Abstract

Labor pain is a major concern for mothers during childbirth, particularly in the active phase of the first stage of labor. Effective and safe pain management is essential in midwifery care. One recommended non-pharmacological method is the combination of counterpressure techniques and the use of a birth ball. This study aimed to assess the effectiveness of implementing counterpressure with a birth ball in reducing labor pain at BaliMed Hospital Denpasar. A pre-experimental study with a one-group pretest-posttest design was conducted involving 30 mothers in the active phase of the first stage of labor, selected through accidental sampling. Pain intensity was measured using the Visual Analogue Scale (VAS) before and after the intervention. The results showed that the mean pain score decreased from 5.37 ± 1.402 (moderate pain) to 3.60 ± 1.329 (mild pain). The Wilcoxon Signed Rank Test indicated a statistically significant reduction in pain (Z = -4.869, p < 0.05). These findings suggest that counterpressure combined with birth ball therapy is effective in reducing labor pain. This method can be recommended as a simple, safe, cost-effective, and non-pharmacological alternative for managing pain during labor in clinical midwifery practice.

Abstract

The success of implant insertion is influenced by the skills of midwives as health workers. Factors such as length of service and level of knowledge are thought to be related to midwives' skills in implant insertion. The purpose of this study was to determine the relationship between length of service and midwives' knowledge of contraceptive implants with their skills in implant insertion. This study was a quantitative correlation study with a cross-sectional design. The population in this study was all midwives in the Darek Community Health Center working area, and the sample was determined using total sampling technique, where the entire population was used as the sample. The number of samples obtained was 49 midwives in the Darek Community Health Center working area. The data were analyzed using Fisher's exact test. The results of this study indicate a significant relationship between length of service and skills (p-value = 0.002) and knowledge and skills (p-value = 0.002). This study proves that there is a significant relationship between length of service and midwives' knowledge and skills in implanting contraception. The longer the work experience and the better the midwife's knowledge, the better the skills in implanting contraceptive implants tend to be.

Abstract

Sleep is an essential need in children's health, but sleep disorders can negatively impact children's health and growth and development. This study aims to assess the relationship between sleep quality and the growth and development of toddler-age children in the Karangdadap Pekalongan, Central Java. A correlative descriptive study with a cross-sectional approach was conducted on 267 children aged 1-3 years. Data collection from October to December 2023 used scales, microtoise, KMS GRAPHS, DDST II, and BISQ questionnaires. Data were analyzed using descriptive statistics and the Chi-Square test. The majority of respondents (82.4%) had good sleep quality. The study showed that there was a significant relationship between sleep quality and child growth; children with sleep disorders had a higher risk of undernutrition and overnutrition (p = 0.0001; OR = 5.260). Sleep quality was significantly associated with child development; children with sleep disorders were at a higher risk of suspected developmental issues (p = 0.0001; OR = 37.217). Sleep quality is highly related to the growth and development of preschool children. Sleep disorders have the potential to reduce the quality of child growth and development, making it important to manage and address sleep disorders to support children's health.

Abstract

Speech delay is one of the most common developmental disorders in toddlers, potentially affecting communication and social skills. The age of 1–2 years is a golden period of brain development, where appropriate language stimulation is crucial. This study aimed to determine the effectiveness of Applied Behavior Analysis (ABA) therapy on language development in toddlers aged 1–2 years with speech delay. A pre-experimental design was used with one group pre-test and post-test. The sampling technique applied was total sampling, involving 15 toddlers at Rumah Terapi Ridha Sidoarjo. Language development was assessed using the Capute Scales. ABA therapy was administered weekly over 8 weeks, involving structured activities such as vowel recognition, syllable repetition, and word formation. Prior to intervention, all participants (100%) were categorized as poor, with an average score of 67.17. After therapy, 40% of children moved to the fair category, with an average score of 79.82. The Shapiro-Wilk test confirmed normal data distribution, and the paired t-test showed a significant difference (p = 0.001). These findings indicate that ABA therapy significantly improves language development in toddlers with speech delay and may serve as a practical intervention for early language support.

Abstract

The global exclusive breastfeeding rate currently stands at 48%, slightly below the 50% global goal for 2025. In Indonesia, although the coverage rate has improved to 74.73%, it remains considerably below the national target of 80%, as stipulated by national policy and Minister of Health Decree No. 450/Menkes/IV/2004. This study aimed to evaluate the feasibility of an educational medium: an animated video called MAPS (Manajemen ASI Perah Sehat), designed to enhance mothers' knowledge about expressed breast milk and support exclusive breastfeeding practices. The research employed a Research and Development (R&D) approach through six stages. The subjects were 10 breastfeeding mothers with infants aged 0–3 months, selected via purposive sampling. Evaluation instruments included expert assessment questionnaires and user trials.The validation results confirmed the medium's high feasibility: Media 93% and Material 97.2%. User testing yielded a score of 90.38%, while the small-scale product trial resulted in an excellent score of 98.3%. The conclusion is that the MAPS animated video is highly feasible to be used as an educational tool. It effectively enhances mothers' understanding of expressed breast milk management, thereby supporting the success of exclusive breastfeeding, especially for working mothers.

Abstract

Preterm birth, defined as delivery before 37 completed weeks of gestation, is one of the leading causes of neonatal morbidity and mortality worldwide. This condition can be influenced by various factors, including obstetric and sociodemographic factors. This study aimed to identify the factors associated with preterm birth at the Regional Public Hospital (RSUD) of Buleleng Regency. A case-control study design with a retrospective approach was employed from January to March 2025. The accessible population included all preterm births at RSUD Buleleng from January to December 2024. A total of 268 respondents (134 cases and 134 controls) were selected using simple random sampling. The results showed significant associations between maternal age and preterm birth (p < 0.001; AOR 6.565; 95% CI 3.119-13.817), employment status (p = 0.007), parity (p = 0.002), premature rupture of membranes (p < 0.001; AOR 6.207; 95% CI 2.123 – 18.148), and threatened preterm labor (p < 0.001; AOR 4.150; 95% CI 2.137 – 8.059). No significant association was found between educational level or history of preeclampsia and preterm birth. Maternal age was identified as the most dominant predictor. These findings highlight the importance of early detection by healthcare providers to prevent preterm birth and its associated complications.

Abstract

Hepatitis B infection during pregnancy remains a significant public health issue due to its maternal and neonatal complications. This study aimed to identify factors associated with Hepatitis B infection among pregnant women in Denpasar City. A case-control design was conducted from February to April 2024 in four randomly selected community health centers, involving 128 pregnant women (64 HBsAg-positive and 64 HBsAg-negative) chosen through multistage random sampling. Data were collected using questionnaires and health center records, and analyzed with bivariate tests and logistic regression. Bivariate analysis showed significant associations between education level, parity, water source, and abortion history with Hepatitis B infection. Multivariate analysis revealed that women with primary/secondary education had a 3.4-fold higher risk of infection (AOR = 3.398; 95% CI: 1.272–9.075), and those using well water had nearly a threefold higher risk (AOR = 2.979; 95% CI: 1.343–76.607). History of blood transfusion increased the risk threefold (AOR = 3.022; 95% CI: 1.180–7.724), while abortion history was the strongest predictor (AOR = 26.267; 95% CI: 2.868–240.601). In conclusion, low education, unsafe water sources, blood transfusion, and abortion were key risk factors. Strengthening maternal education and infection control in reproductive health services is essential.

Abstract

Language ability is a crucial aspect of child development, yet language delay remains a common issue both globally and nationally, including in the Tanon Primary Health Center area. This study aimed to analyze the relationship between parental verbal stimulation and language development in infants aged 6–12 months. An analytic observational study with a cross-sectional design was conducted in May–June 2025, involving 41 parents and their infants selected through proportionate stratified random sampling. Data were collected using a validated verbal stimulation questionnaire (α = 0.767) and expert-validated language development observation sheets (validity score = 0.8295). Results showed that parental verbal stimulation was categorized as good (48.78%), fair (43.9%), and poor (7.32%). Infant language development outcomes were very well developed (36.59%), as expected (26.83%), emerging (21.95%), and not yet developed (14.63%). Spearman Rank correlation indicated a significant association between parental verbal stimulation and infant language development (p = 0.02). The findings highlight the importance of consistent early verbal stimulation supported by healthcare workers through education and counseling at community health posts (Posyandu) to prevent language delay.

Abstract

Adolescent pregnancy is associated with increased risks of obstetric complications (anemia, preeclampsia, and delivery problems) and adverse perinatal outcomes such as low birth weight, prematurity, and neonatal death. Specific health interventions, including nutritional supplementation, counseling, health education, and group based antenatal care (ANC), have been proposed to mitigate these risks. This study aimed to identify and analyze the impact of specific health interventions including nutritional supplementation, counseling, group ANC, and health education on obstetric and perinatal outcomes in adolescent pregnancies through a systematic literature review (SLR). This review followed the PRISMA 2020 framework, encompassing four stages: identification, screening, eligibility, and inclusion. Articles were searched from Scopus and Google Scholar using defined keywords. Of 241 records identified, 41 full-text articles were reviewed, and 7 met the inclusion criteria (quantitative studies or systematic reviews evaluating specific interventions among pregnant adolescents). Nutritional supplementation, particularly iron and folic acid, effectively reduced anemia and improved birth weight. Counseling and health education enhanced adolescents’ knowledge, attitudes, and ANC compliance. Group ANC models reduced low birth weight and preterm birth while promoting social support. Specific health interventions significantly improve obstetric and perinatal outcomes among adolescent pregnancies. Further experimental studies with rigorous designs are needed to strengthen causal evidence.

Abstract

Breast milk production plays a vital role in supporting infant growth and development, yet many postpartum mothers face challenges during the early lactation period. In Indonesia, exclusive breastfeeding coverage remains suboptimal at 52.5% (Riskesdas), with insufficient milk production being one of the contributing factors. This study aimed to evaluate the effectiveness of rolling massage and warm compress in enhancing breast milk production. A quasi-experimental design was used with intervention and control groups, each consisting of 10 purposively selected respondents. The intervention was provided once daily for three consecutive days, 15 minutes per session, and breast milk volume was measured using the pumping test method. Results showed a significant difference in mean breast milk production between the intervention group (63.1 ml) and the control group (50.3 ml). The increase in the intervention group was 24.9 ml compared to 12.1 ml in the control group. A paired sample t-test yielded a p-value of 0.000 (<0.05), confirming the effectiveness of rolling massage and warm compress in improving breast milk production. These findings suggest that incorporating simple, non-invasive techniques such as rolling massage and warm compress into maternal health programs may help promote the success of exclusive breastfeeding.
