and delivery method of health education content). Meanwhile in medical consultation, doctor’s performance (tangible factor), responsiveness and empathy to elderly health problem contributed equally important to elderly satisfaction. Medical consultation contributed more in triggering word of mouth to promote the program and institution (SEM r=0.81) rather than of health education (SEM r=0.58)Conclusion: Not only doctor’s performance and health content delivery method, but responsiveness and empathy were important key factors to provide successful health education and services for elderly. Elderlies highly appreciated pension bank’s initiative in providing health education and consultation while optimizing time in waiting room, therefore this model can be applied in further health education program.

Abstract

Introduction: Clinical measurement of Tibiofemoral (TF) angle and Intermalleolar/Intercondylar (IM/IC) distance are quick, low cost, reliable, and radiation-free way to screen for pathological genu varus/valgum. TF Angle and IM/IC distance varied between countries and races. Meanwhile, research regarding normal TF Angle and IM/IC distance in Indonesia has not been reported before.Aim: To find the average TF angle and IM/IC distance on medical students of Medical Faculty of Airlangga University batch 2013.Methods: Descriptive epidemiologic study were conducted. The data is collected by direct measurement on the sample.Results: We measured 168 students which fulfilled the inclusion criteria. 65% of the samples were 20 years old and 61% of the samples were females. Amongst the samples, 64% had normal BMI, 11% were underweight, and 26% were overweight and obese. Average BMI is 22,9 ± 4,1. The average TF angle were -7,50 ± 3,350 valgum while the average IM/IC distance were -4,4 ± 33,09 mm intermalleolar. Compared to previous studies, there is slight differences in TF angle and IM/IC distance but were comparable.Conclusion: Amongst medical students of Faculty of Medicine Airlangga University, the TF angle and IM/IC distance were comparable to other studies and mostly had valgus angle.

Abstract

Background: Pulmonary cancer is the most prevalent malignancy in the world. Management of cancer in pregnancy creates a dilemma. Patients who choose to continue her pregnancy have to delay her cancer treatment in order to protect the fetus.Objectives: To review the case of pulmonary cancer in pregnancy presented in M. Djamil Central General Hospital Padang.Case: A 24-year-old woman came to the M. Djamil Central General Hospital Padang on September 12th 2017 with diagnosis of G2P1A0L1 37-38 weeks of term pregnancy + history of previous CS + Pulmonary Cancer. Patients have been diagnosed with Pulmonary Cancer based on bronchoscopy. Chemotherapy was initially planned for 9 times, but after the third chemotherapy the patient was known to be pregnant. Patient was encouraged to terminate her pregnancy but the patient refused and chose to continue the pregnancy.Conclusions: Pregnancy with Pulmonary cancer raises its own problems in its management. Practitioners must provide optimal treatment for the mother while protecting the fetus.

Abstract

Introduction : Conjoined twin is a form of congenital anomaly that take the attention of the medical world. Approximately 75% of conjoined twins are women and 70% are attached to the chest (thorachophagus) or abdomen (omphalophagus). Conjoined twins can be divided into 2 categories, which are symmetrical twins and asymmetric twins. The longer the embryological separation takes place, the more complicated the twin attachments that will occur.Case Presentation: A 31-year-old female patient was referred from Fetomaternal Polyclinic to M. Djamil Hospital Padang ward on August 28, 2017 at 11.00 with a diagnosis of G4P3A0H3 31- 32 weeks gravid + Conjoined Twins (Dicephalus Parapagus Dibrachius) + Polyhydramnios.Conclusions: Multifetus pregnancy is considered high-risk because it increases maternal and perinatal morbidity and mortality. In conjoined twins cases, multidisciplinary approach needs to be done for a better prognosis for the mother and baby.

Abstract

Anemia is commonly found in patients with chronic diseases and causing increased in morbidity and mortality. The prevalence of anemia of chronic disease is the second highest after iron deficiency anemia. Anemia of chronic disease oftenly occurs with iron deficiency anemia therefore both of them showed low in serum iron level. However, we should differentiate these two kinds of anemia in order to give the specific treatment. Some iron parameters have to be extracted to differentiate them. The main management for anemia of chronic disease is to treat the underlying causes.

Abstract

Introduction: Dengue infection is endemic to Indonesia and remains a public health problem. Severe dengue infection can cause rapid death, especially in children. Aim: To determine the epidemiology of children with severe dengue infection in Dr. Soetomo Hospital. A cross-sectional study was conducted at the pediatric ward of Dr. Soetomo Hospital. Data were derived from medical records of children aged ≤18 years which hospitalized with severe dengue from March to April 2019. Of 135 children admitted with DHF, obtained 48 samples with characteristics greater than 5–12 years old (72.9%), male (58.3%), residence Surabaya (89.6%), elementary school (60.3%), referral (81.3%), hospitalized ≤5 days (66.7%), day of illness ≥4 days (89.6%), moderate malnutrition (35.4%), had severe plasma leakage consisting of DHF grade III (73%) and IV (27%). Two patients died (4.2%). No patient with congenital disease and malignancy. This study is not in line with the tendency of shifted dengue incidence to older age groups (greater than15 years old) in Indonesia. All patients had severe plasma leakage. No severe dengue patient with severe hemorrhage and organ impairment. Most patients were male, had moderate malnutrition, from referral, day of illness ≥4 days, hospitalized ≤5 days.
