Abstract

Objective: Since December 2019, COVID-19 has become a global concern causing more than thousands of deaths worldwide. The major challenge in fighting against this new emerging disease is the limited data regarding the heterogeneity of its clinical and epidemiological features. We aim to fill this knowledge gap by evaluating a mild case of COVID-19 infecting a health worker in Indonesia.Method: We report a 31-year-old male doctor who was hospitalized for mild pneumonia with positive SARS-CoV-2 swab testResult: This case presents an estimated incubation period of 2 days. The patient showed mild upper respiratory symptoms without significant pneumonia symptoms, however mild pneumonia was confirmed by the presence of fever and pulmonary infiltrates on imaging. Despite the seemingly mild symptoms, there was a complex systemic reaction as thrombocytopenia and elevated aminotransferase enzyme occurred. The patient is a health worker susceptible to SARS-CoV-2 transmission with a close contact history with a suspected COVID-19 patient, without wearing an adequate personal protective equipment (PPE).Conclusion: Patients with mild presentation of COVID-19 could exhibit a complex systemic reaction and organ injury. The use of an appropriate PPE among health workers is crucial to prevent patient-to-doctor transmission.

Abstract

Introduction: Early detection to diagnose PCOS as a promotive and preventive effort is needed in PHC.Method: By using a descriptive research design that was conducted on one of the general practitioners in the Maternal and Child Health Polyclinic (MCH) in 30 PHC was targeted by PCOS from August - October 2019. Result: Thirty general practitioners participated in filling out the questionnaire. Obtained results based on risk factors divided into three categories 33.33% good, 50% sufficient and 16.67% deficient, for causes 6.67% good, 40% sufficient and 53.33% deficient and clinical symptoms 6.67 good%, 40% sufficient and 53.33% deficient. Conclusion: General practitioners at the PHC had a sufficient level of knowledge for risk factors categorized, causes were lacking and symptoms of clinics were lacking. This was an important finding so that it was necessary to increase the specific knowledge of PCOS for the main steps of early detection in PHC.

Abstract

Introduction: Candida krusei inhabit the human body as saprophyte but can turn into pathogen and causes mycosis. Candida krusei usually primarily resistant to fluconazole but laboratory data in Jakarta has different results. We found fluconazole-susceptible isolates which encouraged us to do prospective studies using the different isolates of C. krusei.Method: The study was conducted in 2019-2020 in the Laboratory of Parasitology, Faculty of Medicine, Universitas Indonesia. The total of 15 C. krusei isolates were tested by disc diffusion method. Before being tested, the isolates were refreshed and purified to obtain a single colony. Result: The isolates used, derived from many clinical samples i.e. stool, sputum, broncho-alveolar lavage (LAB) and one isolate isolated from lung abscess. The result of susceptibility test showed nine fluconazole-resistant isolates, one intermediate isolate and five fluconazole-susceptible isolates. Conclusion: It can be concluded, in vitro, some Candida krusei isolates in Jakarta are susceptible to fluconazole.

Abstract

What’s our status? As per 17 April 2020, Indonesia has the most confirmed cases (5.923 cases) of Coronavirus Disease (COVID-19) among Southeast Asian Countries.1 Considering the daily increase of cases, we are considered nowhere near the plateau of the epidemic curve. It was predicted that the peak may be reached from May to June.2 Predictably, this will not end soon. COVID-19 contracted patients may also vary in severity. The patient may present with no symptoms, mild to moderate, and severe condition. Mild (81%) depicted as COVID-19 patients with no evidence or mild pneumonia, severe (14%) for COVID-19 patients with hypoxia and greater than 50% lung involvement within 24-48 hours, critical (5%) if the patient develops shock, acute respiratory distress syndrome, myocarditis, or even organ failure which need intensive care, and 2.3 % were fatal which end up in death.3,4

Abstract

Background: Thalassemia major ranks 5th among catastrophic diseases according to the National Health Insurance, BPJS. Health workers are expected to have good knowledge about thalassemia to raise public awareness for thalassemia carrier early detection. Aim: To explore the knowledge about thalassemia among medical students and general practitioners (GPs) in Bandung, and the prevalence of β-thalassemia carrier.Methods: Analytic study was designed using questionnaire to compare knowledge. The prevalence of anemia due to iron deficiency or β-thalassemia carrier was described using Mentzer Index (MCV/RBC) and Shine and Lal index (MCV*MCV*MCH/100). Results: The knowledge about thalassemia among medical students (n597) and GPs (n171) was moderate, however, there was a significant difference; GPs greater than 30 years old had lower knowledge compared to students. Students attended blood examination had better knowledge (p 0.036). Type of anemia among students (7.9%) was different according to Mentzer index vs. Shine and Lal index; iron deficiciency anemia (5.3% % vs. 3.2%) dan β-thalassemia carrier (2.6% vs. 4.7%). Conclusions: Knowledge of thalassemia among medical students and GPs need to be enhanced, since 6-10% of Indonesian population are thalassemia carriers. Further examination is needed to confirm thalassemia carriership.

Abstract

Background: Postgraduate program for Anatomical Pathology Specialist in Faculty of Medicine Universitas Indonesia use competence-/outcome-based curriculum approach. However, until now, the program has not yet adopted a structured, recorded, formative workplace-based assessment, for example, case-based discussion.Method: This was an explorative qualitative research regarding the use of formative assessment case-based discussion during educational process in postgraduate program for anatomical pathology specialist in FMUI. Interview and focused group discussion to the program manager, teaching staff and the residents were performed. Teaching staff was asked to perform three times case-based discussion (CbD) formative assessment toward the resident. Post-intervention, interview and focused group discussion to the staf and resident were conducted. Result: The staffs and residents of Anatomical Pathology Specialist Program of FMUI showed positive response toward CbD formative assessment. CbD formative assessment enabled deeper discussion between the staffs and residents regarding establishing diagnosis. The staffs were able to monitor the residents’ learning process and giving specific feedback toward the residents. The residents were able to learn about a case in a more comprehensive way and acquiring specific feedback and summative assessment simulation.Conclusion: CbD formative assessment is useful in the process of acquiring competence in diagnosis in a postgraduate education that uses competence- or outcome-based curricula.

Abstract

Since the beginning of 2020 there has been an increase in the number COVID-19 virus infection, previously known as 2019-nCov. It is known that up to February 2020, more than 70,000 positive cases of COVID-19 infection have been confirmed. COVID-19 is known to be a virus from the coronaviridae family, which is also a family of the SARS-CoV and MERS-CoV viruses that caused an outbreak in 2002 and 2012. The exact pathogenesis of COVID-19 is not clearly known, but is thought to be similar to the pathogenesis of SARS-CoV and MERS-CoV. The average COVID-19 mortality was 2.3%, lower than the SARS-CoV and MERS-CoV mortality. The COVID-19 outbreak was also thought to have started from a local seafood sales market in winter, an environment that was almost the same as the SARS virus outbreak. This scientific article will discuss in depth the operational definitions that can be used in the management of patients with suspected COVID-19 infection. It will also discuss clinical manifestations and management in patients with suspected and confirmed COVID-19 infection.

Abstract

Background: Stroke is the chief cause of long-term disability in the world. Predominantly, the disease is known as a degenerative disease on geriatrics age group, yet several studies have proven that incidence on productive ages are also increasing. Objective: The study aimed to know whether there are any difference in Modified Shah Barthel Index (MSBI) score in different age group, geriatric and productive age group and to know independence score in daily activities based on age. Method: The study utilized the cross-sectional design and analytical observational method. Twenty five samples of chronic stroke patients was recruited and grouped into two groups productive and geriatric age group. MSBI score was calculated and compared between two groups. Result: Bivariate analysis proves that there is a significant association between MSBI score and age of stroke patients on chronic phase (p=0.017). Discussion: There is a significant association between age and MSBI score on stroke patients in chronic phase. Conclusion: There was a significance difference between productive and geriatric age with daily activities using MSBI in chronic stroke patients. The productive age group has a higher Modified Shah Barthel Index score than the geriatric age group.

Abstract

Anxiety has manifestations involving excessive fear and anxiety that impact everyday life. The prevalence of anxiety was estimated reaching 10.6% in a year and 16.6% for life in the world. Psychobiotics become one of the promising therapies for anxiety. This paper was made by finding sources from journals and textbooks related to the pathogenesis of anxiety, relationship between brain and the gastrointestinal tract, microbiota and psychobiotics, and also the relationship between psychobiotics and anxiety. Anxiety is caused by the dysregulation of neurotransmitters such as gamma-aminobutyric-acid, serotonin, and noradrenaline. Tryptophan metabolism via kynurenine pathway and hormones secreted by digestive tract also play roles in anxiety. Apart linked by the N. vagus, the inflammatory factor is produced by cortisol in hypothalamus-pituitary-adrenal axis connecting digestive tract and brain in terms of anxiety. Psychobiotics play a role in producing neurotransmitters that are dysregulated during anxious conditions. The production of short-chain fatty acids, tryptophan metabolism, and microbe-associated molecular patterns in psychobiotics creates a protective factor against intestinal leakage and reduces inflammation produced by pathogenic bacteria. Short-chain fatty acids interact with cholecystokinin peptides, tyrosine peptide tyrosine, and glucagon like peptide-1. Relationship between psychobiotics and anxiety is mediated by N. Vagus. Thus, psychobiotics play a role in helping reduce anxiety.

Abstract

Digestive surgery service including surgical management of gastrointestinal disease and digestive cancers are experiencing the impact of COVID-19 pandemic. Therefore it is necessary to formulate recommendation for digestives surgery service, as guidelines to engage in case-by-case assessment of particular patients with digestive diseases. We are aware that the knowledge and science of COVID-19 are still evolving, with new progression every day. This recommendation reflect actual condition and are subject for future adjustment in the future.
