Abstract

Coronavirus disease 2019 (COVID-19) is an acute respiratory infection which is caused by SARS-CoV-2 virus. Studies showed that the transmission of the virus is from human to human the primary transmission of SARS-CoV-2 infection is via droplets produced by sneezing or coughing. There is a possibility that COVID-19 can be transmitted through aerosol or airborne transmission. The incubation period of this infection is predicted to be between 3-14 days after close contact. Patients with COVID-19 could have various clinical manifestations from asymptomatic to respiratory failure. Although a few cases revealed that COVID-19 could result in critical condition, COVID-19 is relatively mild in children. Physicians must diagnose and assess the clinical severity of the patient with SARS-CoV-2 infection as it affects the management of the disease. Several anti-viral medicines that might be effective in treating COVID-19 patients such as Remdesivir, Chloroquine/Hydroxychloroquine, Lopinavir-Ritonavir, and Tocilizumab are currently still on trial. It is also crucial to prevent the spread of COVID-19 by encouraging people to apply hand-hygiene, practice social distancing, and disinfect all possible sources.

Abstract

Introduction: Low back pain is a condition characterized by pain and stiffness on lumbosacral region. Whole body vibration is a mechanical vibration transmitted to whole body. Whole body vibration is a risk factor for low back pain. To determine the relationship of whole body vibration with low back pain.Method: Using cross-sectional method for 27 online ojek driver in Jakarta. Respondent filled the Nordic Musculoskeletal Questionnaire (NMQ). Vibration from the motorcycle measured with smartphone application. Data analysis using Fisher Exact test.Result: There are seven respondent with LBP and vibration exceeding ELV, four respondent with LBP and vibration not exceeding ELV, four respondent without LBP and vibration not exceeding ELV and 12 respondent without LBP and vibration not exceeding ELV.Conclusion: There is correlation between whole body vibration and low back pain.

Abstract

Introduction: Atopic Dermatitis (AD) is an inflammatory skin disorder characterized by chronically relapsing inflammation accompanied by pruritus and itching. Inflammatory mechanism and many body immune responses are involved in AD. There are several diagnostic tests to diagnose AD. NLR, MLR and relative eosinophil count are parameters that can be obtained easily and cheap from the economic side. The purpose of this study is to determine the role of neutrophil-to-lymphocyte ratio (NLR), monocyte-to-lymphocyte ratio (MLR) and relative eosinophil count in AD patients with analyzing the sensitivity and specificity.Methods: This research is done by a cross-sectional method with peripheral blood smear. The number of sample taken were 61 patients with AD who had fulfilled the inclusion and exclusion criteria and 61 healthy controls. The data is then analyzed using Komolgorov-Smirnov and Saphiro-Wilk normality test, Independent T test, Mann-Whitney U.Result: The data analysis result shows a not statistically significant relationship between NLR in AD patients and healthy controls, p=0,858. This study also shows a significant relationship between MLR and relative eosinophil count in AD patients and healthy controls with p=0,000 and p=0,013. The cut-off point of MLR and relative eosinophil count each are less than 0,0488 (sensitivity 70,5%, specificity 72,1%, AUC 72,5%) and greater than 2,25 (sensitivity 70,5%, specificity 77%, AUC 78%).Conclusion: MLR and relative eosinophil count can be proposed as diagnostic tools in assisting the diagnosis of AD, however NLR can not be used as AD diagnostic tool

Abstract

Introduction: Physical Medicine and Rehabilitation Specialists (Physiatrists) usually take care of patients with Spinal Muscular Atrophy Type II in an early childhood. The progression of respiratory dysfunction should be considered as the most disabling condition in patient with Spinal Muscular Atrophy Type II and need further respiratory rehabilitation approaches.This case describes an adult Spinal Muscular Atrophy Type II that is considered a late or neglected case as a lost case in the management of respiratory rehabilitation but his remain functional reserve must be optimized to reach a manageable respiratory outcome in daily living.

Abstract

Introduction: Diabetic retinopathy is a complication of diabetes and is one of the leading causes of blindness. Diabetic retinopathy occurs when the blood vessels in the retina are damaged and this is associated with hyperglycemia and other conditions in diabetes. Bevacizumab is used to treat diabetic retinopathy and other diseases that affect the retina. Bevacizumab injection is used to prevent vision loss by blocking the abnormal development of blood vessels in the retina. This study aims to examine the characteristics and visual acuity of diabetic retinopathy patients before and after bevacizumab injection.Methods: This type of research is a retrospective descriptive conducted by collecting data from medical records that meet the inclusion criteria. Data were collected from January 1, 2019 to December 31, 2019, totaling 69 eyes from 40 patients as the study sample.Results: The results showed that most of the eyes had NPDR, which amounted to 55 (80%) eyes. Prior to bevacizumab injection, visual acuity of 6/6 – 6/18 was 18%, visual acuity of 6/19 – 6/60 was 37.7% and visual acuity of 5/60 – 1/∞ was 43.5%. After injection of bevacizumab, visual acuity of 6/6 – 6/18 was 27.5%, visual acuity of 6/19 – 6/60 was 42% and visual acuity of 5/60 – 1/∞ was 30.4%. In this study, eyes that did not experience changes in visual acuity were 52.2%, eyes that had improved 34.8% and eyes that had decreased 13%.Conclusion: In conclusion, after the injection of bevacizumab, the eyes that did not experience changes in visual acuity were more after 1 month follow-up, namely 52.2%.

Abstract

Patients in the intensive care unit (ICU) usually have muscle weakness problems that are not related to their primary diseases when admitted ICU. This problem may become a clinical syndrome that we called ICU-Acquired Weakness (ICU-AW). There is currently no specific treatment for ICU-AW, therefore preventing patients from ICU-AW by controlling associated risk factors such as immobility, becoming a critical approach. Early mobilization (EM) has many benefits in the ICU setting for patients with or without mechanical ventilation. Besides that, the realization of EM needs consider the safety criteria and the barriers to their application. Thus, there are guidelines for the realization of EM to give the best outcomes while minimalizing the adverse events.

Abstract

Introduction: Intelligence affects someone in thinking and solving problems. Frisson sensation increase dopamine hormone production, activity and blood flow to the brain. Aim: To prove the association between frisson sensations on music stimulus and intelligence performance change. Methods: A quasy experimental study with one group pre- and post-test design was done to female medical students of Diponegoro University. A total of 30 participants were selected by purposive sampling technique. The stimulation was by listening to mp3 of 5 researcher-selected songs that last for 15 minutes in 3 consecutive days. The frisson measured by using a stopwatch. Intelligence performance before and after the frisson sensation was measured using Ravens Progressive Matrices, so that the difference could be calculated. The collected data was then analyzed using Wilcoxon test and simple linear regression. Result: The pre-test score had a median of 50.50 (30-56) and a post-test score of 54 (33-59), with a significant increase in RPM scores (p less than 0.001). The duration of frisson sensation had positive effect on RPM scores differences (p less than 0.001). Association between the duration and the score improvement could be formulated as: y = 0.047 + 2,711x. Conclusion: The frisson sensation on music stimulus can improve intelligence performance. The frisson that lasts longer will have more significant improvement on intelligence performance

Abstract

Introduction: Research on the motivation to choose medicine as a profession and future career plans as a doctor are very important for policy makers and educators. The study of doctor’s career choice has been carried out in many countries so that it can provide an overview of the proportion of career choices from various perspectives. Objective: Analyzing career choices and determinants of career choice for internship doctors in DKI Jakarta Province.Methods: This research is a quantitative study using a descriptive observational design that is a cross-sectional study. This research was conducted on Internship Doctors in DKI Jakarta Province in 2020 with a sample of the study was the Internship Doctors of DKI Jakarta Province Batch III, totaling 154 people. Multivariable logistic regression analysis was used to determine the relationship between the career choice of interns and the determinants of career choice.Results: The results of the study found that 77.3% of internships chose a career as a specialist and only 16.2% were general practitioners. As many as 72.1% of internship doctors chose to work in hospitals and only 5.2% at Puskesmas. The results of the bivariate analysis with the chi-square test showed that the choice of place of work, prospect income, economic and professional work had a significant relationship with the career choice of an internship doctor. The results of multivariate analysis with multinominal logistic regression test showed that there were several independent variables that had a statistically significant relationship to the internship doctor’s career choice, including gender, university origin, place of work, income, lifestyle and family demands.Conclusions: There are more external determinants that are significantly related to the doctor’s career choice than the internal determinants.

Abstract

Introduction: Diabetes and obesity are health problems that affect human life from health, social and economic perspective. Complications caused by diabetes are metabolic problems and lead to diabetic nephropathy. White turmeric rhizome (Curcuma zedoaria) has benefits including antioxidant, anti-inflammatory, anti-proliferative, anticancer, and antimicrobial. Objective: This study is to determine the effect of white turmeric rhizome extract on blood lipids in Wistar rats with diabetic nephropathy. Methods: This research is an experimental study with post-test only control group design, and took place in Pharmacological Laboratory of Pharmacy Faculty, Histological Laboratory of Medical Faculty of Universitas Sumatera Utara and Biomolecular Laboratory of Universitas Prima Indonesia on Apirl until September 2020. Results: Wistar male white rats were divided into six groups; namely the normal control group (P1), the negative control group (P2), the positive control group (P3), and three groups treated with white turmeric each with dose of P4 = 100, P5 = 200, P6 = 400 mg / kgBW which previously induced by alloxan. Data was analyzed using One Way Anova test, which has significance in group given 100 mg/BW white turmeric extract. Conclusions: In conclusion, white turmeric extract can be used to stabilize serum cholesterol and triglyceride levels in diabetic nephropathy rats.

Abstract

Introduction: Currently COVID-19 continues to show an increase in prevalence and mortality. Fever, dry cough, fatique and progression to dyspnea even respiratory failure and ARDS are the symptoms of COVID-19. “Happy hypoxia” is an indication that certain patients do not express breathlessness but have impaired oxygenation. It is important to determine the degree of oxygenation an all COVID-19 patients that will predict the severity of the disease and provide an outline of further management plans.Objective: This study aimed to describe the parameters of blood gas analysis in hospitalized patients and analyze its correlation with degree of severity of the diseaseMethods: We conducted observational analysis, cross sectional, single-center study including 71 laboratory-confirmed patients in Dr. Saiful Anwar General Hospital, Malang, Indonesia from April-Juni 2020. Statistical analysis was performed to determine the determine the relation of blood gas analysis with disease severity. Result: Subjects were divided to 25 patients (35,21%) in the mild-moderate group and 46 patients (64,79%) in severe group. Analysis of demographic and clinical characteristic showed that age, history of smoking, dyspnea and oxygen delivery were associated with disease severity (p less than 0,005). Statistical analysis of blood gas analysis showed associated degree of oxygenation with disease severity which is assessed by PaO2, SaO2 BGA, PaO2/FiO2 and original PaO2 (p less than 0,005). Conclusion: In our cross sectional study we found that degree of oxygenation was associated with disease severity in hospitalized COVID-19 patients.
