Abstract

Noise-induced hearing loss (NIHL) is one of the most prevalent occupational diseases worldwide, particularly among workers exposed to traffic noise. Street cleaners are at high risk of sensorineural hearing loss from prolonged exposure to environmental noise without adequate hearing protection. This study aimed to determine the relationship between traffic noise exposure and the occurrence of sensorineural hearing loss among street cleaners in West Jakarta. The method of this study was a cross-sectional study conducted from August 2024 to March 2025 among 55 street-cleaning workers aged 18–50 years in Cengkareng District, West Jakarta. Data were collected using structured questionnaires, environmental noise measurements, and audiometric examinations. Bivariate and multivariate logistic regression analyses were performed to identify factors associated with hearing loss. The study found that 50.9% of respondents had sensorineural hearing loss, while 49.1% had normal hearing. Most participants were male (89.1%), aged 28–38 years (67.3%), and had worked for ?8 years (89.1%). Bivariate analysis showed that age was significantly associated with hearing loss (p=0.069). Multivariate analysis revealed that workers aged 39–49 years were less likely to experience hearing loss than younger workers (OR=0.241; 95%CI: 0.061–0.955; p=0.043), and those with ?8 years of service also had a lower risk (OR=0.085; 95%CI: 0.007–0.978; p=0.048). Duration of work, noise intensity, and PPE use were not statistically significant. In conclusion, age and length of service were significantly associated with the incidence of sensorineural hearing loss among street cleaners in West Jakarta. Strengthening preventive measures—such as regular noise monitoring, provision of standardized ear protection, and annual audiometric surveillance—is essential to reduce the burden of occupational hearing loss in this high-risk group.

Abstract

Background: Benign Prostatic Hyperplasia (BPH) is a common urological disorder in older men and may cause urinary elimination dysfunction after catheter removal or procedures such as transurethral resection of the prostate (TURP) and prostatectomy. Patients may experience urinary incontinence, urgency, nocturia, urinary retention, and impaired bladder control. Structured bladder training has emerged as a non-pharmacological nursing approach to support continence and bladder recovery. Objective: This systematic review analyzed the effectiveness of structured bladder training nursing interventions for urinary elimination recovery and reduction of urinary incontinence symptoms in patients with BPH or post-catheterization urinary dysfunction. Methods: A systematic review was conducted using the PRISMA 2020 framework. Literature searches were reported in the source document across ScienceDirect, PubMed, EBSCOhost, Cochrane Library, and ProQuest, with additional manual searching. Eligible studies included randomized controlled trials, quasi-experimental studies, intervention studies, and clinical trials published in 2020–2025 that evaluated bladder training or related continence-rehabilitation interventions. Methodological quality was assessed using the Joanna Briggs Institute Critical Appraisal Checklist for Randomized Controlled Trials. Results: Ten studies were included. The reviewed interventions improved urinary continence and bladder control and reduced urinary retention, urgency, nocturia, frequency, residual urine, and recatheterization in various populations. PFMT combined with urgency suppression significantly reduced urinary frequency, urgency, and IPSS (p0.001). Biofeedback combined with PFMT produced greater reductions in urinary incontinence than standard exercise. Conclusion: Structured bladder training and related continence-rehabilitation interventions show potential as evidence-based nursing strategies for improving urinary elimination recovery and urinary continence in patients with BPH and post-urological-procedure dysfunction.

Abstract

Medication errors remain one of the most significant preventable threats to patient safety, particularly in outpatient settings where medication-use processes involve multiple healthcare professionals and depend heavily on electronic medical record (EMR) systems. Conventional incident reporting is largely reactive and often fails to detect latent system vulnerabilities before adverse events occur. This study aimed to identify and evaluate potential failure modes in outpatient medication-use processes using the Healthcare Failure Mode and Effect Analysis (HFMEA) approach at an Indonesian university hospital. This study employed a qualitative prospective risk assessment design conducted between March and May 2025 in the outpatient pharmacy department of a university hospital in Indonesia. An interdisciplinary HFMEA team consisting of 11 healthcare professionals performed process mapping through focus group discussions, semi-structured interviews, and direct observations. Potential failure modes were analyzed using severity–probability scoring and HFMEA decision-tree analysis to determine priority risks requiring corrective action. The analysis identified six major medication-use processes, 22 subprocesses, and 75 potential failure modes, of which 23 required immediate intervention. The highest-priority risks were associated with EMR and network accessibility, electronic prescription entry, medication administration, prescription review and labeling, and medication preparation. The principal contributing factors included unstable digital infrastructure, incomplete electronic prescriptions, inconsistent adherence to standard operating procedures, ineffective interprofessional communication, and inadequate pharmacy facilities. HFMEA proved to be an effective proactive risk assessment tool for identifying latent failures before patient harm occurs. Strengthening digital infrastructure, standardizing medication workflows, enhancing multidisciplinary collaboration, and reinforcing patient safety practices are essential to improve medication safety and healthcare quality in outpatient services.

Abstract

Abstract

Background: Internalized HIV stigma occurs when people living with HIV (PLWH) internalize negative social beliefs about HIV and apply them to themselves. It may be associated with psychological distress and reduced engagement in HIV care. Psychosocial interventions have been developed to address this problem, but findings across studies are not consistent. Objective: This systematic review aimed to synthesize evidence on the effectiveness of psychosocial interventions compared with control or usual care for reducing internalized HIV stigma among PLWH. Methods: A systematic review was conducted using the PRISMA framework. Literature was searched in PubMed, ScienceDirect, and SAGE Journals using combinations of HIV, psychosocial intervention, and internalized stigma terms. Randomized controlled trials evaluating psychosocial interventions and reporting internalized HIV stigma were included. Methodological quality was assessed using the Joanna Briggs Institute critical appraisal checklist for randomized controlled trials. Results: Five RCTs were included. The interventions comprised peer mentoring, a video-based stigma-reduction intervention, iENGAGE, group-based cognitive behavioral therapy, and peer advocacy training. Four studies reported greater reductions in internalized HIV stigma in the intervention group than in the comparison group, whereas one study did not find a statistically significant between-group effect. Conclusion: Psychosocial interventions show potential for reducing internalized HIV stigma among PLWH, although effects are not uniform across settings and populations. Interventions incorporating peer support, shared experience, psychosocial skills, self-compassion, and empowerment may be useful components for nursing and HIV care.
