Abstract

Pasien dalam kondisi sakit kritis sering kali mengalami risiko luka tekan akibat imobilisasi yang berkepanjangan. Luka tekan disebabkan oleh tekanan, gesekan, kelembapan, serta kondisi internal seperti malnutrisi dan anemia. Masalah ini memerlukan intervensi yang efektif untuk pencegahan. Tujuan: Studi ini bertujuan mengevaluasi efektivitas pijat effleurage menggunakan virgin coconut oil (VCO) sebagai bagian dari perawatan keperawatan dalam mencegah luka tekan pada pasien ICU. Metode: Metode yang digunakan pada penelitian ini adalah studi kasus yang berfokus pada Ny. N, seorang pasien ICU dengan risiko tinggi luka tekan (skor Braden 9). Intervensi berupa pijat effleurage menggunakan VCO dilakukan dua kali sehari selama empat hari. Tindakan ini disertai dengan asuhan keperawatan lainnya seperti pengelolaan ventilasi mekanis dan perawatan kulit. Hasil: Setelah empat hari perawatan, Ny. N tidak menunjukkan tanda-tanda luka tekan meskipun memiliki risiko tinggi. VCO terbukti membantu melembapkan kulit, mencegah iritasi, dan mengurangi risiko tekanan dari imobilisasi. Kesimpulan: Pijat effleuragedengan VCO merupakan intervensi keperawatan yang efektif, mudah dilakukan, dan dapat diandalkan dalam mencegah luka tekan pada pasien dengan imobilisasi berkepanjangan. Perawat memiliki peran strategis dalam penerapan metode ini untuk meningkatkan kualitas perawatan. Kata Kunci: dekubitus, massage effleurage, tirah baring Prevention of Pressure Ulcers Using Effleurage Massage and Virgin Coconut Oil (VCO): A Case Study ABSTRACT  Patients in critical illness often face a high risk of developing pressure ulcers due to prolonged immobilization. Pressure ulcers are caused by pressure, friction, moisture, and internal conditions such as malnutrition and anemia. This issue requires effective intervention for prevention. Objective: This study aims to evaluate the effectiveness of effleurage massage using Virgin Coconut Oil (VCO) as part of nursing care to prevent pressure ulcers in ICU patients. Methods: The study employed a case study approach focusing on Mrs. N, an ICU patient at high risk for pressure ulcers (Braden score of 9). The intervention involved effleurage massage using VCO, performed twice daily for four days. This intervention was accompanied by other nursing care measures, including mechanical ventilation management and skin care. Results: After four days of care, Mrs. N exhibited no signs of pressure ulcers despite her high-risk status. VCO was found to effectively moisturize the skin, prevent irritation, and reduce the risk of pressure-related damage from immobilization. Conclusion: Effleurage massage with VCO is an effective, simple, and reliable nursing intervention for preventing pressure ulcers in patients with prolonged immobilization. Abstract

Students were often stressed because of their exciting lecture and practical schedules, the difficulties of living in a dormitory, and other factors. Nursing students face various pressures in their personal lives and academic roles. Diaphragmatic breathing exercises are a form of therapy that can be used to reduce stress. Objective: The objective of the study was to find out how diaphragmatic breathing affected the stress level of nursing students at a private university in Tangerang, Banten. Methods: This study used a pre-experimental quantitative design with a one-group pretest and posttest research design. The method used is consecutive sampling, with a sample size of 225 respondents. This study used the Depression Anxiety Stress Scale (DASS-42) questionnaire administered between February and April 2024. Data analysis techniques include univariate and bivariate analyses using the Wilcoxon signed rank test. Results: The largest number of differences is at the normal level, which previously amounted to 104 people (46.2%) increased to 147 people (65.3%), meaning that there was an increase of 43 students (19.1%). While the least amount of difference is at a very severe level which only increased by 4 students (1.8%). The results of the bivariate analysis obtained a sig value (2-tailed) < 0.001 or equal to Sig < α 0.05, which means that there is a significant influence of diaphragmatic breathing exercises on students' stress levels. Discussion: The tight schedule of lectures and the pressure of dormitory life risk causing students to experience stress and can affect students in studying. One very simple and easy way to overcome stress is the diaphragmatic breathing technique. Conclusion: It is concluded that the provision of diaphragmatic breathing exercises has a significant effect on student stress levels. Keywords: diaphragmatic breathing, students, stress.

Abstract

Patient violence poses a real risk to themselves, others and environment. Violent behaviors can be controlled through restraint and seclusion at mental hospital and confinement at community. There are no specific policies and legal aspects concerning restraint, seclusion and confinement in Indonesia.  Purpose of this study: This study aimed to analyze policies and legal aspects concerning restraint, seclusion and confinement in Indonesia and foreign countries. Method: It was a qualitative study using thematic analysis that analyzed 8 foreign documents and 7 Indonesian documents. It analyzed the similarities and differences of legal aspects concerning restraint, seclusion and confinement based on the documents. Results: Themes from foreign countries were (1) restraint, seclusion and confinement as the last intervention; (2) family empowerment;(3) humanizing patients with restraint, seclusion and confinement; (4) safety and comfort of restraint, seclusion and confinement intervention for patients; (5) safety and comfort of restraint, seclusion and confinement intervention for nurses. Themes from Indonesia were (1) safety and comfort of restraint, seclusion and confinement intervention for patients and nurses; (2) protection against human. Discussion:Indonesia must have specific regulation about restraint, seclusion and confinement for mental disorder. Conclusion: This study provides an overview of the policies implemented in Indonesia. Keywords: Legal Aspect; Confinement; Policies; Restraint; Seclusion

Abstract

Kualitas tidur yang buruk dapat memengaruhi kejadian excessive daytime sleepiness (EDS). Kejadian EDS merupakan kantuk yang berlebihan pada siang hari. Tujuan: Penelitian ini bertujuan untuk mengidentifikasi hubungan antara kualitas tidur dan kejadian EDS pada perawat. Metode: Penelitian kuantitatif dengan desain analitik korelasional cross sectional. Sampel yang digunakan sebanyak 174 perawat yang bekerja di salah satu rumah sakit di Kota Depok dengan teknik purposive sampling dan kriteria inklusinya perawat yang melaksanakan dinas kerja shift. Kualitas tidur diukur dengan instrumen Pittburgh Sleep Quality Index (PSQI) dan kejadian EDS menggunakan instrumen Epworth Sleepiness Scale (ESS). Uji hipotesis menggunakan uji chi square. Hasil: Sebanyak 98 perawat (56,3%) memiliki kualitas tidur yang buruk dan 24 perawat (13,8%) mengalami EDS. Sebanyak 13,4% perawat yang mengendarai kendaraan sendiri mengalami kejadian EDS. Terdapat hubungan yang signifikan antara kualitas tidur dan kejadian EDS (p = 0,015; OR = 3,4). Perawat yang kualitas tidurnya buruk sebanyak 3,4 kali berisiko untuk mengalami EDS. Diskusi: Kualitas tidur yang buruk pada perawat dapat menyebabkan terjadinya EDS yang berisiko terhadap kecelakaan terutama saat berkendara. Kesimpulan: Perlunya manajemen untuk mengatasi kualitas tidur yang buruk dan meningkatkan kualitas tidur perawat seperti menghentikan penggunaan barang elektronik (misalnya, handphone) 30 menit sebelum tidur, mengurangi kebiasaan konsumsi kopi setiap malam hari terutama saat shift malam, dan memperbaiki gaya hidup dengan tidak merokok, tetapi memakan permen sebagai pengganti rokok. Waktu libur sebaiknya digunakan untuk beristirahat atau melakukan kegiatan yang disukai sebagai koping adaptif sehingga terhindar dari stres. Waktu istirahat di rumah sakit juga perlu dimanfaatkan dengan sebaik-baiknya. Kata Kunci: excessive daytime sleepiness, kualitas tidur, perawat  Sleep Quality and the Occurrence of Excessive Daytime Sleepiness among Hospital Nurses in Depok ABSTRACT Poor sleep quality can affect the occurrence of Excessive Daytime Sleepiness (EDS). EDS refers to excessive daytime drowsiness. Objective: This study aims to identify the correlation between sleep quality and the occurrence of EDS among nurses. Methods: This quantitative study employed a correlational analytic cross-sectional design. A total of 174 nurses working at a hospital in Depok City were selected using purposive sampling, with the inclusion criterion being nurses who work in shifts. Sleep quality was measured using the Pittsburgh Sleep Quality Index (PSQI), and the occurrence of EDS was assessed using the Epworth Sleepiness Scale (ESS). The hypothesis was tested using the Chi-Square test. Results: A total of 98 nurses (56.3%) were found to have poor sleep quality, and 24 nurses (13.8%) experienced EDS. Among nurses who commuted by driving themselves, 13.4% experienced EDS. There was a significant correlation between sleep quality and EDS occurrence (p = 0.015; OR = 3.4). Nurses with poor sleep quality were 3.4 times more likely to experience EDS. Discussion: Poor sleep quality among nurses may lead to EDS, which increases the risk of accidents, especially while driving. Conclusion: There is a need for effective management strategies to address poor sleep quality and improve the sleep quality of nurses. Suggested interventions include avoiding the use of electronic devices (e.g., mobile phones) 30 minutes before bedtime, reducing coffee consumption at night, particularly during night shifts, and adopting healthier lifestyles by replacing smoking with alternatives like candy. Rest days should be utilized for relaxation or engaging in enjoyable activities as adaptive coping mechanisms to prevent stress. Abstract

ABSTRAK Sekitar lima dari seratus siswa penyandang disabilitas mengalami cedera setiap tahunnya di sekolah. Guru adalah penanggung jawab anak-anak yang berada di lingkungan sekolah sehingga setiap guru perlu dan harus mampu memberikan pertolongan pertama pada anak ketika mengalami cedera atau kondisi buruk di lingkungan sekolah. Tujuan: Menganalisis pengetahuan guru sekolah luar biasa tentang pertolongan pertama pada cedera. Metode: Scoping review dengan pencarian artikel menggunakan empat database, yaitu Proquest, EBSCOHost, Scopus, dan Wiley Online serta melalui Google Scholar dan manual search melalui Google. Kriteria inklusi antara lain (1) subjek penelitian adalah guru sekolah luar biasa; (2) membahas hal terkait pertolongan pertama; (3) literatur berupa artikel ilmiah yang dipublikasi dalam 12 tahun terakhir (2013–2024); (4) open access; (5) artikel dalam bahasa Inggris atau bahasa Indonesia; dan (6) penelitian dengan metode kuantitatif atau kualitatif. Seleksi artikel menggunakan diagram PRISMA flow. Rayyan Ai digunakan untuk melakukan proses penjaringan artikel yang duplikat dan membantu proses seleksi artikel. Hasil: Sebanyak enam artikel memenuhi kriteria untuk dilakukan analisis. Sebagian besar artikel menjelaskan bahwa pengetahuan guru terkait pertolongan pertama masih rendah (66,6%), sebagian besar responden belum pernah mendapatkan pelatihan pertolongan pertama (50%), dan umumnya jenis pertolongan pertama di lingkungan sekolah adalah kasus cedera dan kegawatdaruratan medis (83,3%). Diskusi: Rendahnya pengetahuan guru sekolah luar biasa dalam memberikan pertolongan pertama dapat menghambat pemberian pertolongan pertama di sekolah. Adanya beberapa program, seperti pelatihan pertolongan pertama berupa edukasi dan praktik skill dapat meningkatkan pengetahuan guru terkait pertolongan pertama. Kesimpulan: Tingkat pengetahuan pertolongan pertama pada guru sekolah luar biasa mayoritas rendah. Direkomendasikan adanya program pelatihan pertolongan pertama dan pedoman yang dapat dimanfaatkan guru dalam memberikan pertolongan kepada siswa.   Kata Kunci: guru sekolah luar biasa, pengetahuan, pertolongan pertama cedera Special Needs School Teachers’ Knowledge of First Aid for Injuries: A Scoping Review ABSTRACT  Approximately five out of every 100 students with disabilities experience injuries annually in schools. Teachers are responsible for children under their supervision at school, making it essential for every teacher to possess the skills and ability to provide first aid when children experience injuries or adverse conditions at school. Objective: To analyze the knowledge of special needs school teachers regarding first aid for injuries. Methods: A scoping review was conducted by searching articles across four databases: ProQuest, EBSCOHost, Scopus, and Wiley Online, as well as Google Scholar and manual searches via Google. Inclusion criteria included: (1) study subjects being special needs school teachers; (2) discussing first aid; (3) literature in the form of scholarly articles published within the last 12 years (2013–2024); (4) open access; (5) articles written in English or Indonesian; and (6) studies using quantitative or qualitative methods. Article selection was guided by the PRISMA flow diagram. Rayyan AI was used to screen duplicate articles and assist in the article selection process. Results: Six articles met the criteria for analysis. Most articles revealed that teachers' knowledge of first aid was low (66.6%), the majority of respondents had never received first aid training (50%), and common first aid cases in schools included injuries and medical emergencies (83.3%). Discussion: Limited knowledge among special needs school teachers regarding first aid may hinder the provision of appropriate care in schools. Several programs, such as training that incorporates education and practical skills, can enhance teachers' knowledge of first aid. Conclusion: The majority of special needs school teachers have low levels of knowledge about first aid. It is recommended to implement first aid training programs and provide guidelines to support teachers in providing aid for students effectively.   Keywords: special needs school teachers, knowledge, first aid for injuries

Abstract

Objective: to examine the relationship between psychological changes and sleep quality with the incidence of hypertension in the elderly.Method: This cross-sectional study, conducted with a purposive sample of 126 elderly participants, assessed psychological changes using the Depression, Anxiety, and Stress Scale (DASS-42) questionnaire and sleep quality using the Pittsburgh Sleep Quality Index (PSQI) questionnaire. Blood pressure was measured using a sphygmomanometer to assess hypertension. Data analysis included independent t-tests and chi-square tests, with logistic regression used to identify predictors of hypertension. Results: The independent t-test revealed significant differences in stress levels between participants with and without hypertension (t = -6.614, p = 0.020). Similarly, significant differences in depression levels were observed between these two groups (t = -0.044, p = 0.014). Elderly individuals with poor sleep quality had a 5 times higher risk of experiencing hypertension (OR = 4.625, p = 0.008). Multivariate analysis indicated that age was a predictor of hypertension incidence (β = 0.208, p = 0.004), and sleep quality was also a predictor (β = -1.501, p = 0.023). Discussion: The findings suggest that psychological factors and sleep quality influence hypertension development in the elderly, likely through physiological mechanisms such as heightened sympathetic activity and vascular stress. These results highlight the importance of addressing both psychological health and sleep quality in managing hypertension risk. Conclusion: Sleep quality is a predictor of hypertension incidence in the elderly. Those with poor sleep quality have a 5 times greater risk of developing hypertension compared to those with good sleep quality. Keywords: elderly, hypertension, psychological changes, sleep quality
