This research aims to determine the implementation of storing and returning medical record documents at Usu Hospital. This research is a descriptive research/qualitative approach. Determination of informants was carried out using purposive sampling. The informants in this study were the head of medical records and the medical records officer. Data collection by observation, interviews and documentation. The results of the research show that the management of medical record data at the North Sumatra Hospital in Medan is: First, making medical records, namely filling out forms for new patients, while old patients only show their treatment cards to be searched for medical record status by the filling officer manually. Second, Medical Record Storage (Filling), namely receiving files that are complete and arranged using a centralized system and using a direct numbering system for the time required to retrieve medical record files, namely 10-20 minutes. Third, destruction of medical records, which is carried out if the patient has not come for five years, is destroyed by shredding inactive files and then burning them. Fourth, maintaining the confidentiality of medical records, namely the custody of medical record files only allows medical record officers with legal interests. So the management of outpatient medical record data at USU Hospital has been carried out in accordance with Minister of Health Regulation Number 269/MENKES/KES/III/2008.

Occupational safety and health is an instrument that aims to protect companies, jobs, and society from hazards resulting from work accidents. Radiation workers are classified as hazardous jobs and have a high risk of exposure to radiation, which can cause stochastic and deterministic effects. One way to prevent and minimize radiation received by radiation workers is with a radiation safety management system in radiology installations, which consists of a radiation protection organization, individual dose monitoring, radiation protection equipment, medical examinations, document storage, quality assurance, as well as education and training. , as an effort to reduce the impact of radiation risk and maintain occupational safety and health. This study describes the radiation safety management system at the Medan Pulmonary Hospital, with a qualitative approach descriptive research design. The samples taken were 7 components of the Radiation Safety Management System at the Radiology Installation of the Medan Pulmonary Hospital .The research results obtained are radiation protection organizations led by the head of the hospital, radiation protection officers totaling 1 person, and radiation workers totaling 12 people whose role is to reduce the risks that occur due to radiation exposure in radiology installations. Individual dose monitoring tools are owned by all staff working in radiology installations which are read quarterly. Radiation protection equipment only uses 4 aprons, while other radiation protection equipment has not been fulfilled. Periodic medical examination of each radiation worker once in 1 year. Documentation, namely records of dose results, and occupational health cards, are always kept for five years from the time the radiation workers stop working. X-ray quality assurance in the radiology installation at the Medan Lung Special Hospital is made in the quality assurance program for the planning, construction, operation and maintenance of the installation. Not all education and training in the radiology installation at the Medan Lung Special Hospital regarding radiation safety protection have been received by radiation workers. It can be concluded that the radiation safety management system is well implemented for patient, environmental and staff safety. It is recommended the fulfillment of other radiation protection equipment and education and training for all radiation workers.

Hospitals as a health service are expected to provide satisfaction for patients. A hospital is a health service institution that provides comprehensive individual health services that provide inpatient, outpatient and emergency services. The health services provided are comprehensive services at the level of perfection of health services in creating a sense of self-satisfaction in each patient. The service in generating satisfaction is seen using five dimensions of service quality, namely the tangible dimensions of confidence, reliability (trust), responsiveness (ability). , assurance (Certainty), and empathy (empathy). To identify patient satisfaction with services at the outpatient registration site at the North Sumatra University Hospital. The type of research used is descriptive research with a quantitative approach and uses a cross-sectional data design. The samples taken were 98 patients using random sampling techniques. The data collection technique uses a questionnaire. Data analysis techniques use univariate analysis. based on research conducted from 98 respondents, patient satisfaction results showed that 5.10% of respondents said they were very satisfied, 87.76% said they were satisfied, 7.14% said they were quite satisfied. No respondents stated that they were dissatisfied or very dissatisfied. The highest satisfaction is in the tangibles dimension (85.71%) and the lowest satisfaction is in the empathy dimension (71.43%). In general, patients who receive services at the Outpatient Registration Center at the University of North Sumatra Hospital are satisfied with the services provided by registration officers, as evidenced by the results of patient satisfaction in the tangibles dimension of 85.71%, the reliability dimension of 73.47%, the responsiveness dimension was 74.49%, the assurance dimension was 80.61%, and the empathy dimension was 71.43%.

Not finding medical record files when they are needed for service is a problem in the storage department. The failure to find medical record files is not only influenced by the storage system, but also by other departments. This is because medical record files that have been used for patient care will undergo processing first before they are returned to storage (filing). This study aims to determine the absence of BRM on the Diling shelf and what are the factors in the length of waiting time for repeat patients at USU Hospital. Research subjects are informants. This means that people in the research setting are used to provide information about the situation and conditions of the research setting. The main data source in qualitative research. Based on the results of observations of the activities of filing officers, they have not implemented tracers in the process of storing medical record files. The results of observations from 27 August 2018 to 29 August 2018 saw an average of 174 outpatient medical record file requests. In requesting medical record files, the filing officer was quite good at compiling the existing BRM. Of the 174 medical record files requested, an average of 7 medical record files or 4.0% of medical record files were not found in one day. Based on the results of observations in the filing section. Based on the results of the research above, no factors were found in the storage shelves that cause medical record files, ranging from the highest active files to the wrong file arrangement and the doctor's integrity being the lowest.

The International Commission on Radiation Protection (ICRP) divides the effects of ionizing radiation on the human body into two, namely stochastic effects and deterministic effects. After knowing that X-ray radiation is dangerous for the human body, this can create a different perception for everyone, especially nurses. Nurses who do not have special knowledge about the dangers of X-ray radiation will have a different perception regarding the dangers of X-ray radiation. This study aims to determine the relationship between knowledge and nurses' perceptions about the dangers of X-ray radiation.The type of research used is analytical quantitative research with a survey method using a cross sectional approach. Data was collected using a questionnaire involving 100 respondents. Next, the data was analyzed using the Spearman Rank correlation test. The results of calculating the correlation between the knowledge and perception of nurses delivering patients showed an r value of 0.424 with a significance level of 0.006, so Ho was rejected, meaning there is a relationship between knowledge and perception. The results of this research show a positive and significant relationship between knowledge and perception. This means that there is an influence between the knowledge and perception of patient delivery nurses at the Radiology Installation of North Sumatra University Hospital. Meanwhile, the strength of the relationship is moderate. This means that knowledge is a factor that influences perception as well as level of education and age.

Officer performance is a statement of an activity or pattern of behavior or application of learning abilities to complete a process related to predetermined specifications or standards. Use of tracer or outguide and guidance cards used to replace medical record files taken for various staff needs in the storage of North Sumatra University Hospital. The population was 4 people and the sample was 4 people as the total sampling. The instrument in this study used a questionnaire, the respondents were storage officers and unit heads at the North Sumatra University Hospital. From the research results it was found that the influence of officer performance according to the majority of respondents in the category of strongly agree was 2 people (50.0), and 2 people agreed (SO.O) with manual calculations. Based on the results of the research conducted, it can be concluded that the influence of officer performance in taking outpatient medical records on the use of tracers at North Sumatra University Hospital.

One of the clinical conditions in the thorax is pleural effusion, which is an abnormal accumulation of pleural fluid caused by the formation of pleural fluid faster than the absorption process. Technique. The aim of this study was to determine the thorax examination procedure in clinical pleural effusion at the Radiology Installation of Sundari General Hospital. This type of research is qualitative using a descriptive method, a case study approach. Data collection methods are, observation, interviews, and documentation. From the results of the research, the management of thorax examination in clinical pleural effusion at the Radiology Installation at Sundari General Hospital, thorax examination in clinical pleural effusion requires the patient to be in an erect or standing position. According to the interview results that have been obtained, chest examination in clinical pleural effusion in the PA and lateral positions can confirm the diagnosis of clinical pleural effusion.

Head injury or traumatic brain injury (TBI) is defined as a mechanical trauma to the head directly or indirectly that causes impaired neurological function, namely physical, cognitive, psychosocial functioning, both temporarily and permanently. The national prevalence of head trauma according to Riskesdas in 2013 increased 0.7% to 8.2% where men are twice as likely to experience head injuries than women. The diagnosis of head injury is established based on anamnesis in the form of post-traumatic disorders of the head followed by the presence or absence of disturbances of consciousness or lucid intervals, otorrhea or rhinorrhea, and post-traumatic amnesia. Neurological clinical assessment is necessary to assess whether there are signs of focal deficits. Supporting examinations in the form of imaging with a plain photo of the head in the AP/lateral/tangential position or with a CT scan of the head. 61-year-old man came with a history of fainting for five minutes which was preceded by a headache and profuse vomiting after being hit by a tree branch on the back of his head. On physical examination he was conscious of compos mentis and found a 4cm hematom in right occipital region. On ECG found sinus bradycardia. General status and other neurological status within normal limits. Radiological results showed an extracranial hematoma in the occipitoparietal region, a subdural hematoma in the right occipital lobe and a fracture of the right occipital bone. Patient received nasal cannula oxygenation, asering and mannitol infusion, tranexamic acid, citicholin and piracetam injection. The condition of the head injury in this case was a severe head injury as indicated by the presence of a subdural hematoma and fracture in the right occipital region even though the patient's consciousness from GCS was normal. The patient's condition improved after administration of osmotic diuretics, antifibrinolytics and neuroprotectant agents.

Physical fitness is a description of a person's physical condition or functional condition in carrying out various jobs or daily activities. The higher the level of physical fitness, the better the physical condition, so that they are able to do various jobs or workloads given and will be successful in completing the work. That way the ability or level of productivity is higher. To be able to do work or daily activities well, it takes good physical fitness as well. Healthy living habits are one of the factors that must be considered and can affect physical fitness, one of these habits is sleep. Sleep is a basic human need. Every human being needs time to rest and sleep to be able to restore stamina or good body condition so that they can do various jobs or daily activities. The purpose this research are to determine the relationship between sleep quality and physical fitness level for students of the Faculty of Medicine, University of Muhammadiyah North Sumatra, Class of 2019. Using an analytical method with a cross sectional approach, that is, each research subject will only be used once to determine the relationship between sleep quality and fitness level. physical student of the Faculty of Medicine, University of Muhammadiyah North Sumatra. The data of this study were obtained using the PSQI questionnaire and the Harvard Step Test. Data analysis using Chi Square test. The results showed that most of the respondents had poor sleep quality (64.4%) and most of them had a very poor level of physical fitness (57.8%). And there is a relationship between sleep quality and physical fitness level (p = 0.001).

Background: A very uncommon subtype of chronic cholecystitis is follicular cholecystitis (FC). Hyperplastic lymphoid follicles and noticeable germinal centers are its defining features. About 2% of standard cholecystectomies involve it. A very uncommon form of chronic cholecystitis is follicular cholecystitis. At least three lymphoid follicles per cent of the gall bladder tissue and an inflammatory infiltrate that is almost entirely made up of dispersed, well-formed lymphoid follicles define it. To prevent misinterpretation of lymphoma, the pathologist needs to be knowledgeable with this condition. Objective: To educate pathologists about this entity, this case is being presented. Case description: We describe a case of FC in a 42-year-old woman in this article. Her abdominal pain was clinically determined to be calculus cholecystitis, and laparoscopic cholecystectomy was used to treat it. It was grossly observed that the gall bladder wall had thickened. A histopathological study showed that the gall bladder wall was heavily infiltrated with lymphocytes, which produced lymphoid follicles with distinct germinal centers. We therefore determined that FC was the cause. This growth should not be confused with lymphoma. A thorough histological analysis is diagnostic, and immunohistochemistry may be used in some cases. Conclusion: This case was used to highlight the rarity of follicular cholecystitis. It is a benign and extremely uncommon entity that frequently looks like lymphoma. FC does not seem to be associated with autoimmunity, lymphoma, or obstructive pathologies.
