Abstract

Congenital heart disease (CHD) is a disorder both in the structure of the heart or large blood vessels and heart function that is obtained since it is still in the womb and can be a major cause of death in the first year of life. The prevalence of CHD events worldwide reaches 9.4 per 1000 live births including in Indonesia. Of these, around 300,000 cases are categorized as severe CHD which sometimes requires gradual surgery in order to survive. The purpose of this paper is to increase the understanding of the signs and symptoms of CHD by general practitioners with simple modalities that can be used in the initial screening of CHD in peripheral areas. Early detection of CHD in primary health services can be done with history taking, physical examination, and simple modalities such as pulse oximetry, electrocardiography, and chest X-ray. Early detection of CHD is expected to reduce the morbidity and mortality rate of CHD in Indonesia.

Abstract

Introduction: Depression is the most common mental disorder. Depression of outpatients in primary health care is frequently unrecognized. It might affect sleep quality, which is associated with worse quality of life, poor medication adherence, higher morbidity, and mortality. The study aimed to determine the association between depression and sleep quality of outpatients in Penjaringan Primary Health Care, Jakarta. Method: This research was observational analytic using a cross-sectional approach involving 434 outpatients in Penjaringan Primary Health Care, Jakarta. The study was conducted from August until September 2019. Instruments used in this research were the demographic questionnaire, the Zung Self-Rating Depression (SDS) questionnaire, and the Pittsburgh Sleep Quality Index (PSQI) questionnaire. Data analytics were bivariate. Results: There were 19.1% of respondents had depression, including 12.2% mild depression, 5.8% moderate depression, and 1.2% severe depression. There were 53.5% of respondents had poor sleep quality. There was a significant relationship (p less than 0.001, OR=7.814) between depression and sleep quality in outpatients. The results also showed that depression scores were moderately correlated with sleep disturbances (r=0.434). Conclusion: This study showed a high prevalence of depression and poor sleep quality among outpatients. Depression was associated with a 7.8 times higher risk of having poor sleep quality and also correlated with the components of sleep quality especially sleep disturbance.

Abstract

The pandemic of Systemic Acute Respiratory Syndrome-coronavirus-2 (SARS-CoV-2) could not be stopped until mid-June 2020. The difficulty in finding a panacea for this disease made the vaccine very valuable. At present there are more than 100 vaccine candidates worldwide, with at least 10 have begun undergoing clinical trials in humans. The vaccine at its farthest stage is from the University of Oxford, which has entered phase 3. There are multiple approaches in vaccine research, and some methods have never been used before. Some of the vaccines in the study were originally intended for the previous SARS-CoV and Middle East Respiratory Syndrome-coronavirus (MERS-CoV). The main target in general is the protein spike S. Several obstacles need to be considered, such as the absence of a vaccine for the corona virus, the antibody-dependent enhancement (ADE) mechanism, and the possibility of immunopotentiation. In Indonesia there are 4 teams with vaccine research, and 3 other companies that work with international institutions abroad to directly enter the clinical trial phase 2 or 3. After the vaccine has been found, it is necessary to guarantee equality in gaining access among various countries.

Abstract

Objective: COVID-19 is a new emerging disease since December 2019 that later was announced by WHO as a global pandemic. Radioimaging appearance of COVID-19 is varied yet dominantly showed as a ground glass appearance in CT-imaging. Lung abscess in COVID-19 is an extremely rare case, therefore it needs further discussion. Method: We report a 39 year-old male patient of COVID-19 with lung abscess.Result: This case report describe a COVID-19 patient who showed a very rare clinical manifestation; lung abscess. The patient showed negative result of TB molecular test. Chest tube insertion procedure was performed in this patient who showed an improvement after abscess being drained. Conclusion: Lung abscess may occur in COVID-19. Abscess drainage procedure may improve clinical condition of COVID-19 patient.

Abstract

Background: Adolescents’ lifestyle has been changing overtime, impacting their body weight, height, body mass index, and waist hip ratio. This changes also outgrow with the changes of sagittal curvature of thoracic kyphosis and lumbar lordosis. Thoracic hyperkyphosis and lumbar hyperlordosis is a disease not only affecting convenience, but also psychologically impacted (body image disorder). The only way to stop this is by aiming for preventive measure, by knowing the correlation between these variables to thoracic hyperkyphosis and lumbar hyperlordosis.Method: A cross sectional study was conducted in two schools, SMAN 2 Jakarta and SMA Katolik Diakonia. This study used weight scales to measure body weight and scaled-tape to measure body height. The sagittal curvature was measured by flexicurve.Result: Out of 154 respondents, 49.35 % were male students and the rest 50.65% were female students. Hyperkyphotic students were found 24.68%, and hyperlordotic students were found 24.02%. No correlation was found between variables but IMT to lumbar hyperlordosis. Weak correlation also found between waist and hip circumference and lumbar hyperlordosis.Conclusion: Weak correlation was found between Body Mass Index (BMI) and lumbar hyperlordosis in high school students. No correlation found between waist-hip ratio and lumbar hyperlordosis, but weak correlation was found between waist and hip circumference and lumbar hyperlordosis. No correlation was found between variables and thoracic hyperkyphosis.

Abstract

Introduction: High myopia correction with clear lens extraction, with or without IOL implantation, remains controversial and associated with a high risk of postoperative complications, especially retinal detachment. However, advanced technology development in the cataract surgery field had resulted in excellent surgical outcomes with a very low complication rate. In this article, we will present an outcome of clear lens phacoemulsification for high myopia correction in four eyes from two patients.Case presentation: Two women with high myopia undergone clear lens phacoemulsification with supracapsular tilt and tumble technique. IOL calculation using SRK-T formula. Both patients had 20/20 postoperative visual acuity in both eyes, which remains until one year postoperative. No retinal abnormalities found in both patients.Conclusion: Clear lens phacoemulsification for high myopia correction is an effective procedure and can prove excellent visual outcome for a long-term period. It can be considered as an alternative method for high myopia management.

Abstract

Konsep dan Peran Dokter Keluarga di Indonesia Kasus positif Covid-19 pertama di Indonesia terdeteksi sekitar awal Maret 2020. Sejak saat itu, telusur dan penguatan kapasitas pelayanan kesehatan terus dilakukan termasuk penguatan fungsi dokter keluarga di layanan primer. Peran layanan primer dalam sistem kesehatan nasional adalah sebagai ujung tombak pengelolaan kesehatan masyarakat dengan sifat pelayanan holistik (memandang pasien sebagai bagian dari lingkungan), bersinambung, berorientasi pencegahan dengan pemberdayaan pasien, keluarga, dan masyarakat.

Abstract

Background: Chronic obstructive pulmonary disease (COPD) is an important disease worldwide in both high-income and low-income countries. Dust has been known to increase COPD risk. During sweeping activity, sweepers are exposed to dust. The street sweepers are exposed to dust particles, bioaerosols, and various harmful gases. In this study we evaluates the prevalence of COPD among street sweepers in Jakarta Method: This is a cross-sectional study among 153 street sweepers in Jakarta, Indonesia with age more than 40 years old with working period more than 2 years. Subjects were collected by cluster sampling method based on working location correlated with Jakarta regional district area. COPD was diagnosed by using questionnaires of COPD Assessment Test (CAT), The Modified British Medical Research Council (mMRC), spirometry examination based on Pneumobile Project Indonesia, and bronchodilator test if there was obstructive results. Results: A total of 153 subjects was selected for spirometry examination. The prevalence of COPD among street sweepers in Jakarta, Indonesia was 10 of 153 subject (6.5%). Six of them were males (60%), do not use face mask (80%), working years greater than 10 years (70%), smokers (60%), and BMI ≤25 kg/m2 (80%). There was a statistically significant relationship between age and COPD (p less than 0,05). Conclusion: Prevalence of COPD among street sweepers in Jakarta is 6.5%. Factor related to the occurrence of COPD is age.

Abstract

Background: Blood transfusion is an important part of modern health services. When used properly, transfusion can save patient's life and improve health. Blood transfusion has risk of transmitted infectious diseases through blood transfusion (TTI) especially HIV / AIDS, Hepatitis C, Hepatitis B, and Syphilis, and Malaria, Dengue Fever (DHF) can only be done in endemic areas, as well as other transfusions which can be life threatening. Method: This was descriptive study with cross sectional approach and sampling techniques using secondary data with total population method. From the sample of 23,875 samples (12,305 (2017) and 11,573 (2018)), it was seen which results of TTI reactive from donor data to be included in finding research objects. Results: In 2017 HbSAg reactive TTI parameters 1.7%, Anti HCV 0.5%, HIV Ag / Ab 0.7%, and Treponema 4% and TTI reactive results in donors in 2018 HbSAg parameters 1.3%, Anti HCV 0.3%, HIV Ag / Ab 0.25% and Treponema 2.2%. Comparison of donor TTI screening results in Blood Center of North Aceh-Indonesia in the period 2017-2018 changes in the ratio of TTI reactive numbers in the HbSAg parameter of 0.4%, Anti HCV 0.2%, HIV Ag / Ab 0.45% and Treponema 1.8% in period 2018. Conclusion: Although the reactive rate in 2017 is relatively high, in 2018 the reactive TTI level shows a decline. This condition can be caused by two factors. ; (1) Implementation of donor selection was good with good data collection and documentation system. (2) The inspection method carried out affects the results of the existing inspection.
