Abstract

Background: Morbidly adherent placenta is a condition which placenta adheres and invades deeply into the myometrium and, in some cases, until uterine serosa, thus contribute to peri-partum hemorrhage and significant maternal mortality.Purpose: To identify prevalence, clinical characteristics and their odds ratio to increasing se-verity of morbidly adherent placenta case in RSUP Persahabatan from 2015 to 2018.Methods: Cohort retrospective study.Results: From 2015 to 2018, prevalence of morbidly adherent placenta was around 0.8%. The risk factors which contribute to increasing severity of morbidly adherent placenta were: age above 35 years (OR 1.6, 95% CI 0.41-6.24) and history of Caesarean section more than 2 times (OR 1.63, 95% CI 0.41-6.46). The presence of increasing severity of morbidly adherent placenta were related to volume of blood loss more than 1000 ml during surgery (OR 2.13, 95% CI 0.52-8.76). Conclusion: The prevalence of morbidly adherent placenta at Persahabatan General Hospital in 2015-2018 was 0.8%. Age, history of Caesarean section more than 2 times, volume of blood loss, and duration of surgery were not statistically associated with adherent placenta case.

Abstract

Background: Pulmonary Aspergillosis is an opportunistic infection in the immunocompromised population. The polymerase chain reaction (PCR) method is expected to be a substitute for sputum mushroom culture with faster and more accurate results. Method: Searched in PubMed, CENTRAL, EbscoHost, and ProQuest from October 1-13 using the keywords “Aspergillosis”, “Pulmonary Aspergillosis”, “sputum”, “PCR”, “sputum fungal culture”. The search results are evaluated using inclusion and exclusion criteria. The selected study is then obtained by the full text and re-evaluated. The final results of the selection were then critically reviewed by validity, importance, and applicability by the three authors. Results: Four studies were obtained, with level of evidence 4, which were then critically reviewed. Three studies used PCR in sputum samples as index tests, while one study used it as a reference test. Output from three studies reported sensitivity and specificity of PCR with sputum samples, while one study reported a proportion of PCR results. Two studies reported PCR sensitivity in sputum samples of 100%, while one study reported a sensitivity of 38%. Two studies also reported specificities above 70%, while one study reported a specificity of 38.5%. Conclusion: Studies show promising values of sensitivity and specificity, especially with results obtained quickly. However, the low level of evidence and its costs are quite expensive, causing detection by the PCR method that cannot be used as a routine diagnostic modality.

Abstract

Introduction: Fetal hydrops is excessive accumulation of fluid inside fetal body. Fetal hydrops can be presented with fetal and placental edema, ascites, pleural and/or pericardial effusion. In past years, most fetal hydrops was caused by severe fetal erythroblastosis related to Rhesus isoimmunization. In recent years, it mostly caused by other condition (non-immune fetal hydrops). Cystic hygroma was an etiology of fetal hydrops. Cystic hygroma was connected with chromosomal abnormalities. This case was presented a woman in her 21 weeks of gestational age with fetal hydrops and cystic hygroma suspected caused by chromosomal abnormality. This case report was highlighting the connection between fetal hydrops and cystic hygroma and the role of genetic evaluation to diagnose aneuploid.

Abstract

Introduction: Chronic Obstructive Pulmonary Disease (COPD) is a global health problem that frequently result in disability. Disability can be caused not only by COPD effect on lung, but also it’s systemic effect. One of the effect is on the skeletal muscle. Strength training is one og rehabilitation method that effective in maintaining functional capacity of COPD patient. This review discuss systemic and biomolecular aspect of COPD, strength training and how strength treaining can affect functional capacity of COPD patient.

Abstract

Introduction: Stroke is the fifth leading cause of disability-adjusted life years (DALYs) in the world. Cognitive impairment is one of the disabilities found in the acute phase of stroke and persists in long-term outcomes which can be assessed using the Mini-Mental State Examination (MMSE). However, a clinical classification to predict the cognitive outcome remained unclear. This study is aimed to identify differences of MMSE results in stroke patients between right and left hemisphere lesions to ensure the mentioned location classifications may contribute to cognitive outcome prediction.Method: With the cross-sectional analytic observational design, 32 acute phase patients hospitalized in the Neurology Department Soetomo General Hospital from October–December 2019 were assessed using the Indonesian version of MMSE with purposive sampling and analyzed using the chi-square test.Result: There was no significant difference between MMSE scores in right or left hemisphere lesion. This might happen because (1) MMSE was insensitive and not a domain-specific test; (2) a more specific infarct location was needed to predict cognitive outcome post-stroke, including microarchitecture of the brain especially those involved in the cortico-striato-thalamocortical loop.Conclusion: The right or left hemisphere lesion classification did not contribute significantly to predict cognitive impairment.

Abstract

Konsultasi dan Rujukan adalah salah satu bentuk pelayanan kedokteran integrasi dengan tenaga-tenaga kesehatan lain baik di tingkat yang sama maupun ke tingkat yang lebih tinggi dalam rangka mengoptimalkan penatalaksanaan pasien. Tujuannya adalah untuk meningkatkan keilmuan, meningkatkan hubungan dengan sejawat, rekan kerja, dan pasien, serta menjamin tatalaksana pasien secara komprehensif dan berkesinambungan. Proses konsultasi dan rujukan memungkinkan pelayanan kedokteran menjadi tidak terkotak-kotak. Namun hal ini membutuhkan koordinasi dan komunikasi efektif antar dokter, pasien dan tenaga kesehatan lain.

Abstract

Introduction: Being in the ring of fire have caused Indonesia prone to emergency and disasters. This condition should make a positive correlation to level of citizen’s knowledge and attitude towards emergency and disaster situation. However, there is no study to assess this condition. This study aims to assess the knowledge and attitude of Indonesian in emergency and disaster using a cross-sectional study collected from questionnaire interviewed by trained medical students in four centers (Jakarta, Depok, Padang, and Makassar).Method: Samples were collected using random cluster sampling. Out of 570 samples participated, most of the respondents had a poor knowledge (56.1%) and attitude (60.7%). Result: There is a significant relationship between knowledge and attitude (p less than 0.001). Age, education, and training on disaster can affect both knowledge and attitude towards emergency and disaster situation (p less than 0.05). However, disaster experience only had a significant contribution towards knowledge (p less than 0.05) but not the attitude (p = 0.856). Conclusion: Most of the population in this study had poor knowledge and attitude in emergencies. These presented data also indicate that training on disaster is urgently needed to give impact on citizen’s awareness. Moreover, further research is needed.

Abstract

Introduction: Good nutrition in the workforce can affect work productivity, but many workers have not paid attention to their nutritional balance, including helper workers in container company. Based on data from the International Labor Office (ILO) in 2005, unbalanced diet on labor can reduce productivity by up to 20%. The purpose of this study was to find out the effect of snack intervention toward working duration among helper workers in container company.Method: This study used pre-experimental design which was conducted for four weeks. Eighteen respondents who were helper workers were given 300 kcal snack once a day for four weeks. The respondent’s food intake was recorded with food record every week (two working days and one day off) and analyzed by Nutrisurvey application and manual calculation. Result: There was significant mean difference in total working duration (reduced two hours and eight minutes) before and after intervention (p less than 0.001). Conclusion: Once a day snack intervention for four weeks can shorten the working duration of helper workers.

Abstract

Introduction: Determination of the primary tumor of a malignant pleural effusion is needed for an appropriate management. The most common etiologies of malignant pleural effusion are cancer of the lung, breast, lymphoma, etc. Mammaglobin (MGB) and gross cystic disease fluid protein 15 (GCDFP15) are currently used as breast specific markers in Indonesia. GATA binding protein 3 (GATA3) has recently reported to have a better diagnostic accuracy. Purpose: To compare the accuracy of GATA3 and MGB/GCDFP-15 as breast specific marker in determining the primary tumor of a malignant effusion cytology specimen.Methods: We searched Pubmed, Cochrane, Proquest, Scopus, EBSCOHost, and found 7 relevant articles to be appraised.Result: With various positive cutoffs, the accuracy of GATA3 for cell blocks of effusion is significantly better than MGB and GCDFP-15, with sensitivity of 90-93,5% and specificity of 88,5-96,9%. The staining intensity of GATA3 was also higher, with less background staining so that reading and interpretation could be done easier.Conclusion: The accuracy of GATA3 as a breast specific marker to determine the primary tumor of a malignant effusion cell block is better than MGB and GCDFP-15. Thus, GATA3 may be considered to be used as a breast specific marker to replace MGB and GCDFP-15.

Abstract

Background: Effects of antipsychotics in the elderly (elderly) is associated with cardiovascular side effects. Some cardiovascular side effects can occur in the elderly and can cause sudden death. The type of antipsychotics used is thought to be related to the incidence of cardiovascular mortality.Methods: A structured search was conducted at Pubmed, EBSCO, and Science Direct using keywords based on the Boolean operators concept that was suitable for clinical questions. The target population is elderly patients who use antipsychotics and effects on the cardiovascular system.Results: After filtering 68 titles and abstracts using inclusion and exclusion criteria, 2 articles were obtained. Furthermore, a critical review is based on validity, importance of results, and application. Both articles showed a significant association between the use of antipsychotics and the incidence of cardiovascular mortalityConclusions: The use of antipsychotics in the elderly can lead to cardiovascular mortality risk, it is necessary to assess the profile of the benefits and risks of using antipsychotic drugs as well as dose adjustments and evaluation of side effects.
