Abstract

Appendicitis is part of the emergency cases that often occur in the abdominal area. Appendicitis is a prototype disease that takes place through inflammation due to obstruction and ischemia with the main symptoms which is pain that reflects the state of the disease. Appendicitis requires surgical appendectomy to reduce the risk of perforation. Apendectomy surgery is one type of wound that is an incision wound. The time of wound healing can be determined by the distinguishing between types of acute or chronic wounds. Appendectomy that does not have a post-surgical infection is categorized as an acute wound, physiologically the acute wound will heal ± 0-21 days. However, if the provision of nutrition is not fulfilled properly it will be almost the wound healing process. One factor that can influence the wound healing process is nutritional status. This study aim to determine the correlation of nutritional status with the wound healing process of post-appendectomy patients in RSUD Dr. H. Abdoel Moeloek, Bandar Lampung. This study used observational analytic with cross sectional approach. The sampling technique used was accidental sampling. The data were analyzed by using Chi-square test. From the Chi-square test found a relationship of nutritional status with a significant wound healing process (p <0.05) which is obtained p = 0.004, and the value obtained (OR = 10.5) can be said that adequate nutritional status is 10.5 times more likely to experience good wound healing when compared to undernourished nutritional status. There is a correlation between nutritional status and wound healing process in post-appendectomy patients.

Abstract

Viral Load (VL) is a direct indicator of the total amount of cells produced by the virus in a person infected with HIV. TB becomes a challenge for controlling Acquired Immunodeficiency Syndrome (AIDS) because it is the most opportunistic infection in people living with HIV / AIDS (PLWHA). TB can increase HIV progression and the risk of death for people living with HIV. Immunosuppression can increase the likelihood that dormant TB germs reactivate. The pathogenesis of TB infection in HIV patients is directly related to the decline in the immune system, specifically CD4 T lymphocytes. HIV infection will cause a decrease in CD4 T lymphocytes thereby reducing the immunological response to Mycobacterium tuberculosis. This will result in reactivation from the latent period of TB to an active infection. This study conducted to discover the relationship between the amount of viral load and the tuberculosis case in HIV / AIDS patients that undertake ARV therapy at RSUD Dr. H. Abdul Moeloek Lampung Province in 2019. Observational analytic approach with cross sectional design was used in this present study. The data taken is secondary data from the medical record of HIV / AIDS patients in Dr. H. Abdul Moeloek Lampung Province in 2019. The sampling technique used was a purposive sampling method and it was obtained a total sample of 196 people. The data was analyzed through Chi Square test. There was a significant relationship between the amount of viral load in HIV / AIDS patients and the tuberculosis case (p-value = 0.004). From the analysis above, it was obtained OR value = 2.52 which represents that patients who have a viral load ≥ 10,000 have a risk of 2.52 times to encounter TB. A high amount of Viral Load can cause immunosuppression in the host and increase the virulence of TB microbes.

Abstract

Appendicitis is one of the causes of mortality rates 0,2 – 0,8% in the world. In Indonesia itself, appendicitis occupies the fourth most patients who undergo hospitalization. The worse this type of appendicitis, the longer the patient should undergo hospitalization in the hospital's surgical treatment room. The duration of the patient post-surgery is varied depending on the type of appendicitis suffered. This study aims to describe the difference in the length of stay in post appendectomy patients between acute appendicitis and perforated appendicitis. The research is a comparative analytical research retrospective with a cross-sectional approach. In this study, the samples were recorded from medical records of appendicitis patients in 2016-2019, totaling 60 respondents of acute appendicitis and 60 respondents of perforation appendicitis. The sampling techniques in this study used a total sampling technique. Analysis of data is using univariate analysis and Independent bivariate tests. It is known that the average length of inpatient hospitalization is acute for 9 days. The shortest hospitalization is for 2 days, and the longest is 23 days. The respondent with an appendicitis perforation with an average length of stay is 34 days, the shortest time hospitalization is 4 days, and the longest is 90 days. The Mann-Whitney U test showed a significant difference (p = 0,000 / p<0,005) of hospitalization length in acute appendicitis and perforation appendicitis of an appendectomy patient.

Abstract

The interaction between TB and HIV infection is complex. In HIV-infected individuals, this situation will decrease the immune system and increase vulnerability to TB incidence. HIV causes reactivation, reinfection, and progression of latent TB infection to active tuberculosis. It also led to differences in the clinical presentation of TB in HIV patients, complications, and response to anti-TB treatment than in HIV negative patients. The purpose of this study To Know Correlation CD4 counts in patients with HIV / AIDS in hospitals Dr.H. Abdul Moeloek Lampung Province 2018. Research methods: cross-sectional approach, in which several variables tested in this research object data got from the medical record. Analysis of the data used is the Mann-Whitney statistical test. Abstract

Advances in the treatment and use of chemotherapy have been shown to improve the life expectancy rate for colorectal cancer patients. Studies conducted in China and Hongkong have shown that CapeOX combination chemotherapy regimens are more commonly used than in Europe and the United States. However, the use of chemotherapy drugs containing oxaliplatin and capecitabine can cause side effects such as hematological toxicity, which is one of them is anemia. This study aims to determine the difference in the form of a decrease in the average levels of hemoglobin and the degree of hemoglobin toxicity in colorectal cancer patients undergoing CapeOX chemotherapy. The Design in this study is a historical (retrospective) cohort. This study sample was 70 colorectal cancer patients who received CapeOX chemotherapy for 6 cycles at RSUD Dr. H. Abdul Moeloek in 2018-2019. Consecutive sampling is used in the sampling method. The statistical analysis is using Paired T-Test. There is a significant difference in the average hemoglobin level of colorectal cancer patients (p-value = <0.005), which receive CapeOX chemotherapy for 6 cycles. Besides, there is an increase in the number of patients who get hemoglobin toxicity and the chemotherapy cycle. In the first cycle, 59 patients (84.3%) got hemoglobin toxicity after chemotherapy, and the number continued to increase to 69 patients (98.6%) in the sixth cycle. There was a decrease in hemoglobin levels in colorectal cancer patients who received CapeOX chemotherapy with p-value = <0.05 and increased patients who got hemoglobin toxicity.
